Background: The aim of this study was to determine whether surgical treatment is more effective than conservative treatment in terms of functional outcomes in elderly patients with distal radius fractures (DRFs)., Methods: An electronic search of the Medline, Central, Embase, PEDro, Lilacs, CINAHL, SPORTDiscus, and Web of Science databases was performed, from inception until July 2021. The eligibility criteria for selecting studies were randomized clinical trials that compared surgical versus conservative treatment in subjects older than 60 years with DRFs. Two authors independently performed the search, data extraction, and assessed risk of bias (RoB) using the Cochrane RoB tool., Results: Twelve trials met the eligibility criteria, and nine studies were included in the quantitative synthesis. For volar plate versus cast immobilization at 1-year follow-up, the mean difference (MD) for PRWE was -5.36 points (p=0.02), for DASH was -4.03 points (p=0.02), for grip strength was 8.32% (p=0.0004), for wrist flexion was 4.35 degrees (p=0.10), for wrist extension was -1.52 degrees (p=0.008), for pronation was 2.7 degrees (p=0.009), for supination was 4.88 degrees (p=0.002), and for EQ-VAS was 2.73 points (p=0.0007), with differences in favor of volar plate. For K-wire versus cast immobilization at 12 months, there were no statistically significant differences in wrist range of motion (p>0.05)., Conclusions: There was low to high evidence according to GRADE ratings, with a statistically significant difference in functional outcomes in favor of volar plate versus conservative treatment at 1-year follow-up. Various plating systems are available to fix distal radius fractures, each with a specific design. The purpose of this study was to compare radiological outcome and complications of the Variable Angle LCP Plate 2.4-mm (DePuy Synthes) with the VariAx volar locking plate (Stryker). One hundred patients (103 wrists) operated on for a distal radius fracture were retrospectively reviewed with a mean follow-up of 3.5 years. Seventy-three wrists were treated with a DePuy Synthes plate and 30 with a VariAx plate. The overall complication rate was 32%. Nineteen cases underwent revision surgery, 18 had malunion and 3 complex regional pain syndrome. Complicaton rate was 43% with DePuy Synthes plates and 27% with Variax plates, but the difference was not significant.

Purpose: Wrist fractures are common, contribute significantly to morbidity in women with postmenopausal osteoporosis, and occur predominantly at the ultradistal radius, a site rich in trabecular bone. This exploratory analysis of the phase 3 ACTIVE study evaluated effects of abaloparatide versus placebo and teriparatide on forearm bone mineral density (BMD) and risk of wrist fracture., Methods: Forearm BMD was measured by dual energy X-ray absorptiometry in a subset of 982 women from ACTIVE, evenly distributed across the three treatment groups. Wrist fractures were ascertained in the total cohort (N = 2463)., Results: After 18 months, ultradistal radius BMD changes from baseline were 2.25 percentage points greater for abaloparatide compared with placebo (95% confidence interval (CI) 1.38, 3.12, p < 0.001) and 1.54 percentage points greater for abaloparatide compared with teriparatide (95% CI 0.64, 2.45, p < 0.001). At 18 months, 1/3 radius BMD losses (versus baseline) were similar for abaloparatide compared with placebo (-0.42; 95% CI -1.03, 0.20; p = 0.19) but losses with teriparatide exceeded those of placebo (-1.66%; 95% CI -2.27, -1.06; p < 0.001). The decline with abaloparatide was less than that seen with teriparatide (group difference 1.22%; 95% CI 0.57, 1.87; p < 0.001). The radius BMD findings, at both ultradistal and 1/3 sites, are consistent with the numerically lower incidence of wrist fractures observed in women treated with abaloparatide compared with teriparatide (HR = 0.43; 95% CI 0.18, 1.03; p = 0.052) and placebo (HR = 0.49, 95% CI 0.20, 1.19, p = 0.11)., Conclusions: Compared with teriparatide, abaloparatide increased BMD at the ultradistal radius (primarily trabecular bone) and decreased BMD to a lesser extent at the 1/3 radius (primarily cortical bone), likely contributing to the numerically lower wrist fracture incidence observed with abaloparatide.

