Simple Summary: Soft-tissue sarcomas (STSs) are rare cancers that are difficult to diagnose and treat due to their variability. In 2009, Denmark introduced the Cancer Patient Pathways for sarcomas (CPPs) to improve the survival of sarcoma patients by accelerating the diagnosis and treatment processes. This study examined whether the CPPs improved survival in patients with high-grade STSs. Our findings show that survival has improved since the CPPs were introduced and treatment delays have been reduced. This research highlights the importance of streamlined cancer care in improving patient outcomes. Background: Soft-tissue sarcomas (STSs) are rare and challenging to diagnose due to their heterogeneous presentation. In 2009, Denmark introduced the Cancer Patient Pathways for sarcomas (CPPs) to improve sarcoma treatment by streamlining diagnostic and therapeutic processes. The primary objective of this study is to evaluate the impact of the CPPs on the overall survival of patients with deep-seated, high-grade STSs, comparing outcomes from before and after CPP implementation. Methods: A retrospective cohort study was conducted using data from 712 patients diagnosed with high-grade STSs in the extremities or trunk wall between 2000 and 2018. Patients were grouped into pre-CPP (2000–2008) and post-CPP (2010–2018) cohorts. Overall survival was analyzed using Kaplan–Meier estimates. Results: The five-year overall survival improved from 43% in the pre-CPP cohort to 52% post-CPP (p = 0.05). Time-to-treatment was significantly reduced in the post-CPP cohort, with a median decrease of 3 days (18 vs. 15 days, p < 0.001). We found only a very weak tendency toward larger tumor sizes in the pre-CPP cohort and no difference regarding the percentage of patients that had distant metastases at diagnosis between cohorts. In the post-CPP cohort, the percentage of whoops operations decreased and the use of oncological services increased. Conclusions: After the introduction of the CPPs for the sarcoma patients, overall survival improved and time to treatment was reduced. Simple Summary: Angiosarcoma is a rare type of soft tissue sarcoma with a progressive and unpredictable disease course. Currently, surgery is the only well-defined treatment option. Adjuvant oncological treatment remains inconclusive. In effort to provide evidence for improved management and follow-up, the purpose of this study was to assess treatment outcomes and identify prognostic factors. We identified a nation-wide cohort of 154 patients with complete follow-up. Adjuvant oncological treatment did not benefit overall survival, risk of metastasis, or recurrence. Cutaneous tumors, surgery, and negative margin showed improved overall survival, yet without reducing the risk of recurrence and metastasis, resulting in a grave overall survival rate. Angiosarcoma is a rare aggressive and understudied soft tissue sarcoma with pending evidence-based treatment guidelines due to varying study cohorts and inconsistent outcome measures. Surgery with wide resection is currently considered to be the cornerstone in management. In a population-based cohort identified from Danish National Health Registers between 2000 and 2017, this study aimed to define prognostic factors in patients with newly diagnosed soft tissue angiosarcoma. Kaplan–Meier survival analysis demonstrated 5-year overall survival of 28%. Competing risk analysis demonstrated cumulative incidence of local recurrence of 30% and metastasis of 43%. Multivariable Cox models among 154 included patients demonstrated age above 60 years and metastasis to be independently associated with worse overall survival. Cutaneous tumors, surgery, and negative resection margin were independently associated with improved overall survival. Adjuvant oncological treatment did not improve overall survival, risk of metastasis, or recurrence. Negative margin was not associated with lower risk of recurrence and metastasis. Background: Transcriptome analysis enables classification of breast tumors into molecular subtypes. BRCA1/2 predisposed patients are more likely to suffer from a basal-like subtype and this group of patients displays a more distinct phenotype and genotype. Hence, in-depth characterization of this separate entity is needed. Material and methods: Molecular subtyping was performed on a consecutive and unselected series of 1560 tumors from patients with primary breast cancer. Tumors were classified by the 256 gene expression signature (CIT) and associated with basic clinical characteristics and aggregated expression levels in the