A newly found leaf arrangement to reduce self-shading was observed in a Japanese warm-temperate forest. For monoaxial trees that deploy leaves directly on a single stem, leaf arrangements involving progressive elongation of the petiole and progressive increase in deflection angle (the angle between stem and petiole) from the uppermost to the lowermost leaves act to reduce self-shading. However, the progressive reduction in petiole length and deflection angle from the uppermost to the lowermost leaves should also result in the reduction of self-shading. Nevertheless, the latter leaf arrangement has not been reported previously for any tree species. Four Araliaceae species, namely, Gamblea innovans, Chengiopanax sciadophylloides, Dendropanax trifidus and Fatsia japonica, which are typical monoaxial tree species in Japan, were studied. We examined the crown structure of saplings growing in the light-limited understorey in a Japanese warm-temperate forest. Two evergreen species, Dendropanax trifidus and F. japonica showed progressive petiole elongation and progressive increase in the deflection angle from the uppermost to the lowermost leaves. In contrast, saplings of deciduous species, G. innovans and C. sciadophylloides had a leaf arrangement involving progressive reduction in petiole length and deflection angle from the uppermost to the lowermost leaves. The leaf arrangement has diversified among members of the same family, but all four studied species develop a crown with little self-shading that is adapted for growth in the light-limited understorey. Although trees are likely to be under the same selective pressure to reduce self-shading, this study revealed that there is flexibility in its morphological realisation, which has been poorly appreciated previously., (© 2024. Phyllostachys nigra var. henonis, a monocarpic bamboo with a 120-year flowering interval, is next predicted to flower in Japan in the 2020s. Because a huge area of the country is presently covered by stands of this species, post-flowering dieback of these stands and ensuing drastic changes in land cover may cause serious social and/or environmental problems. No study on the regeneration of this bamboo species was conducted during the last flowering event in the 1900s, and the regeneration process of this species is thus still unknown. In 2020, we encountered a localized flowering of P. nigra var. henonis in Japan and used this discovery as a rare opportunity to study the initial regeneration process of the species. Over 3 years, more than 80% of culms in the study site bloomed, but no seed was produced. In addition, no established seedlings were located. These facts strongly suggest that P. nigra var. henonis lacks the ability to produce seeds and cannot undergo sexual regeneration. Some bamboo culms were produced after flowering but died within 1 year of emergence. Small, weak culms (dwarf ramets) also appeared after flowering, but most died within 1 year as well. Three years after flowering, all culms had died, with no sign of regeneration detected. According to our 3 years of observation, this bamboo appears to be hard to regenerate-an idea completely contradicted by the fact that this species has long persisted in Japan. Glycemic control is an important issue in gestational diabetes mellitus (GDM) and in diabetic pregnant women. We determined the reference intervals of glycated albumin (GA) and hemoglobin A1c (HbA1c) as glycemic control markers in healthy Japanese pregnant women and analyzed their time courses and factors that influence these variables during pregnancy. 676 women were screened for the present study. After the exclusion of non-pregnant and puerperal women, 574 women were studied to determine the reference intervals. HbA1c, GA, casual plasma glucose, urinary glucose, urinary protein, and body mass index (BMI) (non-pregnancy) were measured. HbA1c levels significantly decreased in the second trimester of pregnancy and increased in the third trimester, while GA levels significantly decreased towards the third trimester. Casual plasma glucose levels decreased in the first trimester and subsequently remained constant. The reference intervals of GA and HbA1c in the healthy pregnant women were 11.5-15.7% and 4.5-5.7%, respectively. GA levels were lower (p<0.01) and HbA1c levels were higher (p<0.05) in pregnant women with proteinuria. In the obese group, GA levels were lower (p<0.01) than those of the control group (18.5≤ BMI <25kg/m²), and HbA1c levels were higher (p<0.01) than those of the control group. On the basis of the results of this multicenter study, the reference intervals of GA and HbA1c in healthy Japanese pregnant women were determined. Strict glycemic control is essential to reduce perinatal complications. GA appears to be a useful marker for pregnant women, since it can be measured easily and changes rapidly and markedly.

Women's employment in Japan has increased substantially in recent decades, however little large scale research has been done on the impact of various types of working conditions on women's health. The aim of this study was to assess the menstrual cycle and menstrual pain problems of female workers and to investigate the factors that relate to them. The questionnaire was distributed to 8,150 women and 2,166 responded (26.6%). An anonymous self-administered questionnaire was used to get information about demographics, menstrual cycle status, the degree of menstrual pain, and employment and environmental factors. Irregular cycle menstruation was experienced in 17.1% of responded workers. We discovered the relationship between irregular menstrual cycles and stress, smell of cigarettes, age and smoking habits. Some degree of menstrual pain was experienced in 77.6% of responded workers. This study showed the relationship between menstrual pain and stress, high temperature and humidity, age, BMI, and number of births. In conclusion, we found that stress is thought to be an important factor related with menstrual cycle irregularities and menstrual pain among Japanese female workers.

