The main objective of the present review is to evaluate the knowledge and attitudes of physical education (PE) teachers concerning dental trauma first-aid through a systematic appraisal of the literature, meta-analysis and meta-regressions. The entire content of PubMed and ISI/Web of Science was mined. Eligibility criteria for selecting studies were studies evaluating dental trauma first-aid knowledge and/or attitudes and/or the effectiveness of mouthguards use by PE teachers. Articles written in any language and published or accepted by peer-reviewed journals were considered. Methodological quality was assessed using an adapted version of the Downs and Black instrument. Of 15 selected articles, three were of strong quality, three were moderate, and the remaining nine were rated as weak. The majority of studies showed that PE teachers had an inadequate knowledge of the initial management of dental trauma. Specifically, there was a lack of knowledge concerning an appropriate washing and transporting medium and the extra-alveolar period of an avulsed tooth. Due to the inadequate knowledge of PE teachers regarding dental trauma management, specific education should be added to PE classes to improve the emergency treatment of dental injuries.

The researchers designed, implemented and analyze a project to teach the contents of First Aid and Waves in English as a foreign language in Brazil using the Content and Language Integrated (CLIL) approach. Seventy participants belonging to two intact classes in a Navy Boarding School took part in the study. Both content and language teachers collaborated to plan and teach the contents. Data collection was carried out through questionnaires, class observation, and entries to teachers' diary. Overall results of the analysis of participants' perceptions on the approach used show that CLIL may be a relevant possibility in that context if certain aspects are considered carefully. Regarding the aspects related to the language of instruction, the study showed that CLIL should be implemented gradually and after students develop a certain level of proficiency in the foreign language. Regarding the aspects related to the content, it was found that participants' perception on CLIL in the First Aid class was much better than in the Waves class because the latter is perceived as being a more difficult content. This finding suggests that difficult contents might not be taught through CLIL depending on participants' language proficiency level. Finally, the study suggests that the implementation of CLIL poses many challenges for teacher practice and education that might be partially overcome through teacher collaboration.

First-aid methods recommended for snakebites are included in Science textbooks in Brazil. Books published before 1996 provided misleading information in this respect and it is possible that such recommendations derived from North-American experiences conducted at the beginning of the 20th Century. The official evaluation of textbooks carried out by the Ministry of Education in 1996 initiated a major public discussion on the several mistakes which had been present in Brazilian educational materials, mainly those relating to mistaken snakebite first-aid procedures. As from 1996, a significant decrease in the number of casualties from snakebites was reported, both in the total number of deaths and among the patients who had received medical assistance. The official data showed this decrease was consistent and remained so for the following ten years. The number of deaths might be associated with the reduced number of victims who developed complications resulting from improper first-aid procedures. This article is intended to relate the changes implemented in textbooks as a result of the first national evaluation and the great impact caused by the publication of the serious mistakes found in these materials, with the reduction in the number of casualties from snakebites in the same period. Objective: To evaluate the effect of an educational intervention, in the context of high school, on the theme cardiorespiratory arrest in adults. Methods: Quasi-experimental study, before and after, with quantitative approach developed between February and November 2019 in a public school in the city of Fortaleza, Ceará. Results: Initially, 30 students were recruited, but 10 met the inclusion criteria, which was the final sample. To carry out the study, a structured questionnaire was used, authored by the researchers, composed of 10 objective questions. After the educational intervention and data analysis, an average of 7,225 correct answers were obtained in the post-test, proving through the numbers the efficiency of health education performed, since initially the average presented was 4,375. Conclusion: It was found that the students had knowledge related to insufficient basic life support, but with great learning capacity, and the school was a favorable environment for educational practices on the subject. Objective: To identify the knowledge of early childhood education professionals about airway obstruction by foreign bodies in children in the school environment. Methods: Descriptive research, with a qualitative approach, carried out in a philanthropic institution in the city of Rio de Janeiro, Brazil, through the application of questionnaires to daycare and pre-school professionals, with the textual