Calcifying lesions in the jaws are usually solitary and can be diagnosed by intraoral, panoramic, and conventional extraoral radiographs. It is extremely rare for these calcifying lesions to be multiple, unless they are associated with certain systematic diseases. Herein, we report a rare case of multiple odontomas in a 4-year-old girl. The lesions were scattered throughout both jaws on the panoramic image. The reconstructed CT image showed lesions in the left maxilla that occupied the sinus and affected the floor of the left orbit. The lesions in the mandible were widespread and extended to the tooth-bearing area. Enucleations of the maxillary parts were performed, and the pathological diagnosis was a mixture of complex and compound odontomas. The 3-year postoperative follow-up by panoramic imaging was uneventful. Preoperative CT was useful for the differential diagnosis by depicting the internal calcifying lesions and revealing the extent and complex relationships among the lesions. [ABSTRACT FROM AUTHOR]

Several attempts have been made to classify odontogenic tumors; however, the need for a uniform international classification system led the World Health Organization (WHO) to present a classification of odontogenic tumors in 1971. We aimed to evaluate the number and types of odontogenic tumors examined at the Tokyo Dental College Hospital in Japan to determine the frequency and types of odontogenic tumors, based on the 2017 WHO classification system, as this information has not been reported previously in Japan. We also compared the results of our evaluation with those reported in previous studies. We conducted a clinicopathological evaluation of odontogenic tumors examined at the Tokyo Dental College Hospital between 1975 and 2020. This included an analysis of 1089 cases (malignant, n = 10, 0.9%; benign, n = 1079, 99.1%) based on the 2017 World Health Organization Classification of Head and Neck Tumors. We identified 483 (44.3%), 487 (44.7%), and 109 (10.0%) benign epithelial odontogenic, mixed odontogenic, and mesenchymal tumors, respectively. The most common tumor types were odontoma (42.5%) and ameloblastoma (41.9%). Of the 1089 cases, 585 (53.7%) and 504 (46.3%) were male and female patients, respectively. Ameloblastoma and ameloblastic fibroma occurred more commonly in male patients, whereas odontogenic fibroma and cemento-ossifying fibroma affected female patients primarily. The age at diagnosis ranged from three to 87 (mean, 29.05) years. In 319 (29.3%) patients, the age at diagnosis ranged from 10 to 19 years. Ameloblastoma and odontoma were the most common tumor types among patients in their 20s and those aged 10-19 years, respectively. In 737 (67.7%) and 726 (66.7%) patients, the tumors were located in the mandible and posterior region, respectively. Ameloblastoma was particularly prevalent in the posterior mandible. Abstract: In the present report, we review the clinical features and biophysiological roles, including gene expression, of nevoid basal cell carcinoma syndrome (NBCCS) in Japanese cases. The frequency of clinical findings of Japanese cases (n =186) was reported in NBCCS including keratocystic odontogenic tumors (KOTs). The syndromes resulted from mutation in the patched (PTCH1) gene in both neoplastic lesion basal cell carcinoma (BCCs) and KOTs. BCCs originate from hair follicles and express Gli gene; KOTs, classically known as odontogenic keratocysts, originate from odontogenic epithelial rest cells and considerable variation in the recurrence rate after surgery. PTCH1 gene alteration may have significant roles in the pathogenesis of NBCCS and sporadic KOTs. However, their histogenetic origin was found to differ: basal cell carcinoma in the skin originated from hair follicle cells and KOTs originated from odontogenic epithelial rest cells in the jaw. Objectives. We rarely find multiple cystic lesions in the jaw in panoramic radiographs. Most reports of multiple cystic lesions have described keratocystic odontogenic tumors (KOTs) in patients with basal cell nevus syndrome. In this study, we performed radiographic and clinical examinations of patients with multiple cystic jaw lesions found during their first visit to our department to determine the proportion of basal cell nevus syndrome among multiple cystic lesions. Methods. Patients who had a panoramic radiograph taken at Okayama University Hospital between 1993 and 2000 were examined. Twenty-three patients with multiple cystic jaw lesions on a first panoramic radiography examination were entered into this study. Results. Of the 23 patients, 12 (52.2%) were diagnosed with basal cell nevus syndrome and 11 (47.8%) were not. Of the 14 patients with multiple KOTs, 12 (85.7%) were diagnosed as having basal cell nevus syndrome. Of the nine remaining patients with multiple cystic jaw lesions, only seven had multiple dentigerous cysts and two had multiple simple bone cysts. Cleft palate was observed in four of the 12 patients (33.3%) with basal cell nevus syndrome. Conclusion. Diagnosing basal cell nevus syndrome based on multiple cystic jaw lesions carries a high risk of misdiagnosis. A strong possibility exists that patients with multiple jaw KOTs have basal cell nevus syndrome. Furthermore, cleft palate should be considered an important oral finding in basal cell nevus syndrome because its occurrence was relatively frequent in our study. [ABSTRACT FROM AUTHOR]

Florid cemento-osseous dysplasia (FCOD) with concomitant simple bone cysts is not common. We report a case of FCOD involving three quadrants of the jaws and associated with two large cystic lesions of the mandible in a 40-year-old Japanese woman. Microscopic examination revealed unencapsulated fibroblastic proliferation with formation of bone and cementum, showing different developmental stages, and cystic lesions resembling a simple bone cyst.
