Purpose: It is known that cerebral palsy (CP) children's caregivers suffer from burden, depression, and stress, impairing their quality of life (QoL). The more severe the CP, the more burden the caregiver has. Psychosocial support, education, therapies, and financial support are inversely related to the level of stress of the caregiver. Most parents of CP patients submitted to selective dorsal rhizotomy (SDR) report improvement not just on spasticity, but also in the functional role of the children, what can impact on caregiver's QoL. Our objective was to evaluate the burden of CP children's caregivers with and without previous SDR. Methods: Spastic CP children caregivers were divided into two groups: those who take care of children without previous SDR (control group) and those that children were previously submitted to SDR (surgical group). The burden index was compared between groups using Burden Interview Questionnaire (BIQ). For statistical analysis, we used SPSS. Results: The control group had enrolled 31 participants and the surgical group 36. The mean GMFCS level on the control and surgical groups was 3.94 ± 1.26 and 3.74 ± 1.12 (p = 0.61), respectively. The surgical group caregivers presented less burden related to the feeling that they should be doing more to their child (p = 0.003) and if they could do a better job in caring (p = 0.032), compared to controls. The total BIQ index was not significantly different between groups (surgical 32.14 ± 12.34 vs. control 36.77 ± 12.77; p = 0.87). Low economic status had a weak correlation to a higher BIQ index (R2 = 0.24). After age-matching, there was a significative higher BIQ index in the control group (p = 0.008). Conclusion: Caregivers of spastic CP children who were previously submitted to SDR presented less burden related to feeling of the amount of given care than those without previous surgery. The impression that they could do a better job with their kids was higher in the control group. The severity of CP and low economic status were related to more burden in both groups. After pairing groups by age, the control group had a significative higher BIQ index compared to the SDR group. Multiple sclerosis (MS) is a demyelinating disease of the central nervous system, with fatigue being one of the symptoms. Objectives: To evaluate the result of performing a virtual task, Labyrinth, by people with multiple sclerosis who present physical fatigue and to understand if even in the presence of fatigue it is possible for the patient to perform a virtual task. Method: Cross-sectional research with 20 people with MS, in Civil Social Institution, Brazil. The psychological tests were: Figuras Complexas de Rey - FCR (memory), Beck depression inventory (BDI) and anxiety (BAI), a questionnaire about fatigue perception and virtual task (Labyrinth). Results: Sample between 25 to 57 years (M 44.4, SD 11.03), 60% female, 90% with computer use experience, 70% with EDSS (expanded disability scale) from 0 to 4, 55% recognize fatigue, physical 75%, mental 80% and visual 60%. Statistically there is a correlation between physical fatigue and coping on FCR (0.019), muscle weakness and spasticity (0.053); Mental fatigue interferes with the copy on FCR (0,059); Visual fatigue has weaker significance on FCR copy, but is present (0.065) and in BAI (0.029). No correlation was found between fatigue, depression and virtual task. Conclusion: The correlation between fatigue and virtual task has not been demonstrated. [ABSTRACT FROM AUTHOR]

