Soil CO 2 efflux (F soil ) is a significant contributor of labile CO 2 to the atmosphere. The Himalayas, a global climate hotspot, condense several climate zones on account of their elevational gradients, thus, creating an opportunity to investigate the F soil trends in different climate zones. Presently, the studies in the Indian Himalayan region are localized to a particular forest type, climate zone, or area of interest, such as seasonal variation. We used a portable infrared gas analyzer to investigate the F soil rates in Himalayan tropical to alpine scrub forest along a 3100-m elevational gradient. Several study parameters such as seasons, forest types, tree species identity, age of trees, distance from tree base, elevation, climatic factors, and soil physico-chemical and enzymatic parameters were investigated to infer their impact on F soil regulation. Our results indicate the warm and wet rainy season F soil rates to be 3.8 times higher than the cold and relatively dry winter season. The tropical forest types showed up to 11 times higher F soil rates than the alpine scrub forest. The temperate Himalayan blue pine and tropical dipterocarp sal showed significant F soil rates, while the alpine Rhododendron shrubs the least. Temperature and moisture together regulate the rainy season F soil maxima. Spatially, F soil rates decreased with distance from the tree base (ρ =  - 0.301; p < 0.0001). Nepalese alder showed a significant positive increase in F soil with stem girth (R 2  = 0.7771; p = 0.048). Species richness (r, 0.81) and diversity (r, 0.77) were significantly associated with F soil , while elevation and major edaphic properties showed a negative association. Surface litter inclusion presented an elevation-modulated impact. Temperature sensitivity was exorbitantly higher in the sub-tropical pine (Q 10 , 11.80) and the alpine scrub (Q 10 , 9.08) forests. We conclude that the rise in atmospheric temperature and the reduction in stand density could enhance the F soil rates on account of increased temperature sensitivity., (© 2023. Heavy metals beyond their permissible limits are major contaminants and causes of concern due to their persistent nature and health hazards. Several studies validated the utilization of plants as biomonitors and bioaccumulators for heavy metal pollution. Therefore, this study was conducted to examine the proficiency for heavy metal monitoring and mitigation by commonly found plant species in urban areas of Delhi. For this objective, four sites (viz., control, residential, commercial, and industrial) and sixteen usually growing plant species were selected. Four heavy metals, i.e., lead (Pb), cadmium (Cd), copper (Cu), and zinc (Zn), were analysed for soil and dust from each site, and leaf samples from each plant, at all sites, and various other parameters were calculated. Among the four elements, Pb and Cd showed maximum geoaccumulation and contamination at all sites. Pollution load index (PLI) values for both soil and dust indicated that industrial sites (1.78, 2.15) were most contaminated followed by commercial (1.52, 1,87), residential (1.41, 1.67), and control (1.22, 1.37) sites. Concentrations of Pb, Cu, and Zn were found to be highest in Morus alba, while Cd concentrations were found to be highest in Millettia pinnata (L.) Panigrahi. From various analyses, we concluded that commonly found plant species such as Ficus religiosa L., Terminalia arjuna (Roxb. Ex DC.) Wight &Arn., Morus alba L., Prosopis juliflora (Sw.) DC., and M. pinnata proved to be exceptional biomonitors and bioaccumulators for heavy metals in urban areas. The rise in urbanization has led to an increase in dust pollution which is hazardous to the health of living beings. The role of roadside plant species in intercepting particulate matter and improving air quality is well reported. Hence, this study was carried out to determine the ability of various plant species to intercept atmospheric dust and withstand the abiotic stress triggered by dust deposition. In the present investigation, three sites (viz., control, commercial, and industrial) differing in anthropogenic activities and vegetation were selected. Sixteen plant species entailing both trees and shrubs that are commonly occurring at all three sites were selected to estimate their dust interception capacity (DIC). The impact of dust pollution on foliage biochemistry and their tolerance in winter and summer seasons were analyzed. Based on biochemical, biological, and socio-economic parameters, air pollution tolerance index (APTI) and anticipated performance index (API) were evaluated. Both dust load and DIC were found to be two times higher in winter than in the summer season. Terminalia arjuna, Ficus benghalensis, and Plumeria alba were the best dust accumulators, while Prosopis juliflora accumulated least. The highest DIC was observed at the industrial site, for Terminalia arjuna (0.025 mg/cm 2 /d) in winter and Plumeria alba (0.023 mg/cm 2 /d) in the summer season. Photosynthetic pigments showed a negative correlation with dust load, while pH, ascorbic acid, electrolytic leakage (E.L.), and proline content showed a positive correlation. In the present study, APTI and API values were highest for Ficus religiosa, Ficus benghalensis, Alstonia scholaris, Dalbergia sissoo, and Terminalia arjuna. Such plant species with wide canopy, large and rough leaf surface area with perforated veins are found to be more suitable and, hence, recommended for the development of greenery to improve air quality in urban areas like Delhi., (© 2022. Prof B Ramamurthi was a pioneer of Indian neurosurgery and a major force in the development of Indian neuroscience. Founding the Madras Institute of Neurology and later the A Lakshmipathi Neurosurgical Centre (ALNC), both at Madras (or Chennai as it is now called), he developed centres of excellence in his career that spanned over five decades. During this period of time he made Madras, a destination for neurosurgery and neuroscience. Along with his colleagues a large number of publications were produced which influenced the world. Notable among his contributions were those in Stereotaxy for movement disorders, epilepsy, pain and psychiatric illness. He also had notable contributions in brain tumours especially acoustic neurinomas and pituitary tumours. His papers on the low incidence of aneurysms is still quoted widely. Head injuries formed a major part of the neurosurgical work and major contributions were made in that field too. As a developing country with socio-economic issues, infections of the nervous system were seen commonly. His publications on tuberculomas of the brain are noteworthy. He was intrigued by the neurophysiological basis of consciousness. He writings on the subject reflect his attempt to bring together ancient eastern thoughts and concepts of consciousness and life and western science. In the later part of his career he spoke on ethics in and the changing milieu of neurosurgery. While contributions to spinal surgery were not seen in the first half of his career, he along with his colleagues from ALNC published original articles on spinal surgery especially tumours and OPLL. Prof B Ramamurthi, has not only influenced, taught and mentored, during his lifetime, a great many neuroscientists, but he also continues to do so through his publications which continue to be relevant in todays world. A glimpse into his contributions show us how without the technology of today a lot was achieved - and we need to see that, to inspire us to achieve more and to strive for greater heights., Competing Interests: None

