Coronavirus disease 2019 (COVID-19) has had a substantial impact on the incidence of cardiac arrest and survival. The challenge is to find the correct balance between the risk to the rescuer when undertaking cardiopulmonary resuscitation (CPR) on a person with possible COVID-19 and the risk to that person if CPR is delayed. These guidelines focus specifically on patients with suspected or confirmed COVID-19. The guidelines include the delivery of basic and advanced life support in adults and children and recommendations for delivering training during the pandemic. Where uncertainty exists treatment should be informed by a dynamic risk assessment which may consider current COVID-19 prevalence, the person's presentation (e.g. history of COVID-19 contact, COVID-19 symptoms), likelihood that treatment will be effective, availability of personal protective equipment (PPE) and personal risks for those providing treatment. These guidelines will be subject to evolving knowledge and experience of COVID-19. Background: Plant poisonings are uncommon in adults, and rarely fatal. Oenantha crocata is a toxic plant growing in the humid grasslands of North America and Western Europe., Case Report: We report here two adults who ingested Oenantha crocata roots, having mistaken them for edible tuberous radish. One developed vomiting and needed only 24 h symptomatic treatment, whereas the other suffered prolonged convulsions, cardiac arrest, and severe metabolic acidosis, leading to a fatal outcome. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: Cases of poisoning with Oenantha crocata are rare and destabilizing. Sudden cardiac death is an unresolved problem which causes significant mortality and morbidity in both the community and in-hospital setting. Cardiac arrest is often caused by ventricular tachyarrhythmias which may be mostly interrupted by cardioversion or defibrillation. The single most critical factor for survival is the response time. Over the last 30 years, there have been virtually no procedural changes in the way hospitals address in-hospital resuscitation. A unique device has been developed that eliminates human intervention and assures defibrillation therapy is administered in seconds. This is accomplished with a fully automatic, external bedside monitor defibrillator designed to be prophylactically attached to hospitalized patients at risk of ventricular tachyarrhythmia. The safety and efficacy of the device has been demonstrated in multicenter US and European trials. Thus, this device allows a new scenario which may increase survival and enables meaningful redistribution of health resources.

Introduction: This is the first study to assess T wave alternans (TWA) analyzed from routine ambulatory electrocardiograms (AECGs) to identify postmyocardial infarction (post-MI) patients at increased risk for arrhythmic events., Methods and Results: The new method of modified moving average (MMA) analysis was used to measure TWA magnitude in 24-hour AECGs from ATRAMI, a prospective study of 1,284 post-MI patients. Using a nested case-control approach, we defined cases as patients who experienced cardiac arrest due to documented ventricular fibrillation or arrhythmic death during the follow-up period of 21 +/- 8 months. We analyzed 15 cases and 29 controls matched for sex, age, site of MI, left ventricular ejection fraction, thrombolysis, and beta-blockade therapy. TWA was reported as the maximum 15-second value at three predetermined times associated with cardiovascular stress: maximum heart rate, 8:00 A.M., and maximum ST segment deviation. TWA increased significantly from baseline in both leads at each time point (P <<0.01) in cases and controls. TWA in V5 increased more in cases than controls during peak heart rate (P = 0.005) and at 8:00 A.M. (P = 0.02). A 4- to 7-fold higher odds of life-threatening arrhythmias was predicted by TWA level above the 75th percentile during maximum heart rate in leads V1 (odds ratio [OR] 4.2, 95% confidence interval [CI]: 1.1-16.3, P = 0.04) and V5 (OR 7.9, 95% CI: 1.9-33.1, P = 0.005). TWA at 8:00 A.M. also predicted risk in leads V1 (OR = 5.0, 95% CI: 1.2-20.5, P = 0.02) and V5 (OR = 4.2, 95% CI: 1.1-16.3, P = 0.04)., Conclusion: TWA measurement from routine 24-hour AECGs is a promising approach for risk stratification for cardiac arrest and arrhythmic death in relatively low-risk post-MI patients.

Background: The primary aim of U-CARE (Unexplained Cardiac Arrest Registry of Europe) is to collect clinical information on survivors about a documented episode of idiopathic ventricular fibrillation (IVF) and to follow these patients (pts) prospectively to acquire information on 1) recurrence of malignant arrhythmias or cardiac arrest, 2) development of a previously non obvious organic heart disease, 3) potential difference in outcome in pts treated with different drugs or devices., Methods and Results: Within April 15th, 1994, eighty-six pts have been enrolled, 65 males and 21 females. The mean age at the time of the first cardiac arrest was 35 +/- 15 years. Clinical evaluation revealed "minor" functional or anatomical abnormalities in 14 subjects and they were excluded from the analysis. In the remaining 72 pts, no abnormalities were found at echocardiogram, Holter, angiography, exercise stress test. At the electrophysiologic study 35/68 pts were inducible. Thirty-eight pts received pharmacologic therapy, 28 an implantable defibrillator (ICD), three pts received both an ICD and drug therapy and three were left untreated. Follow-up data are available for 37 pts with a mean follow-up of 4.4 +/- 2.6 years. No patient had evidence of structural heart disease. Twenty-three pts remained asymptomatic, 12 (32%) had a recurrence of syncope or cardiac arrest: three died suddenly and 2 were defibrillated by the ICD. This study that represents the largest experience in IVF, shows: 1) all patients remained free from any organic heart disease at follow-up, 2) they have a high risk of recurrence of major arrhythmic events., Conclusions: An ICD implant would be appropriate in this population, at least until data on the efficacy of the pharmacologic therapy will be available.

This paper reviews experience over 11 years at Waikato Hospital of an aggressive approach to ruptured abdominal aortic aneurysms. The total hospital mortality was 51 percent. Of those reaching operation 34 percent died in hospital. This was reduced to 28 percent over the last five years of the survey. Features relating to the presentation, operative and postoperative management, and subsequent progress of patients are discussed.
