A diagnosis of autism typically depends on clinical assessments by highly trained professionals. This high resource demand poses a challenge in low-resource settings. Digital assessment of neurodevelopmental symptoms by non-specialists provides a potential avenue to address this challenge. This cross-sectional case-control field study establishes proof of principle for such a digital assessment. We developed and tested an app, START, that can be administered by non-specialists to assess autism phenotypic domains (social, sensory, motor) through child performance and parent reports. N = 131 children (2–7 years old; 48 autistic, 43 intellectually disabled and 40 non-autistic typically developing) from low-resource settings in Delhi-NCR, India were assessed using START in home settings by non-specialist health workers. The two groups of children with neurodevelopmental disorders manifested lower social preference, greater sensory interest and lower fine-motor accuracy compared to their typically developing counterparts. Parent report further distinguished autistic from non-autistic children. Machine-learning analysis combining all START-derived measures demonstrated 78% classification accuracy for the three groups. Qualitative analysis of the interviews with health workers and families of the participants demonstrated high acceptability and feasibility of the app. These results provide feasibility, acceptability and proof of principle for START, and demonstrate the potential of a scalable, mobile tool for assessing neurodevelopmental conditions in low-resource settings. Autism is diagnosed by highly trained professionals– but most autistic people live in parts of the world that harbour few or no such autism specialists and little autism awareness. So many autistic people go undiagnosed, misdiagnosed, and misunderstood. We designed an app (START) to identify autism and related conditions in such places, in an attempt to address this global gap in access to specialists. START uses computerised games and activities for children and a questionnaire for parents to measure social, sensory, and motor skills. To check whether START can flag undiagnosed children likely to have neurodevelopmental conditions, we tested START with children whose diagnoses already were known: Non-specialist health workers with just a high-school education took START to family homes in poor neighbourhoods of Delhi, India to work with 131 two-to-seven-year-olds. Differences between typically and atypically developing children were highlighted in all three types of skills that START assesses: children with neurodevelopmental conditions preferred looking at geometric patterns rather than social scenes, were fascinated by predictable, repetitive sensory stimuli, and had more trouble with precise hand movements. Parents' responses to surveys further distinguished autistic from non-autistic children. An artificial-intelligence technique combining all these measures demonstrated that START can fairly accurately flag atypically developing children. Health workers and families endorsed START as attractive to most children, understandable to health workers, and adaptable within sometimes chaotic home and family environments. Introduction: Incidence & prevalence of OPMDs & OSCC is increasing day by day, thereby escalating the burden of oral cancer in India. Oral cancer ranks in the top three of all cancers in India and is quickly becoming a health priority. This study aims to assess prevalence and associated factors of OPMDs and OSCC in patients attending dental OPD and its association with age, gender, habit (type & duration), clinical presentation and site of involvement. Material and Methods: A prospectively 12 months study was conducted in the outpatient department. Patient's data whether suspected or proven cases of OPMDs & OSCC and fits in clinical criteria were reviewed and analysed for demographic data, oral adverse habit, clinical presentation and site of involvement. Results: Overall 38,588 patient's data were analysed for 12 month time duration. Out of this 552 (1.43%) cases of OPMDs and 58 (0.15%) cases of OSCC were reported. Out of 552 maximum patients were reported with OSMF (34.4%), followed by other lesions and minimum with LP (7.9%). Age group most commonly affected was above 45 years (44.9%) of age. Males (81.1%) were affected more than females. OPMDs (92%) and OSCC (96.5%) were mostly associated with smokeless or smoking form of tobacco. Conclusion: Present study evaluated the prevalence rates and associated factors of OPMDs & OSCC, which is beneficial for general practitioner in early diagnosis, formulating better treatment plan and to educate general population about risk factors, early signs and symptoms of these lesions. Background: Environmentally sustainable diets are represented in the EAT‐Lancet recommendations developed by the Lancet Commission on Planetary Health in 2019. Very few studies have compared Indian diets with the EAT‐Lancet recommendations. The present study performed such a comparison using primary dietary consumption data from adults in north and south India. Methods: Data from 8762 adults (52.4 ± 11.7 years) residing in Sonipat and Vizag India were collected on sociodemographic characteristics, wealth index (household assets) and dietary intake (food frequency questionnaire: nine food groups). The quantity consumed and energy from each food group was compared with the EAT‐Lancet recommendations. We investigated the likelihood of deficit or excess in consumption compared to the EAT‐Lancet recommendations by different sociodemographic factors. Results: Half of the participants were women and half resided in rural areas. Vegetables and fruits were consumed in lower quantity, whereas dairy and added fats were consumed in higher quantity than recommended by the EAT‐Lancet recommendations. For whole grains, female gender and poorest wealth index were the factors associated with deficit or no consumption, whereas, for vegetables and fruits, it was poorest wealth index and residence in rural areas (p < 0.05, all). Rural residence and poorest wealth index were associated with excess consumption of dairy and added fats (p < 0.05, all). Conclusions: The diets of the study participants were mainly plant‐based, and high in dairy but lacking in nutrient‐rich foods such as vegetables and fruits. Appropriate policy actions for making healthy sustainable diets and micronutrient‐rich foods available and affordable to all with a particular focus on the poor and rural populations are warranted. Purpose: The objective of this study was to find the association between fetal hemoglobin (HbF) concentration and retinopathy of prematurity (ROP) in preterm infants. Methods: In this observational, prospective, longitudinal study, a total of 410 preterm infants with <36 gestational weeks and <2.5 kg birth weight, who were attending ROP clinic in a tertiary care hospital of central India for 1 year duration were included. Dilated fundus examination was done as per ROP screening guidelines, and ROP was staged as per international classification for retinopathy of prematurity (ICROP) classification, 2021. HbF (%) was measured with high-performance liquid chromatography, and data was analyzed statistically. The relationship between HbF (%) and ROP was evaluated. Those infants who had ROP were further divided into treatment-requiring and non-treatment-requiring groups and HbF was compared in these groups at the first visit and after 1-month follow-up period. The outcome of ROP was studied with HbF levels. Results: A total of 410 preterm infants were included, out of which 110 infants had ROP (26.8%). Infants with ROP had significantly lower percentage of HbF with gestational age groups and birth weight groups, compared to infants without ROP. Higher percentage of HbF was associated with a lower prevalence of ROP. Higher concentration of HbF was found in the ROP infants who regressed spontaneously without treatment and less concentration was found in those who progressed to a severe disease and those who required treatment. The predictive ability of HbF (%) was 0.976 for ROP. Conclusion: Low fraction of HbF was found to be significantly associated with the development and progression of ROP. [ABSTRACT FROM AUTHOR]

