We studied the comparative in vitro activity of ertapenem, a new carbapenem, against 240 aerobic and 180 anaerobic recent clinical bite isolates using an agar dilution method and an inoculum of 10(4) cfu/spot for aerobes and 10(5) cfu/spot for anaerobes. Ertapenem inhibited 410/420 (98%) of the isolates tested at < or = 4 mg/L with only 4/5 Campylobacter gracilis and 1/3 Campylobacter rectus strains requiring . or = 16 mg/L for inhibition. Ertapenem was only moderately active (MIC 8 mg/L) against 4/6 Enterococcus faecalis and 1/11 Staphylococcus epidermidis strains. All Pasteurella multocida, Pasteurella septica, Pasteurella canis, Pasteurella dagmatis, Moraxella spp. and EF-4 isolates were inhibited at < or = 0.015 mg/L. MIC(90)s for other aerobic genera and species were as follows: Corynebacterium spp., 4 mg/L; Staphylococcus aureus, 0.25 mg/L; Staphylococcus epidermidis, 4 mg/L; other coagulasenegative staphylococci, 0.25 mg/L; Streptococcus milleri group, 0.5 mg/L; Eikenella corrodens, 0.03 mg/L; and Bergeyella zoohelcum, 0.5 mg/L. For anaerobes the range of MICs and MIC(90)s were: Prevotella ssp., < or = 0.015-0.5, 0.125 mg/L; Porphyromonas spp., < or = 0.015-0.03, 0.015 mg/L; Fusobacterium spp., 0.015-0.125, 0.03 mg/L; Bacteroides tectum, 0.03-0.125, 0.125 mg/L; and Peptostreptococcus spp., 0.01-2, 1 mg/L. Ertapenem showed excellent potency against the full range of animal and human bite wound pathogens.

Pasteurella multocida is composed of three subspecies that are often differentiated by fermentation of sorbitol and dulcitol. We studied 35 dulcitol-negative P. multocida isolates from infected dog and cat bite wounds, 16 of which yielded weak and/or conflicting fermentation reactions in Andrades sorbitol, thus making it difficult to distinguish between the two dulcitol-negative subspecies of P. multocida, i.e., P. multocida subsp. multocida and P. multocida subsp. septica. All isolates and two control strains were further analyzed using a PCR fingerprinting technique with a single primer (M13 core) and assessed for alpha-glucosidase (alpha-Glu) activity. Although the PCR fingerprint patterns and alpha-Glu activity did not correlate well with the sorbitol fermentation reactions, they did correlate well with each other. All strains identified as P. multocida subsp. septica were positive for alpha-Glu activity and exhibited the group I PCR fingerprint profile. All strains categorized as P. multocida subsp. multocida displayed either the group II or group III PCR fingerprint profile; 9 of 11 of these isolates were alpha-Glu negative. These data suggest that both PCR fingerprinting and alpha-Glu activity provide reliable means for differentiating P. multocida subsp. multocida from P. multocida subsp. septica, particularly in strains that produce weak and/or discrepant sorbitol fermentation reactions.

Background and Methods: To define better the bacteria responsible for infections of dog and cat bites, we conducted a prospective study at 18 emergency departments. To be eligible for enrollment, patients had to meet one of three major criteria for infection of a bite wound (fever, abscess, and lymphangitis) or four of five minor criteria (wound-associated erythema, tenderness at the wound site, swelling at the site, purulent drainage, and leukocytosis). Wound specimens were cultured for aerobic and anaerobic bacteria at a research microbiology laboratory and, in some cases, at local hospital laboratories., Results: The infected wounds of 50 patients with dog bites and 57 patients with cat bites yielded a median of 5 bacterial isolates per culture (range, 0 to 16) at the reference laboratory. Significantly more isolates grew at the reference laboratory than at the local laboratories (median, 1; range, 0 to 5; P<0.001). Aerobes and anaerobes were isolated from 56 percent of the wounds, aerobes alone from 36 percent, and anaerobes alone from 1 percent; 7 percent of cultures had no growth. Pasteurella species were the most frequent isolates from both dog bites (50 percent) and cat bites (75 percent). Pasteurella canis was the most common isolate of dog bites, and Past. multocida subspecies multocida and septica were the most common isolates of cat bites. Other common aerobes included streptococci, staphylococci, moraxella, and neisseria. Common anaerobes included fusobacterium, bacteroides, porphyromonas, and prevotella. Isolates not previously identified as human pathogens included Reimerella anatipestifer from two cat bites and Bacteroides tectum, Prevotella heparinolytica, and several porphyromonas species from dog and cat bites. Erysipelothrix rhusiopathiae was isolated from two cat bites. Patients were most often treated with a combination of a beta-lactam antibiotic and a beta-lactamase inhibitor, which, on the basis of the microbiologic findings, was appropriate therapy., Conclusions: Infected dog and cat bites have a complex microbiologic mix that usually includes pasteurella species but may also include many other organisms not routinely identified by clinical microbiology laboratories and not previously recognized as bite-wound pathogens.