corpus being subjected to thematic-categorical analysis. Results: 64 early childhood education professionals participated, who pointed out as the main causes of choking food and small objects. From the analysis, 20 thematic units emerged which, by grouping, comprised two categories: surveillance in the activities of playing and eating at school; and, between the right attitude and ignorance in the face of choking at school. Conclusion: It was observed that the knowledge of injury prevention is consolidated by the practice of constant vigilance by professionals in activities that involve eating and playing. The concept of children in contemporary society is the product of long historical structuring which generated children's and adolescents' rights by specific statute (ECA) and the Constitution. It is also necessary that specific laws guarantee specific situations. Act 13.722/2018 (Law Lucas) investigates the trajectory of Law Lucas in the lower house and discusses it with Laws on integral protection and identifies the principles that guide the production and approval of the law. From the perspective of documental analysis, foregrounded on the Bowe and Ball's policies cycle theory and Laclau and Mouffe's Discourse Theory, it may be perceived that most laws in Brazil do not correspond to national needs. They were actually motivated not by popular demands or urgency but depended on latent factors, with mediatic and family influences or political interests that impose a hegemonic discourse instead of a democratic one. Aim: The aim of this study is to offer an overview of the Medical Emergencies (ME) discipline offer in Dentistry graduations in southeastern Brazil and to observe the curricular characteristics of the discipline when present. Methods: This cross-sectional documentary study analyzed the available curricular frameworks in the official websites of Higher Education Institutions (HEI) in southeastern Brazil registered on the Ministry of Education's e-MEC website. The data were analyzed and tabulated using the GraphPad Prism 8.1.2 software, being described by absolute and relative frequencies. Fisher's exact test was used to compare the proportions between public and private institutions. Results: Of the 176 courses in the Southeast, 144 were included in the study for providing access to the curriculum, 19 (13.19%) were public and 125 (86.81%) were private. Only 27 (18.75%) of the HEI present the discipline of ME, with a greater tendency of supply in private HEIs (20.80%) when compared to public HEIs (5.26%), but this difference was not statistically significant (p> 0.05). As a positive aspect, the discipline is predominantly mandatory (88.88%), and the with regard to the teaching methodology is predominantly theoretical (68.18%). The average workload is 50.14 hours (SD=19.54). Conclusions: In only 18.75% of the dental institutions in Southeast Brazil, ME discipline were offered. When offered, the discipline is predominantly theoretical and mandatory. This study raises an important discussion regarding the need to include specific and mandatory subjects on ME in the dentistry curricula in Brazil and reflects the need to update and standardize the national curricular guidelines for dentistry. Objective: To describe an educational action with primary school teachers and to identify possible risky situations for accidents. Methodology: Experience report, using the observation and round table conversation for data collection. Results: two empirical dimensions were set-up, which allowed to verify that the observed structure presents risk factors for IMPAIRMENT, and there is a lack of knowledge about the primary care. Conclusion: educational measures, involved in the dialogue, help strengthen and improve preventive and interventional practice in school and highlights the importance of building a safe school environment. the main objective of this study was to relate the experience of the Heart Rescue Project, in the city of Salvador, Bahia state, in Brazil. Method: the report is about an experience with the Heart Rescue Project developed by SAMU 192, in which the population was trained in cardiopulmonary resuscitation. Also, through the project were distributed defibrillation equipments for health care spaces. Results: were trained 6,287 people linked to places like health services, public agencies, transfer stations and tourist sites. The addressed content, in the theoretical and practical activities, was based on the American Heart Association protocols for cardiopulmonary resuscitation. To the health units that participated in the training, 200 defibrillators were distributed. Conclusions: the population that received the training was stimulated to act in situations of cardiac arrest. The knowledge imparted increases the probability of obtaining a survival without sequelae, therefore it is believed that this would help new courses of training. [ABSTRACT FROM AUTHOR]

