Aims: To describe pediatric ophthalmology surgery activity in private or public practice in metropolitan France in 2016 and to anticipate training needs in surgical pediatric ophthalmology., Methods: We used the French National Health Care System database to identify all pediatric ophthalmology surgical procedures performed in 2016. The study included all children aged ≤14 years who had undergone ophthalmologic surgery. We calculated the incidence of surgeries per 100,000 inhabitants aged ≤14 years, the number of surgeons, the mean age of the practitioners, and the number of surgeons aged >55 years., Results: In 2016, the overall incidence of pediatric ophthalmic surgery activity was 150.42/100,000 children aged ≤14 years. Out of 17,657 pediatric surgeries, 31.3 % were done by private surgeons performing at least 20 surgeries per year, 45.9 % in public centers with ≥20 surgeries per year, and 22.8 % by surgeons (public or private centers) doing <20 surgeries per year. This analysis included 204 surgeons, with a mean age of 41.7 ± 11.2 years (60.8 % female) in the public sector and 51.0 ± 9.5 years (37.3 % female) in private practice. More than a third of the surgeries (37.7 %) were performed in children aged 6-11 years, and the most frequent procedure was strabismus surgery (42.1 %). University regions with the fewest surgeons and regions with surgeons aged >55 years were identified. Within 5 years, to maintain pediatric ophthalmic surgical activity, it will be necessary to train 46 surgeons., Conclusions: This study demonstrated disparities in the geographic distribution of pediatric ophthalmic surgeries in France and identified regions that need increased training capacities and/or incorporate new surgeons to ensure a sufficient activity., Competing Interests: Declaration of competing interest None., (Copyright © 2024 The Authors. Objective: Assessment of suicidal risk is one of the most challenging tasks faced by health professionals, notably in emergency care. We compared telephone suicide risk assessment at prehospital Emergency Medical Services Dispatch Center (EMS-DC), with subsequent face-to-face evaluation at Psychiatric Emergency Service (PES), using French national Risk-Urgency-Danger standards (RUD)., Method: Data were collected for all suicidal adult patients ( N  = 80) who were addressed by EMS-DC to PES between December 2018 and August 2019 and benefited from RUD assessment at both services. Suicidal risk was given a score of 1, 2, 3 or 4, in order of severity., Results: Mean of the differences between the RUD score at EMS-DC and PES was -0.825 (SD = 1.19), and was found to be significant ( p  < 0.01). The average time between RUD assessments was 420 min (SD = 448) and was negatively correlated with the difference in the RUD score ( r  = -0.295, p  = 0.008). Associated suicide attempt increased the odds of a decrease in the RUD score (OR = 2.989; 95% CI = 1.141-8.069; p  < 0.05)., Conclusions: Telephone evaluation of suicidal risk using RUD at EMS-DC yielded moderately higher scores than those obtained by a subsequent face-to face evaluation at PES, with this difference partially explained by the time between assessments, and by clinical and contextual factors.

Purpose: To describe the glaucoma surgery offer in France in 2016., Methods: We used the French National Health Care System database to identify all medical procedures carried out in 2016. The study investigated the entire population aged 30 years and older that had undergone glaucoma surgery, alone or combined with another surgery. We calculated the incidence of surgeries per 100 000 inhabitants 30 years of age and older performed by ophthalmologists carrying out at least 50 procedures annually, the number of surgeons doing these surgeries, the mean age of these practitioners, and the number of surgeons older than 55 years., Results: In 2016, 16 854 glaucoma surgeries were performed in patients aged 30 years and older, for an incidence of 40.8 per 100 000 inhabitants aged 30 years and older. The most frequent procedure performed was trabeculectomy followed by non-penetrating deep sclerectomy (16.7 and 11.7, respectively, per 100 000 inhabitants 30 years of age and older). Private practice glaucoma surgery accounted for 47% of the activity of surgeons performing at least 50 surgeries per year and 60% of the total surgical activity. Of the private practice ophthalmologists performing at least 50 glaucoma surgery procedures per year, 58.5% were over 55 years of age, and 23.5% of public hospital ophthalmologists were over 55 years of age., Conclusions: This study demonstrates that surgeons performing glaucoma surgeries are often older. The main objectives of the reform of the 3rd cycle of medical studies in France that was instituted in 2017 after eight years of preparation, are to train future specialists in a consistent and equitable fashion and to replace the previous time-based qualification by training based on the progressive acquisition of skills. This reform was an opportunity for the 13 different French surgical specialty Colleges involved to share reflections on what a surgeon actually was and to define training in surgical sub-specialties. The current reform is well adapted to these specifications and has fostered training models that are consistent with each other. This article discusses the historical construction of this reform, what will change in the training of future surgeons, as well as some points that warrant caution. The third cycle reform has also triggered a reform of the second cycle, which is expected to come into force for the 2020 academic year. Its objective will be to eliminate the guillotine effect created by the National Classifying Examinations and to allow students to better understand and test their desire and skills for a given specialty. It will be up to these same surgical Colleges to determine how to do this for the sub-specialties of the "surgery" discipline., (Copyright © 2020. Published by Elsevier Masson SAS.)