Background: Appropriate management of ophthalmoplegia demands good anatomy knowledge of cranial nerves involved i.e., III, IV and VI. Where these nerves originate and where they end, clinical features associated with paralysis of each nerve all this knowledge help in proper management of cases. Objective: To study clinical and etiological factors associated with the ophthalmoplegia. Methods: Prospective, Hospital based observational study was carried out among 50 cases of ophthalmoplegia. Detailed history was recorded. Thorough clinical examination including examination of both the eyes was carried out in terms of laterality, pupils affected or not, Central nervous system involvement, cranial nerves III, IV and VI etc. Papilledema, blood pressure, fasting blood sugar were investigated for each case. Results: Most common age group affected was >50 years in 50%. Males were slightly more affected than females. Majority (72%) cases were bilateral. Only nine cases had affected pupils. In ten cases, central nervous system was affected. Hypertension was seen in 32% and diabetes in 44%. 14% had history of trauma. Inflammation, tumor and vascular affection was seen in three cases each. Four cases had viral infection. Among all factors, diabetes was significantly associated with III cranial nerve involvement (p<0.05). Hypertension was significantly associated with IV cranial nerve involvement (p<0.05). But no factor was significantly associated with VI cranial nerve involvement (p>0.05). Conclusion: We conclude that ophthalmoplegia mainly affects elderly and males. It is usually bilateral and sometimes pupils and CNS are affected. Hypertensives and diabetics are at an increased risk of ophthalmoplegia. [ABSTRACT FROM AUTHOR]

Background: Nasolabial cysts are non-odontogenic cysts that arise from the nasal alar area. Sublabial resection of the nasolabial cyst is considered an effective surgical modality of treatment. Objective: This study aimed to evaluate the clinical presentation, diagnosis and treatment of nasolabial cyst at a tertiary care teaching hospital in eastern India. Methods: This was a retrospective study of 38 cases of nasolabial cysts managed between May 2018 and June 2022. The detailed clinical presentations, investigations and treatment of nasolabial cysts of the patients were studied. Results: Out of 38 cases of the nasolabial cyst, 22 (57.9%) were seen on the left side, 13 (34.2%) were seen on the right side and 3 were seen bilaterally. The most common presenting complaint was facial swelling (84.2%). Computed tomography of the nose and paranasal sinuses was done in 24/38 (70.5%) patients studied. Surgical excision of the nasolabial cysts was done by intraoral sublabial approach in 32 (84.2%) cases and 6 underwent endoscopic transnasal marsupialisation. The most common post-operative complication was post-operative facial/perinasal swelling. One case of recurrence was reported who underwent endoscopic transnasal marsupialisation. Conclusions: Nasolabial cysts are an uncommon clinical entity but manifest cosmetic deformity and nasal obstruction. Intraoral sublabial excision and endoscopic transnasal marsupialisation are important surgical techniques for the nasolabial cyst. The post-operative complications and chance of recurrence are rare. Introduction: Bile is deemed a sterile fluid, with the presence of clinical conditions like cholelithiasis, cholecystitis, previous biliary interventions, biliary strictures, and so on, leading to bile stasis, and increases the chances of bacteriobilia. In this study, we recognize the bacterial spectrum of microorganisms isolated from bile samples, diagnostic parameters, and antibiotic sensitivity patterns. Methods: A retrospective observational study was carried out by compiling data from the hospital information system of a tertiary care center from 2021 to 2022 to evaluate biliary infections in patients who underwent surgical procedures related to the biliary tract and associated organs. Results: A total of 234 patients' bile samples were included in our study. The mean age of patients was 48.04 ± 14.74 years, with more patients below the age of 65 years among those with infected bile samples. One hundred and sixty-three (163/234, 69.66%) patients infected by 209 pathogenic microorganisms were recognized. The most common microorganism isolated was Escherichia coli (83/209, 39.71%), followed by Pseudomonas aeruginosa (37/209, 17.7%). Acinetobacter baumannii and Klebsiella pneumoniae isolate owed to more than 90% penicillin, extended-spectrum beta-lactamase, carbapenem, and fluoroquinolone resistance among all isolates. Length of hospital stay, malignant obstruction, and chronic kidney disease were identified as statistically significant risk factors that lead to the isolation of multi-drug-resistant isolates from bile culture. Conclusion: We recognized the spectrum of pathogens causing biliary tract infections at our center along with the antibiotic resistance pattern to guide and facilitate prompt and appropriate treatment by primary health care professionals and family medicine practitioners. Recurrent aphthous stomatitis (RAS) is characterized by painful, oral mucosal ulcers with wide range of prevalence ranging from 2 to 78%. Etiology of RAS is idiopathic and multifactorial. There are numerous gaps in assessment and management of RAS and the absence of guidelines or a consensus document makes the treatment further difficult. The aim of this document is to provide an Indian expert consensus for management of RAS. Experts from different specialties such as Otorhinolaryngology, Oral Medicine/Dentistry and Internal Medicine from India were invited for face to face and online meetings. After a deliberate discussion of current literature, evidence and clinical practice during advisory meetings, experts developed a consensus for management of RAS. We identify that the prevalence of RAS may lie between 2 and 5%. In defining RAS, we advocate three or more recurrences of aphthous ulcers per year as criterion for RAS. Investigation should include basic hematological (complete blood count) and nutritional (serum vitamin B12, and iron studies) parameters. Primary aim of treatment is to reduce the pain, accelerate ulcer healing, reduce the recurrences and improve the quality of life. In treating RAS, initial choice of medications is determined by pain intensity, number and size of ulcers and previous number of recurrences. Topical and systemic agents can be used in combination for effective relief. In conclusion, this consensus will help physicians and may harmonize effective diagnosis and treatment of RAS. Background and Objectives: Blumea lacera (Burm.f.) DC. (Family—Asteraceae) is widely used for treating hemorrhoids by several ethnomedicinal practitioners and tribes in India. Thus, the main objective of the present investigation was to evaluate the potential of B. lacera leaf extract in the treatment of hemorrhoids using a croton oil-induced hemorrhoid rat model. Materials and Methods: Ethanol extract of B. lacera (EBL) leaves was prepared using Soxhlet extraction, optimized using Box–Behnken design (BBD), and quantified using high-pressure liquid chromatography (HPLC). Furthermore, the vasoactive ions were estimated using ion-exchange chromatography. Hemorrhoids were induced in the recto-anal portion of experimental rats, followed by treatment with EBL (100, 200, and 400 mg/kg; par oral (p.o.)) and Pilex granules as a standard anti-hemorrhoid drug for 7 days. The anti-hemorrhoid potential was evaluated on the eighth day by assessing the severity of hemorrhoids, biochemical parameters, and histology of recto-anal tissue. Results: Upon treatment with EBL and Pilex, there was a significant (p < 0.05) reduction in the inflammatory severity index, concentration of Evans blue dye, recto-anal coefficient (RAC), elevated cytokines level, and restoration of altered antioxidant status. Furthermore, the histopathological results revealed a marked reduction in the inflammatory zones along with minimally dilated blood vessels. Conclusion: The present study confirmed the traditional claims of the plant B. lacera in the treatment of hemorrhoids, which may be attributed to its anti-inflammatory and antioxidant potential in EBL, where quercetin could be considered the main contributor. Isoprene, emitted mainly by tree species, is a highly reactive and second most abundant biogenic volatile organic carbon (VOC) in the atmosphere. It produces tropospheric ozone and other secondary organic pollutants. We hypothesize that the soil of tropical dry forests, one of India's major forest types, may serve as a significant sink for isoprene. To address the issue, we enriched the under-canopy soils of isoprene-emitting tree species (Tectona sp., Butea sp., Madhuca sp.) with 10, 40, 70 and 100 ppm isoprene concentrations for a week. Isoprene oxidation (pg g−1 h−1 dws) varied significantly with the season showing maximum oxidation in the rainy season (120.59 to 956.22) followed by winter (106.58 to 842.5) and summer (102.17 to 805.56). However, the effect of tree species on isoprene oxidation was not significant. The qPCR-based bacterial population (16S rRNA gene copy number; copies 109 g−1dws) analysis showed higher value during rainy season (13.75–18.90) followed by winter (2.67–6.19) and summer (2.10–4.51). Isoprene oxidation showed strong