Background: Depression is a significant contributor to disability in Brazil, with most Brazilians affected by depression receiving no treatment. As the community, including family and friends, plays a crucial role in providing support for someone with depression, it is important that evidence-based resources are available to support people who wish to help. The aim of this study was to culturally adapt the English-language mental health first aid guidelines for assisting a person with depression for the Brazilian culture., Methods: A Delphi expert consensus study was conducted, with two expert panels; health professionals (n = 29) and people with lived experience of depression (n = 28). One hundred and seventy-four statements from the English-language guidelines were translated into Brazilian Portuguese and administered as a survey. Participants were asked to rate statements based on how appropriate those statements were for the Brazilian culture and to suggest new statements if appropriate., Results: Data were collected over two survey rounds. Consensus was achieved on 143 statements. A total of 133 statements were adopted from the English-language guidelines, whereas 10 new endorsed statements were generated from suggestions of the two expert panels., Conclusions: There were similarities between the English-language and Brazilian guidelines, mainly related to family involvement and the value of empathy. This is descriptive research, from the field, it's of a qualitative and quantitative nature and transversal, developed with the Special Operations Group - GOE - Military Police / RO, the headquartered being in Cacoal. The officers were selected by convenience, consisted of 22 officers. This study aimed to evaluate whether the police GOE / Cacoal are prepared to provide first aid to members of their team in a situation of urgency / emergency. The survey took place in July 2013 a questionnaire was prepared by the researchers, containing 14 questions, with 09 closed questions and 05 open questions. It was observed that 63.63 % of the officers questioned did not receive first aid training to work in the GOE. In the group of the officers interviewed, 68.18% claim not to have been updated on first aid after entering the GOE; 50% reported that first aid training only occurred every four years or more; 18.18% reported training every two years; 9.09 % reported training every three years; and only 22.72% reported training as annually; 63.63% of the military officers claim that the GOE did not feel prepared to help injured members of their team, because of the first aid equipment in the vehicles of the GOE; 95.45% of the interviewed claim not to have first aid material available to them. Most of the military officers mentioned situations where they witnessed injuries by gunshot fire, automobile accident and stab wounds. Considering the statistical data presented above, it is concluded that the majority of officers interviewed are not prepared to provide first aid care to injured members of their team. Background: Traumatic events increase the risk of mental disorders. In a country with relatively under-developed mental health support systems, services to assist people who have experienced potentially traumatic events may be unavailable. In such situations, people in the community become key sources of support. However, they do not always have the knowledge and skills to offer effective help. This study reports on the cultural adaptation for Brazil of the English-language mental health first aid guidelines for helping someone who has experienced a potentially traumatic event., Methods: A Delphi expert consensus study with two expert panels, one comprising health professionals with experience in the treatment of trauma (n = 33) and the other comprising people with lived experience, (n = 29) was conducted. A questionnaire containing 131 statements from the English language guidelines was translated into Brazilian Portuguese. Participants were asked to rate the importance of actions to be taken to help a person who has experienced a potentially traumatic event and to suggest new items where appropriate., Results: Data were collected over two survey rounds. A total of 149 items were included in the final guidelines (110 items from the English-language guidelines and 39 new items created from expert panel comments, in the second round). Immediate action items were endorsed by both panels, while items related to encouraging victims were rejected by the professional panel. The suggested statements mostly related to providing psychological support and attending to the person's subjective experience rather than providing material or structural support., Conclusion: While there were many similarities with the English-language guidelines for high-income countries, the guidelines also incorporate actions of importance for Brazil, including the emphasis on the first aider's management of the person's subjective experiences. Dental students have little understanding about medical emergencies, and there is very little in-depth data about the importance they place on this important area that is fundamental to their professional training. This study aimed to identify the perceptions of a group of undergraduate dental students about the dentistry-medical emergency interface. Twenty undergraduate dental students at the Federal University of Goias, Brazil, took part in this study. The data were collected through in-depth interviews with these students and were interpreted using qualitative content analysis. Two themes emerged from this data analysis: dentistry as a comprehensive health science, and students' knowledge, feelings, and attitudes about medical emergencies in the dental office. Based on the students' perceptions, an interface between dentistry and medical emergencies in the dental office was proposed that is comprised of the following intertwined concepts: 1) dentistry is a health science profession that should focus on the whole patient instead of being limited to the oral cavity; 2) medical emergencies do occur in the dental office, but students' minimal knowledge about these incidents and their etiology causes feelings of insecurity, dissatisfaction, and a limited appreciation of the dentists' responsibility; and 3) the inability to perform proper basic life support (BLS) technique in the dental office is the ultimate consequence. Undergraduate health courses need to develop strategies to teach professionals and students appropriate behavior and attitudes when facing life-threatening emergencies. [ABSTRACT FROM AUTHOR]