Coagulase-negative staphylococci (CNS) cause the majority of post-cataract endophthalmitis, which can lead to anatomical and/or functional loss of the eye. This study reports the antibiotic susceptibilities of CNS isolates associated with acute post-cataract endophthalmitis cases and correlates antibiotic resistance with severity and outcome of infection in these patients. Clinical data (initial ocular examination, final prognosis, antibiotic treatment) and the antibiotic susceptibilities of the isolated CNS strains were obtained from 68 patients with post-surgical endophthalmitis recruited during a 7-year period by the FRench Institutional ENDophthalmitis Study (FRIENDS) group. The CNS strains displayed 100% susceptibility to vancomycin, 70% to fluoroquinolones, 83% to fosfomycin, 46% to imipenem and 18% to piperacillin. The most effective antibiotic combinations were fosfomycin plus a fluoroquinolone and imipenem plus a fluoroquinolone, which were considered adequate in 80% and 58% of patients, respectively. Methicillin resistance was significantly associated with older age (p 0.001), diabetes mellitus (p 0.004), absence of fundus visibility (p 0.06), and poor visual prognosis (p 0.03). Resistance to fluoroquinolones was significantly associated with absence of fundus visibility (p 0.05) and diabetes mellitus (p 0.02). This large prospective study demonstrates that methicillin resistance and, to a lesser extent, fluoroquinolone resistance in CNS strains causing postoperative endophthalmitis are both prevalent in France and associated with a poorer visual prognosis. These results emphasize the need for an effective surveillance of this antibiotic resistance and the development of new diagnostic tools for rapid detection for early optimization of antibiotic therapy in endophthalmitis patients., (Copyright © 2015 European Society of Clinical Microbiology and Infectious Diseases. Objective: Several reports suggest that glaucoma may be linked to obstructive sleep apnea (OSA). Herein, we investigated this hypothesis in the largest reported sample to date., Methods: Data were from the French multicenter prospective cohort study including OSA-suspected patients from private practice, general and teaching hospitals. Demographics, history, comorbidities and sleep studies from patients aged >50 years were analyzed. Univariate and multivariate logistic regression were used to predict the odds ratio of prevalent glaucoma depending on sleep apnea status and other potential anthropometric, metabolic, cardiovascular and respiratory confounders., Results: A total of 9580 patients aged >50 years were included. Among these patients, 6754 had sleep apnea and 330 had glaucoma. Glaucoma prevalence was 3.55% in patients with OSA and 3.14% in patients without OSA. OSA diagnosis did not significantly influence the risk of glaucoma in univariate analysis (odds ratio [OR], 1.13; 95% confidence interval [CI], 0.87-1.47). The variables significantly influencing the odds of glaucoma with multivariate regression were age >61.4 years (OR, 1.55; 95% CI, 1.23-1.95), body mass index <30 kg/m2 (OR, 1.58; 95% CI, 1.26-1.99), female gender (OR, 1.40; 95% CI, 1.11-1.78), arterial hypertension (OR, 1.32; 95% CI, 1.05-1.67), high triglyceride levels (OR, 2.03; 95% CI, 1.43-2.88) and thyroid dysfunction (OR, 1.52; 95% CI, 1.09-2.11)., Conclusions: When confounders are taken into account, patients with OSA do not have higher odds of glaucoma compared with patients who do not have OSA in a large multicenter prospective cohort., (Copyright © 2014 Elsevier B.V. Acute postoperative endophthalmitis caused by Staphylococcus lugdunensis is infrequently reported in clinical studies. Five cases of acute postcataract surgery endophthalmitis caused by S. lugdunensis were taken from a multicenter prospective study conducted in four university-affiliated hospitals in France (2004 to 2005). These cases were characterized by severe ocular inflammation occurring with a mean delay of 7.6 days after cataract surgery, severe visual loss (hand motions or less in three cases), and dense infiltration of the vitreous. Each of these patients was initially treated by using a standard protocol with intravitreal (vancomycin and ceftazidime), systemic, and topical antibiotics. Given the severity of the endophthalmitis, even though bacteria were sensitive to intravitreal antibiotics, pars plana vitrectomy was needed in four cases. The final visual prognosis was complicated by severe retinal detachment in three cases. The microbiological diagnosis was reached by using conventional cultures with specific biochemical tests and eubacterial PCR amplification followed by direct sequencing.

Background: The cornea donation process often runs into problems of obtaining family consent. A face-to-face interview is often not possible for logistical reasons. We carried out a prospective study on the effectiveness of telephone contact in obtaining donation consent., Material and Methods: Consent was obtained by a single, non medical, hospital coordinator. He contacted families selected on good staff-family relations during the patient's stay. If a face-to-face interview was not possible, a telephone interview was conducted using a standardized procedure., Results: Over 21 months, 334 families were contacted, either in a face-to-face interview (142, 42.5%) or by telephone (192, 57.5%). Donation consent was obtained in 66.5% of cases, 106 times by telephone (47.7%) and 116 times in the face-to-face interview (52.3%). The acceptance rate was 55.2% by telephone and 81.6% face to face (p<0.001)., Conclusions: The telephone interview was an effective method for obtaining consent for cornea donation. Although the acceptance rate using this method is lower than the face-to-face interview, using the telephone should not be overlooked as this enabled procurement of nearly half the corneas in our hospital.