dependence on bacterial population (r = 0.93; p < 0.05). Higher values of kinetic constant K during rainy season also supported higher oxidation of isoprene in this season. Overall, the findings suggest that tropical dry forest soil favors growth and activities of bacterial populations during warm and wet periods of the year which consume significant amounts of isoprene. Access to inexpensive, safe, consistent, and clean energy is a critical necessity for all to achieve the SDGs. India's renewable energy (RE) currently accounts for more than a third of the 482 GW of installed capacity and more than 40 percent of power production (including large-scale hydropower). Reforms such as the establishment of a single national power grid have improved access to electricity for people, and the ambitious development of renewable energy, which is the world's third-largest energy generator and third-largest electricity user, has helped in achieving these aims. As a result, the expansion of national targets signifies and reflects the country's optimism and goal for the forthcoming generation. Standardization of the guidelines and development of the stable grid and transmission networks will only enable the country to achieve the ambitious target of 500 GW of green and clean energy by 2030. This paper highlights the important development in the power sector regarding the energy security of India. As well as specifically examining the initiative of NSMs for achieving the 2030 targets, the key challenges, and the way forward to increase the cumulative installed capacity, comprehensive studies of various policies and government initiatives are also discussed. Furthermore, the key challenges usually faced by the developers in the industry, along with the steep decline and rise in the tariffs of solar projects and the previous trends in capacity installation, are also pointed out. This research work also highlights the potential key challenges to achieving the targets, and will thus provide a focus for power developers, policy makers, researchers, and industry practitioners and help with their planning. Background: It has been proposed that, thrombocytopenia is the most common haematological abnormality in pregnancy after anaemia. The incidence of severe immune thrombocytopenia (ITP) in pregnancy has been difficult to report because of the rarity of the disease. Aims and objectives: Objectives were to determine the prevalence, pregnancy outcomes, treatment modalities of ITP mothers in a tertiary health care hospital in West Bengal, India. Methods: Our study was a retrospective record study. Records of the in-patient medical record department (MRD) folders of patients with ITP who delivered at Dept of G&O, N.R.S. Medical College and Hospital, Kolkata, West Bengal, India from November 2018 to November 2022. Statistical data were analysed by using SPSS V.20 software. Results: Most common age group of the mother found to 19-25 years 10 (43.5%). Gravida 1 was 11 (47.8%), followed by gravida 2 was 8 (34.83%). IUFD was 1 (4.3%), preterm was 7 (30.4%), SA was 2 (8.7%), term was 12 (52.2%) and neonatal death was 2 (8.7%). Associated Haematological Disorder of B Thal Trait and HbE Carrier was 3 (10.3%) each. Platelet Count during AN Period as <49000 was 13 (56.5%) and 50000-99,999 was 10 (43.5%). Myasthenia gravis during pregnancy was 7 (30.4%), Methylprednisolone was 3 (13.0%), thyroid disorder was among 5 (21.7%). Conclusion: Chronic ITP in pregnancy poses more risks to mother and foetus as seen with the higher chance of PPH etc. Mothers with ITP should be screened antenatally as the chances of anomalies are high in foetus. Background and Objectives: Chronic rhinosinusitis (CRS), a prevalent ENT condition, often presents treatment-resistant cases. Gastroesophageal Reflux Disease (GERD) causing Laryngopharyngeal reflux (LPR) is implicated in its etiology. This study aims to assess LPR prevalence in refractory CRS and the impact of combining proton pump inhibitors (PPIs) with functional endoscopic sinus surgery (FESS). Methods: 50 refractory CRS patients were evaluated, split into study (PPIs + FESS) and control (FESS only) groups in the Department of ENT at a tertiary care medical college in India. CRS indicators were assessed, and post-surgery, study patients received intranasal steroids and PPIs. A 3-month follow-up tracked symptom relief. Result: Majority of refractory CRS patients had LPR, mainly affecting the posterior larynx. Nasal obstruction, discharge, and polyps were common CRS indicators. Study group with PPIs showed greater improvement in symptoms, notably by the third postoperative month. Conclusion: The study highlights a significant link between refractory CRS and GERD. PPIs, when used alongside FESS, offer effective refractory CRS management, emphasizing combined treatment's advantages. [ABSTRACT FROM AUTHOR]

Chryseobacterium spp., is non motile, non fermenter, Gram Negative Bacillus (GNB) showing enzymatic activity of catalase, oxidase, and indole production. Chryseobacterium spp. are found widely in the soil and aquatic milieu. It is also capable of surviving in chlorinated water, which can be attributed to the spread of infection in hospital settings hosting profusely immunosuppressed individuals. Newer diagnostic modalities like Matrix-Assisted Laser Desorption Ionisation-Time Of Flight-Mass Spectrometry (MALDI-TOF-MS) and Vitek-2 facilitate early identification and treatment can alleviate the infections caused by them. Present series discusses three cases of Chryseobacterium indologenes (C. indologenes) Urinary Tract Infection (UTI) Case 1-was a 61-year-old hypertensive and diabetic male patient diagnosed with renal failure with pyelonephritis. Case 2- was a female patient of 64-year-old with left upper ureteric calculus. Case 3-was a 31-year-old male patient having left mid ureteric calculus. All the patients were catheterised with a urinary catheter and developed UTI by C. indologenes. C. indologenes infection in UTI patients is uncommon but these cases of complicated UTIs demonstrate C. indologenes as a potential cause of UTI in hospitalised patients using invasive equipment like urinary catheters. As the organism was Multidrug-Resistant (MDR), appropriate antibiotic treatment and accurate identification can alleviate infection by this organism. [ABSTRACT FROM AUTHOR]

The present paper focused on investigating the potential of indigenous Pseudomonas fluorescens strain CD5 isolated from manure-enriched soil as a bioremediation agent for chlorpyrifos biodegradation in semi-arid soil of Bathinda district of Punjab, India. The soil had sandy loam texture with slightly alkaline pH and low organic matter content of 0.36%. The growth pattern, alkaline phosphatase enzyme production and chlorpyrifos utilization capability of Pseudomonas fluorescens strain CD5 were screened in mineral salt medium and the results revealed degradation of 78.19% of 500 mg L−1 chlorpyrifos within 15 days. The pattern of chlorpyrifos degradation was further studied in natural conditions of semi-arid soil by the Pseudomonas fluorescens strain CD5 and photolysis by sunlight for a period of 105 days. The biodegradation of chlorpyrifos at 500 mg kg−1 sterile soil by Pseudomonas fluorescens strain CD5 was 59.68% in comparison to 26% of photodegradation and enhancing total degradation. The residual chlorpyrifos in sterile soil after biodegradation by Pseudomonas fluorescens strain CD5 under natural light condition was 14.33%. The alkaline phosphatase enzyme activity in soil was found positively correlated with the chlorpyrifos degradation. Apis mellifera, a priceless bee species is known to produce various nutritional products. Royal Jelly (RJ) is one such bee product which has high nutritive value, functional and biological qualities. However factors such as bee species, environment, season, collection technique and larval age affect the composition of RJ at macro and micro levels thus there is a need to evaluate Indian Royal Jelly (IRJ) for its chemical properties. So in the present work IRJ samples collected from Southern, Central and Northern regions of India were evaluated for physicochemical parameters (nature, color, appearance, odor, and solubility,) residual content (moisture content, ash value and pesticide content), chemical attributes (total polyphenol content, total flavonoid content,), nutraceutical potential (total fat, protein, carbohydrate content, and energy), and antioxidant activity (DPPH assay, ABTS assay and reducing power,). The total flavonoid content and total phenolic content were in the range of 0.119-0.321 mg quercetin/g of IRJ and 25.2844-68.203 mg Gallic acid/g, respectively. High energy, protein and low fat value suggested IRJ as a suitable nutraceutical agent. Antioxidant activity (IC50) of IRJ samples was found to be in the order