With neoliberal ideologies having inspired social policies for families across the world, the discourse on childrearing as the sole responsibility of individual parents has become dominant. The neologism 'parenting' refers more and more to a de-contextualized performance, with targets to be reached, and necessary competences to be learned. Currently, discourse on parenting thus entails what has been defined as 'parental determinism' which in turn leads to blaming parents, often described as incompetent and in need of guidance. It is, therefore, not surprising that parents' voices have seldom been considered and systematically explored. This paper presents a qualitative study, carried out in a northern Italian province, which compared representations of childrearing in different contexts by mothers and fathers, on the one hand, and by practitioners involved in various capacities with parents, on the other. In total, 55 mothers and fathers and 49 professionals were interviewed. The resulting data enabled the identification of common views, as well as clashes in perspectives and instances of parental resistance to dominant discourses on parenting. [ABSTRACT FROM AUTHOR]

The COVID-19 pandemic and the associated socio-economic crisis are posing unprecedented challenges both globally and locally, raising many unknowns and imposing difficult trade-offs. One of the side effects of this pandemic, with the ensuing physical distancing rules and quarantine, has been the condition of isolation that is affecting vulnerable populations disproportionately across the world. Against this background, one of the major tasks for social work is to keep up connections, find new ways to sustain relations and networks, and mitigate against inescapable feelings of loneliness and their consequences. Social workers themselves are affected personally and professionally by this crisis, facing new living and working conditions. They experience feelings of uncertainty, fear, and the risk of isolation. The aim of this paper is to present examples of practices intended to promote a sense of community, as such a shared emotion can support present and future social workers. These examples serve to underline how, now more than ever, a central challenge for social work is to strengthen the professional community, find ways to overcome isolation and 'to take care of those who care'. Purpose: The measurement of quality in social work practice has become an area of growing interest and relevance in the social services field. Our starting point is that quality in interventions with human beings has to be defined in ways that incorporate the multiple perspectives of all the subjects involved. Methods: The study, adopting qualitative and quantitative methods, explored issues of quality in social services provision in South Tyrol in Italy from the point of view of the main stakeholders. Results: It was possible to identify four dimensions of quality that stakeholders considered important: the political role of practitioners, the ability to take an active role in the organization, the capacity to connect with other professionals, and the quality of direct relationships with users. One of the core dimensions of social work practice, identified since its establishment as a professional endeavour, is concerned with political action. Nonetheless, policy practice, which entails that social workers should connect their work with individuals, as in traditional casework, to wider political action, is often perceived as marginal in everyday practice. This paper connects views on social work policy practices to the context provided by the ways social policies are actually constructed in Italy. A research study on governance processes across Italy, addressing the main actors involved in social policy-making, reveals that in fact practitioners’ political action is differently represented within different frameworks, ranging from being seen as a near impossibility, to being perceived as a crucial factor in the policy-making processes. If context and cultures play such a relevant role, synergistic work at different levels and by all the different actors in the social work community is required if we want social workers to be able to express their potential fully in the political arena. From the North to the South of Europe, embedded in local contexts, we can find interesting projects of cooperative knowledge production in social work settings. In this article we present case studies from research projects with social