DNA repair genes are crucial in maintaining the integrity of the whole genome. Single nucleotide polymorphisms (SNPs) in DNA repair genes have been attributed to the development of various cancers. SNPs of DNA repair genes (ERCC1 and ERCC2) have been implicated in the causation of various cancers as well as inter-individual variability in the therapeutic outcomes of platinum based therapy. Thus establishing the frequency of these functional SNPs in the healthy population is of significance. The present study was aimed to establish the allele and genotype frequencies of ERCC1 (19007C>T, rs11615; 8092C>A, rs3212986) and ERCC2 (Asp312Asn, rs1799793) genes in South Indian healthy population and to compare the data from HapMap populations. The study population consisted of 128 healthy South Indian unrelated individuals of either sex aged between 18 and 60 years. Standard phenol-chloroform method was used to extract DNA from peripheral leukocytes. The genotype of DNA repair gene polymorphisms was determined by quantitative real-time polymerase chain reaction using TaqMan genotyping assay. The observed frequency of the studied polymorphisms followed Hardy-Weinberg equilibrium (p>0.05). The frequencies of the minor alleles of the SNPs rs11615 (T), rs3212986 (A) and rs1799793 (A) were 43.8%, 29.3% and 35.6%, respectively. Gender-based analysis showed no significant difference in the frequency pattern. The observed allele and genotype frequencies showed significant ethnic difference between South Indians and other HapMap populations. This is the first study to provide the normative frequency data of allele and genotype distribution of three SNPs of ERCC1 and ERCC2 in South Indian healthy population. India and China have booming chemical, agrochemical, and pharmaceutical industries. Both countries also represent expanding markets for foreign chemical and healthcare companies. All such products require reproductive toxicity testing before marketing. The ICH testing guidelines for medicinal products are not applicable in China and India. Nonetheless, reproductive toxicity studies designed and run to ICH principles are generally acceptable for submission. The Chinese guidelines take into consideration traditional Chinese medicines, which are usually mixtures. Likewise, the specific recommendations of India and China for the reproductive toxicity testing of chemicals and pesticides differ from those of the OECD and the USEPA. Again, studies performed in accordance with internationally recognized principles are usually acceptable for submission in both countries. The Chinese guideline for the reproductive toxicity testing of agrochemicals is currently under revision; the new version is expected to resemble more closely the requirements of the OECD and the USEPA. As a member of the OECD, India has conducted Good Laboratory Practice (GLP) inspection, accreditation, and monitoring activities since 2004. China has made several attempts to join the Council Decisions on Mutual Acceptance of Data in the Assessment of Chemicals since 2005. Currently 47 laboratories in China have been certified by the national GLP authorities. Several laboratories in China have also been recently been certified by OECD member countries as GLP compliant. In India, there are currently 23 GLP-Certified laboratories; about six of these are also AALAC accredited. The specific study designs specified in the guidelines of China and India for reproductive toxicity studies are described in detail in this chapter.