of IRJ II > IRJ IV > IRJ I > IRJ III. Air pollution is one of the environmental concerns which is a threat to the health of our plants and animals. Little knowledge exists in the literature about its impact on trees and crops. The objective of the present study was to assess the impact of air pollutants on the biochemical parameters of 19 tree and crop species from five different locations in Patna, Bihar, India. Air pollution tolerance index value showed that Ficus religiosa, Zea mays, Carthamus tinctorius and Cajanus cajan were more tolerant compared to the other crops. Anticipated performance index value showed that trees like F. religiosa, Azadirachta indica and Mangifera indica and crops like C. cajan, Z. mays and Triticum aestivum were most suitable under air pollution condition. The de-focalisation of reporting and the focalisation of post-truth ideology is antithetical to the tenets of democracy. It reverts to the traditional centralised propagation of one truth, one reality, and one perspective while contracting, negating, or antagonising the possibility of alternative and parallel narratives. The right wing has created a narrative of suspicion, which is based on the conviction that anything deviating from the traditional cultural patterns is a contamination of the ethos of one's own native values, a deviation from the nativity, and hence an influence that needs to be disposed of. The ongoing Covid pandemic has magnified the narrative of post-truth through the use of sensationalism as an effective modus operandi. The blinding of the truth is rampant as news outlets are being guarded by right-wing gatekeepers and sensationalism is used to reflect the surging emotions of a public already gripped by a precarious reality. This paper examines the three primary and seemingly independent threats to democracy during the pandemic in India: de-focalisation of reporting, coerced homogeneity based on religion and language, and sensationalism. This study demonstrated the influence of downscaling using the regional climate model (RCM) driven by Era-Interim reanalysis (EIN) in simulating different aspects of the Indian summer monsoon (ISM). It is also examined, whether increasing the horizontal resolution of RCM will inevitably be capable of adding more information to ISM characteristics and its spatio-temporal variability. In this regard, two RCM (at 50 km: Reg50 and 25 km: Reg25) simulations were conducted for six years from 2000 to 2005 for the South Asia Coordinated Regional Downscaling Experiment (CORDEX) domain. The added value (AV) is found to be strongly dependent on region and considered metrics. A slight improvement towards increasing spatial resolution is observed in the simulation of the mean ISM characteristics, while considerable improvements are noticed for the frequency distribution of extremes. The notable improvement in the daily climatology of precipitation is observed over the region of northeast India (~ 35%) and the Hilly region (~ 32%) and the lowest improvement over north-central India (~ 8%). The reduction of anomalously strong northeasterly flow over the southeastern Arabian Sea and strengthening of the moisture leaden southeasterly wind flow from the Bay of Bengal in Reg25 compared to Reg50 is consistent with the reduction of dry bias over India in Reg25. The robust improvements are noticed for the heavy precipitation events (probability density function: PDF tails) and mean precipitation due to extreme precipitation events, particularly over the areas characterized by complex topographical features (e.g., the Western Ghats, Indo-Gangetic plains, and northeast India and Hilly regions) as well as over the areas having substantial bias (e.g., central India), indicating its strong sensitivity towards model resolution. The increasing latent heat flux in Reg25 contributes to increasing the moisture and hence rainfall over India. Both simulations apparently simulate many of the ISM characteristics better than the EIN, thereby emphasizing the usefulness of finer resolutions in the better simulation of the Indian monsoon, especially for heavy rainfall. However, the RegCM bias is comparable to or even greater in some places than the EIN bias. This suggests that high-resolution models are important for improving performance; however, it does not necessarily mean that they can have AV for every aspect and all places. Apart from this, the substantial difference in the AV over different regions or aspects highlights the importance of carefully selecting AV matrices for the different areas and characteristics being investigated. RegCM exhibits some systematic biases in precipitation despite substantial improvement due to misrepresentation of dynamical and thermodynamical processes, including northward and eastward propagating convective bands. Context: Knowledge of epidemiology of bacterial isolates and their anti-biograms in hospital settings is necessary for prompt empirical anti-microbial therapy of neonatal sepsis. Aims: To study risk factors, bacteriological profiles, and anti-biograms of blood culture isolates of both early and late onset neonatal sepsis. Settings and Design: It is a prospective observational study conducted from January 2020 till July 2021 at our tertiary care center. Material and Methods: Neonates (0–28 days) admitted to this neonatal intensive care unit clinically suspected with sepsis were subjected to blood cultures, and the isolates were identified both biochemically and by the matrix-assisted laser desorption/ionization time-of-flight mass spectrometry system. Antibiotic susceptibility testing (AST) was performed as per CLSI guidelines. Statistical Analysis: Chi-square test was used. Results: Out of 280 suspected cases of neonatal sepsis, 43 (15.3%) cases showed positive blood culture. Of these, the majority (30, 69.8%) had late-onset neonatal sepsis. Major pre-disposing risk factors were pre-term birth and a low birth weight (26, 60.5%). Gram-negative bacteria and Gram-positive bacteria were isolated in 25 (58.1%) and 18 (41.9%) blood cultures, respectively. Klebsiella pneumoniae (37.5%) was the most predominant pathogen in both early-onset (23.1%) and late-onset (46.7%) sepsis. Coagulase negative Staphylococcus (34.8%) was the second most common organism and was more common in late onset (23.2%) neonatal sepsis. A high level of antibiotic resistance was noted in Klebsiella pneumoniae isolates, even to amikacin (76.5%) and carbapenems (66.7%). In this study, we identified and characterize a plant pathogenic isolate, collected from naturally infested bean (Phaseolus vulgaris var. Anupama) seedlings presenting root rot symptoms from an agricultural land of the Prayagraj district, Uttar Pradesh, India. Based on morphological features, growth characteristics and the ITS sequencing‐based molecular data, the isolate was assumed very closed to the species of the Pythium genera. However, ITS sequencing data and the BLAST results for gene annotation clustered the identified isolate more closely to the Pythium catenulatum (p‐value e‐value), and therefore, confirmed as P. catenulatum. The pathogenicity assay confirmed the role of the isolate as a root rot causing pathogen. Furthermore, the isolate was tested for its growth under in‐vitro conditions against several environmental parameters including temperature, pH, salt, drought and metals. The Biolog FF MicroPlate method assessed the carbon utilization profile and reported the ability of the isolate in utilizing both carbohydrates and amino acids as a primary energy source. Moreover, in‐vitro colony growth inhibition assay performed with different agrochemicals (fungicides and insecticides) and the dyes determined their efficacy in suppressing the growth and development of the isolated pathogen. The dual culture assay of the isolate along with several fungal and bacterial strains confirmed the antagonistic potential of some tested microbes in delimiting the growth of the pathogen. Background: Transurethral resection of the prostate (TURP) is one of the best available treatments for benign hypertrophy of the prostate. Many of the complications in TURP are associated with the absorption of irrigation fluid. In BIPOLAR TURP, normal saline is used as an irrigant which is more physiological. Aims and Objectives: Our aim of the study is to evaluate the correlation between post-operative electrolyte changes to perioperative factors in bipolar TURP. Materials and Methods: One hundred and ten patients enrolled in this study who were admitted to Meenakshi Mission Hospital, Madurai, Tamil Nadu, India. Results: Mean age of the participants was 65.68 (±8.50) years with a median of 66 years. mean hemoglobin drop was 1.21 (±1.36) g/dl. The mean serum sodium drop was 1.55 (±3.01) mEq/L. The mean serum potassium change was 0.06 (±0.74) mEq/L. There was a