Training and support will be vital in developing and maintaining a skilled disability workforce in the roll-out of individualised disability funding across Australia. Technology has the potential to deliver training and support that is more accessible and cost-efficient in geographically isolated regions. The current qualitative study explored participants' experience of an autism-specific webinar and individual online support programme delivered to a range of early childhood staff in regional and remote areas. Researchers used purposive sampling to select and interview 11 participants enrolled in the training and support programme. Rigour and trustworthiness of the data were enhanced using member checking and a data analysis check on 20% of the transcripts. Several categories of responses were identified under three broad themes; impact of the training programme, interaction with technology, and barriers to access. Findings indicated that the programme had a positive impact on access to autism-specific learning and support. The interactive nature of webinars and individual online consultations (synchronous learning) was highly valued; however, barriers including extensive travel and busy caseloads resulted in participants relying on recorded webinars (asynchronous learning). Findings from the current study will inform the development of future training programme at an individual, organisational and policy level.

Purpose: The rollout of individualised disability funding in Australia resulted in an increased demand for services for people with a disability, particularly support for the high percentage of Australians with autism spectrum disorder. Continuing professional development is one way to grow and maintain a skilled workforce; however, face-to-face opportunities can be limited in remote areas of Australia. Technology may provide a low-cost and widely accessible platform for providing education and support for staff in these areas. Design/methodology/approach: The current study evaluated a novel webinar training and individual online support program for 36 allied health, education and community support staff. Data were collected via a survey on changes in perceived "knowledge and skills" and "confidence" in supporting children with autism, as well as mode of participation, and accessibility of the program. Findings: Participants reported a significant increase in their "knowledge and skills" and "confidence", in working with children with autism. This increase was positively related to the number of webinars the participants accessed. The mode of webinar access was predominantly via watching recordings of webinars (asynchronous learning). Synchronous learning via individual online sessions was accessed by a small number of participants, mostly allied health professionals. Workload and scheduling were identified as barriers to engaging in the program. The technology platform was found to be accessible and acceptable. Originality/value: A predominantly asynchronous mode of delivery was successful, thereby increasing program access and flexibility for remote staff. Technology was not a barrier to accessing the program regardless of remoteness or job role.

The Access Consortium New Active Substance Work-Sharing Initiative, or "Access" for simplicity, allows regulatory authorities (RAs) of the Access Consortium countries to jointly review applications for the registration of new active substances or for new indications. Using a survey developed by the pharmaceutical industry trade associations of the five Access Consortium countries—Australia, Canada, Singapore, Switzerland, and the United Kingdom (UK)—this study gathered insights into the perceptions and experiences of the Access pathway held by affiliates of pharmaceutical companies. Understanding industry perceptions of Access is important for the success of the initiative, as participation is voluntary. Findings indicate that affiliates who participated in Access had mostly positive experiences with this pathway; most affiliates were satisfied with their interactions with the Access RAs and appeared willing to continue to participate in the initiative. Affiliates' reasons for not having yet participated in Access included a lack of opportunity to do so and perceived barriers, such as the Access pathway being too complicated to manage. Recommendations to improve Access cover six key areas: ensure predictability, increase guidance and transparency, streamline processes, maintain flexibility, increase harmonization, and advance RA-industry cooperation. This study should facilitate informed discussions among relevant stakeholders on how to improve Access to maximize efficiencies, accelerate approvals, and improve patient access to innovative medicines. Previous research has emphasized the importance of active citizenship in the early years for the development of a tolerant and cohesive Australian society. This paper presents findings related to young children's beliefs about exclusion based on gender and race. The findings draw from a larger study exploring the development of children's moral and social values and teachers' beliefs and practices related to teaching for moral development in the early years of school in Australia. This current study examined reasoning about exclusion in early childhood with children aged five-eight years. One hundred children from seven schools (Preparatory to Year 3) answered questions relating to two scenarios where the children had to make a decision about whether to include others of different gender or race in their play. The majority of children believed that others should be included in their play, regardless of their gender or race. When asked to explain, the children primarily gave reasons related to moral concern and fairness. They were then asked whether they would continue to include or exclude if their friends (social consensus) or teachers (authority) suggested otherwise. Most maintained their beliefs when beliefs to the contrary were voiced by their peers and teachers. The implications of these responses are discussed. (Contains 2 tables and 1 footnote.)

Current theories of learning hold dominant assumptions about the type and scope of knowledge and skills taught in formal courses that prepare novices for professional practice at work. In performing arts educational contexts, a common emphasis continues to hone individual performance skills in order to gain technical mastery and to differentiate competitively against others. This paper analyses a case study of an alternative educational programme developed by a major orchestra, which serves to induct young players into becoming professional orchestral musicians. This analysis reveals the multiple kinds of learning that it is claimed can only be gained from context-sensitive participation in orchestral practice at work. Six distinctive features of this practice-based learning are discussed and implications drawn for how well the various main theories of workplace learning account for the types of learning identified in the case study. This paper concludes with some observations on what these findings suggest for Vocational Education and Training (VET) in general. (Contains 2 tables.)