Objective: This paper illustrates a retrospective study of the outcome of those pregnancies that continued from an initial episode of bleeding in first trimester., Methods: Neural networks is used for the prediction of preterm delivery, using various inputs such as the age of women, gestational age when the bleeding occurred, duration of the bleeding days, amount of bleeding, number of episodes, presence or absence of hematoma and placentation position., Results: The success rate of prediction obtained using the feed forward backpropogation network is 70%. Hence, this model can be used to identify women at the risk of premature delivery for planning antenatal care and clinical interventions in pregnancy.

Ethnopharmacological Relevance: Leaves of Caesalpinia bonduc (L.) Roxb. (Caesalpiniaceae) have been used by traditional Siddha healer of Malabar region for psoriasis treatment., Aim of the Study: To evaluate the Caesalpinia bonduc decoction (CBD), Caesalpinia bonduc hydroalcoholic extract (CBHA) for antipsoriatic activity., Materials and Methods: Mouse tail test for psoriasis was used for the evaluation of antipsoriatic activity. Extracts were tested at a dose of 500 mg/kg b.w. and fractions at 250 mg/kg b.w. in Swiss albino mice. Parameters studied in the mouse tail test were changes in epidermal thickness and percentage orthokeratotic values. In vitro antiproliferant assay on HaCaT cell lines and in vitro lipoxygenase inhibition were also carried out., Results: Butanol fraction of Caesalpinia bonduc hydroalcoholic extract (CBHAB) and water fraction of Caesalpinia bonduc hydroalcoholic extract (CBHAW) produced significant orthokeratosis (p<0.001). In relative epidermal thickness, a significant (p<0.05) reduction with respect to control was observed in groups treated with retinoic acid, CBD, butanol fraction of Caesalpinia bonduc decoction (CBDB), water fraction of Caesalpinia bonduc hydroalcoholic extract (CBHAW). Maximum antiproliferant activity was shown by CBHA (IC(50), 77.5±12.7 μg/ml). In lipoxygenase inhibition assay, water fraction of Caesalpinia bonduc decoction (CBDW) showed maximum activity with an IC(50) value of 164.71±4.57 μg/ml., Conclusions: Among all the tested samples only CBHAW showed good activity in the mouse tail test, antiproliferant activity in HaCaT cells and lipoxygenase inhibition assay. Other extracts and fractions showed varying degrees of activity. Aim of the Study: Based on research findings this study is aimed to generate database on ethnobotanical aspects, sustainable utilization by value addition and awareness generation through outreach programme related to Hippophae salicifolia D. Don. (Elaeagnaceae) in the higher Himalayan zone of Uttarakhand in Central Himalaya, India., Materials and Methods: An in-depth survey from June 2004 to July 2006 followed by 480 interviews with the help of semi-structured questionnaires was carried out in 24 Hippophae growing locations in 12 different valleys of Uttarakhand., Results: Plant has immense multipurpose properties and is traditionally utilized for food (20(Min)-90%(Max)), medicine (10(Min)-60%(Max)), veterinary (20(Min)-100%(Max)), fuel (10(Min)-80%(Max)), fencing (20(Min)-80%(Max)), agricultural tools (20(Min)-50%(Max)) and dye mordant (60%). Besides, awareness programmes and value added product demonstration resulted in economical upliftment of local inhabitants of Central Himalaya., Conclusion: The present manuscript will certainly provide an ethnobotanical statistics' impact on the modern scientific societies regarding conservation, cultivation and popularization of this underutilized wild edible species at mass scale. Background: We