workers in Helsinki, Trento and Antwerp. We describe how the new paradigm of knowledge production (Mode 2) is implemented in this kind of social work research. We look for the specificity of the research process in these projects, the benefits for the researchers and the practitioners, and the necessary conditions to get satisfactory results. Using Italian social workers' representations of their profession three core issues are identified that recur in inter- national debates. Background/aims: HFE protein controls iron absorption and cycling, and HFE mutations influence iron status. The aim was to evaluate the effect of the HFE genotype on the need for iron and erythropoietin in Italian hemodialysis patients.Methods: Ninety-six prevalent patients were evaluated at the time of enrolment and prospectively followed for 3 years. Patients were given r-HuEPO and Fe3+-gluconate according to guidelines. The HFE genotype was determined by restriction analysis.Results: Three patients (3%) carried the C282Y mutation, 4 (4%) were homozygous and 18 (19%) heterozygous for the H63D mutation, and 71 (74%) were negative for both. At enrolment, subjects positive for HFE mutations had higher iron stores (ferritin 617 +/- 663 vs. 423 +/- 386 ng/ml, p = 0.05), were receiving less iron (82.5 +/- 66 vs. 110 +/- 154 mg/month, p = 0.05) and a lower r-HuEPO dosage (98 +/- 83 vs. 142 +/- 138 U/kg/week, p = 0.03). Consistently during the study period, patients positive for HFE mutations received a lower amount of r-HuEPO (94.5 +/- 63 vs. 186 +/- 344 U/kg/week, p = 0.01) and iron (97 +/- 63 vs. 121 +/- 68 mg/month, p = 0.07). Upon Cox regression analysis, after adjustment for confounding variables, the presence of HFE mutations was associated with a reduced risk of death (HR 0.6, 95% CI 0.34-1.03, p = 0.06).Conclusion: HFE mutations reduce the amount of r-HuEPO and iron necessary to support erythropoiesis in hemodialysis. Summary: Two different frameworks are identified with respect to social work practice and views of professional powers. Findings: Accounts of social work practice, gathered in Italy, illustrate two styles of narrating and constructing the work experience. These are analysed following Mannheim's seminal work on romantic and enlightenment styles of thought. Practitioners' accounts differ in how sequences are constructed, in the meaning attributed to crucial aspects such as the understanding of the situation, and in the place attributed to theory. Two conceptions emerge of the relationship of professional social workers with clients, and of their power. Applications: The article contributes to the debate on the relation between theory and practice in social work. Theoretical models are not viewed as potential sources of practical suggestions, but as whole perspectives with which to engage. They are ways of seeing and doing `things' which stimulate critical reflection on one's own work. The two ways of conceiving professional powers that emerge from this study appear embedded in different cognitive orders. This invites those seeking to identify new ways of conceiving professional powers to look at the whole social work culture, without trying to isolate single aspects or elements of it. This article aims to examine how, and to what extent, the process of Europeanisation has affected the reshaping of territorial representation in Italy. To explore this research question, it was considered how European Cohesion Policy and structural funds exercise a potential influence over the key dimensions of representation by providing regions with the opportunity to strengthen their roles as both ‘actors’ and ‘arenas’ of representation. Empirical research conducted through interviews and the analysis of documents in all twenty Italian regions revealed how each region has reacted to structural funds in a distinctive way due to different internal factors, such as the legacy of pre-existing policy styles and political structures, and the attitudes of the political/administrative elite. The article discusses this point in detail by focusing on the changes that occurred (or did not occur) at the regional level in light of the aforementioned dimensions of representation. A priori , Italy and France would appear as unfit for activation policies. Yet, when activation is seen as a diversified strategy of restructuring welfare states across Europe, and not only the 'activation' of individuals from assistance, then the empirical picture over the last 20 years is much more complex. One of the main differences