Background: The number and proportion of the elderly population have been continuously increasing in China, leading to the elevated prevalence of chronic diseases and multimorbidity, which ultimately brings heavy burden to society and families. Meanwhile, the status of multimorbidity tends to be more complex in elderly inpatients than community population. In view of the above concerns, this study was designed to investigate the health status of elderly inpatients by analyzing clinical data in Chinese People's Liberation Army (PLA) General Hospital from 2008 to 2019, including the constitution of common diseases, comorbidities, the status of multimorbidity, in-hospital death and polypharmacy among elderly inpatients, so as to better understand the diseases spectrum and multimorbidity of elderly inpatients and also to provide supporting evidence for targeted management of chronic diseases in the elderly. Methods: A clinical inpatients database was set up by collecting medical records of elderly inpatients from 2008 to 2019 in Chinese PLA General Hospital, focusing on diseases spectrum and characteristics of elderly inpatients. In this study, we collected data of inpatients aged ≥ 65 years old, and further analyzed the constitution of diseases, multimorbidity rates and mortality causes in the past decade. In addition, the prescriptions were also analyzed to investigate the status of polypharmacy in elderly inpatients. Results: A total of 210,169 elderly patients were hospitalized from January 1st, 2008 to December 31st, 2019. The corresponding number of hospitalizations was 290,833. The average age of the study population was 72.67 years old. Of the total population, 73,493 elderly patients were re-admitted within one year, with the re-hospitalization rate of 25.27%. Malignant tumor, hypertension, ischemic heart disease, diabetes mellitus and cerebrovascular disease were the top 5 diseases. Among the study population, the number of patients with two or more long-term health conditions was 267,259, accounting for 91.89%, with an average of 4.68 diseases. In addition, the average number of medications taken by the study population was 5.4, among which, the proportion of patients taking more than 5 types of medications accounted for 55.42%. Conclusions: By analyzing the constitution of diseases and multimorbidity, we found that multimorbidity has turned out to be a prominent problem in elderly inpatients, greatly affecting the process of healthy aging and increasing the burden on families and society. Therefore, multidisciplinary treatment should be strengthened to make reasonable preventive and therapeutic strategies to improve the life quality of the elderly. Meanwhile, more attention should be paid to reasonable medications for elderly patients with multimorbidity to avoid preventable side effects caused by irrational medication therapy. Frequent magmatism was a major event causing the mass extinction across the Permian/Triassic Boundary. In the current study, we determined magmatism characteristics from the Pingdingshan section at the Permian/Triassic Boundary using conodont-based geochemical proxies at a high-resolution scale (~ 50 kyr). Integrated trace elements (Mn, Sr, Rb, and Th) and stable/radioactive isotopes (δ18O, δ13C, and 87Sr/86Sr) revealed that conodonts provided an ideal proxy for chemostratigraphic signatures of ancient seawater. The conodont-based, high-resolution 87Sr/86Sr isotopes from the studied interval (250.50–252.00 Ma) displayed three decreasing cycles upwardly against a long-term increasing background, reflecting three episodes of magmatism. As a contrast, the conodont-based, high-resolution δ18O isotopes from this interval exhibited no episodic pattern, indicating that the δ18O isotopes of conodonts were limitedly influenced by marine magmatism. The high-resolution δ13C values of micrites displayed a pattern consistent with the trend of long-term background, revealing that the δ13C signatures of episodic magmatism were not inherited by micrites. The magmatism associated with the EPME event exerted great influences upon the Chihsian source rocks by introducing massive heat through hydrothermal fluids and causing pervasive oceanic anoxia. Uncertain provenance and tectonic setting of shale has constrained the exploration and exploitation of natural gas in the Wufeng–Longmaxi Formation in the southern Sichuan Basin, China. Therefore, this study analyzed the mineral petrology and geochemistry to assess the effects of sorting, recycling, weathering, and paleoclimate on the deeply buried shale. The findings revealed