Distal radial fractures account for up to 18% of all fractures in patients over 65 years of age, and osteoporosis is a predominant factor in these fractures. Background: Fractures of the radial shaft with disruption of the distal radial ulnar joint (DRUJ) or Galeazzi fractures are treated with reduction of the radius followed by stability assessment of the DRUJ. In rare instances, the reduction of the DRUJ is blocked by interposed structures requiring open reduction of this joint. The purpose of this study is to review all cases of irreducible Galeazzi fracture-dislocations reported in the literature to offer guidelines in the diagnosis and management of this rare injury., Methods: A search of the MEDLINE database, OVID database, and PubMed database was employed using the terms "Galeazzi" and "fracture." Of the 124 articles the search produced, a total of 12 articles and 17 cases of irreducible Galeazzi fracture-dislocations were found., Results: The age range was 16 to 64 years (mean = 25 years). A high-energy mechanism of injury was the root cause in all cases. More than half of the irreducible DRUJ dislocations were not identified intraoperatively. In a dorsally dislocated DRUJ, a block to reduction in most cases (92.3%) was secondary to entrapment of one or more extensor tendons including the extensor carpi ulnaris, extensor digiti minimi, and extensor digitorum communis, with the remaining cases blocked by fracture fragments. Irreducible volar dislocations due to entrapment of the ulnar head occurred in 17.6% of cases with no tendon entrapment noted., Conclusions: In the presence of a Galeazzi fracture, a reduced/stable DRUJ needs to be critically assessed as more than half of irreducible DRUJs in a Galeazzi fracture-dislocation were missed either pre- or intraoperatively.

There is not enough evidence in the literature to support the use of any classification system for distal radius fractures (DRF) in adults. However, there is a need for identification of more homogeneous groups of patients with DRF so that the extent of preoperative workup and sophistication of treatment would best match the needs of the patient. The authors propose an innovative method to analyse and stratify acute DRF in adults. A one-page chart includes criteria related to the patient (P), the energy of the accident (A) and the pathology of the fracture (F). Analysis of the pathology includes not only the distal radius itself but also the associated ulnar and carpal lesions. Radiological suspicion of associated carpal of distal radio-ulnar joint ligamentous injuries is included in the analysis. The preliminary results of the use of this chart in 1610 consecutive adult patients (16-102 years) with unilateral acute DRF are presented. A total of six homogeneous groups of patients are described, and the relevance of this classification regarding the therapeutic options is discussed.

