BACKGROUND: To solve the complications such as screw cut-out, loosening, and insufficient holding force that may occur during internal fixation of osteoporotic fractures, a new cement-reinforced screw combined with PHILOS plate is currently used to treat osteoporotic fracture of the proximal humerus. However, there are few reports on the clinical efficacy of this technique in the treatment of osteoporoticfractures of the proximal humerus in China. OBJECTIVE: To compare the clinical efficacy of a novel cement-reinforced screw combined with locking plate fixation and artificial humeral head replacement in the treatment of osteoporotic fractures of the proximal humerus. METHODS: Twenty-two patients with osteoporotic fractures of the proximal humerus admitted to at Guizhou Provincial People's Hospital from February 2017 to March 2019 were enrolled, including 8 males and 14 females, aged 68-88 years. Ten patients underwent open reduction using new cement-reinforced screws combined with locking plate internal fixation (internal fixation group), and 12 patients underwent humeral head arthroplasty (humeral head arthroplasty group). The operation time, intraoperative blood loss, and intraoperative and postoperative complications were compared. The Visual Analogue Scale and Constant scores of the shoulder joint were detected at 6 months after surgery. All patients received a postoperative anti-osteoporosis treatment. The study was approved by the Ethics Committee of Guizhou Provincial People's Hospital, approval No. 2017(02). RESULTS AND CONCLUSION: (1) Twenty-two patients were followed up for &-15 months, an average of (9.0±1.6) months. (2) No toxicity reaction of bone cement or embolism occurred in both groups. One case of postoperative shoulder stiffness occurred in the internal fixation group. There were no complications in both groups, such as incision infection, heterotopic ossification, delayed healing, and screw cut-out. (3) The operation time and intraoperative blood loss in the internal fixation group were significantly less than those in the humeral head arthroplasty group (P < 0.05). (4) The Visual Analogue Scale and the excellent and good rate of Constant scores of the shoulder joint at 6 months after surgery showed no significant differences between two groups (P > 0.05). (5) In summary, the novel cement-renforced screw technique combined with locking plate internal fixation has similar clinical efficacy with humeral head arthroplasty. BACKGROUND: Simple posterolateral tibial plateau collapse fracture is very rare in the clinic. The displaced collapse fracture in this area must be anatomically reduced, filled with suitable materials, and fixed with internal fixators. OBJECTIVE: To evaluate the stability, clinical results and biocompatibility of modified posterolateral approach to treat simple posterolateral tibial plateau collapse fracture wtth beta-trlcalclum phosphate combined with T-shaped locking plate. METHODS: Fifteen patients with simple posterolateral tibial plateau collapse fractures who received surgical treatment between June 2013 and December 2017 in Zhangjlagang Hospital Affiliated to Soochow University. These patients Included 8 males and 7 females, aged 25-53 years. All of them received surgical treatment with beta-trlcalcium phosphate combined with T-shaped locking plate through the modified posterolateral approach. After surgery, they were followed up for 12 months. X-ray examination was performed to evaluate fracture healing and internal fixation. Reduction effect was evaluated by Rasmussen radiology score. Knee function and stability were evaluated by HSS knee function score, Lachman test, pivot-shift test and lateral stress test. This study was approved by Medical Ethics Committee, Zhangjiagang Hospital Affiliated to Soochow University, China (approval No. 201305KS001). RESULTS AND CONCLUSION: (1) All patients underwent anatomical reduction of knee fractures, and the fracture healing time was 9-14 weeks. At 12 months after surgery, β-tricalcium phosphate was completely absorbed and replaced by new bone, and repair of bone defect was satisfactory. (2) There were no significant differences in posterior tibia angle, varus angle, Rasmussen imaging collapse score and Rasmussen imaging total score between 12 months after surgery and immediately after surgery in 15 patients (P > 0.05). (3) At 12 months after surgery, the HSS score of the knee joint was 89-100, and Lachman test, pivot-shift test and lateral stress test results were negative In 15 patients. (4) During the follow up period, infection around implants, allergic reaction, immune reaction, or rejection reaction was not observed. (5) These findings suggest that treatment of simple posterolateral tibial plateau collapse fracture through the modified posterolateral approach with β-tricalcium phosphate combined with T-shaped locking plate exhibits good repair effects and biocompatibility. Background: To analyze the risk factors of revision operation after the treatment of distal femoral fracture with lateral locking plate (LLP)., Methods: Retrospective analysis of the clinical data of 152 cases with distal femoral fracture treated in our hospital from March 2005 to March 2019. The SPSS 26.0 software (univariate analysis and logistic regression analysis) was used to analyze the general condition, fracture-related factors, operation-related factors, and construct characteristics of internal fixation., Results: Sixteen of 152 patients who were included in the study underwent revision surgery, with a revision rate 10.5%. Univariate analysis showed that there were significant differences in age, body mass index (BMI), fracture type, supracondylar involved or not, type of incision, quality of reduction, ratio of length of plate/fracture area (R1), the ratio of the length of the plate/fracture area above the condylar (R2), ratio of distance between proximal part of fracture and screw/working length of proximal plate (R3) between the two groups (P < 0.05). Logistic regression analysis showed that age [OR for age > 61.5 group is 4.900 (1.071-22.414)], fracture type [OR for A3 fracture is 8.572 (1.606-45.750), the OR for periprosthetic fracture after TKA is 9.073 (1.220-67.506)], poor reduction quality [OR is 7.663 (1.821-32.253)], and the ratio of the length of the plate/fracture area above the condylar were the possible risk factors (P < 0.05)., Conclusion: Age, fracture type (A3 and periprosthetic fracture after TKA), poor reduction quality, and the ratio of the length of the plate/fracture area above the condylar were the possible risk factors of the revision in distal femoral fractures treated with lateral locking plate. The appropriate application of the locking plate and operation strategy are the key to reduce the revision rate in distal femoral fractures.
