Recognizing the central role of mental health in global health and affirming the complexity of the universalization of care policy, this paper discusses aspects of global mental health in Brazil and Italy - two countries that are a potential reference for exchange between the global North and South. Using ethnographic and action research methodologies, we conducted a study of a virtual community of practice composed of Brazilians and Italians interested in community mental health care. The results are presented in scenes that provide insights for the international debate in three categories: the doctor-centered approach to care; the institutionalization of care and medicalization of suffering; and the contribution of community practices and non-specialized local knowledge. The locally situated scenes cast light on globally shared critical knots, elucidating a plural set of relationships that run through work processes and mental health care. The sharing of knowledge and experiences highlight what should be universalized: opportunities for horizontal exchange, rather than the production of national identities that radiate universalizing practices and policies. This descriptive study sought to analyze the mental health status of 48 Northeastern municipalities in Brazil with quilombola communities and agrarian reform settlements in their territories, considering the living conditions, health service supply and the profile of morbidity and mortality in mental health. Gender, class and race aspects are articulated to understand the production of health inequities in these territories. By means of a quantitative analysis based on secondary data collected from different open databases, the research unveiled the intersection and combination of factors that influence the mental health status of municipalities with rural and forest populations: the precariousness of living and working conditions, insufficient support from psychosocial care services, and ethnic-racial and gender inequalities in psychiatric morbidity and mortality. (1) Background: This study aims to examine and describe the policies of three Latin American countries: Colombia, Brazil, and Spain, and identify how they implement their support systems for health, mental health, mental health for children and adolescents, and juvenile justice systems that support judicial measures with treatment and/or therapeutic approaches specialized in mental health. (2) Methods: Google Scholar, Medline, and Scopus databases were searched to identify and synthesize of the literature. (3) Results: Three shared categories were extracted to construct the defining features of public policies on mental health care in juvenile justice: (i.) models of health and mental health care, (ii.) community-based child and adolescent mental health care, and (iii.) mental health care and treatment in juvenile justice. (4) Conclusions: Juvenile justice in these three countries lacks a specialized system to deal with this problem, nor have procedures been designed to specifically address these situations within the framework of children's rights.

This paper is based on a qualitative research study within a Community Mental Health Centre in Brazil. It addresses professional actions within mental health services as a sensitive sphere in which to discuss deadlocks and critical strategies to expand practices towards deinstitutionalization. The idea of subjective development from a cultural-historical standpoint is discussed as a theoretical way to promote institutional practices which articulate education and mental health care. Subjective development is regarded as a non-universal, non-deterministic, and context-sensitive process, having the subject configuration as its unit. We argue that such discussion has heuristic value for understanding mental health as a living process, beyond hermetic diagnostic entities, overcoming the objectualization and hierarchical aspect which frequently characterize the relationship between service users and workers. Moreover, we discuss how professional actions geared toward subjective development could enhance dialogical relations capable of supporting individuals and groups to actively position themselves as subjects in their life pathways. From this point of view, individuals are not considered as an epiphenomenon of social forces, such as the result of the effects of power, but as a crucial moment of social experience. Contemporary society is a reflection of a series of transformations in the social, political, cultural and economic sphere that alter the forms of establishing social ties. The know-how in Mental Health has also undergone important and significant transformations in Brazil. It has evolved, definitively, from a model centered on the hospital reference to a model of diversified attention, community territorial base, found in the Centros de Atenção Psicossocial (Centers for Psychosocial Attention/CAPS) one of its main references. The purpose of this paper is to demonstrate how the assembly of this service suffers the effects of the described changes in the name of post modernity. The malaise, described by Freud, finds in contemporary perspective new forms of expression. This implies in new forms of modulating the psychic suffering. In this context, the body becomes the privileged stage to support these different forms of presentation of malaise, is sometimes captured by the language game of a neurobiological clinic that excludes the dimension of the subject, thus silencing its suffering. The article presents a case study that investigates the use of motivations in the mental healthcare governance of socially disadvantaged ethnic minority users in Portugal. The topics discussed include the factors that motivate them to be involved in mental healthcare governance and the effectiveness of the mutual incentives theory (MIT) in motivating them to participate in the said activities.

Introduction: Community-based psychiatry as basis for the psychosocial organizational care is a political perspective that many health services adopted after processes of desinstitutionalization. We analyze two community-based networks in depth: Rio de Janeiro's (Brazil) and Legane's (Madrid, Spain) Mental Health Services. Its contrasts and divergences are striking both structurally and performatively. Objectives: We present an analysis of the organization and implementation of care for people with severe mental illness in Rio de Janeiro and in Leganés. We theorize about the possible causes of the observed differences, considering cultural, social and political factors. Methods: An analysis of the relevant literature published by prominent community-based psychiatry authors in Spain and Brazil. Special attention is paid to the discrepancies between the theoretical principles held between different countries and the practical institutional implementation. Results: A predominance of a socio-cultural approaches is observed in the Brazilian psychosocial network compared to a more clinical orientation in the Spanish one. Conclusions: We hypothesize that the differences observed are supported by complex sociocultural aspects. In the case of Brazil: greater primary-care resources which allow for a better coordination, the overlapping of assistance with the private healthcare system as well as Río de Janeiro's vulnerable population marginalized by the state, to name only a few. All of the above creates the need for a socially-centered approach which is less centered in the purely clinical approach. These characteristics result in a comprehensive and community-based support of the individual in which the clinical psychopathological understanding becomes blurred and relegated to second place.