significant positive association between operative time and serum sodium drop (P<0.001). There was no significant correlation between any of the other parameters prostate size, resection time, resected volume were noted, and post-operative electrolyte April 2019-March 2021. Patient pre-operative changes and hemoglobin drop is observed. Correlation is assessed with Spearman Rho. Conclusion: Our study concluded that as operative time increases sodium drop and hemoglobin drop significantly increases. High-resolution seismic data of the Ramasetu region revealed three subaerial unconformities S1, S2, and S3. S1 is the youngest subaerial unconformity, whereas S3 is the oldest. These subaerial unconformities have been determined based on the presence of channel-incision signatures. Identified buried channels originated in the Palk Strait and debouched in the Gulf of Mannar. Youngest buried channels became most active during MIS 3 (~60-29 ka) and were buried entirely at ~7.0 ka. The study suggests that the Palk Strait evolved as a large multicentered freshwater reservoir during the last glacial period and remained a freshwater reservoir until seawater started encroaching the region at ~8.5 ka. The Ramasetu region was a potential habitat zone for foragers, with fresh and saline water ecosystems available at opposite banks. During the Microlithic and terminal Pleistocene, Homo sapiens needed to cross high-energy rivers using floaters/rafts in wet seasons of MIS 2 and 3. At the same time, they used a direct walkable land link in the extreme dry season of MIS 2. During the early to mid-Holocene, foragers required floaters/rafts or artificial structures (such as bandh/bridge) to cross shallow, low-energy estuarine patches of the Ramasetu. Introduction: Acinetobacter is a non fermenting, Gram negative bacillus, a causative pathogen of hospital-acquired infections due to its inherent Multidrug-Resistant (MDR) property. It is held responsible for the majority of nosocomial meningitis in patients undergoing neurosurgical procedures. Aim: To identify the clinical characteristics, drug-resistance and mortality rate among the patients suffering from meningitis caused by Acinetobacter baumannii. Materials and Methods: This retrospective, single-centre study was carried out in the Bacteriology section of the Department of Microbiology at Sanjay Gandhi Postgraduate Institute of Medical Sciences, Lucknow, Uttar Pradesh, India, from February 2019 to February 2022. A total of 150 Cerebrospinal Fluid (CSF) samples from routine bacterial culture-confirmed patients were included in the study. All clinical data were extracted from the Hospital Information System (HIS). All the isolates were identified by Matrix Assisted Laser Desorption/Ionisation-Time of Flight-Mass Spectrometry (MALDI-TOF-MS) assay and antibiotic sensitivity testing was performed according to Clinical and Laboratory Standards Institute (CLSI) guidelines. Results: The study included 150 (7.55%) cases of culture-proven bacterial meningitis among 1986 CSF samples collected from February 2019 to February 2022. There were 45 (30.0%) cases of Acinetobacter meningitis. Thirty-five (77.8%) patients had undergone neurosurgical procedures for the removal of spaceoccupying lesions from the brain parenchyma. Forty-two (93.3%) isolates were resistant to amikacin and a cumulative resistance of almost 93.3-95.6% was observed among cephalosporins. Fluoroquinolone resistance was observed in 43 (95.6%) patients and carbapenem resistance was observed in 42 (93.33%) isolates. Overall, 12 (26.7%) succumbed to their infections. Conclusion: Acinetobacter meningitis causes delay in the recovery of the patient undergoing intracranial surgery, amounting to a delay in brain parenchyma healing in the case of neurosurgical patients. Introduction: India is a major contributor to both maternal and neonatal death worldwide. Majority of these fatalities can be averted by adequate utilization of Antenatal care (ANC) services. Large scale surveys like National Family Health Survey follow hieratical characters in which subjects within the clusters are often correlated. The ordinary logistic model ignores this correlation and provide compromised estimate of effect size of predictors. Multilevel model that incorporates correlation is the appropriate method. Aim: To demonstrate the adequacy of multilevel logistic model over ordinary logistic model in hieratical data sets with ANC visits >4 and delivery assisted by Skilled Birth Attendant (SBA). Materials and Methods: This retrospective, cross-sectional study was conducted at the Institute of Medical Sciences, Banaras Hindu University, Varanasi, Uttar Pradesh, India, from October 2021 to March 2022. The data of 174607 women, who delivered the children within five years, obtained using cluster sampling, from country wide survey during 20th January 2015 to 4th December 2016 (India's National Family Health Survey-IV) was used. Firstly, the model adequacy of ordinary and multilevel logistic models was evaluated for ANC ≤4 visits and delivery assisted by SBA, the same data set of fourth round National Family Health Survey by considering the outcomes;. Thereafter, predictors of late or no registration of pregnancy were identified using three level logistic model. Results: Because of high Variance Partition Coefficients (VPC) at state and district levels, the multilevel model applied on components ANC ≥4 visits and delivery assisted by SBA suggested better accuracy of the multilevel logistic model (-2Log L=189334 for ANC ≥4 visits and 141148 for delivery assisted by SBA) than the ordinary logistic model (-2Log L=220268 for ANC ≥4 visits and 151978 for delivery assisted by SBA). For the late (registration in third trimester of pregnancy) or no registration of pregnancy, each predictor was found significantly associated in which the most important were women's education, child birth order, caste and wealth quintile. Conclusion: The present study concluded that multilevel logistic model in clustered design data was useful instead of individual level for more precise estimate of effect size of the predictors. Head and neck infection (HNI) can lead to life-threatening complications, including death. The purpose of this study is to look at the entire clinico-demographic profile of patients with HNI as well as the microbiologic profile of recurring bacterial infection cases with a variety of symptoms. A retrospective cross-sectional study was conducted on 1080 HNI patients in a tertiary care hospital in Bhubaneswar, Odisha, India, from January 2018 to December 2022. Of the 1080 cases, 771 (71.39%) were males, 309 (28.61%) were females, and 603 (55.83%) were from rural areas reporting to a tertiary care hospital. 62% of the cases were between the ages of 31 and 60. Neck abscesses account for 570 (52.78%) of all cases, with parotid abscesses accounting for 233 (21.57%), peritonsillar abscesses accounting for 170 (15.74%), otitis media 32 (2.96%), and oral cavity infection accounting for 26 (2.41%). In 854 (79.07%) cases, the etiology was odontogenic, followed by sinus in 188 (17.41%) and otogenic in 38 (3.52%). The most common presenting features were neck swelling in 537 (49.72%) cases and face swelling in 238 (22.04%) cases, followed by jaw pain in 26 (2.41%) cases and others. Patients were hospitalized for an average of 11.82±4.38 days. Treatment and recurrence had a strong significant relationship (p 0.001). Microbiologic investigation of recurrent patients revealed 12 microorganisms, including bacteria and fungus, mainly multidrug-resistant in given ascending order Staphylococcus aureus (26.74%), Klebsiella pneumoniae, Pseudomonas aeruginosa, Acinetobacter baumannii, Escherichia coli, Candida albicans (4.65%), Aspergillus fumigatus, A. flavus, A. niger, C. tropicalis, C. glabrata, C. krusei. Apart from colistin, almost all antibiotics were highly resistant to gramnegative bacteria, whereas against S. aureus, benzylpenicillin, and oxacillin showed 100% resistance, followed by erythromycin (91.3%), levofloxacin (86.96%), and ciprofloxacin (82.61%). This study using a high resolution (~ 25 km) regional coupled earth system model ROM (REMO-OASIS-MPIOM), addresses the future projections of heatwave characteristics (frequency, duration, and severity) and associated dynamics over India for the AMJ (April–May–June) season. The near-surface maximum temperature (Tmax) is used to identify the prominent regions of large fluctuations using rotated empirical orthogonal function analysis. The averaged heatwave characteristics simulated by ROM is compared against the Indian Meteorological Department (IMD) Tmax observational data for the historical period 1980–2005. The ROM simulated heatwave characteristics largely agree with observations though few deviations are also noticed. The typical synoptic