PURPOSE OF THE STUDY The purpose of our retrospective study is to evaluate 5-year functional and radiological outcomes in patients following corrective osteotomy of the distal radius and ulnar osteotomy for malposition after a distal radius fracture, to identify differences in the outcomes of corrective osteotomies depending on the type of the original fracture according to the AO classification, the grade of arthritis of radiocarpal (RC) joint, surgical approach and the way of stabilisation of the osteotomy. MATERIAL AND METHODS The followed-up group of 22 patients (8 men and 14 women) underwent osteotomy for malposition of distal radius in the period 2007-2011. The age of patients in the followed-up group ranged from 21 to 72 years, with the mean age of 51 years at the time of surgery. The indications for corrective osteotomy due to distal radius deformity were the following: functional limitation, pain and radiological parameters confirming the deformity. The opening wedge osteotomy of distal radius performed through volar or dorsal approach, isolated shortening osteotomy of ulna and a combination of both the methods were used. The observations included: the original type of fracture according to AO/OTA classification, functional outcomes - Disabilities of the Arm Shoulder and Hand (DASH) score, Mayo Wrist Score (MWS), range of motion, grade of wrist arthritis and specific complications. The follow-up period was 5 years. RESULTS Average results were obtained in the group of patients before/after the osteotomy: DASH score- 35/14, MWS- 54/77, flexion- 44°/64°, dorsiflexion- 48°/61°, supination- 75°/79°, pronation- 72°/83°, ulnar duction- 20°/23°, radial duction- 9°/16°, grip strength in percentage- 59%/83%. After osteotomy, a statistically significant increase was observed in flexion, dorsiflexion, supination, pronation, radial duction, ulnar duction, grip strength in percentage, MWS, while in the DASH score a statistically significant decrease was reported. Based on the statistical evaluation of differences in functional outcomes after osteotomy, in patients with type A and C original distal radius fractures no difference in range of motion parameters was found after osteotomy. As to the mean values, the flexion and dorsiflexion range of motion was greater by 10° in patients after extra-articular osteotomy for malposition following the type A original fracture as compared to the type C fracture. By measuring the Joint Space High (JSH) ratio, no statistically significant changes were found regarding the progression of arthrosis of the radiolunate and radioscaphoid part of the RC joint as against the arthrosis in patients up to 5 years after corrective osteotomy of the distal radius. In patients with distal radius malposition and RC joint grade 1-2 arthritis according to the Knirk and Jupiter classification, better functional outcomes were achieved than in the limited and total wrist arthrodesis. In our patients, at 5 years after osteotomy no worsening was observed of the existing wrist arthritis and no arthritis was newly diagnosed. Specific complications were found in 4 cases (18.2 %). In 2 patients after radial osteotomy from dorsal approach (extensor tendon irritation, rupture of the long extensor tendon of the thumb), removal of osteosynthesis material was necessary in both the patients. In one patient after the isolated ulnar shortening osteotomy, an intraoperative fissure of distal ulnar fragment was detected, which healed without any further complications. In one patient an iatrogenic fracture of anterior superior iliac spine was observed after harvesting the corticospongious graft from the ala of the ilium. DISCUSSION Corrective osteotomy is a well-established method for treating distal radius deformities following a fracture. Even at present, there are various opinions regarding the indications, contraindications, timing of the surgery, osteotomy technique and the need to use a bone graft. Limited or total wrist arthrodeses in the area of wrist arthrosis and deformities bring good results with respect to the pain relief, but a limited range of motion occurs mainly in younger patients. Bearing this in mind, in grade 1-2 wrist arthritis in patients with distal radius deformities, a better functional outcome can be achieved by osteotomy. By using dorsal or volar approach, comparable outcomes can be obtained, but with the dorsal approach there is higher frequency of complications and the need to remove the osteosynthesis material. CONCLUSIONS Corrective osteotomies resulted in an improved functional outcome in the treatment of deformities after a distal radius fracture. Apart from deformity correction, the treatment has been affected also by the RC joint arthritis. The study confirmed neither statistically significant differences in the osteotomy outcomes in patients with the original type A or type C distal radius fracture according to the AO classification, nor the statistical significance of the RC joint arthritis. Our results have proven better functional outcomes of deformity correction achieved by osteotomy in case of grade 1 and 2 arthritis than by the limited wrist arthrodesis. In patients showing arthritic changes, no progression was reported within five years after the osteotomy. Key words: distal radius fracture, distal radius deformity, osteotomy, functional results.