Objectives: We studied the clinical management and prognosis of pregnant women with a history of abruption, as well as the associated risk factors., Methods: We reviewed the cases of 23 patients with a history of abruption and 66 patients with abruption., Results: The recurrence rate of abruption was 4.3%. Intentional care prolonged gestational age in most patients. Although the incidence of abruption was low (0.44%), the consequences could be perinatal death and maternal disseminated vascular coagulation (DIC)., Conclusion: To prevent abruption recurrence, careful monitoring during hospitalization is important. Both clinical findings and transabdominal echography are useful in diagnosing abruption.

Background: Geminin negatively regulates Cdt1 and induces the formation of prereplicative complexes by loading mini-chromosome maintenance proteins (Mcm) onto chromatin and limiting DNA replication to once per cell cycle. Recent studies have suggested that geminin expression is a marker of the S/G2/M phase of the cell cycle and is associated with a poor prognosis in various human malignancies. This study aimed to clarify the pathobiological role of geminin in intestinal-type gastric carcinoma, and its relationships with minichromosome maintenance 2 (Mcm2) and Ki67 expression., Methods: We performed western blot analysis of seven human gastric cancer cell lines, and immunohistochemical analysis of 72 gastric mucosal lesions and 128 surgically removed advanced intestinal-type gastric carcinomas. Double-labeling immuno-fluorescence was performed to identify the coexpression of geminin and Ki67., Results: Geminin was detected in all cell lines. Geminin labeling indices (LIs) in hyperplastic polyps, low-grade adenomas, high-grade adenomas, and intestinal-type adenocarcinomas were 3.9%, 10.5%, 18.6%, and 27.2%, respectively. The equivalent LIs for Ki67 and Mcm2 were 17.7%, 42.2%, 52.6%, and 59.7%; and 26.7%, 70.0%, 67.8%, and 77.8%, respectively. Double-labeling immunofluorescence revealed coexpression of geminin and Ki67 in both normal and tumor cells. The LI for geminin was significantly correlated with N stage, International Union Against Cancer (UICC) stage, Mcm2 LI, and Ki67 LI. Patients in stages I-IV and stage III with higher LIs for geminin (>25%) had significantly worse prognoses (P < 0.05 and P < 0.04, respectively). Univariate Cox regression analysis indicated that the overall survival of stage I-IV tumors was significantly correlated with high geminin LIs (relative risk [RR] = 1.94; P = 0.04)., Conclusions: Geminin expression might reflect the biological nature of gastric intramucosal neoplasms and could be a possible prognostic marker in advanced intestinal-type gastric carcinomas.

Background: Although pulmonary thromboembolism (PTE) has been considered relatively uncommon in Japan, its incidence has been on the increase in recent years., Methods and Results: To verify the incidence of PTE in Japan, PTE cases of obstetrics and gynecology were investigated among 102 facilities throughout Japan between 1991 and 2000. A total of 254 cases were enrolled, showing a 6.5-fold increase over the past 10 years. PTE occurred in 0.02% of total births; 0.003% after vaginal deliveries and 0.06% after cesarean births (C/S), of which 14.5% resulting in fatality. The mortality rate was 2.5 per 100,000 deliveries. The incidences among gynecological cases were 0.08% of total operations; 0.03% in benign diseases and 0.42% in malignant diseases of which 13.5% resulting in fatality. The mortality rate was 10.8 per 100,000 operations. The risk was 22 times higher in C/S compared with vaginal deliveries, 16 times higher in malignant diseases compared with benign diseases., Conclusions: As our present survey has shown, PTE has been on the rise in Japan in recent years. C/S and malignant diseases are strong risk factors in obstetrics and gynecology.

This study is a part of our series of studies of nonpatient, Japanese children. In total, 346 children were administered the Rorschach. Thirty-two children had fewer than 10 responses, 112 children had more than 10 but fewer than 14 responses, and 12 children were maladjusted or mentally retarded, for a total of 156. In this study, we analyzed the remaining 190 children not excluded by the above criteria in the following age groups: 5 years (n = 24), 8 years (n = 43), 9 years (n = 42), 12 years (n = 42), and 14 years (n = 39). Japanese children showed few responses generally, with an average of 18 ~ 20 responses. In addition to having fewer responses, other features of Japanese children are high Lambda, low EA, very high X-%, and low X+%. The fact that the Rorschach test depends on verbal expression may be one factor in all these features, and we should consider this study in view of these Japanese cultural factors. Interrater reliability findings and Rorschach Comprehensive System (CS; Exner, 1995; Matsumoto, 2003; Matsumoto et al., 2002) data are presented.

Objectives: The incidence of poor neurologic outcomes was studied in term newborns who had suffered severe asphyxia at birth., Methods: Subjects were 152 newborns admitted to the NICU with a low Apgar score at 1 or 5 minutes. A 1-year prospective follow-up of neurological outcomes was carried out by a questionnaire survey concluded between April 1, 1996 and March 31, 1998., Results: 1) The incidence of a poor neurologic outcome, including 15 neurologic sequelae and 6 deaths, was 13.8% among the subjects. 2) The risk of a poor outcome was increased by 13-fold in neonates with adverse neurological signs and 31-fold in those with hypoxic ischemic encephalopathy., Conclusion: The incidence of poor neurologic outcome was very high among term infants with low Apgar scores. These infants were 10 times to 20 times more likely to die, or to survive with permanent disabilities, than were infants without low Apgar scores.