The activity of trovafloxacin and five other oral agents against 250 aerobic and 137 anaerobic strains isolated from human and animal bite wounds was determined by an agar dilution method. Trovafloxacin was active against all aerobic and fastidious facultative isolates at < or = 0.5 mg/L and all anaerobes at < or = 2 mg/L (Bacteroides tectum, Porphyromonas salivosa and Prevotella heparinolytica, < or = 0.25 mg/L; Porphyromonas spp., < or = 0.5 mg/L; Prevotella spp. and peptostreptococci, < or = 2.0 mg/L), except Fusobacterium nucleatum and other fusobacteria (MIC90 < or = 4 mg/L). Levofloxacin was generally one to two dilutions more active than ofloxacin, while ciprofloxacin was active against aerobes (MIC < or = 1 mg/L) but less active against anaerobic strains (MIC90 < or = 16 mg/L).

The activities of DU-6859a, levofloxacin, ofloxacin, sparfloxacin, and ciprofloxacin against bite wound isolates were determined by the agar dilution method. DU-6859a was the most active compound (MICs, < or = 0.125 microg/ml) against all Pasteurella species, Staphylococcus aureus, and streptococci; anaerobes were susceptible to < or = 0.5 microg/ml, except fusobacteria, which were susceptible to < or = 2 microg/ml. Against aerobes, levofloxacin was more active than ofloxacin (MIC at which 90% of isolates are inhibited [MIC90], < or = 1.0 microg/ml for both) and sparfloxacin and ciprofloxacin were also active (MIC90s, < or = 0.25 and < 1 microg/ml, respectively).

Saccharolytic, nonpigmented, anaerobic gram-negative rods isolated from infected dog and cat bite wounds in humans have been poorly characterized, and most are not included in the databases of kits used for anaerobic identification; thus, they are problematic for clinical laboratories to identify. Fifty strains isolated from such wounds were characterized with commercial kits for preformed-enzyme detection, carbohydrate fermentation, and other biochemical tests. PCR fingerprinting was performed on these strains to further characterize subgroups within these species. Bacteroides tectum is a frequent isolate in bite wounds and resembles Prevotella bivia in colony morphology and saccharolytic activity, except that it grows in 20% bile and hydrolyzes esculin. Profile numbers generated by various kits associate B. tectum with P. bivia, Prevotella oralis group, or Prevotella melaninogenica. PCR fingerprinting identified at least four subgroups and confirmed the heterogeneous nature of this species. Prevotella heparinolytica was also frequently isolated from these bite wounds. It produces indole and generates a profile number in preformed-enzyme kits that is usually associated with Bacteroides uniformis. However, it is bile sensitive and quite distinct from the Bacteroides fragilis group of anaerobes. The PCR fingerprint profiles generated by strains of P. heparinolytica were very similar to that of the type strain and to each other. Prevotella zoogleoformans, occasionally isolated from dog and cat bite wounds in humans, resembles P. heparinolytica except for a negative indole test. Clinical laboratories should be aware of the characteristics of these animal species when identifying isolates from animal bite wounds in humans.

We isolated 40 strains of Porphyromonas (formerly Bacteroides) species from 29 of 102 cat and dog bite wounds in humans. P. salivosa, P. gingivalis, and P. canoris were the most frequent isolates. A comparison of the RapID ANA II system (Innovative Diagnostic Systems, Norcross, GA), An-IDENT panels (bioMérieux, St. Louis), and API ZYM strips (bioMérieux) showed that the latter kit best characterized these isolates because it included tests for trypsin and chymotrypsin activity; however, the tests for glycosidase activity in this kit were less sensitive than were those in the other kits. None of the biochemical systems was able to identify all species. Arbitrarily primed-polymerase chain reaction fingerprinting with a nonspecific single primer, T3B, yielded distinct profiles for type strains and for the clinical isolates, suggesting that some of the isolates represented previously undescribed species. Growth of these species took > or = 5 days; therefore, laboratories should incubate anaerobic plates from bite wound cultures for > or = 7 days to assure isolation of these common pathogens.

Three patients with simian bites and resultant infection are described. The bacteriology of the wounds was diverse and included alpha-hemolytic streptococci and other streptococci in all wounds, enterococci, Staphylococcus epidermidis, and Enterobacteriaceae. A literature review revealed brief mentions of 132 cases of simian bites in some of which Bacteroides species, Fusobacterium species, and Eikenella corrodens were isolated. Infection, despite antimicrobial therapy, and complications, such as osteomyelitis and flexion contractures, occurred frequently.