Orthopedic trauma surgeons often encounter Smith's fracture in patients who report that they have fallen on the palms of their hands. The aim of this study was to clarify the pathogenesis of Smith's fracture in basic clinical aspects. First, a survey was conducted for investigating the mechanism of injury and arm position at the time of injury among patients with Smith's fractures who consulted at our outpatient clinic. Second, we created three-dimensional finite element models (FEMs) to predict the influence of arm position on the type of injury resulting from a fall. These predictions were then used in ten freshly frozen cadavers to provide experimental proof of Smith's fractures resulting from the impact on the palmar side. Twenty-six patients (5 males and 21 females) with Smith's fractures were enrolled in this study. Injury resulting from a fall on the palm of the hand, the dorsum, or ulno-dorsum of the hand, and fisting handle was observed in 16 cases (61%), 3 cases (12%), and 1 case (4%), respectively. Six patients were uncertain of their arm position at the time of injury. FEM analysis showed that Smith's fractures occurred when the angle between the long axis of the forearm and the ground was 30°-45° in the sagittal plane and 75°-90° in the coronal plane. Smith's fractures occurred in 7 of 10 wrists in the experimental study, whereas no Colles' fractures were observed. Recently, many studies have emphasized the importance of the comprehension of detailed functional anatomy of the distal forearm and wrist joint, and their biomechanics. A significant contribution which yields good functional outcomes of surgical treatment was the development of the locking plate technology; this technology has facilitated the improvement of the surgical technique for the fixation of fractures. This article reviews the locking mechanism and design of the fixation screws and plate, and the details of the surgical technique including the double-tired subchondral support procedure as it is applied to common fractures. Arthroscopic-assisted surgical procedures can be used to reduce the intra-articular fracture fragments after realignment of the distal radius with the locking plate. This technique is also useful at the time of fixation to assess soft tissue injury. The combination of arthroscopic-assisted reduction and locking plate fixation is now indicated for AO type C2 and C3 intra-articular comminuted fractures., (Copyright © 2016 The Japanese Orthopaedic Association. Background: The wrist/forearm complex is one of the most commonly fractured body regions, yet the impact tolerance of the wrist is poorly understood. This study sought to quantify the injury tolerance of the adult male forearm-wrist complex under loading simulating axial impact to an outstretched hand., Methods: Fifteen isolated cadaveric forearm/wrist specimens were tested. Loading was applied via an instrumented drop tower device designed to impact the palmar surface of the hand with the wrist extended to approximately 90 degrees. Impact severity was modulated by adjusting the boundary condition of the elbow. Elbow reaction force and deformation of the specimen (deflection of the palmar surface of the hand toward the elbow) were measured. Bone-implanted strain gauges were used to detect the time of fracture. Injury risk functions were developed using parametric survival analysis with a cumulative Weibull distribution., Results: Of 14 specimens, 10 exhibited a fracture to the wrist or forearm after test (one specimen was excluded from the analysis). Injury severities varied from nondisplaced fractures of the radius to severely displaced fractures and/or fracture-dislocations of the carpal bones. Of the potential predictors studied, the specimen deflection expressed as a percentage of the initial specimen length produced the injury risk model of best fit (50% risk of fracture at 1.69% deflection; 95% confidence interval, 1.38-2.07% deflection). The value of the elbow reaction force corresponding to a 50% risk of injury was 4.34 kN (3.80-4.97 kN)., Conclusion: These results provide information for the prediction of wrist and forearm injury in biomechanical models simulating impacts in the field and provide tolerance information for the development of injury mitigation countermeasures.

Objective: Distal radius fractures are among the most common fractures encountered in clinical practice. Although multiple epidemiological studies have been done in Western populations, there is a lack of data within Asia. Our study aims to fill this knowledge gap., Methods: This study included all patients aged 16 years and above with distal radius fractures seen between November 2008 and May 2009 by the Department of Hand and Reconstructive Microsurgery at the Singapore National University Hospital. A retrospective analysis was done. Information captured included clinical data, hand dominance, AO classification of fractures, mechanism of injury, associated injuries as well as treatment modalities., Results: Of the 431 fractures, 238 were males, 181 were females. These fractures occur at all ages, peaking at the 50 to 60 years age group. The peak incidence of distal radius fractures in females occurred in the perimenopausal age group, whereas the incidence for males peaked between age of 30 to 50. By AO classification, 53% were type A; 13% were type B and 32% were type C. A fall on the outstretched hand from a standing position is the most common mechanism of injury. As the intensity of the injury mechanism increased, the percentage of type C fractures increased correspondingly. Older patients were more likely to be treated conservatively compared to younger patients. AO severity classification correlated well with the clinician's decision to surgically fix the fracture. No correlation between hand dominancy and fracture site found was found., Conclusion: We compared our data with that of previous similar studies, and found similarities as well as differences in results. Unlabelled: The purpose of this study was to assess changes in epidemiology and reasons for wintertime excess of distal radius fractures in Oulu, Finland. Our results showed that age-specific incidence of distal radius fractures in elderly women in Finland has increased compared with a previous study. Slippery pavement surfaces assessed by a new meteorological model partly explained wintertime excess of fractures, but factors beyond weather are contributing to seasonality., Introduction: In this report, we describe the epidemiology and seasonal variation of distal radius fractures in Oulu, Finland, with a focus on the effect of weather and slippery pavement conditions., Methods: Records of patients aged ≥16 years living in Oulu with a distal radius fracture during the year 2008 were reviewed. Demographic data and details of the injury were assessed from medical records, and fractures were classified according to AO classification. Population data for Oulu during the year 2008 were used to calculate crude incidence as well as sex- and age-specific incidence rates. The number of wintertime fractures was compared with those related to pavement surface slipperiness using a unique weather and pavement condition model of the Finnish Meteorological Institute., Results: The crude incidence was 258/100,000 person-years. Sex- and age-specific incidence rates rose to 1,107/100,000 person-years for females and 466/100,000 person-years for males aged ≥80 years. Poisson regression analysis showed that the number of fractures was 2.5 (95% confidence interval (CI), 1.6 to 4.0; P < 0.001) times greater on slippery winter days compared with non-winter days whereas on normal winter days fractures were 1.4 (95% CI, 1.1 to 1.9; P = 0.01) times greater. Both low- and high-energy injuries resulted in similar fracture patterns by AO classification., Conclusions: Our results suggest that the epidemiology of distal radius fractures in elderly women in Finland has changed compared with a previous study. Weather analysis showed that the slipperiness of the pavement could partly explain the wintertime excess of distal radius fractures.