Purpose: This paper aims to examine the nature of learning discovered by recent graduates participating in a symphony orchestra-initiated development program that is designed to nurture them through the transition to becoming professional orchestral musicians. Design/methodology/approach: Using a qualitative case study approach, the authors interviewed developing musicians and professional musician mentors individually and in small groups using a semi-structured protocol. Interviews were audio-taped and transcribed. The authors also observed musicians working together in rehearsal and in concert performance and reviewed documents on the development program, the organisation and the Australian performing arts industry. Findings: The findings suggest that learning is better conceptualised as an embodied constructed experience with others in context. What the authors call "guided contextualising" differs from conventional discussions of skill-based novice learning and mentorship. For musicians, the competency that is being developed is one of learning how to become, forming a sense of identity as broader musical citizens as well as becoming members of more instrumental communities. Practical implications: The design and structure of the program (and alternatives emerging overseas) suggest possibilities for new collaborations towards "a living curriculum" between higher education and industry. Originality/value: Rather than "employability", the concept of "graduateness" for young adults is formative and transformative, a process that involves the seeking of various forms of identity and contextualised learning that transcends self. "Becoming" practitioners together in generative ways enhances fitness for professional practice and develops a commitment to lifelong learning.

Access to timely behaviour support services for children and adolescents with a disability in rural and remote regions of Australia is problematic due to the inadequate supply of specialised staff providing complex behaviour support in their local area. Technology has the potential to provide a timely, quality, low-cost option that extends access to behaviour support services for these children and their support teams in rural and remote areas. The purpose of this narrative review was to explore policy and practice guidelines on the delivery of behaviour support via telepractice for children and adolescents with a disability in Australia, and more specifically for those on the autism spectrum. Secondly, the review aimed to establish the evidence base of this model of service for children and adolescents with autism, in order to reflect on how it aligns with current Australian policy and practice. Practice and policy documents were drawn from selected websites relevant to the delivery of behaviour support via telepractice in Australia. Peer-reviewed literature (2004–2019) on the delivery of behaviour support via telepractice was sourced via four databases. While telepractice appears to have promising utility for the provision of support for children and adolescents with autism who present with challenging behaviour, there was very little policy documentation or guidelines found that related specifically to the delivery of this support. There is a need for further rigorous research to inform policy, establish efficacy, and develop practice guidelines that ensure delivery of high quality telepractice behaviour supports under the National Disability Insurance Scheme. Simple Summary: Squamous-cell carcinoma of the anus, being a rare cancer, requires national and international collaborations, networking, organizational proficiency and leadership to overcome barriers towards the implementation of clinical trials to establish improved standards of care treatment strategies and the conduction of translational research projects to shed light into its biology and molecular characterization. The purpose of the present study is to obtain a global frame of the scientific literature related to anal cancer, through a bibliometric analysis of the published articles during the last 20 years (2000–2020), exploring trends and common patterns in research, tracking collaboration and networks to foresee future directions in basic and clinical research. Squamous-cell carcinoma of the anus (ASCC) is a rare disease. Barriers have been encountered to conduct clinical and translational research in this setting. Despite this, ASCC has been a prime example of collaboration amongst researchers. We performed a bibliometric analysis of ASCC-related literature of the last 20 years, exploring common patterns in research, tracking collaboration and identifying gaps. The electronic Scopus database was searched using the keywords "anal cancer", to include manuscripts published in English, between 2000 and 2020. Data analysis was performed using R-Studio 0.98.1091 software. A machine-learning bibliometric method was applied. The bibliometrix R package was used. A total of 2322 scientific documents was found. The average annual growth rate in publication was around 40% during 2000–2020. The five most productive countries were United States of America (USA), United Kingdom (UK), France, Italy and Australia. The USA and UK had the greatest link strength of international collaboration (22.6% and 19.0%). Two main clusters of keywords for published research were identified: (a) prevention and screening and (b) overall management. Emerging topics included imaging, biomarkers and patient-reported outcomes. Further efforts are required to increase collaboration and funding to sustain future research in the setting of ASCC. Purpose: Measurement practices have long been considered vital for informing the management of performance in organisations. Their application to local governments is a more recent, yet multi-decade phenomenon facilitated by New Public Management trends. This paper aims to review the landscape of publications that discuss performance measurement (PM) practices in Australian and New Zealand local government contexts and identify implications for future research. Design/methodology/approach: A systematic review methodology was used to identify a shortlist of publications. Next, a rating-based researcher appraisal process was applied. Multiple iterations of search and appraisal were conducted to form the basis for inductive thematic analysis and synthesis. Findings: Analysing 65 PM publications, two interrelated themes, namely, discourses of performance as efficiency, accountability or strategic growth and change were identified, which influence the adoption of local PM tools and frameworks. As demands for strategic growth and more complex service delivery increase, strategic and localised adaptation of PMs may be required to integrate learning and communicative competencies with technical and operational capabilities. Research limitations/implications: The systematic review methodology has been applied to address some of the limitations of publication and reporting biases in literature. This research provides a starting point for future investigations and broadening of discourse in local government contexts. • Four spheres of confluence are found between the modern market economy and diverse economies. • Ecovillages may be places where speheres of confluence between economic approaches may be incubated, experienced and experimented. • Mixed methods research is used to study a recently formed Australian ecovillage. • Community is identified as the principle motivation for living in an ecovillage, this is reflected in the social economic transactions enacted. The practices of ecologically-minded alternative communities illuminate differing perspectives on what might constitute an economy. This paper explores the extant literature and presents a framework for considering four spheres of confluence between the modern market economy and diverse economies: economic production practices, attitudes towards growth, environmental responsiveness and the socialrelational context of transactions. Drawing on evidence from an Australian ecovillage, the paper adopts the framework presented as a means of understanding the experiences of cooperatively negotiating various practices necessary to establish an intentionally sustainable community economy. Through this analysis, the paper provides insights into ways ecovillages may operate in the context of the modern market economy without adopting all of its practices. Ecovillages may indeed be places where convergence between economic approaches may be incubated, experienced and experimented, and through closer trading with the local external communities opportunities for the diffusion of such practices may ensue. Objectives. It is known that there are difficulties in recruiting and retaining practitioners in rural and remote communities and that access to support and professional development can be key in breaking this cycle. Technology provides a possible solution not only for increasing access to these opportunities, but also in building community capacity to support children with autism. The aim of the present study was to investigate the current learning and support needs within rural and remote professionals prior to setting up a model of support. Methods. An online survey was used to gather information from service providers in rural and remote communities on their demographics, current skills and confidence in working with clients on the autism spectrum, current supervision and professional development, identified learning and support needs, and the availability and uptake of technology for accessing professional development. Results. Respondents reported below average levels of perceived confidence and skills when working with children with autism, most notably children with challenging behaviour. Half the respondents do not currently attend supervision sessions, with only 15% receiving regular supervision (fortnightly or more often), and 66% of respondents had travelled more than 3 h to access professional development workshops. The majority of participants had access to technology and over half had already used this for online training. Conclusion. Overall, service providers in rural and remote areas are generally not currently meeting their needs in terms of frequency of supervision and professional development. The present needs analysis identifies key areas for learning, the ideal frequency of support and the acceptability of using technology to deliver this support. Background: Lymphedema may impact patients' health-related quality of life (HRQoL). The purpose of this study was to evaluate HRQoL after two different treatments to the 12-month follow-up point. Methods and Results: Study participants were patients with moderate lymphedema in the upper or lower limb who attended a Swedish rehabilitation program (RP) undergoing conservative treatment, or those with severe, chronic lymphedema dominated by excess adipose tissue, who underwent liposuction (LS) combined with controlled compression therapy (CCT) in Australia, Scotland, or Sweden. The patients completed the Lymphedema Quality of Life Inventory (LyQLI) before intervention and after 1, 3, 6, and 12 months. Mean values and standard deviations were calculated for total limb volume and excess limb volume. Mean values were also calculated for the three LyQLI domains (physical, psychosocial, and practical). To detect and analyze differences in LyQLI responses in the three domains, the Wilcoxon signed rank test was performed. In the RP, 18 eligible patients completed the LyQLI. The results show improvements in HRQoL in physical ( p  = 0.003) and psychosocial domains ( p  = 0.002) at 1 month after the RP, with results remaining steady for 12 months for the physical domain ( p  = 0.024). Fifty-seven eligible LS patients completed the LyQLI. The results show improvements in HRQoL in all three domains ( p  < 0.001), with results remaining steady up to the 12-month follow-up ( p  < 0.001). The total volume in affected limb and the excess volume decreased significantly in both patient groups 1 month after intervention ( p  < 0.001). Conclusions: Treatment with a conservative RP in moderate lymphedema or with LS combined with CCT in severe lymphedema improves HRQoL.