features associated with the heatwave days for the identified regions show the presence of elevated geopotential height (thickness; Z) with an anomalous anticyclonic structure which forms an atmospheric blocking over each region except south east coast. The daily evolution of potential vorticity and Z suggests eastward propagation of blocks that remain persistent during heatwave days. ROM is able to simulate these features with systematic higher magnitude and minor location changes. The future projections (RCP 8.5) of heatwave characteristics show a gradual increase for different time-slices during 2020–2099. The projected frequency will be doubled, and the average duration will increase by 8–12 days per season at the end of the century. The severity will also increase by 2°–3 °C. Similarly, future dynamical features will be associated with an increase in geopotential height (thickness) in the future along with a gradual decrease in potential vorticity. This study highlights the land use/land cover change, and its implications on the long-term changes in various hydrometeorological parameters in India during 2001–2018. The study exhibited a decrease in grassland (15.1%), barren land (10.2%), and shrubland (9.5%) in contrast to an increase in deciduous broadleaf forest cover (27.4%) and croplands (25.2%) in India. The study exhibited 31.7% of the geographical area observed a low warming (<0.8°C), in contrast to 52.1% of the area observed a low cooling (> 0.8 °C) in India over the observation period. The transformation of (a) barren land to shrubland and cropland (Western India) and (b) cropland, and grassland to forest cover (Central India) led to the cooling effects. While Indo-Gangetic Plain and North-Eastern parts observed warming effects due to greenness and wetness change. The findings highlight the relevance of bio-geophysical and land-climate feedback, which may help in developing integrated and effective climate mitigation and adaptation strategies. Most developing countries have large and persistent informal sector. Historically, the literature was dominated by strict duality viewpoint, where the formal and informal sectors are treated as two separate entities. However, in recent times, a new approach has developed where the formal and informal sectors are not binary but they often co-exist and feed into each other. Most academic work on agglomeration economies is based on developed countries where almost all production takes place in the formal sector. Apriori, there is no reason to believe that agglomeration economies do not extend to the informal sector. Hence, in this paper, we hypothesize that agglomeration economies can arise from the interaction between formal and informal firms as well. To check the validity of the hypothesis, we study the geographic distribution and Coagglomeration pattern between formal and informal manufacturing firms in India. Using the firm level National Sample Survey for unincorporated enterprises and Annual Survey of Industries data, we calculate the Ellison and Glaeser Coagglomeration index at 2-digit NIC level. Further, using the Marshall's theory of agglomeration, we create four kinds of linkages to capture the nature of interdependence between formal and informal firms, i.e. Buyer, Supplier, Labor and Technology. We find that there is heavy clustering of both formal and informal manufacturing firms in India, however, mostly in different areas, with few exceptions like Western Uttar Pradesh and Southern Tamil Nadu. Out of 22 major industries, we find high coagglomeration only in 7. The average value of coagglomeration index at aggregate level decreases from 0.00536 in 2000-01 to -0.0027 in 2015-16. This indicate towards reduced interdependence over time, however in few industries it remains high. The average value of labor linkage increased from 0.1889 in 2000-01 to 0.3651 in 2015-16, during the same period buyer linkage remained constant around 0.2481 and supplier linkage fell from 0.5755 to 0.2749. The technology linkage remained low throughout in the range of 0.02-0.03. Strong buyer and labor linkage mean subcontracting of inputs is the main motive driving the interaction between formal and informal firms. [ABSTRACT FROM AUTHOR]

In this study, weathering intensity, provenance and tectonics setting of Quaternary sediment deposits of the Ganga river, the Yamuna river and their tributaries in and around Haridwar area, Uttarakhand, India is explicated. The channel and levee sediments are well preserved along these rivers. For the purpose, the concentration of major oxides in the channel and levee sediments of rivers were analysed and chemical indices and discriminant functions were determined to understand the mobility pattern of different chemical constituents under various climatic conditions. An overall view of variation in provenance, shifting of channels and mechanism of river sediments transportation and accumulation in varying energy conditions is emphasised and validated from the field evidences and the present condition. Geomorphologically, the area comprises of bedrock strata of various lithologies, small sediments mounds and high hills, and fault scarps. The accumulated sediments and debris in channel bars and levees, and deviated drainages are the prominent features observed along the catchment of studied rivers. The results reveal the distinct concentration of heavy minerals derived from multiple provinces under low to intermediate weathering intensity, due to frequent climatic changes, migration of river channels, activation of thrusts and neotectonic activities. The integrated outcome of the present study can be used to predict the dynamics of the Ganga and Yamuna rivers and their tributaries which may provide a holistic management of headwater region above the area and its influence in lower parts of river sediment deposits in plains. The aim of this study is to lay a framework for evaluating flood vulnerabilities. Specifically, it applies the proposed methodology to evaluate flood vulnerability index (FVI) across 21 districts comprising a significantly large part (drainage area of 85 859 km2) of Narmada River basin in the central Indian state of Madhya Pradesh. The study assesses sensitivity, adaptive capacity indices, and the exposure risk over a chosen set of 22 climatic, demographic, social and economic parameters, and applies principal component analysis to evolve a composite FVI. The results rank various districts for their vulnerabilities to floods. The analysis indicates that, in the study area, around a third of the vulnerability is accounted by two factors: a lack of adaptive capacity of the community and heightened sensitivity to floods. There exists significant scope to modify these two factors by appropriate policy interventions at the social and economic levels. It is expected that the framework would be of use to stakeholders such as planners, policymakers, flood management authorities, field engineers, district administration and local non-governmental organisations who strive to make the community less vulnerable to floods. Objectives: This study examines the association between quality Postnatal Care (PNC) considering timing and providers' type on neonatal mortality. The aim extends to account for regional disparities in service delivery and mortality including high and non-high focus states. Methods: Ever-married women aged 15–49 years (1,87,702) who had delivered at least one child in five years preceding the survey date surveyed in National Family Health Survey (2015–16) were included in the study. Neonatal deaths between day two and seven and neonatal deaths between day two and twenty-eight were considered dependent variables. Descriptive statistics and multivariate regression analysis were conducted. Results: Chances of early neonatal mortality were 29% (OR = 0.71; 95%CI: 0.59–0.84) among newborns receiving PNC within a day compared to ones devoid of it while 40% (OR: 0.60; 95%CI: 0.51–0.71) likelihood for the same was noted if PNC was delivered within a week. Likelihood of neonatal mortality decreased by 24% (OR: 0.76; 95%CI: 0.65–0.88) when skilled PNC was delivered within 24 hours. Receiving quality PNC by skilled providers within a day in a non-high focus state decreased the chances of neonatal mortality by 26% (OR: 0.74; 95%CI: 0.59–0.92) compared to ones who did not receive any PNC. Conclusions: Neonatal deaths were significantly associated with socioeconomic and contextual characteristics including age, education, household wealth, social group and region. Background and purpose: No previous study has assessed the frequency and clinical–radiological characteristics of patients with diabetes mellitus (DM) and acute onset nonchoreic and nonballistic movements. We conducted a prospective study to investigate the spectrum of acute onset