between both countries is with regard to the role of the state, whilst at the same time they share many common features. However, French achievements in matters of activation seem to be important, whereas in Italy institutional features have consistently impaired the development of comprehensive strategies. The article discusses the developments in the Italian welfare state during the second half of the 1990s and sheds light on the distinctive policy-making dynamics and outcomes which differentiate that period's centre-left governments from the technocratic ones of the 1992-95 period. It is argued that, after the 1996 elections, party politics regained ground; although the executive continued to maintain a primary role in policy-making, the weakness and internal divisions in the centre-left coalition increasingly undermined the executive's action and strategy. The evidence presented shows how centre-left governments attempted to counterbalance political constraints by resorting to available institutional opportunities and managed to introduce a wide range of innovative measures and address hitherto neglected social needs. However, their inability to tackle crucial issues -- as the reform of social shock absorbers -- shows that the executive's room for manoeuvre was further restricted by the workers' trade unions. The article supports the interpretation that, whereas during the «transition period» the trade unions were willing and able to play a role which went well beyond traditional interest representation, as the country progressively recovered the unions increasingly engaged in inward-looking strategies, turning social pacts into a liability rather than an asset for centre-left governments. The aim of this study was to analyze the role of HFE mutations in blood donors with iron parameters suggesting iron overload, taking into account the regional distribution of HFE mutations in Italy. We studied 5880 subjects undergoing evaluation for blood donation eligibility, from different areas of Italy. Abnormal iron parameters were defined as transferrin saturation (TS) >50% or >45% and serum ferritin (SF) >300 or >250?µg/ml in males and females, respectively. Subjects with increased TS and/or SF were re-tested and typed for HFE mutations C282Y and H63D. A total of 548 individuals had increased iron parameters at first testing. In total, 179/548 were available for retesting, and in 109 increased TS and/or SF were confirmed. Increased TS was confirmed in 25 individuals, among whom three were C282Y homozygotes and six were compound heterozygotes for C282Y and H63D. Increased TS was more frequent in northern Italy than in southern regions. In individuals with increased TS and/or SF, the frequency of C282Y and H63D was 0.13 and 0.21 in northern-Italy versus 0.05 and 0.45 in southern Italy (P=0.004 for H63D). Nine out of 10 individuals carrying hemochromatosis-associated genotypes (including compound heterozygosity for C282Y and H63D) originated from northern regions. Among controls, the allelic frequencies of C282Y and H63D were 0.037 and 0.16 in the northern regions and 0.015 and 0.16 in the southern regions. Introduction: Women are generally underrepresented in trials focusing on aortic aneurysm. Nevertheless, sex-related differences have recently emerged from several studies and registries. The aim of this research was to assess whether sex-related anatomical disparities existed in fenestrated and branched aortic repair candidates and whether these discrepancies could influence endovascular repair outcomes., Methods: Data from all consecutive patients treated during the 2008-2019 period within the Italian Multicenter fenestrated or branched endovascular aortic repair (F/BEVAR) Registry were included in the present study. Propensity matching was performed using a logistic regression model adjusted for demographic data and comorbidities to obtain comparable male and female samples. The selection model led to a final study population of 176 patients (88 women and 88 men) among the total initial cohort of 596. Study endpoints were technical and clinical success, overall survival, aneurysm-related death, and reintervention rates evaluated at 30 days and during follow-up., Results: Twenty-eight patients (15.9%) received urgent/emergent repair. In most of the cases (71.6%), women received treatment for extensive thoracoabdominal pathology (Crawford type I, II, or III aneurysm rather than type IV or juxta-pararenal) versus 46.6% of men (p=0.001). Female patients presented