The activities of sparfloxacin, levofloxacin, Bay y 3118, azithromycin, cefprozil, loracarbef, and nine other oral antimicrobial agents against 194 aerobic and anaerobic clinical bite wound isolates were determined by the agar dilution method. Sparfloxacin, levofloxacin, and Bay y 3118 were active against all aerobic isolates (MICs at which 90% of the isolates are inhibited [MIC90], < or = 1.0 microgram/ml for sparfloxacin and levofloxacin and 0.1 microgram/ml for Bay y 3118) and many anaerobic isolates, with the exception of the fusobacteria. Azithromycin was more active than erythromycin by 1 to 2 dilutions against many aerobes, including Pasteurella multocida and Eikenella corrodens, and by 2 to 4 dilutions against anaerobic isolates. Cefprozil was more active (MIC90, < or = 1 microgram/ml) than loracarbef (MIC90, < or = 4 micrograms/ml) against aerobic gram-positive isolates, but both had poor activity (MIC90, > or = 16 micrograms/ml) against peptostreptococci. Both cefprozil and loracarbef had MIC90s of < or = 0.5 micrograms/ml against P. multocida.

The in vitro activities of sparfloxacin, temafloxacin, ciprofloxacin, ofloxacin, clarithromycin, erythromycin, tetracycline, cephalothin, penicillin G, and amoxicillin-clavulanic acid against 173 recent clinical bite wound isolates were determined by agar dilution. Sparfloxacin was active against all strains (MIC for 90% of strains tested, < or = 1 micrograms/ml) except for most fusobacteria and one-third of the Prevotella spp. The other fluoroquinolones had similar activities but higher MICs, especially for streptococci. Clarithromycin was more active against many isolates including Pasteurella multocida than erythromycin, with MICs of < or = 2 micrograms/ml (versus 4 micrograms/ml for erythromycin).

One of every two Americans will be bitten by an animal or by another person at some point. Bites account for approximately 1% of all visits to emergency rooms; injuries inflicted by dogs are most common. The bacteria involved in infection of bite wounds include Pasteurella multocida, Staphylococcus aureus, Staphylococcus intermedius, alpha-hemolytic streptococci, Capnocytophaga canimorsus, and other members of the oral flora. Anaerobic bacteria are present in approximately one-third of bite wounds and are associated with the formation of abscesses and with relatively serious infections. P. multocida is found in infections of cat bites more than 50% of the time. The bacteriology of bite wounds inflicted by exotic animals reflects the animals' oral flora. Bites inflicted by humans are often more serious than those inflicted by animals. Infections of human bites are associated with alpha-hemolytic streptococci, S. aureus, Eikenella corrodens, Haemophilus species, and (in more than 50% of cases) anaerobic bacteria. The principles of management of bite wounds are discussed.

An unusual gram-negative rod (RMA 1571) was isolated from a hand infection following a pig bite. This unclassified isolate was characterized by growth requirements, microscopic examination, biochemical characteristics, antimicrobial susceptibility tests, and cellular fatty acid analysis. It was indole positive and produced yellow-pigmented growth, which placed it in the genus Flavobacterium, but its other features, including cellular fatty acid analysis, did not appear to be those of a named species.

Staphylococcal gingival flora was characterized in cultures from 135 dogs. Staphylococcus intermedius was isolated in 39% of the cultures, S. aureus was isolated in 10%, and both were isolated in 2.0%. S. aureus was isolated more often from dogs of working breeds with weights of greater than 40 lb (ca. 18 kg) and with outdoor habitats than was S. intermedius, which was associated with dogs of nonworking breeds with weights of less than 40 lb and indoor habitats. S. intermedius was distinguished from S. aureus by the following characteristics: coagulation of rabbit plasma at 4 h (26 versus 100%, respectively), hemolysis of sheep blood at 24 h (30 versus 79%, respectively), and mannitol fermentation at 24 h (4 versus 93%, respectively). A clear separation of the two species was apparent only with the acetoin (modified Voges-Proskauer) reaction (100% of the S. aureus isolates versus 0% of the S. intermedius isolates) and beta-galactosidase activity on the API Staph-Ident strip (0% of the S. aureus isolates and 100% of the S. intermedius isolates). Susceptibilities of S. intermedius and S. aureus were 72 and 7%, respectively, to penicillin G, and 100% of both species to oxacillin. Fourteen previously collected strains of coagulase-positive staphylococci from infected canine-inflicted human wounds were reanalyzed; 3 of 14 (21%) isolates were S. intermedius. We conclude that S. intermedius is a common canine gingival flora and is responsible for some canine-inflicted human wound infections, thus representing a newly recognized zoonotic pathogen.

