Intra-abdominal infections (IAIs) are common surgical emergencies and are an important cause of morbidity and mortality in hospital settings, particularly if poorly managed. The cornerstones of effective IAIs management include early diagnosis, adequate source control, appropriate antimicrobial therapy, and early physiologic stabilization using intravenous fluids and vasopressor agents in critically ill patients. Adequate empiric antimicrobial therapy in patients with IAIs is of paramount importance because inappropriate antimicrobial therapy is associated with poor outcomes. Optimizing antimicrobial prescriptions improves treatment effectiveness, increases patients' safety, and minimizes the risk of opportunistic infections (such as Clostridioides difficile) and antimicrobial resistance selection. The growing emergence of multi-drug resistant organisms has caused an impending crisis with alarming implications, especially regarding Gram-negative bacteria. The Multidisciplinary and Intersociety Italian Council for the Optimization of Antimicrobial Use promoted a consensus conference on the antimicrobial management of IAIs, including emergency medicine specialists, radiologists, surgeons, intensivists, infectious disease specialists, clinical pharmacologists, hospital pharmacists, microbiologists and public health specialists. Relevant clinical questions were constructed by the Organizational Committee in order to investigate the topic. The expert panel produced recommendation statements based on the best scientific evidence from PubMed and EMBASE Library and experts' opinions. The statements were planned and graded according to the Grading of Recommendations Assessment, Development and Evaluation (GRADE) hierarchy of evidence. On November 10, 2023, the experts met in Mestre (Italy) to debate the statements. After the approval of the statements, the expert panel met via email and virtual meetings to prepare and revise the definitive document. This document represents the executive summary of the consensus conference and comprises three sections. The first section focuses on the general principles of diagnosis and treatment of IAIs. The second section provides twenty-three evidence-based recommendations for the antimicrobial therapy of IAIs. Purpose: Our aim was to evaluate the evolution of 20 patients with H1N1 pneumonia, focusing our attention on patients with severe clinical and radiological findings who developed post-acute respiratory distress syndrome (post-ARDS) pulmonary fibrosis., Materials and Methods: Twenty adult patients (nine women and 11 men; mean age 43.5 ± 16.4 years) with a diagnosis of H1N1 infection confirmed by pharyngeal swab came to our attention from September to November 2009 and were followed up until September 2010. All patients were hospitalised in consideration of the severity of clinical findings, and all underwent chest X-ray. Twelve of them underwent at least one computed tomography (CT) scan of the chest., Results: In 75% of cases (15/20), there was complete resolution of the clinical and radiological findings. Twenty-five percent of patients (5/20) developed acute respiratory distress syndrome (ARDS), which progressed to predominantly peripheral pulmonary fibrosis in 10% (2/20; one died and one had late-onset pulmonary fibrosis, documented on day 68). Moreover, in one patient with a CT diagnosis of pulmonary fibrosis, we observed progressive regression of radiological findings over 4 months of follow-up., Conclusions: In patients with H1N1 pneumonia, post-ARDS pulmonary fibrosis is not a rare complication. Therefore, a CT scan should be performed in all patients with severe clinical findings. Our study demonstrated that in these patients, fibrosis could present a different spatial distribution and a different temporal trend, with delayed late onset; moreover, in one case, the signs of interstitial lung disease partially regressed over time. Therefore, CT should be considered not only in the diagnostic stage, but also during the follow-up.

The evidence for HLA-Cw antigens' involvement in the modulation of the immune response in bone marrow transplantation, NK alloreactivity and the susceptibility and follow-up for different diseases has been growing in the recent years, but very few data on HLA-Cw distribution in healthy Italian subjects are available to date. This report presents an updated description of HLA-Cw frequencies in Italy, comparing data from the northern (Lombardia) and southern (Campania and Puglia) parts of the country. A total of 1101 healthy subjects of Italian origin were genotyped, and the results showed that HLA-Cw*04, Cw*07, Cw*12, and, in particular, Cw*0401, Cw*0701, Cw*1203, were the most frequent alleles found in all three regions analysed. Nevertheless, statistically significant differences were observed in Cw*07 distribution, which was more frequent in the southern than in the northern part of Italy (28.8% vs 22.4%; p=0.001; OR: 1.4; 95%CI: 1.14-1.73), and in Cw*12 distribution, which was more frequent in the north than the south (17.0% vs 12.4%; p=0.007, OR: 1.4; 95%CI: 1.10-1.91). These results, which give an improved pattern of distribution of HLA-Cw alleles in the Italian population, would be useful in bone marrow transplantation and anthropological studies. Moreover, due to the important role of HLA-Cw antigens in modulation of the immune response and NK alloreactivity, these data would be of interest in studies on susceptibility, follow-up and/or protection against different diseases.