that the mineral composition is dominated by biogenic quartz, terrigenous clay minerals, and carbonate minerals. Combinations of clay lamina and silty lamina occur in black shale. The geochemical characteristics of these sedimentary rocks remain unaffected by sedimentary sorting and recycling processes. The clastic sediments primarily originate from the felsic igneous rock source that underwent moderate weathering during its initial deposition in the collision environment. Based on the comprehensive analysis of the paleoclimate and paleoprovenance characteristics, Wufeng–Longmaxi Formation characteristics are considered to have undergone six stages, among which the stage of relative enrichment of organic matter corresponds to LM1, LM2–3, and LM4. Following the Hirnantian glacial period, the era has witnessed global warming, marked by glacier melting, a rise in global sea level, and a decrease in terrestrial debris supply, collectively facilitating organic matter enrichment. During the LM5 and LM6–8 periods, the increase in source supply caused by the Leshan–Longnvsi and Qianzhong–Xuefeng uplift inhibited organic matter enrichment. [ABSTRACT FROM AUTHOR]

After more than 10 years of exploration, development, research, and practical efforts, China has opened up new perspectives for the commercial exploitation of marine shale gas. While high shale gas production is a main driver for energy security and economic development in China, there have been few attempts to systemically scientific analysis the challenges, prospect, development strategies, and goals for shale gas. Here, we present a detailed comparison of the differences in shale gas between the Sichuan Basin and North America from multiple dimensions, explain how and to what extent recent advances have been made, discuss the current challenges, and provide strategies to deal with these challenges. We demonstrate that a total of 13 graptolite zones developed in the Wufeng–Longmaxi Formations, achieved by representative cores from 32 coring wells and 7 outcrop profiles, can establish the chronostratigraphic framework in the Sichuan Basin, which leads to the potential impact of high-quality reservoir distribution and shale gas production. Shale gas is still faced with the challenges of complex underground and surface conditions, low single-well EUR, and immature deep development engineering technology. To circumvent these issues, here, we propose several strategies, including sweet-spot optimization, low-cost drilling techniques, and efficient fracturing technologies. Our results strengthen the importance of adopting fundamental theoretical research and practical and feasible development goals to realize more commercial discoveries of shale gas of diverse types and higher growth of shale gas reserves and production. High graphitization is responsible for low-resistivity shale development with poor reservoir quality. This paper provides an explanation of organic matter graphitization and determines the impact of high graphite content on low-resistivity shale reservoir quality at the Wufeng-Longmaxi Formation in the Southern Sichuan Basin. Fine veins are frequently developed at shale samples with Ro > 3.5%, graphitized organic matter > 25%, and resistivity < 5 Ω•m, which are dominated by three mineral assemblages: brunsvigite, barite-hyalophane-barium feldspar-potassium feldspar-anhydrite, and calcite-ankerite. These filling minerals are characterized by an Eu positive anomaly and high Ba, Fe, and Mn contents, suggesting that low-resistivity shale was modified by magmatic-related low-temperature hydrothermal fluid. Temperature measurements of brine inclusions and a semi-empirical geothermometer of chlorite show that low-temperature hydrothermal fluid experienced the chlorite stage (150–180 °C), the low-sulfidation stage (120–150 °C), and the low-temperature calcitization stage. Paleozoic fault systems and late Permian hydrothermal activities associated with the Emeishan mantle plume control the graphitization of low-resistivity shale. The water formation and seawater infiltrated into the deep crust along the Paleozoic basement faults under gravity, developing alkaline hot brine through mantle plume heating and then causing a water-rock reaction with basement rocks. They migrated upward along deep and large Paleozoic faults through convective thermal circulation in the Tiangongtang area, the Shuanglong-Luochang area, and the Xuyong area. Cation exchange and redox reactions occurred during the interaction between high-temperature hydrothermal fluid and cool wall rocks. The migration of