Abstract: Objective: The Chinese Head Trauma Data Bank (CHTDB) has been established, which includes 7145 hospitalised cases with acute head trauma patients in 47 hospitals. Methods: We explored factors that might affect the outcome of acute traumatic brain injury. Results: There was no statistical difference in the mortality rate between male (7.5%) and female (7.2%) patients (P >0.05). The mortality rate in children (<18 years), adults (18–65 years) and elderly (>65 years) was 7.3%, 7.2% and 9.0%, respectively (P >0.05). The mortality rate of patients with mild (2.7%), moderate (5.0%) and severe (21.8%) head trauma was significantly different (P <0.001). The mortality rate of patients with unilateral tentorial herniation, bilateral tentorial herniation and tonsillar herniation was 24.2%, 60.2% and 78.8% respectively (P <0.001). The mortality rate of patients with intracranial pressure (ICP)<20mmHg, 20–40mmHg and >40mmHg was 6.3%, 21.4% and 93.1%, respectively (P <0.001). The mortality rate of patients with no cerebral contusion, single cerebral contusion and multiple cerebral contusions was 3.9%, 7.8% and 14.8%, respectively (P <0.001). The mortality rate of patients with and without traumatic subarachnoid haemorrhage (tSAH) was 9.5% and 5.4%, respectively (P <0.001). The mortality rate of patients with no intracranial haematomas, single intracranial haematoma and multiple intracranial haematomas was 5.8%, 8.4% and 20.6%, respectively (P <0.001). Conclusion: The CHTDB, the first head trauma data bank in China, has one of the largest numbers of cases of any head trauma data bank in the world. Our major findings on mortality may be helpful to neurosurgeons for predicting the outcome of acute head trauma patients. [Copyright &y& Elsevier]

Earthquake is one of the most devastating natural disasters that threaten human lives. Worldwide more than 3 million deaths have been caused by earthquakes in recent 20 years. To analyze clinical features of head injuries after Sichuan earthquake. From May 12 to June 12, 2008, Departments of Neurosurgery in major Hospitals in Sichuan Province admitted 1368 patients with head injuries caused by the Sichuan earthquake; the epidemiology, mechanism, severity, complications, treatments and outcome of head injury were retrospectively analyzed. Of the 1368 patients, 755 were men and 613 women. Collapsing building was the most important cause of head injury. Most of the patients, 85% had mild to moderate head injury. The type of injury was open scalp injury in 65% of patients. About 47% of the head-injured patients were admitted within 72 h after earthquake. Skeletal bone fracture was the most common associated injury (9%). Only 98 patients received surgery. Glasgow Outcome Scale on discharge or transfer was: 5 in 1121 (82%) patients, 4 in 173 (13%) patients, and 3 or less in 74 (5%) patients. Overall 33 (2%) patients died. The characteristics of Sichuan earthquake-related head injury are quite distinct. Early standardized treatment is important to have better outcomes. [ABSTRACT FROM AUTHOR]

Background: There have yet to be any large-scale studies in China on headaches after traumatic brain injury (TBI). We evaluate the incidence of headache after TBI and investigate risk factors and functional outcome in a large tertiary center with a high caseload., Methods: A total of 543 patients (82% men, 18% women) with a mean age of 48.4 ± 18.6 years presenting with TBI were prospectively enrolled in this study between March 2011 and July 2013. Patient demographics, severity of TBI, incidence and classification of headache, and treatment information were collected during initial hospitalization and at 3, 6, and 12 months follow-up., Results: Of our 543 patients (82% men, 18% women), 62% were injured in motor vehicle collisions and 27% in falls. Most patients (97%) were considered to have mild TBI. Follow-up rates at 3, 6, and 12 months were 91%, 75%, and 61%, respectively. Only 12% of patients reported pre-TBI headaches, whereas 58% of respondents reported headache at 3 months follow-up, 54% at 6 months follow-up, and 49% at 1 year follow-up. No statistically significant correlations between age, sex, or TBI severity and posttraumatic headaches were observed., Conclusions: We present the findings of the first study on headaches after TBI in China. Headaches were found to occur in most patients with TBI and persisted through the first year after injury.