aimed to analyse treatment outcomes of patients receiving first-line antiretroviral therapy (ART) through the national AIDS control programme of India., Methods: Using routinely collected programme data, we analysed mortality, CD4 evolution and adherence outcomes over a 2-year period in 972 patients who received first-line ART between 1 October 2004 and 31 January 2005 at 3 government ART centres. Cox regression analysis was used to identify independent predictors of mortality., Results: Of the 972 patients (median age 35 years, 66% men), 71% received the stavudinellamivudine/nevirapine regimen. The median CD4 count of enrolled patients was 119 cells/cmm (interquartile range [IQR] 50-200 cells/ cmm) at treatment initiation; 44% had baseline CD4 count <100 cells/cmm. Of the 927 patients for whom treatment outcomes were available, 71% were alive after 2 years of treatment. The median increase in CD4 count was 1 42 cells/ cmm (IQR 57-750 cells/cmm; n=616) at 6 months and 184 cells/cmm (IQR 102-299 cells/cmm; n=582) at 12 months after treatment. Over 2 years, 124 patients (13%) died; the majority of deaths (68%) occurred within the first 6 months of treatment. Those with baseline CD4 count <50 cells/cmm were significantly more likely to die (adjusted hazard ratio 2.5, 95% confidence interval 1.3-3.2) compared with patients who had baseline CD4 count >50 cells/cmm. Over the 2-year period, 323 patients (35%) missed picking up their monthly drugs at least once and 147 patients (16%) were lost to follow up., Conclusion: Survival rates of HIV-infected patients on first-line ART in India were comparable with those from other resource-limited countries. Most deaths occurred early and among patients who had advanced disease. Earlier initiation of HIV treatment and improving long term treatment adherence are key priorities for India's ART programme.

Consumption of greens is a major source of vitamins and micro-nutrients for people using only vegetarian diets rich in carbohydrates. In remote rural settlements where vegetable cultivation is not practiced and market supplies are not organized, local inhabitants depend on indigenous vegetables, both cultivated in kitchen gardens and wild, for enriching the diversity of food. Knowledge of such foods is part of traditional knowledge which is largely transmitted through participation of individuals of households. A total of 123 households in six villages of Nanda Devi Biosphere Reserve buffer zone was surveyed using a schedule to assess the knowledge, availability and consumption pattern of wild leafy vegetables. Quantity estimations were done using regular visits with informants from 30 sample households of the six study villages during the collections. Monetization was used to see the value of wild leafy vegetables harvested during a year. The diversity of wild leafy vegetables being use by the local inhabitants is 21 species belonging to 14 genera and 11 families. This is far less than that being reported to be used by the communities from Western Ghats in India and some parts of Africa. Irrespective of social or economic status all households in the study villages had the knowledge and used wild leafy vegetables. The number of households reported to consume these wild leafy vegetables is greater than the number of households reporting to harvest them for all species except for Diplazium esculentum and Phytolacca acinosa. The availability and use period varied for the species are listed by the users. The study indicated that the knowledge is eroding due to changing social values and non participation of younger generation in collection and processing of such wild leafy vegetables.

