Introduction: Latin America and the Caribbean have reduced the maternal mortality rates. However, some of these countries still have high rates, which indicates the inequity in the access to the health services. The human resource training competent in primary health care is one of the main strategies to approach this problem. Objective: To describe the experience of planning, implementing and evaluating a training program in maternal health in countries of this region (Bolivia, Colombia, Ecuador, Guatemala, Honduras, Nicaragua, Haiti and Peru). Methodology: Between 2016 and 2019, the Department of Health Systems and Services and the Human Resource Unit of the Pan-American Health Organization, together with some collaborative centers, universities and the Latin-American Center for Perinatology and Women's Health, carried out a survey on training needs, developed a formation plan and implemented the training among both professional and non-professional staff in relation to maternal health. Results: The training needs identified were Health Leadership, Prenatal Care, Labor, Postpartum Care, Obstetric Emergencies, and Family Planning. These needs are addressed both in the online and in-person training with 71 trainers from seven countries, who reproduced them for more than 4000 people. The evaluations were very positive and the health impact in future Project is yet to be determined. Conclusions: It is crucial to identify the needs and to do collaborative work in order to carry out the pertinent training in our countries. Introduction: Breastfeeding promotion is one of the most effective strategies to prevent child malnutrition; it reduces costs to families, health services and society. In Colombia, exclusive breastfeeding is practiced only by 10% of women. Objective: To identify factors associated with the duration of exclusive breastfeeding. Methods: A cohort of 438 primiparous women was followed during 6 months by means of 8 home interviews, in order to determine the duration of exclusive breastfeeding. Individual, family and health service factors were studied; and survival analysis was carried out. Results: At 8 days, only a few more than half of the participants maintained exclusive breastfeeding; at month 6 of follow-up, this proportion was reduced to 1.4%. The duration of exclusive breastfeeding was determined by: initiation of breastfeeding in the first 4 hours after delivery (HR= 4.07, 95% CI: 0.96-16.67), selfperceived sureness for breastfeeding (HR= 1.28, 95% CI: 1.04 -1.58), positive opinion of the baby's father regarding breastfeeding (HR= 1.26, 95% CI: 1.01-1.57), and newborn weight (HR= 1.23, 95% CI: 1.00-1.53). Conclusion: There are factors before delivery and in the immediate puerperium that determine, partially, the duration of exclusive breastfeeding. [ABSTRACT FROM AUTHOR]

Objective: To characterise the population of women who died as a result of suicide during pregnancy, delivery and the postpartum period in the Department of Antioquia between 2004 and 2014, and to estimate the proportion of deaths due to suicide. Materials and methods: Descriptive study of suicide deaths during pregnancy, delivery and the postpartum period in women living in the Department of Antioquia, in central Colombia, identified between 2004 and 2014. All the cases identified from the National Public Health Surveillance (SIVIGILA) reporting system and death certificates were included. Data were taken from clinical records and charts. Clinical and sociodemographic characteristics were assessed. Risk factors taken into consideration included a personal or family history of mental disorder, the use of psychoactive substances, attempted suicide, domestic violence and intimate partner violence. Categorical variables were grouped according to frequency in terms percentages; medians and inter-quartile ranges were calculated for continuous variables. The proportion of suicide deaths was calculated over the total number of maternal deaths which occurred in the Department during the study period, and the mortality ratio was calculated specifically by age groups taking into consideration the number of deaths identified in the research by age group and the total number of live births reported by DANE in women of that age during the 11-year period, expressed as number of maternal deaths/1000 live births. Results: Between 2004 and 2014 there were a total of 476 maternal deaths and 24 cases of suicide death, for a proportion of 4.8%. Data were available for descriptions in 18 cases. The highest number of suicide deaths occurred in the age group between 20 and 29 years, single women, housewives or students, living in rural areas, affiliated to the subsidised healthcare regime and with less than complete secondary education and one-half of the patients had attended prenatal care visits. Deaths were due to poisoning and 30% were suicide by hanging. There was a similar distribution between the deaths that occurred in the first two trimesters of pregnancy and the late postpartum period. Here were no data in the personal history of risk factors for suicide in 44.4% of cases, or in the family history in 72.2% of cases. Conclusions: Pregnancy-related suicide deaths are an important cause of potentially avoidable maternal mortality. Identification of vulnerable patients is required in order to offer active treatment and reinforce follow-up.