Background: Distal radial fractures occur earlier in life than hip and spinal fractures and may be the first sign of osteoporosis. The aims of this case-control study were to compare the prevalence of osteopenia and osteoporosis between female and male patients with low-energy distal radial fractures and matched controls and to investigate whether observed differences in bone mineral density between patients and controls could be explained by potential confounders., Methods: Six hundred and sixty-four female and eighty-five male patients who sustained a distal radial fracture, and 554 female and fifty-four male controls, were included in the study. All distal radial fractures were radiographically confirmed. Bone mineral density was assessed with use of dual x-ray absorptiometry at the femoral neck, total hip (femoral neck, trochanter, and intertrochanteric area), and lumbar spine (L2-L4). A self-administered questionnaire provided information on health and lifestyle factors., Results: The prevalence of osteoporosis was 34% in female patients and 10% in female controls. The corresponding values were 17% in male patients and 13% in male controls. In the age group of fifty to fifty-nine years, 18% of female patients and 5% of female controls had osteoporosis. In the age group of sixty to sixty-nine years, the corresponding values were 25% and 7%, respectively. In adjusted conditional logistic regression analyses, osteopenia and osteoporosis were significantly associated with distal radial fractures in women. Osteoporosis was significantly associated with distal radial fractures in men., Conclusions: The prevalence of osteoporosis in patients with distal radial fractures is high compared with that in control subjects, and osteoporosis is a risk factor for distal radial fractures in both women and men. Thus, patients of both sexes with an age of fifty years or older who have a distal radial fracture should be evaluated with bone densitometry for the possible treatment of osteoporosis.

Previous studies show that identification and treatment of osteoporosis in patients with minimal trauma fractures treated as outpatients are poor. Our aim was to test two interventions designed to increase rates of identification and treatment. This prospective, action research study, using explicit medical record review and scripted telephone interview, was conducted at emergency departments (ED) of three hospitals from April 2007 to February 2008. Participants were patients aged over 50 years who were treated as outpatients with a minimal trauma wrist fracture. Data collected included demographic and fracture details, bone density testing and osteoporosis-related medication change. There were two interventions staff education in ED and fracture clinic and information provided to patients by telephone by a research nurse. These interventions were applied to all patients sequentially. The outcome measure of interest was the proportion of patients who underwent bone density testing (DEXA scans) in the follow-up period, analysed by intervention (clinic or phone). One hundred and seventeen patients were studied. Eighty-six per cent were female; median age 64 years. Ten per cent (12/117) of the ED/clinic intervention group had undergone testing at follow up. At follow up after the telephone intervention 55% (65/117) had undergone testing (P < 0.001, chi(2)). Patients undergoing testing were significantly more likely to have an osteoporosis-related medication change (relative risk 6.8, 95% CI 2.8-17.9). A brief telephone intervention and provision of information pack significantly improved testing rates for osteoporosis after minimal trauma wrist fracture. An ED/clinic-based intervention resulted in low rates of testing. Treatment of clinical osteoporosis remains suboptimal.

