As life course research has long recognized, work and careers are what Pearlin (1988; p.259) describes as "durable arrangements" that serve to "organize experience over time." However, understanding (a) the specific impacts of the alienations and contradictions of work and society under capitalism as well as (b) the analytic details of how the processes of learning are involved in the relationship of work experience and life course remain less well understood. An approached referred to as Mind in Political Economy is explained. It is based on a synthesis of several socio-cultural learning theories which allows the effective use of the concept of dramatic perezhivanie. This approach is then applied to a study of workplace learning in the context of a chemical production plant in Canada with a focus on the life history of one subject. It concludes that, based upon evidence of the realization of dramatic perezhivanie in relation to the contradictory object-motives of occupational autonomy/control as well as labour autonomy/control more generally, work-life learning in activity affected the quality of work experiences, the nature of development across employment history, and had carry-over effects beyond work. Moreover, it is shown that work-life learning could play a role in retrieving, reconstructing and making use of early life experience iteratively in the course of biographical meaning-making through the creation and refinement of biographical artefacts across the life course by a process of double stimulation. Gaps in health-care services for women with polycystic ovary syndrome (PCOS) are not isolated incidents. They are systemic and disadvantage women. In this study, we explored the juxtaposition of power and agency in diagnosis narratives collected from 72 Canadian women diagnosed with PCOS. Using Braun and Clarke's inductive codebook thematic analysis, three themes of power were constructed: physicians as gatekeepers, oppression as intersectional, and antagonistic provider communications, leading to emotional and cognitive impacts. Themes of agency centered on education, active resistance strategies, and collective action. Implications for these findings are discussed in relation to women's health equity. Objectives: The COVID-19 pandemic has led to death and hardship around the world, and increased popular discourse about end-of-life circumstances and conditions. The extent to which this discourse and related pandemic experiences have precipitated advance care planning (ACP) activities was the focus of this study with a particular emphasis on sexual orientation. Methods: A large, national online survey was conducted between 10 August and 10 October 2020 in Canada. The final sample of 3923 persons aged 55 and older was recruited using social media, direct email, and Facebook advertising and in conjunction with community groups. Women comprised almost 78% of the sample; just more than 7% of the sample identified as lesbian, gay, and bisexual (LGB). Measures included demographic variables and a series of questions on ACP, including documents and discussions undertaken prior to the pandemic and since its onset. Results: Descriptive analyses revealed few gender or sexual orientation differences on documents and discussions prior to the pandemic; since its onset, LGB persons have completed or initiated wills, powers of attorney, advance directives, representation agreements, and have engaged in ACP discussions in greater proportion than heterosexuals. Logistic regressions reveal the increased likelihood of pre-pandemic ACP engagement by age, gender (women), and education; since the pandemic onset, gender, education, and sexual orientation were predictive of greater ACP engagement. Care discussions were more likely undertaken by women and LGB persons since the pandemic most often with spouses, family, and friends, especially among LGB persons. Discussion: Gender roles and previous pandemic experiences (HIV/AIDS, in particular) are implicated in this pattern of results; opportunities for educational interventions are considered. The article presents a case-based review of ethical and legal principles behind informed consent in Canada. It suggest respecting patient autonomy is an important tenet to the ethical practice of medicine, however, situations arise where there is a strong medical indication for life-prolonging surgery, but the patient refuses, even after a thorough discussion of benefits and harms of the intervention.

The article offers tips for dentists on preparing for the prospect of becoming disabled. There is a need for dentists to review their insurance coverage annually since the amount of disability insurance that one is eligible to purchase depends on one's income. Options are offered by the Canadian Dentists' Insurance Program's Long Term Disability Insurance plan that can enhance disability insurance protection. Another important consideration is to grant a power of attorney to someone who can be trusted in the event of an incapacitating disability.