with more challenging iliac accesses with at least one side considered hostile in 27.3% of the cases (vs 13.6% in male patients, p=0.039). Finally, women had significantly smaller visceral vessels. Women had significantly worse operative outcomes, with an 86.2% technical success rate versus 96.6% in the male population (p=0.016). No differences were recorded in terms of 30-day reinterventions between men and women. The 5-year estimate of freedom from late reintervention, according to Kaplan-Meier analysis, was 85.6% in men versus 81.6% in women (p=ns). No aneurysm-related death was recorded during follow-up (median observational time, 23 months [interquartile range, 7-45 months])., Conclusion: Women presented a significantly higher incidence of thoracoabdominal aneurysms, smaller visceral vessels, and more complex iliofemoral accesses, resulting in a significantly lower technical success after F/BEVAR. Objective: Complex aortic aneurysms (juxtarenal aortic aneurysms [JAAA], pararenal aortic aneurysms [PAAAs], thoracoabdominal aortic aneurysms TAAAs) are treated with increasing frequency through fenestrated and branched endovascular repair (F/B-EVAR); however, the outcome of these procedures is usually reported separately by single experiences and wider overviews are not frequent. The aim of this study was therefore to report an Italian experience analyzing the results obtained in four academic centers to evaluate the predictors of outcomes., Methods: Between 2008 and 2019, all consecutive patients undergoing F/B-EVAR in four Italian university centers were recorded prospectively and analyzed retrospectively. Preoperative comorbidities and postoperative complications were classified according with the Society for Vascular Surgery reporting standard. Postoperative complications and 30-day/in-hospital mortality were assessed as early outcomes. Survival, freedom from reinterventions and target visceral vessels patency were assessed as follow-up outcomes by Kaplan-Meier analysis. Risk factors for 30-day/in-hospital mortality and spinal cord ischemia (SCI) were determined by multivariate analysis. Risk factors for follow-up mortality and reinterventions were evaluated by Cox regression model., Results: Five hundred ninety-six patients underwent F/B-EVAR for 124 JAAAs (21%), 121 PAAAs (20%), and 351 TAAAs (59%). Elective and urgent procedures were performed in 520 (87%) and 76 (13%) cases, respectively. Postoperative cardiac, pulmonary, and renal complications were reported in 41 (7%), 50 (8%), and 80 (13%) patients, respectively. Seven bowel ischemia (1%) and 23 cerebrovascular complications (4%) occurred. Forty-seven (8%) patients suffered SCI with 17 cases (3%) of permanent paraplegia. Crawford's extent I-II-III TAAAs (odds ratio [OR], 13.41; 95% confidence interval [CI], 1.77-101.65; P = .012) and postoperative renal complications (OR, 3.84; 95% CI, 1.70-8.69; P = .001) independently predicted SCI. Thirty-two patients (5%) died in the perioperative period. Preoperative chronic renal failure (OR, 7.81; 95% CI, 7.81-26.31; P = .001), postoperative bowel ischemia (OR, 26.97; 95% CI, 3.37-215.5; P = .002), cardiac complications (OR, 5.77; 95% CI, 1.41-23.64; P ≤ .001), cerebrovascular complications (OR, 28.63; 95% CI, 5.20-157.5; P < .001), and SCI (OR, 5.99; 95% CI, 1.12-32.5; P = .036) were independently correlated with 30-day/in-hospital mortality. The mean follow-up was 25 ± 7 months. Freedom from target visceral vessels occlusion and freedom from reintervention were 96% and 92% at 1 year and 93% and 85% at 3 years, respectively. TAAAs (hazard ratio [HR]. 3.16; 95% CI, 1.68-5.92; P ≤ .001), postdissection TAAAs (HR, 2.20; 95% CI, 1.30-4.90; P = .05) and postoperative bowel ischemia (HR, 11.98; 95% CI, 1.53-93.31; P = .018) were independent predictors of reinterventions. Survival was 88% and 78% at 1 and 3 years, respectively. Preoperative chronic renal failure (HR, 2.39; 95% CI, 1.59-3.59; P ≤ .001), urgent repair (HR, 1.80; 95% CI, 1.03-3.20; P = .039), TAAAs (HR, 2.01; 95% CI, 1.13-3.56; P = .017), postoperative bowel ischemia (HR, 5.55; 95% CI, 2.11-14.59; P = .001), cardiac complications (HR, 3.89; 95% CI, 2.25-6.71; P ≤ .001), and pulmonary complications (HR, 1.97; 95% CI, 1.56-3.35; P = .013) were independent predictors of mortality during follow-up., Conclusions: F/B-EVAR is associated with satisfactory midterm outcomes in a nationwide experience. Objective: We compared the early and midterm outcomes of polytetrafluoroethylene covered stents (CSs) vs bare metal