The Sethu Samudram Canal Project (SSCP), considered as a harbinger of economic growth in India, is a mega engineering project being developed to provide a 260 km long, 300 m wide and 12 m deep shipping channel between the Arabian Sea and the Bay of Bengal. This project impacts the unique biota and biodiversity of a 10,500 km(2) Marine Biosphere Reserve. This man-made link facilitates exchange of water masses between the less saline Bay of Bengal and the more saline Arabian Sea. Initial construction, subsequent dredging for channel maintenance, and the associated ship traffic would result in irreversible changes to the already over-exploited and stressed environment. Additionally, the channel would form a deep ocean route for future tsunamis. However, to provide assurance to the public, particularly the 50,000 fishing folk, in 47 villages in this area, it would be crucial that a long-term environmental monitoring program is instituted. A thorough evaluation of the impending environmental impacts, similar to those addressed by a Before/After and Control/Impact (BACI model) is recommended.

Objective: To measure the economic output/input ratios for the various options of prevention of rheumatic fever/rheumatic heart disease (RF/RHD) and check the viability of primary prevention vis-à-vis secondary and tertiary preventions., Methods: Cost accounting of the various prevention options was calculated for each variable as available in literature. Actual data as obtainable for the financial year ending March 2006 were computed for the Pondicherry population. Both direct and indirect costs (including community/social costs) were worked out using mostly primary data and wherever necessary, secondary data. Certain scientific assumptions were used where exact data was not available., Results: Primary prevention is the definite viable economic option (1:1.56) compared to secondary (1: 1.07) and tertiary (1: 0.12) preventions. In fact, the current stress on only secondary and tertiary preventions is found to be economically unviable., Conclusion: It is postulated that primary prevention as a practical policy in tackling RF and RHD can be recommended.

Differences in static and dynamic volumes may exist between high altitude residents of Indian Himalayas and their South American counterparts, as well as with acclimatized lowlander sojourners. Maximum expiratory flow-volume loops were recorded in healthy native highlanders of Ladakh (NH, N = 75) and in healthy acclimatized lowlanders (AL, N = 32) at an altitude of 3450 m in the western Indian Himalayas. The forced vital capacity (FVC) and forced expiratory volume in the first second (FEV1), both corrected for a height of 168 cm, were significantly higher in NH [FVC: 5.02 (0.51) vs. 3.89 (0.45) L, p < 0.0001; FEV1: 4.27 (0.47) vs. 3.44 (0.37) L, p < 0.0001]. The flow rates at larger lung volumes (PEFR, FEF25, and FEF50) were similar in the two groups. The NH showed significantly higher flow rates at low lung volumes, that is, FEF75 and FEF75-85% [FEF75: 2.03 (0.69) vs. 1.70 (0.52) L/s, p = 0.0092; FEF75-85%: 1.42 (0.54) vs. 1.06 (0.35) L/s, p = 0.0001]. The exact mechanisms allowing the higher flow rates at low lung volumes remain to be elucidated, but it is possible that these findings may indicate an inherited adaptive response in the Ladakhi highlander.

We examined the effect of S-allylcysteine (SAC), a water-soluble garlic constituent, on cytokeratin expression, a sensitive and specific marker for differentiation status during 7,12-dimethylbenz[a]anthracene (DMBA)-induced hamster buccal pouch (HBP) carcinogenesis in male Syrian hamsters. Hamsters were divided into four groups of six animals each. Animals in group 1 were painted with a 0.5% solution of DMBA in liquid paraffin on the right buccal pouches three times a week for 14 weeks. Group 2 animals were painted with DMBA as in group I, and in addition they received orally 200 mg/kg of SAC on days alternate to DMBA application. Group 3 animals received SAC as in group 2. Group 4 animals received neither DMBA nor SAC and served as the control. The hamsters were killed after an experimental period of 14 weeks. Cytokeratin expression was detected by Western blot analysis using monoclonal antibodies AE1 and AE3. In DMBA-induced HBP tumors, the decreased expression of high molecular weight cytokeratins of molecular mass between 55-70 kDa was observed. Administration of SAC (200 mg/kg) to animals painted with DMBA suppressed the incidence of DMBA-induced carcinomas and was associated with restoration of normal cytokeratin expression. The results of the present study suggest that inhibition of HBP tumorigenesis by SAC may be due to its regulatory effects on differentiation, tumor invasiveness, and its ability to migrate and form metastases.