movement disorders in DM. Methods: We recruited all the patients with acute onset movement disorders and hyperglycemia who attended the wards of three hospitals in West Bengal, India from August 2014 to July 2021. Results: Among the 59 patients (mean age = 55.4 ± 14.3 years, 52.5% men) who were included, 41 (69.5%) had choreic or ballistic movements, and 18 (30.5%) had nonchoreic and nonballistic movements. Ballism was the most common movement disorder (n = 18, 30.5%), followed by pure chorea (n = 15, 25.4%), choreoathetosis (n = 8, 13.6%), tremor (n = 5, 8.5%), hemifacial spasm (n = 3, 5.1%), parkinsonism (n = 3, 5.1%), myoclonus (n = 3, 5.1%), dystonia (n = 2, 3.4%), and restless leg syndrome (n = 2, 3.4%). The mean duration of DM was 9.8 ± 11.4 years (89.8% of the patients had type 2 DM). Nonketotic hyperglycemia was frequently (76.3%) detected. The majority (55.9%) had no magnetic resonance imaging (MRI) changes; the remaining showed striatal hyperintensity. Eight patients with MRI changes exhibited discordance with sidedness of movements. Most of the patients (76.3%) recovered completely. Conclusions: This is the largest clinical series depicting the clinical–radiological spectrum of acute onset movement disorders in DM. Of note was that almost one third of patients had nonchoreic and nonballistic movements. Our findings highlight the importance of a capillary blood glucose measurement in patients with acute or subacute onset movement disorders, irrespective of their past glycemic status. Muga silkworm (Antheraea assamensis), one of the economically important wild silkmoths, is unique among saturniid silkmoths. It is confined to the North-eastern part of India. Muga silk has the highest value among the other silks. Unlike other silkmoths, A. assamensis has a low chromosome number (n = 15), and ZZ/ZO sex chromosome system. Here, we report the first high-quality draft genome of A. assamensis, assembled by employing the Illumina and PacBio sequencing platforms. The assembled genome of A. assamensis is 501.18 Mb long, with 2697 scaffolds and an N50 of 683.23 Kb. The genome encompasses 18,385 protein-coding genes, 86.29% of which were functionally annotated. Phylogenetic analysis of A. assamensis revealed its divergence from other Antheraea species approximately 28.7 million years ago. Moreover, an investigation into detoxification-related gene families, CYP450, GST, and ABC-transporter, revealed a significant expansion in A. assamensis as compared to the Bombyx mori. This expansion is comparable to Spodoptera litura , suggesting adaptive responses linked to the polyphagous behavior observed in these insects. This study provides valuable insights into the molecular basis of evolutionary divergence and adaptations in muga silkmoth. Background: The development of barotrauma in COVID-19 patients who were ventilated and admitted to the intensive treatment unit seemed to have been a problematic issue in the COVID era. This study aimed to explore the possibility of developing the barotrauma-related issues with mechanical ventilation in the cases of individuals suffering from COVID-19. Results: Out of 48 patients who developed barotrauma, 30 (62.5%) presented with pneumothorax, 22 (45.8%) with pneumomediastinum, 10 (20.8%) with subcutaneous emphysema, and 2 (4.1%) with pneumopericardium. Of those that developed barotrauma, 45 (93.7%) patients were in acute respiratory distress syndrome. In patients with and without barotrauma, significant factors were white blood cell count (p = 0.001), neutrophil percentage (p = 0.012), and lymphocyte percentage (p = 0.014). There were no statistically significant differences in CRP, procalcitonin, d-dimer test, LDH, or ferritin. Conclusions: Patients infected with COVID-19 have a high risk of barotrauma when on mechanical ventilation. As a result, the death rate in this patient group is higher. [ABSTRACT FROM AUTHOR]

Background: Antrochoanal polyps (ACPs) are benign polypoidal mass originating from the maxillary sinus and they extend into the choana. The etiopathogenesis of ACP is not clear. These are found more commonly in children and young adults. ACP is almost always unilateral and common symptoms include nasal obstruction and nasal discharge. These lesions are extremely rare in the older adult age group. Materials and Methods: This is a retrospective study of 14 cases of ACPs managed between June 2017 and July 2022. Detailed clinical presentations, investigations, and treatment with surgical interventions for ACPs were studied. Results: Out of 14 cases of older adult patients with ACPs, there are 8 (57.14%) cases with a polyp on the left side and 6 (42.85%) cases on the right side of the nasal cavity. The most common symptom was constant and unilateral nasal obstruction in 12 (85.71%) patients, followed by purulent nasal discharge in 7 (50%) cases, clear rhinorrhea in 5 (35.71%) cases, facial pain in 4 (28.57%) cases, snoring in 3 (21.42%) cases, sneezing in 3 (21.42%) cases, and halitosis in 2 (14.28%) cases. Conclusion: ACPs are uncommon in the older adult age group. Diagnostic nasal endoscopy and computed tomography scans are important tools for the diagnosis of ACP. Functional endoscopic sinus surgery and power instrumentation are helpful for the complete removal of ACPs and are considered extremely safe procedures. The way communicable diseases do spread from one person to another, depending upon the specific disease or causative infectious agent. Out of these diseases, some are incurable and the health care workers during their practice or otherwise acquire such infections and transmit them further to innocent patients who are unaware of about the health status of health care workers. The rights of an infected health care worker and patients are protected by many laws but in case of conflict of interests between the individual right of the health care worker and life of a patient, then obviously by the principle of natural justice, saving the life of a person from such incurable infection gets the privilege. Therefore, there is a lot of ethical and professional dilemma, arising out, in such a type of scenario, irrespective of concealed or disclosed health status and the question mark is raised on whether clinical practice may be allowed in such cases. Some of the studies show the actual but very little risk of transmission from infected health care workers to patients. Therefore, in the current scenario, many western countries such as USA and UK are following different guidelines in this regard but the same is lacking in India. So, this article critically analyses the various issues arising out of it and thereby justifies the need to have a uniform infection control policy in this regard apart from legal and ethical binding on infected health care workers. Bioenergy production from marginal and degraded lands is getting worldwide attention due to its potential for renewable energy, land restoration, soil carbon sequestration and also for meeting the United Nations Sustainable Development Goals. However, majority of the literature reported the bioenergy production prospect based on a single bioenergy crop production system. Therefore, the present article utilizes a multi-criteria analysis to identify India's potential marginal and degraded lands (P ml) and their suitability for bioenergy crop production based on a polyculture zonation technique (PZT) with the projections for bioenergy and carbon mitigation potentials. Ten multipurpose bioenergy crops have been studied, out of which seven hardwood tree species were further selected based on ecological and climate suitability. Results suggested that India accounted for 45.13 million ha (Mha) of P ml , and it could produce 96.82 Mt y−1 of biomass and 6.16 Mt y−1 of biofuel, considering half utilization of the plantation areas. Such productions represent 2.22 EJ y−1 (equivalent to 618 TW h y−1) of bioenergy potential. Around 64 Mt C y−1 can also be sequestered by these plantations and additionally help in meeting carbon sequestration over 2800 MtCO 2 by 2030, corresponding to India's nationally determined contribution. According to the spatial validation results via centroid coinciding approach, 14 randomly generated points out of 24 coincided on the centroids of randomly identified land parcels, which were further validated through the ortho-imagery from Google Earth. Our study results can be used for framing suitable strategies for exploring bioenergy production from similar agroecological conditions and also for attaining India's national and international commitments related to land restoration and renewable energy production. Background: The species Marsilea quadrifolia L. belonging to the family Marsileaceae is a semi-aquatic plant widely distributed in the tropical and sub-tropical regions of India. The whole plant of Marsilea