stents (BMSs) used in the kissing conformation for the reconstruction of the aortic bifurcation in aortoiliac obstructive disease., Methods: A multicenter cohort registry (2015-2019) collected data from 1306 patients who had undergone endovascular treatment of aortoiliac arterial obstructive disease. Only patients who had received bilateral iliac kissing stents for TransAtlantic Inter-Society Consensus (TASC) class C and D lesions were included in the present analysis. The 30-day outcomes, midterm primary patency, and limb salvage rates were compared between the CSs and BMSs in matched patient cohorts after propensity score matching. The follow-up results were analyzed using Kaplan-Meier curves. Cox proportional hazards models were used to identify the predictors of primary patency., Results: A total of 336 patients were treated with kissing stents, 201 with CSs (60%) and 135 with BMSs (40%). In the unmatched cohort, patients receiving CSs were more likely to have critical limb ischemia (41% vs 30%; P = .038), complex iliac lesions, such as TASC D (90% vs 56%; P < .01), and iliac occlusions (59% vs 44%; P < .01). After propensity score matching, 220 patients were selected (110 with CSs and 110 with BMSs), without differences in the clinical presentation (critical limb ischemia, 41% vs 33%; P = .167), or anatomic complexity (TASC D, 66% vs 60%, P = .21; iliac occlusion, 48% vs 49%, P = .89). The 30-day mortality was 0%. The early medical (unmatched, 5% vs 4%, P = 1.00; matched, 5% vs 4%, P = .75) and surgical (unmatched, 5% vs 5%, P = 1.00; matched, 5% vs 3%, P = .72) complication rates were similar between the CSs and BMSs. However, the CSs resulted in a lower risk of intraoperative iliac rupture (0% vs 3.5%; P = .013) and greater ankle-brachial index improvement (0.43 ± 0.22 vs 0.36 ± 0.24; P = .02). At 36 months, the overall primary patency (92% ± 7% vs 92% ± 8%; P = .38), secondary patency (98% ± 3% vs 98% ± 4%; P = .50), and limb salvage (93% ± 9% vs 97% ± 5%; P = .20) rates were similar. In cases of moderate to severe iliac calcification, the CSs showed better results in the matched cohort (100% vs 89% ± 9%; P = .048). On multivariate analysis, CS use (hazard ratio [HR], 1.67; P = .45) did not significantly affect primary patency, but older age (HR, 0.93; P = .03) and kissing stent diameter ≥8 mm (HR, 0.25; P = .03) were significantly associated., Conclusions: In the present multicenter study, the use of kissing stents for the treatment of the aortic bifurcation provided good early and midterm results. CSs were preferred for more complex lesions, were protective from iliac rupture, and allowed for greater ankle-brachial index improvement. Aim: The aim of this paper was to report outcomes of endovascular aneurysm repair with percutaneous femoral access (pEVAR) using Prostar XL and Proglide closure systems (Abbot Vascular, Santa Clara, CA, USA), from the multicenter Italian Percutaneous EVAR (IPER) registry., Methods: Consecutive patients affected by aortic pathology treated by EVAR with percutaneous access (pEVAR) between January 2010 and December 2014 at seven Italian centers were enrolled in this multicenter registry. All the operators had an experience of at least 50 percutaneous femoral access procedures. Data were prospectively collected into a dedicated online database including patient's demographics, anatomical features, intra- and postoperative outcomes. A retrospective analysis was carried out to report intraoperative and 30-day technical success and access-related complication rate. Uni- and multivariate analyses were performed to identify factors potentially associated with an increased risk of percutaneous pEVAR failure., Results: A total of 2381 accesses were collected in 1322 patients, 1249 (94.4%) male with a mean age of 73.5±8.3 years (range 45-97). The overall technical success rate was 96.8% (2305/2381). Major intraoperative access-related complications requiring conversion to surgical cut-down were observed in 3.2% of the cases (76/2381). One-month pEVAR failure-rate was 0.25% (6/2381). Presence of femoral artery calcifications resulted to be a significant predictor of technical failure (OR: 1.69; 95% CI: 1.03-2.77; P=0.036) at multivariate analysis. No significant association was observed with sex (P=0.28), obesity (P=0.64), CFA diameter (P=0.32), level of CFA bifurcation (P=0.94) and sheath size >18 F (P=0.24). The use of Proglide was associated with a lower failure rate compared to Prostar XL (2.5% vs. 3.3%) despite not statistically significant (P=0.33)., Conclusion: The results of the IPER registry confirm the high technical success rate of percutaneous EVAR when performed by experienced operators, even in presence of demanding anatomies. Femoral calcification represents the only predictor of percutaneous access failure.