India's health system was designed in a different era, when expectations of the public and private sectors were quite different. India's population is also undergoing transitions in the demographic, epidemiologic and social aspects of health. Disparities in life expectancy, disease, access to health care and protection from financial risks have increased. These factors are challenging the health system to respond in new ways. The old approach to national health policies and programmes is increasingly inappropriate. By analyzing inter- and intra-state differences in contexts and processes, we argue that the content of national health policy needs to be more diverse and accommodating to specific states and districts. More 'splitting' of India's health policy at the state level would better address their health problems, and would open the way to innovation and local accountability. States further along the health transition would be able to develop policies to deal with the emerging epidemic of non-communicable diseases and more appropriate health financing systems. States early in the transition would need to focus on improving the quality and access of essential public health services, and empowering communities to take more ownership. Better 'lumping' of policy issues at the central level is also needed, but not in ways that have been done in the past. The central government needs to focus on overcoming the large inequalities in health outcomes across India, tackle growing challenges to health such as the HIV epidemic, and provide the much needed leadership on systemic issues such as the development of systems for quality assurance and regulation of the private sector. It also needs to support and facilitate states and districts to develop critical capacities rather than directly manage programmes. As India develops a more diverse set of state health policies, there will be more opportunities to learn what works in different policy environments.

The World AIDS Day was declared by WHO in 1988 and the theme is given for this day every year and for the year 2002-03 "Live and let live" is the specific theme. According to UNAIDS there are 40-50 million people living with HIV/AIDS globally. Stigma and discrimination are major obstacles to effective HIV/AIDS prevention. HIV remains unaffected by highly active antiretroviral therapy (HAART). The best hope for prevention of HIV rests on the development of a globally accessible vaccine. There are many complexities associated with conducting AIDS vaccine efficacy trials in the developing world.

The success of conserving biological resources in any Biosphere Reserve or protected area depends on the extent of support and positive attitudes and perceptions of local people have towards such establishments. Ignoring the dependence of the local people for their subsistence needs on resources of such areas leads to conflicts between protected area managers and the local inhabitants. Crop yield losses and livestock depredation were serious problems observed in most buffer zone villages of Nanda Devi Biosphere Reserve. In the present study 10 villages situated in the buffer zone of Nanada Devi Biosphere Reserve (1612 km2 area) in Chamoli district of Uttaranchal, India were studied during 1996-97 using a questionnaire survey of each household (419 = households; 2253 = total population in 1991; 273 ha = cultivated area). Estimates of crop yield losses were made using paired plots technique in four representative villages for each crop species. The magnitude of crop yield losses varied significantly with the distance of agricultural field from forest boundary. The total crop yield losses were high for wheat and potato in all the villages. The spatial distribution of total crop yield losses in any village indicated that they were highest in the area near to forest and least in the area near to village for all crops. Losses from areas near to forest contributed to more than 50% of total losses for each crop in all villages. However, in Lata, Peng and Tolma villages, the losses are high for kidney bean and chemmi (local variety of kidney bean) which varied between 18.5% to 30% of total losses in those villages. Potato alone represents 43.6% of total crop yield loss due to wildlife in Dronagiri village in monetary terms. Among the crops, the monetary value of yield losses are least for amaranth and highest for kidney bean. The projected total value of crop yield losses due to wildlife damage for buffer zone villages located in Garhwal Himalaya is about Rs. 538,620 (US$ 15,389). Besides food grains, horticultural crops i.e. apple, also suffered maximum damage. Major wildlife agents responsible for crop damage were wild boar, bear, porcupine, monkey, musk deer and partridge (chokor). Monkey and wild boar alone accounted for about 50% to 60% of total crop damage in the study villages. Goat and sheep are the major livestock killed by leopard. The total value of livestock losses at prevailing market rates is about Rs. 1,024,520 (US$ 29,272) in the study villages. Due to existing conservation policies and laxity in implementation of preventive measures, the problems for local inhabitants are increasing. Potential solutions discussed emphasize the need to undertake suitable and appropriate protective measures to minimize the crop losses. Change in cropping and crop composition, particularly cultivation of medicinal plants (high value low volume crops), were also suggested. Besides, fair and quick disbursement of compensation for crop loss and livestock killing need to be adopted. Local people of the buffer zone area already have a negative attitude towards park/reserve establishment due to socio-political changes inducing major economic losses and this attitude may lead to clashes and confrontations if proper ameliorative measures are not taken immediately.