Introduction: Osteoporotic fractures involving the hip and wrist are common in the elderly. The incidence of coincident hip and wrist fractures in the same patient is small but may represent a unique subpopulation of elderly with osteoporotic fragility fractures and little information has been published about these patients., Materials and Methods: We performed a retrospective review studying a series of 33 elderly patients who were admitted with concurrent hip and wrist fractures and matched them with patients of similar age, race, gender and co-morbidities with isolated hip fractures analysing their pre-morbid functional status, degree of osteoporosis by the Singh's index, length of stay and re-admission rate., Results: In our cohort of 33 matched pairs, location of fall in the study group consisted of 21 at home and 26 in the control group. The median duration of stay in hospital was 23 days versus 18 days in the control group. Bone density assessment of our study group revealed 18 patients with severe osteoporosis with Singh's index and 21 severe osteoporosis in the control group. The 1-year re-admission rate for the patients in the study group was 7 and 12 in the control group. Ambulatory status at discharge in the study group was 12 and in the control group 21., Discussion and Conclusion: Our study population demonstrated a higher proportion of community ambulators with fewer 1-year re-admission rates and a high cervical:trochanteric ratio of 2.7, all of which signify that they belong to a physiologically younger and more active cohort than our control group of isolated hip fractures. Patients with concurrent hip and wrist fractures were not significantly more osteoporotic, and showed a trend toward longer stay in hospital and incurred a higher cost of stay. The confluent upper limb injury was the likely reason for their post-discharge impaired ambulatory status. The most common location of injury in both groups of patients is at home.

Introduction: Fractures of the distal radius are the most frequent fractures of longbones during childhood and adolescence. A rare mechanism of injury is the attempt of a soccer goalkeeper to save the ball, shot with high velocity., Material and Methods: Reports of all children and adolescents up to the age of 14, suffering from a distal radius fracture from a three year period were evaluated retrospectively and this particular mechanism of injury was identified. All patients were reinvestigated after at least 10 months clinically., Results: Amongst 516 distal radius- and distal forearm fractures, there were 213 (41.3 %) sports injuries. 61(28.6 %) of them resulted from playing soccer, with 48 (78.7 %) after a fall. 13 distal radius fractures were identified as a goalkeepers injury by a sharply shot ball. We found 6 buckle fractures, 4 complete fractures, 2 physeal separations Salter II and 1 metaphyseal greenstick fracture. At the time of the last follow up investigation all injured had regained their level of sports., Discussion: Injuries to the lower extremities are most frequent in soccer. The ball itself may lead to fractures and dislocations of the hand and finger skeleton, even not unfrequently to distal radius fractures. Wrist protectors and an adequate behaviour in game and training may contribute to their avoidance.

Distal radius (Colles') fractures are a common fall-related injury in older adults and frequently result in long-term pain and reduced ability to perform activities of daily living. Because the occurrence of a fracture during a fall depends on both the strength of the bone and upon the kinematics and kinetics of the impact itself, we sought to understand how changes in bone mineral density (BMD) and loading direction affect the fracture strength and fracture initiation location in the distal radius. A three-dimensional finite element model of the radius, scaphoid, and lunate was used to examine changes of +/-2% and +/-4% BMD, and both axial and physiologically relevant off-axis loads on the radius. Changes in BMD resulted in similar percent changes in fracture strength. However, modifying the applied load to include dorsal and lateral components (assuming a dorsal view of the wrist, rather than an anatomic view) resulted in a 47% decrease in fracture strength (axial failure load: 2752N, off-axis: 1448N). Loading direction also influenced the fracture initiation site. Axially loaded radii failed on the medial surface immediately proximal to the styloid process. In contrast, off-axis loads, containing dorsal and lateral components, caused failure on the dorsal-lateral surface. Because the radius appears to be very sensitive to loading direction, the results suggest that much of the variability in fracture strength seen in cadaver studies may be attributed to varying boundary conditions. The results further suggest that interventions focused on reducing the incidence of Colles' fractures when falls onto the upper extremities are unavoidable may benefit from increasing the extent to which the radius is loaded along its axis.