Background: Children with microcephaly associated with the Zika virus are more likely to develop choking and apnea, so it is necessary that mothers of this population receive first aid training to cope with emergency situations that their children may experience., Objective: To understand how an educational first aid intervention with mothers/caregivers of Brazilian children with congenital Zika virus syndrome impacts their management of seizures and choking., Method: A qualitative study conducted in a philanthropic institution with ten mothers/caregivers of children with congenital Zika virus syndrome. Data were collected between August and November 2018 through four focus groups with two meetings for intervention/educational workshops in first aid on situations of choking and seizure. After one month, two more meetings were held to evaluate the effectiveness of this action performed at home. The empirical material was submitted to Content Analysis, and discussed in the light of Paulo Freire's Pedagogy of Autonomy., Results: The mothers/caregivers performed ineffective and harmful behaviors to the children in situations of choking and seizure before the intervention. Thus, the intervention empowered these caregivers through an exchange of experiences which began to present discernment, autonomy, resolving capacity and confidence to act in these experienced situations. In the Brazilian Amazon, snakebites are a significant problem, especially for populations in rural areas, particularly in forests, where victims are a considerable distance from hospital care. Several factors are associated with the severity of the accident, such as the size and age of the snake. This study aims to compare the clinical, epidemiological and laboratory aspects of envenomation to the size of Bothrops atrox snakes. Clinical, epidemiological and laboratory variables were collected from patients bitten by B. atrox and who were admitted to a hospital in the city of Cruzeiro do Sul (western Brazilian Amazon). When the two punctures of the teeth were present in the bite sign, the distance between these was measured. When taken to the hospital, the snake was measured; otherwise, its size was estimated via interviews with patients. In 92 cases, the size of the snake was estimated, and most of these were caused by small snakes. Bites of small snakes occur mainly on the feet, while larger specimens reach the legs or higher regions. Small snakes were associated primarily with mild and moderate snakebites, with more presence of hemorrhagic manifestations, while larger snakes were responsible for severe cases and characterized by local effects (necrosis, edema, flictena, compartment syndrome, and infection) and patients were treated with a higher amount of antivenom and for a longer period of hospitalization. The distance of the punctures was related to the size of the snake and the severity of the local envenomation. The observation of the distance between puncture marks when present, which is correlated with the length of the specimen, as well as the estimation of the snake size by the patient, may provide more support for the health professional on the prognosis of envenomation. The use of boots in activities in rural areas and forests could contribute to a lower rate of cases of snakebites, and health education on preventive measures and first aid for populations is fundamental., (Copyright © 2020 Elsevier Ltd. Objective: To describe elements of vulnerabilities and clinical manifestations of the victims of scorpionism in Santarem-Pará., Method: Quantitative and qualitative study with a theoretical framework of Twenty-eight patients stung by scorpions were interviewed by applying a semi-structured instrument. Data were analyzed according to the technique of discourse analysis., Results: Seventeen patients (60.7%) were male and eight (28.6%) were illiterate. Twenty-two (78.5%) patients were small farmers. This study revealed that these patients had an important individual, social and programmatic vulnerability. Interviews revealed limited knowledge about prevention and first aid after the envenomation. Programmatic vulnerability is highlighted because there is difficulty in accessing health services due to the great geographical extension of the Amazon region and also a lack of experience of the health care workers in caring for scorpion sting victims., Conclusion: This research accurately reveals previously described neuromuscular and sensory nervous system disorders in these victims of scorpionism. As far as we know, this is the first study that addresses the vulnerability of scorpionism using qualitative methodology.