The etiologic agents usually involved in wound infections due to human or animal bites are the aerobic skin flora of the victim, e.g., Staphylococcus aureus, and/or the aerobic oral flora of the biter, e.g., Pasteurella multocida. While anaerobic bacteria are predominant in the normal oral flora of humans and animals, their importance in the pathogenesis of bite-wound infections has not been stressed. Most investigators in this field have either not cultured these wounds for anaerobic bacteria or not utilized optimal culture techniques. In a series of studies on human and animal bite wounds, methods that are optimal for recovery of anaerobic bacteria were used. Anaerobes were found in significant quantities in 39% of animal bite wounds, 50% of human bite wounds, and 56% of clenched-fist injuries. Several species of anaerobes usually were present in the wounds and always were present in mixed culture with aerobic oral flora. The anaerobes most commonly isolated included Bacteroides asaccharolyticus, Bacteroides bivius, Bacteroides disiens, Bacteroides melaninogenicus, Bacteroides oralis, Bacteroides ruminicola, Bacteroides pneumosintes, Bacteroides ureolyticus, Fusobacterium nucleatum, Fusobacterium russii, Peptococcus species, Peptostreptococcus species, and Veillonella species. Initial, empiric antimicrobial therapy for bite wounds should be directed against potential anaerobic as well as aerobic pathogens.

Thirty patients received penicillin (17 of these also received dicloxacillin) and 31 received amoxicillin/clavulanic acid in a randomized study of the outpatient management of bite wounds. A wide variety of aerobic (185 strains) and anaerobic (47 strains) bacteria were isolated from these wounds. The control and study regimens were equally effective. There were four therapeutic failures in all. Aggressive wound management along with antimicrobial therapy led to a 87-96% cure rate. Loose stools or diarrhea was a more frequent side effect with amoxicillin/clavulanic acid than with penicillin +/- dicloxacillin.

Staphylococcus intermedius is a Gram-positive, coagulase-positive coccus that can be distinguished from Staphylococcus aureus by routine microbiological testing. Whereas S intermedius is recognized as flora and pathogen of dogs, it has never been isolated from human infections. We hypothesized that S intermedius may cause human dog bite wound infections and that it has been previously misidentified as S aureus. Fourteen isolates from clinically infected dog bite wounds that were originally identified as S aureus were subjected to further testing; three (22%) were found to be S intermedius. The clinical and microbiological characteristics of these three S intermedius cases are described. All three patients were nonimmunocompromised persons seen within 24 hours for bites on the upper extremity. All patients developed cellulitis within one to three days. All S intermedius isolates were distinguished from S aureus by the lack of acetoin production and by the presence of beta-galactosidase activity. S intermedius was susceptible to a wide range of antibiotics; one isolate was resistant to penicillin. Two patients were treated with penicillin, one with amoxicillin-clavulanate, and all were clinically cured. These are the first three reported human infections involving S intermedius. Further study is necessary to define its clinical importance as a potential human pathogen.

We prospectively studied 26 patients with 27 dog bite wounds who sought medical attention in the emergency department. Two distinct populations were found (Fishers Exact Test, P less than or equal to 0.001): 1) those who presented 8 hr to 12 hr post-injury were concerned about rabies or tetanus or surgical wound repair; and 2) those who presented more than 12 hr post-injury were concerned with infection. There was no difference (P > 0.05) in the bacteriology of the two groups. Of all wounds, 74% had aerobic pathogens isolated, including Streptococcus viridans (12 strains), Staphylococcus aureus (five strains), and Pasteurella multocida (eight strains); 41% had anaerobic pathogens isolated, including Bacteriodes species (five strains) and Fusobacterium species (five strains). Gram stains made from wound swabs were specific but insensitive predictor for bacterial growth. Empiric outpatient therapy with oral penicillin (17 patients) and/or dicloxacillin (four patients) was effective.

Erythromycin has been reported to be active in vitro against most anaerobic bacteria. We found it effective in treating 14 of 17 patients with mild to moderately severe anaerobic or mixed aerobic/anaerobic pleuropulmonary of soft-tissue infections when adjunctive measures (eg, drainage, débridement, and the use of additional antibiotics to treat important aerobic pathogens) were employed. Erythromycin offers a reasonable therapeutic alternative to penicillin in the treatment of a penicillin-allergic patient who has a mild or moderately severe anaerobic or mixed aerobic/anaerobic infection not involving Bacteroides fragilis or fusobacteria.