The most common cause of an arthritically damaged distal radioulnar joint is a malunion of a distal radius fracture. Therapeutically, ulnar head resection, hemiresection-interposition-technique, Kapandji-Sauvé procedure and implantation of an ulnar head prosthesis have been described. None of these procedures is able to restore the complete function of the joint. Therefore, anatomical reconstruction of the joint in acute or secondary correction osteotomy for malunited fractures of the distal radius should be performed to avoid the development of the arthrosis. Numerous clinical studies have demonstrated a similar reduction of the clinical symptoms for all procedures. Therefore, classification of the different procedures has to consider the number of complications. Biomechanically, partial resection of the distal ulna will destabilize the distal radioulnar context and clinically may lead to painful radioulnar and/or dorsopalmar instability of the distal ulnar stump. Biomechanically and clinically, this complication, next to secondary extensor tendon ruptures, has to be expected far more often following complete resection of the ulnar head than in the alternative procedures. We do not see any remaining indication for complete resection of the ulnar head. Clinical results and the occurrence of painful instability of the distal ulnar stump have been reported almost identically for the hemiresection-interposition technique and the Kapandji Sauvé procedure. Therefore, both procedures appear to be equally suitable for the treatment of painful arthrosis of the distal radioulnar joint. In patients with a preexisting instability of the distal radioulnar joint, or a major deformity of the radius or the ulna, we prefer to perform the hemiresection-interposition-technique. In these conditions we consider the remaining contact of the triangular fibrocartilage complex with the distal end of the ulna a biomechanical advantage to reduce the risk of secondary instability. Biomechanically as well as clinically, replacement of the ulnar head using a prosthesis has been shown to either avoid or solve the problem of instability. We therefore consider ulnar head replacement the treatment of choice in secondary painful instability following resection procedures at the distal end of the ulna. Primary ulnar head replacement should be considered in special indications until long-term follow-up results are available.

Two cases of carpal scaphoid fractures associated with distal radius epiphysiolysis are presented. Only three cases of this unlikely combination were found in the English literature. One of the scaphoid fractures had to be confirmed by bone scan. Conservative treatment after gentle reduction of the radius fracture gave excellent results.

Non-union following distal radial fracture is extremely rare. Therefore, the patterns are not completely understood. Recently, it was suggested that an associated distal ulna shaft fracture increases the risk to develop a non-union for comminuted fractures of the distal radius. The purpose of this study was to review our 15 cases with this condition to investigate the role of an associated distal ulna shaft fracture and/or an associated lesion of the distal radioulnar joint. The second goal was to evaluate the success of the surgical treatment in relation to the extent of the metaphyseal subchondral bone supporting the articular surface distal to the site of the non-union. We reviewed our 15 cases which were operated on for non-union of the distal radius since 1992. In all cases, the radiographs could be reviewed. Twelve patients could actually be re-examined. The investigated criteria were: initial, pre- and postoperative X-ray findings, other medical conditions, range of motion, grip strength, and pain relief. Clinical outcome was evaluated using the DASH-questionnaire. According to the AO classification system, there were two type A3 and 13 type C3 fractures. There were seven associated distal ulna fractures. The distal radioulnar joint was involved in twelve cases. The other medical conditions in the patients with non-union after distal radial fracture included allergy, hypothyroidism, diabetes mellitus, peripheral neuropathy, and bronchial asthma. Four patients were tobacco smokers. Only one patient was initially treated non-operatively. In all cases, the distal radius was restored preserving wrist motion. Bony union was achieved in 14 cases. The range of motion has improved in all planes. Grip strength increased but still remained over 50% below the level of the opposite side. Postoperatively, the majority of patients was pain free. In four cases, complications were seen postoperatively. Three of these patients had less than 6 mm of metaphyseal subchondral bone beneath the articular surface. Three of the four patients were tobacco smokers. Inadequate treatment of a comminuted distal radial fracture associated with a special local situation--such as an additional distal ulna shaft fracture and/or an associated lesion of the distal radioulnar joint--combined with a general medical condition, adversely affects fracture healing, increasing the risk for non-union. From the review of the records of these 15 cases, we recommend that non-union with more than 5 mm of subchondral bone supporting the articular surface distal to the non-union site undergo reconstruction of the radius. Non-union with less than 5 mm subchondral bone supporting the articular surface requires an individual decision. A good bone stock and stable fixation allows for reconstruction of the radius, otherwise wrist fusion is a useful salvage procedure.