alkaline hot brine via the Wufeng-Longmaxi shale introduced a subsequent water-rock reaction, resulting in the development of hydrothermal mineral assemblages that intricately filled fractures. It increased formation temperature and enhanced thermal maturity and graphitization of organic matter at the Wufeng-Longmaxi low-resistivity shale, resulting in a wide distribution of low-resistivity shale at the Changning Block. The sediment provenance influences the formation of the shale gas sweet-spot interval of the Upper Ordovician–Lower Silurian Wufeng–Longmaxi shale from the Yangtze Platform, South China. To identify the provenance, the mineralogy and geochemistry of the shale were investigated. The methods included optical microscopy analysis, X-ray diffraction testing, field-emission scanning electron imaging, and major and trace element analysis. The Wufeng–Longmaxi shale is mainly composed of quartz (avg. 39.94%), calcite (avg. 12.29%), dolomite (avg. 11.75%), and clay minerals (avg. 28.31%). The LM1 interval is the shale gas sweet-spot and has the highest contents of total quartz (avg. 62.1%, among which microcrystalline quartz accounts for 52.8% on average) and total organic carbon (avg. 4.6%). The relatively narrow range of TiO2–Zr variation and the close correlation between Th/Sc and Zr/Sc signify no obvious sorting and recycling of the sediment source rocks. Sedimentary sorting has a limited impact on the geochemical features of the shale. The relatively high value of ICV (index of compositional variability) (1.03–3.86) and the low value of CIA (chemical index of alteration values) (50.62–74.48) indicate immature sediment source rocks, probably undergoing weak to moderate chemical weathering. All samples have patterns of moderately enriched light rare-earth elements and flat heavy rare-earth elements with negative Eu anomalies (Eu/Eu* = 0.35–0.92) in chondrite-normalized diagrams. According to Th/Sc, Zr/Sc, La/Th, Zr/Al2O3, TiO2/Zr, Co/Th, SiO2/Al2O3, K2O/Na2O, and La/Sc, it can be inferred that the major sediment source rocks were acidic igneous rocks derived from the active continental margin and continental island arc. The pore structure characterization and its controlling factors in overmature shales are keys to understand the shale gas accumulation mechanism. Organic matter in source rocks is a mixture of various macerals that have their own specific evolutionary pathways during thermal maturation. Pores within macerals also evolve following their own path. This study focused on petrographic characterization and maceral controls on porosity in overmature marine shales in China and the United States. Shale from Ordos Basin in China was also selected as an example of overmature transitional shale for maceral comparison. Organic petrology techniques were used to identify maceral types and describe morphological features in detail; scanning electron microscopy techniques were then used to document the abundance and development of pores within macerals. Helium measurement, mercury intrusion capillary pressure, and CO2 adsorption were especially applied to quantify the pore structure of Wufeng-Longmaxi shale from Sichuan Basin in China. The vitrinite reflectance equivalent of the studied overmature samples is ~2.4%. The macerals within the studied marine shales are composed mainly of pyrobitumen and zooclasts. At this maturity, pyrobitumen develops abundant gas-related pores, and their volume positively correlates to gas content. Three types of pyrobitumen and its related pore structure are characterized in Wufeng-Longmaxi shales. Zooclasts contribute to total organic carbon (TOC) content but little to porosity. When the TOC content is above 1.51% in Wufeng-Longmaxi samples, the TOC content positively correlates to quartz content. Organic matter strongly controls micropore development. Pores of diameter ~ 0.5 nm provide a significant amount of micropore volume. Clay mineral and quartz contents control micro- and macropore increments in organic-lean shales. MICP results indicate that pores within 3-12 nm and 900-2500 nm account for a major contribution to pore volume obtained. Determining the proportions of pyrobitumen to zooclasts within the total organic matter in pre-Devonian organic-rich marine shales is important in predicting porosity and gas storage capacity in high-maturity shales. The Sichuan Basin is rich in shale oil and gas resources, with favorable geological conditions that the other shale reservoirs in China cannot match. Thus, the basin is an ideal option for fully "exploring petroleum inside source kitchen" with