Aims: To investigate whether exposure to antipsychotic medications during pregnancy is associated with gestational diabetes mellitus (GDM) in United Kingdom (UK) and Hong Kong (HK) population cohorts.Methods: Two population-based cohort studies were conducted using data from the UK The Health Improvement Network (THIN) and HK Clinical Data Analysis and Reporting System (CDARS). Nondiabetic women who received any type of antipsychotic medicine before their first pregnancy were included in our cohorts. The exposed group comprised women who continued using antipsychotics from the start of pregnancy to delivery (continuers), while the comparison group included women who were prescribed antipsychotics before the start of pregnancy but stopped during pregnancy (discontinuers). GDM was identified using GDM diagnosis and/or clinicians reported GDM. Odds ratios (ORs) with a 95% confidence interval (CI) were calculated to assess the association between antipsychotic use during pregnancy and GDM. Propensity Score fine-stratification weighting was used to adjust for potential confounding factors.Results: 3114 women with registered first pregnancies (2351 in THIN and 763 in CDARS) were included. 5.49% (2.55% in THIN and 14.55% in CDARS) were diagnosed with GDM. The adjusted OR of GDM in continuers was 0.73 (95% CI: 0.43-1.25) in THIN and 1.16 (95% CI: 0.78-1.73) in CDARS compared with discontinuers.Conclusions: Our results do not suggest an increased risk of GDM in women who continued using antipsychotics during pregnancy compared to women who stopped. Based on these results, women should not stop their regular antipsychotics prescriptions in pregnancy due to the fear of GDM. [ABSTRACT FROM AUTHOR]

Previous studies tend to report the effect of rapid urbanization on temperature, but not on humidity. In this study, we aimed to address this research gap by examining diurnal profiles of temperature and humidity in Hong Kong. Our analysis revealed a more significant rising trend in humidity. In addition, our analysis of equivalent temperature revealed that latent temperature (i.e., moisture) contributes more to the aggravation of urban heat stress than does sensible temperature. The average urban air moisture concentration exceeds that in rural areas for both day and night, confirming the existence of an urban moisture island. A new lumped moisture model is developed to predict the hourly urban air humidity using weather data from the operational suburban weather stations, to better understand and quantify the urban moisture environment. The resulting coupled thermal‐moisture model can capture the main characteristics of diurnal profiles of urban air temperature and humidity in Hong Kong. The modelling results demonstrate that weakened city ventilation tends to lead to an increase in urban air moisture. Furthermore, natural surfaces (vegetation and bare soil) reduce urban heat stress by evapotranspiration. It is noteworthy that although the increase in fractions of natural surfaces can lead to more potential moisture sources, the absolute humidity will start to decrease when the fraction of natural surfaces reaches a threshold. This decreasing moisture trend may be attributable to reduced evaporation owing to the decreased surface temperature, reduced anthropogenic moisture and increased ventilation. Besides, the large wall areas in the compact high‐rise areas of Hong Kong function as both sinks and storage sites, contributing to a noticeable diurnal trend in the urban air humidity profile. In brief, the significant urban moisture island phenomenon in Hong Kong may result from enhanced evapotranspiration, restrained moisture sinks, intensive human activities and a weakened wind environment. Aim: To compare the prevalence and trends of antipsychotic drug use during pregnancy between countries across four continents.Methods: Individually linked health data in Denmark (2000-2012), Finland (2005-2014), Iceland (2004-2017), Norway (2005-2015), Sweden (2006-2015), Germany (2006-2015), Australia (New South Wales, 2004-2012), Hong Kong (2001-2015), UK (2006-2016), and the US (Medicaid, 2000-2013, and IBM MarketScan, 2012-2015) were used. Using a uniformed approach, we estimated the prevalence of antipsychotic use as the proportion of pregnancies where a woman filled at least one antipsychotic prescription within three months before pregnancy until birth. For the Nordic countries, data were meta-analyzed to investigate maternal characteristics associated with the use of antipsychotics.Results: We included 8,394,343 pregnancies. Typical antipsychotic use was highest in the UK (4.4%) whereas atypical antipsychotic use was highest in the US Medicaid (1.5%). Atypical antipsychotic use increased over time in most populations, reaching 2% in Australia (2012) and US Medicaid (2013). In most countries, prochlorperazine was the most commonly used typical antipsychotic and quetiapine the most commonly used atypical antipsychotic. Use of antipsychotics decreased across the trimesters of pregnancy in all populations except Finland. Antipsychotic use was elevated among smokers and those with parity ≥4 in the Nordic countries.Conclusion: Antipsychotic use during pregnancy varied considerably between populations, partly explained by varying use of the typical antipsychotic prochlorperazine, which is often used for nausea and vomiting in early pregnancy. Stack aerosols are generated within vertical building drainage stacks during the discharge of wastewater containing feces and exhaled mucus from toilets and washbasins. Fifteen stack aerosol-related outbreaks of coronavirus disease 2019 (COVID-19) in high-rise buildings have been observed in Hong Kong and Guangzhou. Currently, we investigated two such outbreaks of COVID-19 in Hong Kong, identified the probable role of chimney effect-induced airflow in a building drainage system in the spread of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). We injected tracer gas (SF6) into the drainage stacks via the water closet of the index case and monitored tracer gas concentrations in the bathrooms and along the facades of infected and non-infected flats and in roof vents. The air temperature, humidity, and pressure in vertical stacks were also monitored. The measured tracer gas distribution agreed with the observed distribution of the infected cases. Phylogenetic analysis of the SARS-CoV-2 genome sequences demonstrated clonal spread from a point source in cases along the same vertical column. The stack air pressure and temperature distributions suggested that stack aerosols can spread to indoors through pipe leaks which provide direct evidence for the long-range aerosol transmission of SARS-CoV-2 through drainage pipes via the chimney effect. The urban heat island (UHI) and urban moisture island (UMI) effect can be significant in Hong Kong due to its high-density land utilization, and this can strongly affect building energy performance. While the UHI' energy impact has been rather intensively studied recently, the UMI effect on latent energy is still underexplored, especially for humid subtropical climate like Hong Kong. This study investigated the intensity of UHI and UMI in Hong Kong, and its impacts on the sensible and latent cooling demand of residential buildings in summer. Firstly, a ten-year weather dataset from 2004–2013 for the six stations selected based on the local climate zone (LCZ) scheme was analysed. The results show that the urban area of Hong Kong appears as both a heat and moisture island during summer nights but as cooling and dry islands during daytime, and the nocturnal UHI and UMI intensity vary significantly with different LCZs. Furthermore, the energy performance of a typical residential building in Hong Kong was simulated with measured weather data for the selected stations as an input. The urban building shows a higher sensible cooling demand, approximately twice that of the comparative rural one, and the latent cooling demand could be up to 96% higher. Both sensible and latent cooling energy demand decrease with increasing LCZ grades. Our study highlights that both UHI and UMI effect should be considered in the estimation of building energy in Hong Kong due to their significant impacts on the cooling energy demand. • The UHI and UMI effect among various LCZs in Hong Kong are compared. • The nocturnal UHII and UMII during summer both decreases with the increasing LCZ grades. • The impact of UMI effect on latent cooling demand is investigated in Hong Kong. • The combination of UHI and UMI effect leads to an increase in electricity demand for urban residential buildings.