Objective: This study retrospectively analyzed our 33-year experience with surgical management of popliteal artery aneurysms (PAAs), with particular attention paid to early and long-term results., Methods: From January 1981 to December 2013, 234 open surgical interventions for PAA were performed in 196 patients. Data concerning these interventions were collected in a dedicated database containing main preoperative, intraoperative, and postoperative features. Early (intraoperative and <30 days) results were analyzed for mortality, thrombosis, reintervention, and amputation rates. The follow-up program consisted of clinical and duplex ultrasound examinations at 1 month and yearly thereafter. Patients who did not accomplish follow-up examinations were interviewed by telephone. Additional data regarding long-term survival and major clinical events were obtained from the Regional Health Care database. Follow-up results were analyzed for survival, primary and secondary patency, and amputations rates., Results: Patients were predominantly males (186 [95%]), with a mean age of 68.5 ± 9.9 years. The PAA was asymptomatic in 97 limbs, intermittent claudication was present in 68, and limb-threatening ischemia was present in 62. Aneurysmal rupture occurred in six patients, and venous compression with leg swelling and pain was present in one patient. The intervention consisted of aneurysmal ligation and bypass grafting in 122 interventions, aneurysmectomy with graft interposition was used in 108, and four patients underwent aneurysmectomy with an end-to-end anastomosis. An autologous vein was used in 49 interventions, and a prosthetic graft was used in 181. In 71 interventions a posterior approach was used, and in the remaining 163, a medial approach was preferred. There were two perioperative deaths, with a cumulative mortality rate of 1%. Perioperative thrombosis occurred after 18 interventions (7.7%). A successful reintervention was performed in 10 of those patients, whereas surgical thrombectomy was ineffective in one patient and leg amputation was necessary. The remaining seven patients underwent major amputation without any new surgical attempt. An adjunctive major amputation was necessary in a patient with a patent bypass for irreversible foot ischemia. The cumulative rate of amputations at 30 days was 3.8% (9 of 234 limbs). Mean duration of follow-up was 62 months (range 1-312 months). During follow-up, 31 deaths, 45 thromboses, and 10 amputations were recorded. The estimated 13-year survival rate was 50.8% (standard error [SE], 0.07%); during the same interval, primary patency, secondary patency, and limb preservation rates were 55.1% (SE, 0.05%), 68% (SE, 0.05%), and 86% (SE, 0.04%)., Conclusions: Open surgical repair of PAAs provided good results in our experience, with low rates of perioperative complications and an excellent durability in the very long-term setting, representing the benchmark for alternative techniques such as endovascular repair., (Copyright © 2015 Society for Vascular Surgery. Background: To retrospectively analyze the early and the midterm results of endovascular management of aortoiliac obstructive disease with the kissing stent technique., Methods: From January 2005 to September 2012, 229 consecutive endovascular interventions for aortoiliac obstructive disease were performed; data from all the interventions were prospectively collected in a dedicated database. In 41 patients, the kissing stent technique at the level of aortic bifurcation was performed (group 1), whereas in the remaining 188 it was not (group 2). Perioperative results were compared with chi-squared test. Follow-up results were analyzed with Kaplan-Meier curves and compared with log-rank test., Results: Trans-Atlantic Inter-Society Consensus II C and D lesions were present in 66% of patients in group 1 and in 28.5% in group 2 (P < 0.001), whereas iliac occlusion rather than stenosis was detected in 78% of patients in group 1 and in 50% in group 2 (P = 0.001). The mean number of placed stents was 2.5 in group 1 and 1.4 in group 2 (P < 0.001). Technical success was 100% in group 1 and 98% in group 2 (P = 0.3). The rate of perioperative complications was 7.3% in group 1 and 4.2% in group 2 (P = 0.4). At 30 days, neither deaths nor major cardiovascular complications occurred. There was no thrombosis or significant restenosis at the early postoperative follow-up visit. Mean duration of follow-up was 22 months. Primary patency rates at 4 years were 70.5% (standard error [SE], 0.09) in group 1 and 75.5% (SE, 0.06) in group 2 (P = 0.7). At the same interval, assisted primary and secondary patency and survival rates were also similar; reintervention rates were 15.5% in group 1 (SE, 0.1) and 19.5% in group 2 (SE, 0.06; P = 0.6)., Conclusions: The kissing stent technique provided satisfactory results in patients with obstructive aortoiliac diseases, without an increase in immediate and midterm complications, representing an effective solution in complex anatomies., (Copyright © 2015 Elsevier Inc. Introduction: The aim of this report was to analyse early and mid-term outcomes of endovascular treatment (endovascular aneurysm repair, EVAR) for aorto-iliac aneurysms with the use of an iliac branch device (IBD)., Report: A total of 85 EVAR procedures with IBD were electively carried out in 81 patients between September 2007 and August 2012. Technical success was obtained in 98.7% of the cases. The mean follow-up duration was 20.4 months (SD ± 15.4). There was one IBD occlusion (1.2%). Estimated 48 months' survival, freedom from re-intervention and branch occlusion were 76.7%, 88.3% and 98%, respectively., Conclusions: EVAR for aorto-iliac aneurysms using IBD is an effective procedure with low complication and re-intervention rates at mid-term follow-up., (Copyright © 2013 European Society for Vascular Surgery. Aim of the Study: The aim of this study was to retrospectively analyse early and late results of surgical management of internal carotid artery (ICA) true and false aneurysms in a single-centre experience., Materials and Methods: From January 1988 to December 2011, 50 consecutive interventions for ICA aneurismal disease were performed; interventions were performed for true ICA aneurysm in 19 cases (group 1) and for ICA post-carotid endarterectomy (CEA) pseudo-aneurysm in the remaining 31 (group 2). Early results (<30 days) were evaluated in terms of mortality, stroke and cranial nerves' injury and compared between the two groups with χ(2) test. Follow-up results (stroke free-survival, freedom from ICA thrombosis and reintervention) were analysed with Kaplan-Meier curves and compared with log-rank test., Results: All the patients in group 1 had open repair of their ICA aneurysm; in group 2 open repair was performed in 30 cases, while three patients with post-CEA aneurysm without signs of infection had a covered stent placed. There were no perioperative deaths. Two major strokes occurred in group 1 and one major stroke occurred in group 2 (p = 0.1). The rates of postoperative cranial nerve injuries were 10.5% in group 1 and 13% in group 2 (p = 0.8). Median duration of follow-up was 60 months (range 1-276). Estimated 10-year stroke-free survival rates were 64% in group 1 and 37% in group 2 (p = 0.4, log rank 0.5); thrombosis-free survival at 10 years was 66% in group 1 and 34% in group 2 (p = 0.2, log rank 1.2), while the corresponding figures in terms of reintervention-free survival were 68% and 33%, respectively (p = 0.2, log rank 1.8)., Conclusions: Surgical treatment of ICA aneurismal disease provided in our experience satisfactory early and long-term results, without significant differences between true and false aneurysms. Background: Aim of this study was to retrospectively compare perioperative (<30 days) and 2-year results of open and endovascular management of popliteal artery aneurysms (PAAs) in a single-center experience., Methods: From January 2005 to December 2010, 64 PAAs in 59 consecutive patients were operated on at our institution; in 43 cases, open repair was performed (group 1), whereas the remaining 21 cases had an endovascular procedure (group 2). Data from all the interventions were prospectively collected in a dedicated database, which included main preoperative, intraoperative, and postoperative parameters. Early results in terms of mortality, graft thrombosis, and amputation rates were analyzed and compared by χ(2) text or Fisher exact text. The surveillance program consisted of clinical and ultrasonographic examinations at 1, 6, and 12 months and yearly thereafter. Follow-up results (survival, primary and secondary patency, limb salvage) were analyzed by Kaplan-Meier curves, and differences in the two groups were assessed by log-rank