respect to onshore shale oil and gas in China. This paper analyzes the characteristics of shale oil and gas resources in the United States and China, and points out that maturity plays an important role in controlling shale oil and gas composition. US shale oil and gas exhibit high proportions of light hydrocarbon and wet gas, whereas Chinese marine and transitional shale gas is mainly dry gas and continental shale oil is generally heavy. A comprehensive geological study of shale oil and gas in the Sichuan Basin reveals findings with respect to the following three aspects. First, there are multiple sets of organic-rich shale reservoirs of three types in the basin, such as the Cambrian Qiongzhusi Formation and Ordovician Wufeng Formation-Silurian Longmaxi Formation marine shale, Permian Longtan Formation transitional shale, Triassic Xujiahe Formation lake-swamp shale, and Jurassic lacustrine shale. Marine shale gas enrichment is mainly controlled by four elements: Deep-water shelf facies, moderate thermal evolution, calcium-rich and silicon-rich rock association, and closed roof/floor. Second, the "sweet section" is generally characterized by high total organic carbon, high gas content, large porosity, high brittle minerals content, high formation pressure, and the presence of lamellation/bedding and natural microfractures. Moreover, the "sweet area" is generally characterized by very thick organic-rich shale, moderate thermal evolution, good preservation conditions, and shallow burial depth, which are exemplified by the shale oil and gas in the Wufeng-Longmaxi Formation, Longtan Formation, and Daanzhai Member of the Ziliujing Formation. Third, the marine, transitional, and continental shale oil and gas resources in the Sichuan Basin account for 50%, 25%, and 30% of the respective types of shale oil and gas geological resources in China, with great potential to become the cradle of the shale oil and gas industrial revolution in China. Following the "Conventional Daqing-Oil" (i.e., the Daqing oilfield in the Songliao Basin) and the "Western Daqing-Oil & Gas" (i.e., the Changqing oilfield in the Ordos Basin), the Southwest oil and gas field in the Sichuan Basin is expected to be built into a "Sichuan-Chongqing Daqing-Gas" in China. This paper documents depositional structures and their reservoir characteristics in the Wufeng–Longmaxi shale from outcrops and cores using thin sections, X-ray powder diffraction (XRD) analysis, carbon–sulfur analyzer, helium porosimeter, decay permeameter, and focused ion beam scanning electron microscope (FIB-SEM). In the study area, clayey and silty laminae abound in the shale. Clayey laminae are rich in bedding parallel fractures, microfractures, and organic pore networks. Silty laminae are rich in isolated inorganic pores and limited amounts of bedding non-parallel fractures. Various inter-lamination of clayey and silty laminae form five depositional structure types which are closely related to the ancient hydrodynamics, paleoredox condition, and sedimentation rate and have significant impacts on shale fractures, microfractures, pore types, pore-size distribution, and porosity. For the paper lamination (PL) and grading lamination composed of siltstone and claystone (GL-SC), organic pores account for 71.6% and 61.4% of the total, and dense bedding parallel and non-parallel fractures intersect to form connective networks. In the interlaminated lamination composed of siltstone and claystone (IL), grading lamination composed of claystone (GL-C) and structureless beds (SB), organic pores merely account for 20% to 51.8% of the total and minor isolated bedding parallel fractures occur. Among five depositional structure types, the PL and GL-SC have the highest porosity, permeability, TOC content, siliceous content, organic pore proportions, and ratios of horizontal to vertical permeability, which help them become shale gas exploration and development targets. Abstract: Twelve avian-origin H3N2 influenza strains were isolated from dogs with signs of respiratory disease in northern China during 2009–2010. Phylogenetic analysis showed that eight gene segments of all the isolates had a close relationship with those of avian-origin H3N2 canine influenza viruses (CIVs) from South Korea and southern China. Genetic analysis indicated that these isolates had a PERQTR/G HA cleavage motif, which differed from the PEKQTR/G motif of canine viruses before 2007. Noteworthy, one of our isolates had an additional basic amino acid at position −3 of the HA cleavage site, with a sequence of PERRTR/G which might facilitate the HA cleavage. An insertion of two amino acids at positions 74–75 in the neuraminidase stalk were found in all H3N2 CIVs isolated since 2009. Our findings show the continued evolution of avian-origin H3N2 CIVs and emphasize the necessity of continued surveillance of influenza virus in dogs. [Copyright &y& Elsevier]