Background: The Alto Juruá region, located in the extreme western part of the Brazilian Amazonia, possesses an indigenous and riverine population which is involved in agricultural and forest extraction activities, and is a region that stands out for its high incidence of snakebites., Objectives: To assess the attitudes of the victims, the characteristics of the snakes and the circumstances of the snakebites which occurred in a region where human populations are highly exposed to snakes., Methods: The study was conducted at the Regional Hospital of Juruá in the Municipality of Cruzeiro do Sul (Acre), which regularly attends victims of snakebites in the Alto Juruá region. The snakes that caused the envenomations were identified from clinical and epidemiological diagnosis of the symptoms and signs that patients presented during hospital, and by enzyme immunoassay for venom detection using serum samples of the patients, or by identification of the snake responsible for the envenomation when it was taken to the hospital or photographed. People who suffered or witnessed the snakebite were interviewed to assess the circumstances of the bite, the attitude adopted after the accident and whether they recognized the species of snake that caused the envenomation., Results: There were 133 cases of snakebite (76.24/100.000 inhabitants), mainly involving male individuals living in the rural area and who had a low level of education. The most affected groups were farmers (48%) and children and teenagers (39%). It was observed that 8.3% of them presented a history of recurrence for bites. The lower limbs were the most affected anatomical region (84%). The Bothrops atrox snake, mainly small specimens (mostly juveniles), was the main species involved in the envenomations (83.4%). Snakebites occurred mainly in forest areas, backyards of houses in rural areas and near to aquatic environments, during activities (walking, farming, extractivism, hunting). Most of the time, the snake was on the ground and the bite occurred because of the approximation of the individual, either by trampling or by approximation of a hand. Half of the victims performed some kind of inadequate first aid (not drinking water, use of tourniquet, incision at the site of the bite, use of black stone, drinking a compound "Específico Pessoa")., Conclusions: Snakebite is an important cause of morbidity in the Alto Juruá region. Bothrops bites are mostly caused by small-sized specimens, probably due to the greater abundance of B. atrox juveniles, and also because small snakes are more difficult for people to see. People are more often bitten on the lower limbs probably due to the size of B. atrox (small and medium) and because the snakes are usually on the ground in most situations. Many victims resort to ineffective actions that can cause complications and also delay serotherapy. International data severely underestimates actual drowning numbers. Almost all victims are able to help themselves or are rescued in time. This study aims to report the occurrence of Drowning Chain of Survival actions and resuscitations needed in a fully operational lifeguard service., Methodology: Data was collected from Dec-2009 to Mar-2015 by lifeguards at a 6km-long beach in Brazil. The Drowning Chain of Survival links were summarized into 3 main action-response sections: Prevention; rescue; and provide care. Rescues were classified by severity., Results: Lifeguards reported 1,565,699 actions during the study period. Preventative actions comprised 1,563,300(99.8%) and 2044 (0.1%) involved recognizing a person in stress/distress and rescuing them. Of those requiring rescue, 355(0.02%) needed medical assistance due to respiratory symptoms, isolated respiratory arrest, or cardiopulmonary arrest. Those cases were classified by severity as: Grade 1 = 234(65.9%), grade 2 = 78(22%), grade 3 = 22(6.2%), grade 4 = 7(2%), grade 5 = 4(1.1%), and Grade 6 = 10(2.8%). From all 2044 rescues, 14(0.7%) were grade 5 and 6 and needed respiratory or cardiorespiratory resuscitation. An estimative incident rate for each day at a lifeguarded beach revealed: 1 rescue for every 4.227 beach attendances, 1 drowning for every 24,338 beach attendances, and 1 instance of CPR being performed for every 617,142 beach attendances. The prevalent misconception that majority of drowning require resuscitation is perpetuated by the media and publishers. We are only just seeing the tip of the iceberg and urgently need to look at the problem in its entirety. We aimed to evaluate knowledge of first aid among new undergraduates and whether it is affected by their chosen course. A questionnaire was developed to assess knowledge of how to activate the Mobile Emergency Attendance Service - MEAS (Serviço de Atendimento Móvel de Urgência; SAMU), recognize a pre-hospital emergency situation and the first