hallmark gene sets. Results: Of the 1560 samples, 168 were classified basal-like and 120 patients were screened for BRCA1/2 mutations, resulting in 19 BRCA1/2 carriers, 95 non-carriers and six patients carried variants of unknown significance. The BRCA1/2 carriers were significantly younger and there were no carriers above 60 years of age. The tumors showed a loss in DNA-repair profile, as well as an upregulation in proliferative cancer signaling pathways. A robust molecular signature for identification of the BRCA1/2 - carriers was infeasible in the current cohort. Patients with a basal like breast cancer had the lowest median age and the largest median tumor size. They were almost exclusively diagnosed in disease stage III. Conclusions: Basal-like subtype is indeed a separate entity compared with other molecular breast cancer subtypes and the clinical course for this patient group should reflect the aggressiveness of this cancer. Taken together, patients being diagnosed with a basal-like breast cancer are in the youngest segment of breast cancer patients and are mainly diagnosed in stage III disease. [ABSTRACT FROM AUTHOR]

Introduction. Estrogen receptor (ER) is a prognostic and predictive biomarker, which has been known for 40 years. The detection method has developed over the years from different biochemical assays (BCA) to immunohistochemistry (IHC) on paraffin embedded tissue. The aim of the present study is to describe the development in ER analysis in the Danish Breast Cancer cooperative Group (DBCG), in the period of 1977 to 2006, regarding quantity and method of analyses. To compare BCA with IHC, and to report the prognosis for low-risk breast cancer patients. Patients and methods. In the period of 1991-1993, BCA and IHC were both performed on 2 364 tumours from breast cancer patients in Denmark. Three central laboratories in Copenhagen, Aarhus and Aalborg, respectively, performed BCA, while IHC was done in each of the pathology departments participating in the study. Data on ER status, clinical variables and prognostic factors were obtained from the DBCG database. Prognosis is calculated from the DBCG protocol 89a, regarding recurrence free survival (RFS) and overall survival (OS). Results. We find an increasing frequency of ER positive tumours over time, with correlation to patient age. There is a better RFS and OS for tumours positive in both ER determinations. However, BCA is more sensitive than IHC. We find a significant correlation between positive ER status and other low risk factors, except lymph node status. Discussion. Immunohistochemistry has several advantages compared with BCA; it is decentralised, only requiring small amounts of tumour tissue, with direct light microscopic interpretation of invasive tumour cells. It is less expensive and more rapid than BCA. Results in this study show the same RFS in both ER determinations. We conclude that IHC in analysing ER is a rapid, reliable and easy method, and we recommend the use of external quality control programme. [ABSTRACT FROM AUTHOR]

Invasive lobular carcinoma (ILC) is the second most reported type of breast cancer in the Danish Breast Cancer Cooperative Group (DBCG). Several histological subtypes exist, with reports of different prognosis. The aim was to present the incidence of ILC in DBCG from 1977-2004, and evaluate tumours regarding diagnosis, histological subtype and grade, and relate to prognosis. Eight hundred and sixty tumours from patients with a diagnosis of ILC or ILC/non-ILC, who underwent breast cancer surgery in the period of 1990-1998, were evaluated. The impact of histological malignancy grade on disease-free survival and overall survival was analysed using a multivariate analysis adjusting for tumour size, hormone receptor status, axillary lymph node status and patient age. The incidence of pure ILC has risen from 5 to 12%, the ILC/non-ILC is constant at 2% of all reported breast cancers in DBCG. Most of the tumours were classical ILC grade II. The majority of the grade III tumours were among the non-classical subtypes, showing a statistically significant worse disease-free and overall survival compared to grade II, regardless of type. The prognosis was the same for grade I and grade II tumours. The number of positive axillary lymph nodes and hormone receptor negative tumours increased among grade III tumours. We conclude that histological malignancy grade has an independent significant impact on the prognosis of ILC, and it should be taken into consideration when planning the postoperative treatment in this group of patients.