Background: Ischemic cardio-cerebrovascular disease is the leading cause of mortality worldwide. However, studies focusing on elderly and very elderly patients are scarce. Hence, our study aimed to characterize and investigate the long-term prognostic implications of ischemic cardio-cerebrovascular diseases in elderly Chinese patients., Methods: This retrospective cohort study included 1026 patients aged ≥ 65 years who were categorized into the mono ischemic cardio-cerebrovascular disease (MICCD) (either coronary artery disease or ischemic stroke/transient ischemic attack) (n = 912) and the comorbidity of ischemic cardio-cerebrovascular disease (CICCD) (diagnosed with both coronary artery disease and ischemic stroke/transient ischemic attack at admission) (n = 114). The primary outcome was all-cause death. The mortality risk was evaluated using the Cox proportional hazards risk model with multiple adjustments by conventional and propensity-score-based approaches., Results: Of the 2494 consecutive elderly patients admitted to the hospital, 1026 (median age 83 years [interquartile range]: 76.5-86.4; 94.4% men) met the inclusion criteria. Patients with CICCD consisted mostly of very elderly (79.2% vs. 66.1%, P < 0.001) individuals with a higher burden of comorbidities. Over a median follow-up of 10.4 years, 398 (38.8%) all-cause deaths were identified. Compared with the MICCD group, the CICCD group exhibited a higher adjusted hazard ratio (HR) (95% confidential interval, CI) of 1.71 (1.32-2.39) for long-term mortality after adjusting for potential confounders. The sensitivity analysis results remained robust. After inverse probability of treatment weighting (IPTW) modeling, the CICCD group displayed an even worse mortality risk (IPTW-adjusted HR: 2.07; 95% CI 1.47-2.90). In addition, anemia (adjusted HR: 1.48; 95% CI 1.16-1.89) and malnutrition (adjusted HR: 1.43; 95% CI 1.15-1.78) are also independent risk factors for all-cause mortality among elderly and very elderly patients., Conclusions: Our results thus suggest that elderly patients with ischemic cardio-cerebrovascular disease and anemia or malnutrition may have higher mortality, which may be predicted upon admission. Objectives: Previous studies have examined the benefits of self-compassion for psychological symptoms in breast cancer patients; however, little is known about the role of self-compassion for patients' fear of cancer recurrence (FCR) as well as the underlying mediating mechanisms. This study aimed to examine the effect of self-compassion on FCR, and whether maladaptive cognitive styles mediate this relationship., Methods: This cross-sectional study included 304 females with breast cancer. A self-report questionnaire was used to assess patients' self-compassion, maladaptive cognitive styles (i.e., rumination and catastrophising), and FCR. Parallel mediation analyses were conducted to examine the research questions., Results: Approximately half of the patients with breast cancer reported elevated levels of FCR. Self-compassion was negatively related to FCR, and the relationship between self-compassion and FCR was mediated by catastrophising, whereas rumination did not significantly mediate the relationship between self-compassion and FCR., Conclusions: Our findings suggest that self-compassion and catastrophising are closely associated with FCR in patients with breast cancer, and catastrophising is a mediator between self-compassion and FCR. Evidence of H1N1/2009 influenza virus infection was identified in two domestic dogs in China in November 2009. Virus isolation and sequence analysis of all eight genes of the two isolates showed that they were related closely to the H1N1/2009 influenza virus circulating in humans, indicating that they were probably acquired from humans. To determine the pathogenicity and transmissibility of H1N1/2009 influenza virus in dogs, experimental infection and transmission were performed. Inoculated dogs were able to shed virus in nasal secretions, but symptoms were very mild. Uninoculated dogs were co-mingled to determine the transmissibility of the isolate, and one of three exposed dogs was shown to develop infection. The present findings indicate that human H1N1/2009 can infect dogs, but is transmitted inefficiently between dogs.