aid required for cardiac arrest. The students were also asked about enrolling in a first aid course. Responses were received from 1038 of 1365 (76.04%) new undergraduates. The questionnaires were completed in a 2-week period 1 month after the beginning of classes. Of the 1038 respondents (59.5% studying biological sciences, 11.6% physical sciences, and 28.6% humanities), 58.5% knew how to activate the MEAS/SAMU (54.3% non-biological vs 61.4% biological, P=0.02), with an odds ratio (OR)=1.39 (95%CI=1.07-1.81) regardless of age, sex, origin, having a previous degree or having a relative with cardiac disease. The majority could distinguish emergency from non-emergency situations. When faced with a possible cardiac arrest, 17.7% of the students would perform chest compressions (15.5% non-biological vs 19.1% biological first-year university students, P=0.16) and 65.2% would enroll in a first aid course (51.1% non-biological vs 74.7% biological, P<0.01), with an OR=2.61 (95%CI=1.98-3.44) adjusted for the same confounders. Even though a high percentage of the students recognized emergency situations, a significant proportion did not know the MEAS/SAMU number and only a minority had sufficient basic life support skills to help with cardiac arrest. A significant proportion would not enroll in a first aid course. Biological first-year university students were more prone to enroll in a basic life support course.

Obesity is currently one of the major epidemics, representing an important public health problem that is associated with an increase in chronic disease. The scope of this study is to investigate the prevalence of excess weight and obesity and associated factors in 20 to 59-year-old women attended at First Aid Units of the Unified Health System. Standardized interviews and anthropometric measurements were conducted with 440 women. Factors associated with the presence of excess weight/obesity were investigated using the Poisson model with robust variance. The prevalence of excess weight and obesity (64.3%) as well as central obesity (44.1%) in women attended at First Aid Units is higher than that in the population in general and is associated with an increased incidence of health problems. Insufficient physical activity (PR=1.44; 95% CI 1.24-1.66), a history of three pregnancies or more (PR=1.45; 95% CI 1.05-2.00), age above 50 (PR=1.34; 95% CI 1.00-1.82) and central obesity (PR=1.92; 95% CI 1.64-2.25) are associated with a higher prevalence of obesity. The implementation of public health polices focused on the prevention of obesity would undoubtedly have a major impact on primary prevention of chronic diseases in the female population.

Coatis [including Nasua nasua, the ring-tailed coati], are medium-sized mammals widely distributed in the Americas. They are social animals, whose normal diet includes insects, fruits, and small vertebrates, and rarely prey on larger sized animals. There are, to our knowledge, no reports in the medical literature of attacks on humans. This report describes a coati attack on 2 children in their home. The children sustained deep scratches and bites. The animal may have injured the humans in a defensive strike, but motivation for attack was uncertain. Introduction: The fishes of continental Brazil have socioeconomic importance due to their potential for sport fishing and commercial and subsistence uses, as seen in the Upper Paraguay River Basin, particularly in the municipalities of the Pantanal region, where it is the second largest economic activity. Injuries caused in professional fishermen are common and poorly studied, as in other regions of the country., Methods: Data were obtained from questionnaires and interviews with 100 professional fishermen, 50 in each municipality, between December 2008 and October 2009., Results: All the fishermen reported some kind of injury caused by fish stings (78% of injuries) and fish, alligator and snake bites (22%) on the hands (46% of cases) and feet (35% of cases). Most of the patients had mild symptoms. The most severe cases were associated with secondary bacterial infections and required specific treatment and prolonged recovery associated with social and economic losses., Conclusions: The results of this study indicate that the stressful work conditions, inattention to basic preventive measures and carelessness were factors that contributed to accidents and that the toxicity and ability to inflict mechanical trauma of some aquatic species, plus the ineffective use of first aid and hospital treatment, contributed to the high morbidity and complications in many cases. Data from this study are relevant to the fishing communities of the Pantanal region, since they reveal high rates of accidents, lack of knowledge concerning first aid, initial treatment, injury prevention and lack of medical follow-up of the population.