Vibration exposure of the hand-arm system is associated with an increased risk of upper-limb vascular, neurological and musculoskeletal lesions, or hand-arm vibration syndrome (HAVS). The prevalence of occupational HAVS and upper-limb disorders was studied among 278 Forestry Service workers in Sicily and Calabria. Subjects who used chain-saws (18 weeks/year) had a greater prevalence of peripheral sensory-neural disturbances (28%), upper-limb musculoskeletal disorders (33%) and carpal tunnel syndrome (19%) compared with 260 manual workers from the same Corps not exposed to hand-transmitted vibration. Raynaud's phenomenon was comparable in exposed and control subjects (5.3% vs. 4.7%.) Upper-limb neuropathies were significantly associated with energy-equivalent frequency-weighted acceleration; exposure duration; and cumulative vibration dose (m2/s4h). The variable "years of work with vibrating tools" was strongly associated with peripheral neuropathies; carpal tunnel syndrome; and upper-limb musculotendinous syndromes. Data suggest that in Sicily and Calabria, where the climate is milder than in other areas of Italy, forestry work with hand-held vibrating tools does not entail a greater prevalence of peripheral vascular disorders (Raynaud's phenomenon), while the prevalence of occupational upper-limb neurological and musculoskeletal disorders, in which combined ergonomic and mechanical risk factors have a large pathogenic role, is significantly increased.

Three years from the approval of the law safeguarding pregnant workers and the foetus (D.Lgs. 151/01), awareness of these provisions and management of pregnancy in relation to their job were assessed in 160 pregnant workers using a questionnaire. Respondents were 147 subjects, mean age 29.4 +/- 4.6 years, employed as blue-collar workers (n=26); white-collar workers (n =47); health workers (n=16); school workers (n=14); workers in shops (n=6); policewomen (n =3) housewives (n=25). Moreprovisions and most of them took advantage of maternal leave. Pre-term birth (16%) and low birth weight (9%) were the mostfrequent events. There were no neonatal diseases. Blue-collar, white-collar work and work in the home were not associated with pre-term births or low birth weight. Analysis based on the fathers' job characteristics did not yield significant associations either. Although the small size of our sample does not allow to draw firm conclusions, it is interesting to note that more than 10% of respondents were not aware of the provisions protecting workers' pregnancy.

Recent legislation regarding the safety of workers aboard fishing vessels requires the appointment by ship owners of a Reference Physician in charge of health surveillance, preventive inspections and related tasks. As maritime workers, especially fishermen, have always been excluded from legal protection of occupational health, there are no exhaustive data on the incidence of their occupational disease. Several epidemiological studies of fishermen have evidenced a high prevalence and incidence of occupational conditions, among which noise-related hypoacousia. We report data of a phonometric survey conducted aboard six fishing vessels carrying a crew of less than six fishing in the mid-Adriatic. Measurements were performed during fishing and navigation aboard five vessels fitted with a fixed-pitch propeller and during fishing only aboard one vessel fitted with an controllable pitch propeller. Measurements were conducted: 1) in the engine rooms; 2) in the work area on deck; 3) at the winch; 4) in the wheelhouse; 5) in the mess-room and kitchen; 6) in the sleeping quarters. Results show that the equivalent sound pressure level in the engine rooms consistently exceeded 90 dBA on all vessels. The speed of the vessels fitted with the fixed-pitch propeller is 3-4 knots in the fishing phase and around 10 knots during navigation to and from the fishing grounds; noise emission is lower with the former regimen because of the smaller number of engine revolutions per minute. Our survey demonstrated considerably different noise levels in the various areas of vessels. One key element in workers' exposure, the tasks assigned and the environmental working conditions is of course the type of fishing in which the vessel is engaged. Further phonometric studies are required to assess the daily level of exposure per crew member, which represents the reference for the noise-related risk of each subject. Knowledge of the sound pressure levels in the work environment and the length of daily exposure of each crew member will allow to assess the level of occupational exposure and consequently enact the proper prevention and protection measures by the Reference Physician.