test., Results: There were no differences between the two groups in terms of sex, age, risk factors for atherosclerosis, and comorbidities; PAAs were symptomatic in 48% of cases in group 1 and in 29% in group 2 (P = 0.1). Fifteen patients with mild-to-moderate acute ischemia due to PAA thrombosis underwent preoperative intra-arterial thrombolysis, 13 in group 1 and 2 in group 2. In open surgery group, nine cases were treated with aneurysmectomy and prosthetic graft interposition, and in seven cases, the aneurysm was opened and a prosthetic graft was placed inside the aneurysm. In 27 cases, ligation of the aneurysm with bypass grafting (21 prosthetic grafts and 6 autologous veins) was carried out. In group 2, 20 patients had endoprosthesis placement, whereas in the remaining patient, a multilayer nitinol stent was used. There was one perioperative death in a patient of group 2 who underwent concomitant endovascular aneurysm repair and PAA endografting. Cumulative 30-day death and amputation rate was 4.5% in group 1 and 4.7% in group 2 (P = 0.9). Follow-up was available in 61 interventions (96%) with a mean follow-up period of 22.5 months (range: 1-60). Estimated primary patency rates at 24 months were 78.1% in group 1 and 59.4% in group 2 (P = 0.1). Freedom from reintervention rates at 24 months were 79% in group 1 and 61.5% in group 2 (P = 0.2); estimated 24-month secondary patency rates were 81.6% in group 1 and 78.4% in group 2 (P = 0.9), and freedom from amputation rates were 92.7% and 95%, respectively (P = 0.7)., Conclusions: Endovascular treatment of PAAs provided, in our initial experience, satisfactory perioperative and 1-year results, not significantly different from those obtained with prosthetic open repair in patients with similar clinical and anatomical status. Aim: AnacondaTM is an infrarenal stent-graft for the endovascular treatment (EVAR) of abdominal aortic aneurysms (AAA). The AnacondaTM Italian Registry is an observational multicentre registry that started in Italy on March 2009. The aim of this study is to present the registry protocol and to evaluate the interim perioperative results., Methods: Patient data collected covered both retrospective and new cases. The case record forms are divided into preoperative data (population characteristics and clinical data), preoperative morphological data, intraoperative data, perioperative data (30-days from AnacondaTM implant) and follow-up record forms. The inclusion criteria are: AAA with a maximum sac diameter ≥ 5 cm, AAA with rapid expansion, symptomatic AAA or with a contained rupture, aorto-iliac aneurysms, proximal neck length ≥ 15 mm and diameter of the proximal aortic neck between 16 and 31.5 mm. No exclusion criteria were considered regarding aorto-iliac tortuosity, wall calcification or thrombosis and no exclusion criteria were considered regarding the patients age, comorbidities or life expectancy. Seven postoperative follow-up controls (visit and imaging) were required: at 30-days from the AnacondaTM implant, 6 months, 1 year and then annually up to 5 years. The registry is still open to new case enrollment., Results: The reported results reflect the registry cases as at March 15, 2011. At this date, 787 cases were present in the database: 722 (91.7%) were male and 65 (8.3%) female. The mean patient age was 76.6 years with a range between 50 and 94 years. The ASA class was ≥ 3 in 635 patients (80.6%). The mean aneurysmal sac diameter was 55.73 ± 9.5 mm. The mean proximal aortic neck length was 26.52 ± 12.31 mm. Of the 787 patients treated, 11 cases of immediate surgical conversion were reported (1.3%). The overall 30-day mortality rate was 1.2% (10 cases): but 30-day death occurred in 0.9% of successful endograft implants and in 27.2% of surgical conversions (p<0.001). In fourteen cases (1.7%) an iliac leg thrombosis occurred and a native iliac artery thrombosis occurred in only five cases (0.5%). The 30-day overall occurrence of endoleaks was observed in 95 cases (12%): 5 cases of type I proximal endoleak (0.6%), 89 cases of type II endoleak (11.3%) and 1 case of type III endoleak (0.1%). No cases of endograft dislocation were reported., Conclusion: This is an interim report on the study protocol and on the preliminary early results. Presently there are no definitive conclusions, however the perioperative results show that the AnacondaTM endograft seems to be safe and effective in the treatment of AAA, even in a high risk cohort of patients. Definitive and long-term results are needed.