This study aims to analyze accidents involving foreign bodies among children less than 15 years of age residing in Londrina, Paraná State, Brazil, in terms of first aid, hospitalization, and death (2001). Data were obtained from general hospital records and the Municipal Mortality Database. A total of 434 accidents were analyzed, with a 3.7% hospitalization rate and 0.7% mortality. Boys predominated (53.7%), and the incidence rate was highest among children one to three years of age (7.2 per 1,000 children). Foreign body penetration in natural orifices (eyes, nostrils, and ears) accounted for 94%, inhalation/ingestion of food 2.8%, inhalation/ingestion of objects 2.5%, and aspiration of gastric contents 0.7%, and these causes accounted for all the deaths. The results contribute to epidemiological knowledge on such accidents and indicate the need to restructure health services in order to decentralize care for less complex injuries, besides emphasizing the need for preventive measures.

We studied 386 cases of spinal cord injury to analyze the follow up of the patients admitted most of the time more than four hours, the majority of the injuries happening far from the attending health service and first specialized care received long after the accident. This is a clinical study based on data collected during hospitalization of the patients, operated or not, in a Brazilian public health service. The lesion mainly seen was fracture and dislocation, isolated or on multiple levels, and the most important clinical complications were due to respiratory failure and hypotension, especially because 73.8% were from outside and they were admitted more then four hours after the trauma. The mortality rate was 11.9%, but just 2.1% had undergone a surgery. The complications resulted in major risk of death when the trauma was at the cervical level and the patients were over 50 years old, especially when admitted more than four hours after the trauma. We emphasize the importance of the first health care concerning the clinical treatment, aiming to reduce the mortality rate.

Study Objective: To establish an updated classification for near-drowning and drowning (ND/D) according to severity, based on mortality rate of the subgroups., Materials and Methods: We reviewed 41,279 cases of predominantly sea water rescues from the coastal area of Rio de Janeiro City, Brazil, from 1972 to 1991. Of this total, 2,304 cases (5.5%) were referred to the Near-Drowning Recuperation Center, and this group was used as the study database. At the accident site, the following clinical parameters were recorded: presence of breathing, arterial pulse, pulmonary auscultation, and arterial BP. Cases lacking records of clinical parameters were not studied. The ND/D were classified in six subgroups: grade 1--normal pulmonary auscultation with coughing; grade 2--abnormal pulmonary auscultation with rales in some pulmonary fields; grade 3--pulmonary auscultation of acute pulmonary edema without arterial hypotension; grade 4--pulmonary auscultation of acute pulmonary edema with arterial hypotension; grade 5--isolated respiratory arrest; and grade 6--cardiopulmonary arrest., Results: From 2,304 cases in the database, 1,831 cases presented all clinical parameters recorded and were selected for classification. From these 1,831 cases, 1,189 (65%) were classified as grade 1 (mortality=0%); 338 (18.4%) as grade 2 (mortality=0.6%); 58 (3.2%) as grade 3 (mortality=5.2%); 36 (2%) as grade 4 (mortality=19.4%); 25 (1.4%) as grade 5 (mortality=44%); and 185 (10%) as grade 6 (mortality=93%) (p<0.000001)., Conclusion: The study revealed that it is possible to establish six subgroups based on mortality rate by applying clinical criteria obtained from first-aid observations. These subgroups constitute the basis of a new classification.