In the last 30 years, maternal and perinatal mortality rates have markedly improved in Japan. These results are supported by the advanced technologies and newly developed treatments in obstetric and neonatal medicine, the evidence of which will be described through the authors' experiences. Ultrasound-guided treatments for fetuses and pediatric surgery for extremely tiny infants have been performed with cooperation among cardiac surgeons, anesthesiologists and perinatologists. The remarkable achievements in neonatal medicine are surfactant replacement therapy, application of high frequency oscillation (HFO) respirator, nitric oxide and extracorporeal membrane oxygenation (ECMO) systems and transcutaneous blood gas monitoring. The possibility to have a safe pregnancy and delivery arose for women with severe medical complications such as diabetes mellitus (DM), heart diseases, and hemo-dialysis, factors formerly regarded as contraindications to pregnancy. The accumulative data of these high-risk pregnancies treated in our center are introduced. It is noted that molecular mechanisms in fetal growth have been elucidated to some extent. The insulin-like growth factor I (IGF-I) and IGF-binding protein 1 (IGFBP1) system, and cytokines, are involved in the fetal growth. Newly developed treatments such as urinary trypsin inhibitor (Urinastatin) for prevention of preterm delivery and amnioinfusion to cases of preterm premature rupture of the membrane are summarized. Finally, the reference to advanced medicine and its side effects lead to broad discussions regarding the future development of perinatal medicine.

We conducted prenatal diagnosis by haplotype analysis, using newly developed microsatellite markers, in eight Fukuyama type congenital muscular dystrophy (FCMD) families. In addition to six new families, two previously reported families were reexamined by haplotype analysis including detection of an ancestral founder haplotype (138-183-301) for 3 microsatellite markers closest to the FCMD gene, designated D9S2105-D9S2107-D9S172, the distances of which from the FCMD gene are presumed to be approximately 140, approximately 20, and approximately 280 kb, respectively. Five fetuses from five families were diagnosed as nonaffected, and were subsequently confirmed to be healthy. Three fetuses of the other three families were diagnosed as having a high probability of being affected by FCMD. In the prenatal diagnosis conducted for these eight families, the ancestral founder allele was observed in 13 of 16 (81%) FCMD-bearing chromosomes. Detection of the ancestral haplotype facilitated achieving accurate prenatal diagnosis of FCMD. The brains of all three fetuses prenatally diagnosed as FCMD-affected showed the initial stage of cortical dysplasia, strong evidence of FCMD.

Unlabelled: It is well known that many pathophysiological findings in toxemia of pregnancy are explained by imbalance of coagulation and fibrinolysis system. The purpose of this study is to elucidate a precise role of coagulation and fibrinolysis system in pathogenesis of toxemia of pregnancy., Subjects and Methods: 1) Classification of toxemia of pregnancy. Three hundred and thirty seven of toxemia of pregnancy are classified based on the onset period, and incidence of severity of disease and IUGR, rate of genetic factor of hypertension are compared in each group. 2) Platelet factor 4 (pf4) and beta-thromboglobulin (beta-TG), Fibrinopeptide A (FPA), thrombin-ATIII complex, ATIII fibrinopeptide B beta 15-42, D dimer FDP and plasmin-alpha 2 PI complex are assayed. The levels of PGI2, tissue plasminogen activator (tPA) and thrombomodulin (TM) are measured after venous occlusion. Immunoreactivity and biological activity of TM in urine are analyzed. 3) Aminoacid sequence of TM from normal and toxemia of pregnancy are determined by analyzing cDNA for TM. Moreover, TM are synthesized from recombined DNA and enzymological properties of TM obtained from normal and toxemia of pregnancy are compared. 4) Release of PGI2, tPA and TM by addition of thrombin are observed using monolayer culture of endothelial cells from cord. Enzymological properties of purified placental TM are analyzed., Results: 1) The incidence of severe type, IUGR and the rate of patients who possess genetic factors for hypertension are higher in early onset type, suggesting that hypertension is the predominant characteristics in early onset type and that genetic factors for hypertension are tightly involved. 2) All parameters such as platelets, coagulation and fibrinolysis system are elevated in toxemia of pregnancy compared to those in normal pregnancy. Coagulation index that consists of above parameters is well correlated with clinical index that consists of clinical findings (r = 0.7006, p less than 0.0001). The net increase of PGI2, tPA and TM by venous occlusion are decreased along with severity of toxemia of pregnancy. The potency of production of PGI2 from endothelial cells in maternal omentum is impaired in the severe toxemia of pregnancy. Purified TM in urine from normal pregnancy and toxemia of pregnancy has 63K dalton of single band on SDS-PAGE. However, bioactivity/immunoreactivity ratio of TM in early onset type is lower than those in late onset type, and affinity of TM for thrombin and protein C is decreased in early onset type.(ABSTRACT TRUNCATED AT 400 WORDS)
