Objective: to characterize the notifications of malaria in pregnant women in the municipality of Oiapoque. Method: documentary, descriptive, retrospective and quantitative study, conducted from secondary data of the Epidemiological Surveillance System of Malaria in Oiapoque-Amapá, Brazil, from 2013 to 2017. The following variables of autochthonous cases of malaria in pregnant women are addressed: year, month of occurrence, gestational age, Plasmodium infecting species and notification unit. Data were presented and analyzed using descriptive statistics and formulation of spatial distribution maps, generated by ArcGIS software. Results: notifications predominated in urban areas, especially in the Paraíso neighborhood (74%), with Plasmodium Vivax being the main agent (88%), and with a higher incidence between October and December (33%), in the third gestational quarter (35%). Conclusion: the profile of reports of malaria in pregnant women of this municipality resembles previous studies in this region, regarding the place of concentration and period of greater occurrence. However, the gradual introduction of the protozoan Plasmodium falciparum brings an alert to the mobilization of managers and professionals. [ABSTRACT FROM AUTHOR]

Objective. To identify the factors linked to patients and health services in delays in the diagnosis of tuberculosis. Methods. Epidemiological study in Foz do Iguaçu, Paraná, Brazil, 2009. The Primary Care Assessment Tool, adapted for appraising tuberculosis treatment, was the instrument used. Descriptive statistics techniques were used, such as frequency distributions, central tendency and dispersion measurements (median and interquartile intervals), and odds ratios. Results. There were greater delays in seeking health services for those in the age group 60 years and older, for females, for patients with low levels of education, and for patients with poor knowledge of the disease. Clinical variables (being a new case and HIV infection) and behavioral variables (use of tobacco and alcohol consumption) were not linked with delays in diagnosis. The median time delays before diagnosis attributable to patients and to the health services were 30 days and 10 days, respectively. Emergency 24-hour medical services and primary health care services were not effective in identifying suspicious cases of tuberculosis and requesting tests to confirm the diagnosis, with a high percentage of referrals to the Tuberculosis Control Program clinic. Conclusions. Going to primary health care services for diagnosis increased the time before diagnosis of the disease was reached. The Tuberculosis Control Program clinic was more effective in diagnosis of tuberculosis, due to the training of the staff and to an organized process for receiving patients, including the availability of tests to support the diagnosis. The objectives of this article are to describe the health sector reform process in Latin American countries and its impact on the health systems of Brazil, Colombia, and Peru; analyze the structure of the national health systems of these bordering nations; and assess the constraints and potential for integrating the local health systems along the triple Amazon border. Using official documents and databases, we analyzed health system development and the structure of the services network in this segment of the border located in the heart of the Amazon region. Comparison of the three countries confirmed that Brazil's model has the characteristics of a national public health system, while the Colombian and Peruvian models emphasize insurance systems, with the adoption of compulsory health care plans, the segmentation of users, and limitations on coverage. The three countries are similar in their adoption of a public-private mix in service delivery, but they differ in the degree of access and type of State regulation. We concluded that the characteristics of the health systems in the three countries make the challenge of building a unified health system capable of guaranteeing universal access, equity in care, and comprehensive care a complex undertaking. However, despite the differences identified, we can state that the weight of the public sector in the basic care network makes it possible to foresee integration of the local health systems to provide an organized supply of programmatic health actions at the primary and secondary level of care.