quadrifolia L. is extensively used for its eminent medicinal properties. Objectives: Despite promising therapeutic benefits, the plant seems to be less explored, and consequently, the present investigation focuses on exploring pharmacognostical parameters, quality control standardization and antioxidant studies. Materials and Methods: The microscopical (transverse sections of leaves, petiole, stem, and root.) studies, leaf constants (stomatal number, vein islet number, etc.), and powder microscopic examination were performed. All physicochemical evaluations, like swelling index, foaming index, etc. were accomplished as per the World health organization (WHO) guidelines. TLC profiling and HPLC standardization of ethanolic extract was also performed. In vitro, antioxidant activities of ethanolic extract were carried out by various methods. Results: The transverse sections of all parts of the plant, leaf constants, and powder microscopy were reported with clear identification. Physicochemical parameters, i.e., foaming index (<100), Loss on drying (13.99 w/w), moisture content (13.75 w/w), total ash value (13.5 w/w) were done. Ethanolic extract was standardized with reference to quercetin and quantified HPLC data was calculated to be 2.1805±0.02089 mg/g. The antioxidant potential of ethanolic extract and its IC50 values are reported. Conclusion: We have reported complete pharmacognostical standardization of Marsilea quadrifolia L. as per WHO guidelines. During pandemic, eye banks worldwide faced drastic reduction of corneal retrieval. Since precut tissues are uncommon in India, the onus is on corneal surgeons to maximize the donor corneal usage. The stumbling block making the stromal part unusable in the graft preparation of the Descemet membrane endothelial Keratoplasty is the stromal window creation to place the orientation mark on the stromal side of the folded graft. To overcome this, we simply marked directly on the folded Descemet graft, after partly stripping it from the donor cornea. Then, we unfolded, punched, and created two grafts after completely stripping it, retaining the Descemet part for the endothelial Keratoplasty and the stromal part for lamellar Keratoplasty. Initially, we used a special F marker, and later we marked an L using just a spatula. Background: Parinaud syndrome is a dorsal mid-brain syndrome characterized by upgaze paralysis, convergence retraction nystagmus, and pupillary light-near dissociation. Infarctions or hemorrhages involving the mid-brain are the most frequent causes in older adults. Objective: To report a novel case of a patient who presented with classical parkinsonian signs and Parinaud syndrome. Material and Methods: Patient data were obtained from medical records from the Department of General Medicine, Burdwan Medical College and Hospital, Burdwan, West Bengal, India. Results: A 62-year-old previously healthy man presented with motor and non-motor symptoms of Parkinson's disease (PD) for the past 6 years. The neurological examination revealed an asymmetric resting tremor of the upper limbs with rigidity, bradykinesia, hypophonia, hypomimia, decreased blinking, and micrographia. The neuro-ophthalmological examination showed Parinaud syndrome. He was treated with levodopa-carbidopa and trihexyphenidyl. After 6 months and 1 year of follow-up, his neurological condition was re-assessed; motor symptoms improved substantially, but Parinaud syndrome persisted. Conclusions: Parinaud syndrome can be a potential manifestation of PD. A detailed neuro-ophthamological examination should be carried out even in patients having a diagnosis of classic PD in whom eye-movement abnormalities are distinctly infrequent. More than 8% of the global population is affected by a rare disease and >50% affected are children (30% of whom will die before 5 years). There are 6,000-8,000 classified rare diseases - 80% are genetically based - with less than 500 FDA approved treatments (or <5% have therapies) available to treat them. Lack of standard definition and community-based epidemiological data in India limits the estimation of prevalence and/or disease burden; though extrapolations at 6% to 8% show 72 to 96 million affected individuals. The Government of India approved the "National Policy for Rare Diseases, 2021" on 30th March 2021. Aerococcus viridans is a rare Gram positive microorganism identified largely as environmental or skin contaminants. With the advent of an increase in the immunosuppressed population due to diabetes, the use of steroids and the Coronavirus Disease 2019 (COVID-19) pandemic, this bacteria caused a variety of infections like bacteraemia, urinary tract infections, and endocarditis. The use of Matrix-Assisted Laser Desorption Ionisation Time-of-Flight Mass-Spectrometry (MALDI-TOF MS), a unique technique of microorganism identification, has placed Aerococci among human pathogens, capable of causing infection among immunocompromised patients. The present case was of a 48-year-old female presented with dry cough, high-grade fever associated with chills and rigors, and generalised body ache and weakness for the past one week. She was a known case of bronchial asthma. She tested positive for COVID-19 and over the course of hospital stay, her BACTEC blood culture performed due to high fever which flagged positive indicated her as a case of Aerococcus viridans bacteraemia. Climate change has impacted the nature and the severity of extreme temperatures over India in the recent past. In this study, the characterization in nature and change of hot season (March–May) temperatures across India's various geographical regions have been analysed over the last 69 years. Using canonical correlation analysis (CCA), we elucidate the relationships between various meteorological parameters from the preceding Indian winter (December–February) and the following hot season. A combination of detrended and raw data for CCA has revealed the asynchronous winter parameter's effects on hot season temperatures. The raw data explained the evolution of the relationship between the variables in the warming climate. In contrast, detrended data explains the predictive relationship between hot season temperature and other predictive variables which are not influenced by the trend in the data. The evolving nature of the global and Indian ocean as well as, El‐Niño Southern Oscillation, zonal circulation, and pressure systems, are evident in effecting Indian hot season temperatures. The post‐1980s warming is also tangible from the increasing strength of El Niño in the last few decades. The cross‐validated and mean square skill scores are moderate except for a few regions having an average correlation of ~.5 or more for the study periods. The minimum temperature has shown better skill than the maximum temperature, and the southern peninsular region has shown more predictability than the northern regions. Compared to raw data, detrended has a lesser prediction skill. Background: Human Resources for Health (HRH) are crucial for improving health services coverage and population health outcomes. The World Health Organisation (WHO) promotes countries to formulate holistic policies that focus on four HRH dimensions-availability, accessibility, acceptability, and quality (AAAQ). The status of these dimensions and their incorporation in the National Health Policies of India (NHPIs) are not well known.Methods: We created a multilevel framework of strategies and actions directed to improve AAAQ HRH dimensions. HRH-related recommendations of NHPI-1983, 2002, and 2017 were classified according to targeted dimensions and cadres using the framework. We identified the dimensions and cadres focussed by NHPIs using the number of mentions. Furthermore, we introduce a family of dimensionwise deficit indices formulated to assess situational HRH deficiencies for census years (1981, 2001, and 2011) and over-year trends. Finally, we evaluated whether or not the HRH recommendations in NHPIs addressed the deficient cadres and dimensions of the pre-NHPI census years.Results: NHPIs focused more on HRH availability and quality compared to accessibility and acceptability. Doctors were prioritized over auxiliary nurses-midwives and pharmacists in terms of total recommendations. AAAQ indices showed deficits in all dimensions for almost all HRH cadres over the years. All deficit indices show a general decreasing trend from 1981 to 2011 except for the accessibility deficit. The recommendations in NHPIs did not correspond to the situational deficits in many instances indicating a policy priority mismatch.Conclusion: India needs to incorporate AAAQ dimensions in its policies and monitor their progress. The framework and indices-based approach can help identify the gaps between targeted and needed dimensions and cadres for effective HRH strengthening.