In continuation of their studies on Biodegradation of cellulosic waste materials, the authors have presented their experimental data on biodegradation on waste paper pulp. Greater efficiency in the cow dung and M3 culture experimental setups have been pointed out and discussed. Necessity of detailed data evaluation of temperature, pH, dry weight, cellulose, protein, carbohydrate, Nitrate, Phosphate & Potassium levels is suggested. Derivation of %, absolute and increase/decrease % of BOO, COD and VS have been pointed out for the first time in solid waste Biodegradation. Further data evaluation and derivation of critical reaction rate kinetics have been recommended for further studies in solid waste-degradation to achieve quicker Bioconversion of cellulosic wastes into compost.

Background and Aim of the Study: The Chitra tilting disc valve was developed in India to meet the need for a low-cost cardiac valve. The valve has an integrally machined cobalt-based alloy cage, an ultra-high molecular-weight polyethylene disc, and a polyester suture ring. An important feature of this valve is its soft closing sound, by virtue of a plastic occluder., Methods: Between December 1990 and January 1995, 306 patients underwent isolated aortic (AVR, n = 101) or mitral valve replacement (MVR, n = 205) at six institutions in India. The early mortality rate was 6.9% (seven after AVR; 14 after MVR). A total of 285 survivors was followed up until September 1998; total follow up was 1212 patient-years (pt-yr) (AVR, 445 pt-yr; MVR, 767 pt-yr)., Results: There were 52 late deaths (4.3%/pt-yr; AVR 2.2%/pt-yr; MVR 5.5%/pt-yr). Thirty-five deaths were valve-related (23 were due to unknown causes). One AVR patient (0.2%/pt-yr) and 12 MVR patients (1.6%/pt-yr) developed valve thrombosis, and embolic episodes occurred in 25 patients (seven after AVR, 1.6%/pt-yr; 18 after MVR, 2.4%/pt-yr). Bleeding events and infectious endocarditis occurred infrequently (AVR 0.9 and 0.7%/pt-yr; MVR 0.4 and 0.5%/pt-yr, respectively). There was no incidence of paravalvular leak or structural dysfunction of the valve. Actuarial survival rates at seven years were 82.4+/-4.0% for AVR and 65.2+/-5.0% for MVR. During the same interval, thrombus-free and embolism-free survival after AVR and MVR occurred in 98.9+/-1.1% and 94.1+/-1.9%, and 92.3+/-2.8% and 82.1+/-5.7% of patients, respectively. Freedom from all valve-related mortality and morbidity at seven years was 81.5+/-4.1% after AVR, and 64.2+/-5.1% after MVR., Conclusion: The Chitra valve appears to be safe and to have performance comparable with that of other currently used tilting disc valves. This valve costs substantially less than other valves, and is therefore within reach of a larger subset of Indian patients.