Distal radius impaction fractures are not rare. They present a dual problem for the surgeon: the difficulty of establishing the exact pathological anatomy of the fracture, and of obtaining stable anatomic reduction. The authors report their experience of 18 distal radius impaction fractures in young patients after violent trauma (motor vehicle accident). We subdivided these fractures using Kapandji and Müller's classifications: 12 type 9 or C3, 4 type 5 or B1 and 2 type 4 or C1. The mean depth of impaction of the scaphoid fossa or lunate fossa ("die punch fracture"), or centrally was measured at 5 mm on preop X-rays or on CT scan. After radiographs of the wrist in traction under anaesthesia, open reduction was performed in 14 cases (78%). A volar approach with plate fixation was performed in 3 cases. A dorsal approach with internal fixation with k-wires and an external fixator was performed in the other 11 cases. A bone graft was necessary in 10 cases when the bone defect was significant. The four remaining patients were treated with percutaneous k-wires and external fixation. We reviewed these 18 patients after a mean follow-up of 27 months. The mean age at the time of the accident was 37 years. The results were graded on the Green and O'Brien scale. Results were excellent in 1 case, good in 11 cases, fair in 5 cases, and poor in 1 case. We believe that impaction fractures always require open reduction with or without the addition of bone graft depending on the degree of the impaction. This is because of the risk of arthritic degeneration with this kind of fracture, with articular incongruence (more than 2 mm), and with chondral injuries on the radial or the carpal aspect of the wrist joint. Also, when there is a distal radio-ulnar joint injury (D.R.U.J. dislocation or distal head ulna fracture), the joint must always be stabilized to avoid secondary displacement.

We sought to quantify agreement by different assessors of the AO classification for distal fractures of the radius. Thirty radiographs of acute distal radial fractures were evaluated by 36 assessors of varying clinical experience. Our findings suggest that AO 'type' and the presence or absence of articular displacement are measured with high consistency when classification of distal radial fractures is undertaken by experienced observers. Assessors at all experience levels had difficulty agreeing on AO 'group' and especially AO 'subgroup'. To categorize distal radial fractures according to joint displacement and AO type is simple and reproducible. Our study examined only whether distal radial fractures could be consistently classified according to the AO system. Validation of the classification as a predictor of outcome will require a prospective clinical study.

Distal radius fractures are one of the most common types of injuries treated by an orthopedic surgeon. The overall results may not be as favorable as once thought. As a distinct subgroup, the young adult deserves special consideration. Management of distal radius fractures in these patients is difficult due to the higher energy involved, amount of comminution, and associated soft tissue damage. Functional outcome has been best when an anatomical reduction has been achieved. Many surgical options are available: external fixation, percutaneous pin fixation and open reduction are the mainstays. The complications may be minimized with careful attention to and knowledge of the techniques used and early intervention when closed treatment fails.

Aims: To identify the different types of fractures of the forearm caused by mountain bike accidents, and to assess the physical and social consequences of these injuries., Methods: All the forearm fractures recorded in the Wellington Hospital fracture logs between July 1992 and July 1994 were reviewed in December 1994. Mountain bike accidents were identified in 37 patients. 11 patients could not be contacted and one patient declined participation, leaving 25 patients who participated in the study. Those who took part each completed a questionnaire and were examined. The radiographs of each patient were also reviewed., Results: Nine of the fractures occurred in the proximal third of the forearm and 16 occurred in the distal third. The most common fracture was that of the radial head (9). When functionally assessed 15 patients were graded excellent, 5 satisfactory, 4 unsatisfactory, and 1 patient was graded as poor. The average time off work was 28 days, with no change of occupation being necessitated in any of the patients. Eight patients had to make changes to their recreational activities., Conclusions: The results of the study suggest that mountain bike accidents can result in significant injury. With adequate medical attention it appears that the majority of cases recover well, and experience only minimal discomfort without significant long term physical and social consequences.

In this paper we describe a case of epiphysiolysis of the distal radius epiphysis by a weight lifter and we point out also the importance of x-rays by young athletes.

Winter sport is of great importance in the Alpine countries. In 1980 3600 persons (10.24%) of all new admittances hospitalized in our hospital were injured when practicing winter sports. In 1983 they lessened to 2597 (7.73%) and in 1985 2999 (5.74%). Analysis of all injuries caused by winter sport activities shows that the number of injured persons is decreasing, severity of the injuries however, increasing. A comparison of the distal radius fractures in 1980, 1983 and 1985 indicates that extraarticular metaphyseal fracture types have decreased, intraarticular fracture with multiple fragments however, have increased. Mainly younger patient suffer from severe fractures. We regard the high pace-risk of the age group as the main course of this fact.