In 1997, Italy had the highest number of tetanus cases in the European Union; cases in the Marches region were more numerous than in France or Germany. In a retrospective study of the patients infected in Marches in 1996-1999, subjects or relatives were interviewed to ascertain disease severity, infection mode, occupation and immunization state at the time of infection. There were 32 cases, 29 (90.6%) females and three (9.4%) males, mean age 74.65 years (SD 9.06). The raw annual incidence was 5.6 per million and raw annual mortality 0.5 (n = 3). Twenty-seven patients (84.4%) were agricultural workers and five (15.6%) housewives; 25 (78.2%) had never been vaccinated, two (6.2%) had been immunized several years earlier and five (15.6%) had received only the first dose of vaccine at least 15 years earlier. In Italy, tetanus vaccination is compulsory for agricultural workers. Occupational health physicians should monitor workers' tetanus immunization state during their periodic surveillance visits.

Objective: The present study evaluated the factors that contribute to stress and the psychological difficulties of the Italian military component of the United Nations Mission in Bosnia and Herzegovina. This group was compared with a homogeneous group stationed in Italy., Methods: A cross-sectional study was performed in two groups of career military personnel: 452 subjects who were stationed in Bosnia as peacekeepers and 166 who were stationed in Italy. All subjects completed two multiple-choice questionnaires (a stress self-evaluation test and a socioeconomic questionnaire) during various periods of duty., Results: The responses to the stress self-evaluation test indicated that the peacekeepers did not show a statistically significant increase in stress. Those who were affected by stress, though, indicated that the level was high. The principal variables that were associated with a greater level of stress were length of the mission, lack of recreational or athletic activities during the mission, more than three family members, and unemployment before enrollment., Conclusions: Peacekeepers come from an economically poor environment, have large families, and usually have a history of unemployment. Along with these preenrollment risk factors, the effective time spent in recreational activities and the length of the mission also influence stress levels. Individual and group techniques for stress management should be implemented.

The paper analyses the rate and severity of accidents in a shipyard in the period 1 January-31 December, 1987. Data were collected by consulting the shipyards' accident register, which recorded each single case of injury, and the medical certificates sent to the shipyard management by the worker's physicians. The shipyard employed 996 blue collar workers and 210 white collar workers in 1987, all of whom were subject to obligatory industrial accident insurance. For each of the 1375 accidents that occurred in the period under study, the time, place and cause of the accident and the type and site of the injury were recorded. The aim of the statistical analysis of the data was to assess the risk in each area of the shipyard and the causes of the accidents in relation to the work station and job. The severity rate of injury increased in those workers recently assigned to new jobs; climate had a negative influence since accidents were more frequent in spring/summer. Accident prevention measures are suggested that can be achieved by improving work organisation and by a more appropriate use of personal safety devices.

Blood lead concentration was measured from 1979 to 1989 in 1148 subjects (834 males and 314 females) living in Regione Marche of Italy. Blood concentration lowered in the time from 34.6 mcg/100ml in 1979 to 12.3 mcg/100ml in 1989. The greatest decrease was measured after 1982 and this appears to be related to the reduction, on the basis of CEE dispositions, of lead concentration in gasoline. Blood lead concentration was higher in males than in females (18.2 mcg/100ml vs 16.5 mcg/100ml) and in people living in the cities with more than 50.000 inhabitants (18.3 mcg/100ml). The smoking habit, the wine assumption and the use of canned foods do not influence blood lead concentration, which is instead increased in subjects which usually drink water drawn from artesian wells.