Background: Lymphedema may affect patients' health-related quality of life (HRQoL) and thus a disease-specific questionnaire is needed. The Lymphedema Quality of life Inventory (LyQLI) has been tested for reliability and validity. The purpose of this study was to test the LyQLI for responsiveness and sensitivity., Methods and Results: The study participants were patients with lymphedema in the upper or lower limb, attending a Swedish rehabilitation program (RP) and undergoing conservative treatment without surgery (RP-sample), or undergoing liposuction (LS) in Australia, Scotland, and Sweden (LS-sample). Mean values for the three LyQLI domains (physical, psychosocial, and practical) were calculated before and 1 month after intervention, and the paired t-test was used to detect group differences. Responsiveness was calculated using standardized response mean (SRM), and sensitivity was analyzed with box plots and an independent t-test to detect group differences. Eighteen patients in the RP-sample and 50 in the LS-sample were studied, and both samples demonstrated increased HRQoL after intervention. The changes were significant in all domains, except for the practical domain in the RP-sample. The SRM for the three LyQLI domains (physical, psychosocial, and practical) were 0.8, 0.8, and 0.4, respectively, in the RP-sample and 0.7, 1.2, and 0.9 in the LS-sample. Statistical testing indicated that the LyQLI was sensitive enough to reveal differences between patient groups., Conclusions: LyQLI responsiveness and sensitivity indicated that the tool can be used to evaluate patients undergoing conservative or surgical lymphedema treatments.