Background: Based on the premise that spasticity might affect gait post-stroke, cryotherapy is among the techniques used to temporarily reduce spasticity in neurological patients. This effective technique would enhance muscle performance, and ultimately, functional training, such as walking. However, understanding whether a decrease in spasticity level, if any, would lead to improving muscle performance and gait parameters is not based on evidence and needs to be clarified. Objectives: to investigate the immediate effects of cryotherapy, applied to spastic plantarflexor muscles of subjects post-stroke, on tonus level, torque generation capacity of plantarflexors and dorsiflexors, and angular/spatiotemporal gait parameters. Methods: Sixteen chronic hemiparetic subjects participated in this randomized controlled crossover study. Cryotherapy (ice pack) or Control (room temperature sand pack) were applied to the calf muscles of the paretic limb. The measurements taken (before and immediately after intervention) were: 1) Tonus according to the Modified Ashworth Scale; 2) Torque assessments were performed using an isokinetic dynamometer; and 3) Spatiotemporal and angular kinematics of the hip, knee, and ankle (flexion/extension), obtained using a tridimensional movement analysis system (Qualisys). Results: Cryotherapy decreased plantarflexor tonus but did not change muscle torque generation capacity and did not affect spatiotemporal or angular parameters during gait compared to control application. These findings contribute to the evidence-based approach to clinical rehabilitation post-stroke. Conclusions: The findings of this study suggest that cryotherapy applied to the calf muscles of subjects with chronic hemiparesis reduces muscle hypertonia but does not improve dorsiflexors and plantarflexors performance and gait parameters. Objective: The aim of the study was to evaluate the immediate effects of cryotherapy (using an ice pack) on ankle joint position sense and the degree of spasticity after chronic hemiparetic stroke. Design: We used a sham-controlled crossover design. Sixteen chronic hemiparetic patients were randomly assigned to two groups: (1) those who received cryotherapy followed by a control intervention 15 days later (cryotherapy group) and (2) those who received the control intervention followed by cryotherapy 15 days later (control intervention group). Ankle joint position sense was measured on the paretic side using a Biodex Multi-joint System 3 dynamometer before and after 20 mins of either application on the calf muscles. Lower absolute error scores were calculated for data analyses and were used to determine joint position sense. The degree of spasticity of the plantar flexor muscles was scored according to the Modified Ashworth Scale. Results: Sixteen patients completed the crossover experiment; however, data analysis was successfully conducted in 15 participants. Cryotherapy reduced the degree of spasticity of the plantar flexor muscles without altering ankle joint position sense. PurposeWe propose a visual myofeedback protocol as a coadjuvant therapy to standard rehabilitation of post-stroke spastic patients. We also argue in favor of the tonic stretch reflex threshold (TSRT) as a more sensitive unit for quantifying subtle changes in the spastic response that may be induced by biofeedback training.MethodSixteen volunteers with ischaemic stroke were divided into an experimental group (EG), subjected to myofeedback training in parallel with conventional physical therapy and a control group (CG), receiving only conventional physical therapy. The EG subjects underwent a six-week myofeedback training, with two sessions weekly. Both groups followed the same treatment schedule for physical therapy. The TSRTs of the volunteers’ spastic muscles were assessed before the beginning of the experimental protocol and 3 weeks after it ended.ResultsBoth groups showed some degree of improvement in the level of spasticity when the final TSRT values were compared to the initial values. However, the percentage of improvement (after–before) of the experimental group (38.59%) was significantly higher than that in the control group (18.58%).ConclusionThe myofeedback training provided a significant contribution to conventional treatment, allowing for a better improvement of the spastic condition.Implications for rehabilitationBiofeedback is an effective means of improving motor control of post-stroke spastic patients.The Tonic Stretch Reflex Threshold is a more sensitive quantitative measure to assess upper-limb post-stroke spasticity.Spastic patients who participate in myofeedback training along with physical therapy can improve faster then those who participate only in traditional physical therapy rehabilitation protocols. The purpose of the present study was to evaluate the effects of conventional physical therapy and task specific training associated with electromyographic (EMG) feedback to recover upper limb motor function in patients with hemiparesis caused by stroke. Twelve patients were evaluated and then randomly assigned to either of two groups: Group 1 - Conventional physiotherapy; Group 2 - EMG feedback. Patients attended three weekly 50-min sessions, for eight weeks. The evaluated aspects were motor impairment, motor function, and spasticity, using the Fugl-Meyer Assessment (FMA) test, TEMPA, and the Modified Ashworth Scale (MAS), respectively. Results showed that, in both groups, there were no changes in spasticity in the groups, but there was a significant improvement in FMA values after the rehabilitation programs. Regarding TEMPA scores, only Group 2 presented significant gains. The comparison of the MAS and FMA gains showed no differences between groups. However, group 2 showed greater gains in the two tasks of TEMPA. The results suggest that functional training using EMG feedback can have positive results in improving arm function. However, due to small differences between groups, concluded that EMG/Biofeedback therapy was not superior to other form of treatment. [ABSTRACT FROM AUTHOR]

A letter to the editor is presented regarding the frequency and mutation spectrum of SPG-related hereditary spastic paraplegia in Brazil.

Botulinum toxin type A (BoNT-A) is an effective treatment for post-stroke spasticity; however, some patients cannot access treatment until ≥1 year post-stroke. This Brazilian post-marketing study (NCT02390206) assessed the achievement of person-centered goals in patients with chronic post-stroke spasticity after a BoNT-A injection. Patients had a last documented stroke ≥1 year before study entry and post-stroke upper limb (UL) spasticity, with or without lower limb (LL) spasticity. Patients received BoNT-A injections at baseline (visit 1) and visit 2 (3-6 months). Primary endpoint was responder rate (achievement of primary goal from Goal Attainment Scaling (GAS)) at visit 2. Overall, 204 patients underwent GAS evaluation at visit 2, mean (SD) age was 56.4 (13.2) years and 90.7% had LL spasticity. Median (range) time between first stroke and onset of spasticity was 3.6 (0-349) months, onset of spasticity and first injection was 22.7 (0-350) months and waiting time for a rehabilitation appointment was 9.0 (1-96) months. At visit 2, 61.3% (95% CI: 54.4, 67.7) of patients were responders, which was similar for UL and LL primary goals (57.8% [95% CI: 49.9, 65.3] vs. 64.1% [95% CI: 48.4, 77.3]). This study provides evidence to support the effectiveness of BoNT-A treatment for chronic post-stroke spasticity.

The study aims to investigate the influence of different postures on spasticity results by pendulum test in patients with spinal cord injury (SCI). The setting was at the University of Campinas (UNICAMP), Campinas, SP, Brazil. Five individuals with SCI and five individuals in the control group were included. All individuals went through the pendulum test in three different positions: supine, semi-supine at an angle of 30°, and sitting up at an angle of 60°. An electrogoniometer was attached to the right leg for measurement of knee joint angles. All situations were performed five times. Blood pressure was monitored during tests. Relaxation index (RI), normalized relaxation index (RIn), test duration in seconds, initial flexion angle, and resting angle were analyzed at three different positions. Results were compared between different positions, and statistically no differences were found. In individuals with SCI, RI (1.83 ± 0.2), RIn (1.14 ± 0.13), and test duration values (13.95 ± 4.14), in sitting up position, were similar to the control group results. In sitting up position, patients showed spasticity reduction. However, the other two postures produce pain and increase blood pressure in patients with tetraplegia.