Objectives: The aim of this study was to look to for a relationship between pulmonary artery pressure (PAP), left atrial pressure (LAP), mitral valve area (MVA) and transmitral gradient (Gr) on the one hand and the spirometric data on the other, in cases of mitral stenosis., Methods: The spirometry values measured were forced vital capacity (FVC), forced expiratory volume in one second and as percent of FVC (FEV1, FEV1/FVC%), forced expiratory flow between 25% to 75% of FVC (FEF 25-75%), at 50% of FVC (FEF 50%) 75% of FVC (FEF 75%) and peak expiratory flow rate (PEFR). The PAP, LAP, MVA and Gr were measured at echocardiography and cardiac catheterisation., Results: Analysis of results was done of 60 patients in four groups divided on basis of mean PAP: Group 1 with least and group 4 with highest mean PAP. FVC values were reduced in direct proportion to PAP, LAP, MVA and Gr in three out of four groups. FEV1% as a percent of predicted value showed uniform reduction in all the groups, PEFR was moderately affected in group 1 and most severely in group 4. Values of FEV1/FVC% were within normal range in all the groups. FEF 25-75%, and FEF 50% showed serial reduction in values in keeping with the elevation of the PAP. FEF 75% was also moderately affected in group 1 and most severely in group 4., Conclusion: We found that a moderate restrictive defect and a small airway defect which is found in cases of mitral stenosis, directly correlates to the PAP, LAP, MVA and Gr. There was no significant involvement of the larger airways.

Relationship between body size and bioaccumulation of copper, zinc, lead and cadmium in the fouling bivalve, M. sallei (Recluz) in Visakhapatnam harbour was studied. While concentration of copper, zinc and lead decrease with increasing size, no such relationship is observed for cadmium.

Hydatid antigen was demonstrated for the first time in the urine of patients with hydatid disease by countercurrent immunoelectrophoresis (CIEP). The antigen was detected in the concentrated urine of 7 of 16 (43.75% positive) patients with surgically confirmed hydatid disease, 4 of 10 (40% positive) patients with ultrasound-proven hydatid disease (daughter cysts or prominent septation and hydatid sands demonstrated by ultrasound), and 8 of 14 (57.14% positive) patients with clinically diagnosed (presumptive) hydatid disease. No antigen was detected in the concentrated urine from 24 patients with parasitic diseases other than hydatid disease. However, antigen was detected in 2 (8% false positive) of 25 concentrated urine samples collected from healthy control subjects (blood donors and students). These result suggest that the detection of hydatid antigen in the urine by CIEP is a simple, rapid, and noninvasive method of diagnosis of hydatid disease.

Six children, aged upto one year, with volvulus of the stomach are reported. Vomiting and regurgitation of feeds were the main presenting symptoms. Eventration of the left hemidiaphragm was present in four cases. Barium meal confirmed the diagnosis. Five cases were operated on through an abdominal approach. Plication of the diaphragm (3 cases), colonic displacement (2) and gastrostomy (3) were resorted to. One child needed reoperation for a missed Ladd's band. There was no operative mortality.

Plasma glucagon levels were measured in young individuals with severe, insulin-dependent, juvenile-onset diabetes mellitus to study whether differences in glucagon secretion were related to ketosis proneness and resistance. Fasting glucagon levels were similarly elevated in both classical, ketosis-prone, type I diabetic subjects and ketosis-resistant, type "J" subjects [70 +/- 7 pmol/L (mean +/- SEM) and 81 +/- 10 pmol/L, respectively] compared with nonobese, nondiabetic controls (36 +/- 3 pmol/L, P less than 0.01). After oral glucose administration, however, glucagon responses were strikingly dissimilar in the two groups. In type I diabetic individuals, glucagon rose paradoxically during OGTT, by 21 +/- 4 pmol/L, an increase of 33 +/- 10%; on the other hand, glucagon levels in type "J" diabetic individuals fell by 28 +/- 7 pmol/L, a decrease of 33 +/- 5%. There was no measurable increase in plasma free insulin during OGTT in either group. Postprandial glucagon suppressibility may be relevant to the ketosis resistance that is characteristic of type "J" diabetes.
