Background: Tuberculous meningitis is a major public health issue, despite showing low incidence, tuberculous meningitis causes substantial mortality. For better clinical management, identification of prognostic factors is crucial to reduce health risk of Malaysian society. Therefore, the aim of this study was to determine the prognostic factors of adult tuberculous meningitis in peninsular Malaysia. Materials & methods: Initially, a retrospective cohort study and one-year of follow-up period was carried out. In addition, a total of 217 adult tuberculous meningitis patients treated or had follow-up in four tertiary hospitals in peninsular Malaysia were recruited. Cox proportional hazards regression was employed to perform multivariable analysis. Results: The overall survival probability of adult tuberculous meningitis was 36.8% with median survival time 244 days. Significant prognostic factors were Glasgow coma scale score (aHR=0.71, 95% CI=0.65, 0.76; p<0.001), HIV status (aHR=1.94, 95% confidence interval [CI]=1.19, 3.15; p=0.008), headache (aHR=0.48, 95% CI=0.31, 0.76; p=0.002) and meningeal enhancement (aHR=0.47, 95% CI=0.30, 0.74; p=0.001), nausea (aHR=2.21, 95% CI=1.33, 3.66; p=0.002), and vomit (aHR=0.58, 95% CI=0.36, 0.93; p=0.023). Conclusions: Evidently, the survival of among adults with tuberculous meningitis was low. Since HIV positive has a significant influence in mortality; early screening, diagnosis, and prompt treatment in this subgroup of patients play a key role in survival. [ABSTRACT FROM AUTHOR]

Substantial comorbidity exists between posttraumatic stress disorder and sleep disturbances/disorders. Such comorbidities are understudied in minority groups, including Asian Indians residing in countries outside India. Thus, we synthesized the existing literature specific to this group of Asian Indians to determine (a) prevalence estimates of posttraumatic stress disorder (PTSD) and sleep disturbances/disorders; and (b) PTSD-sleep comorbidity estimates. For this systematic review, we searched four databases (PubMed, PsycInfo, PTSDpubs, Web of Science) using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Of 3,796 screened articles, 9 articles (10 studies) met inclusion criteria. Study sample sizes ranged from 11 to 2,112 Asian Indians; studies were conducted in Singapore or Malaysia. No reviewed study examined PTSD. All studies examined sleep disturbances/disorders among Asian Indians; prevalence estimates were: 8.3% to 70.4% for short sleep duration, 2.0% to 22.9% for long sleep duration, 25.9% to 56.3% for poor sleep quality, 3.4% to 67.5% for insomnia diagnosis or probable insomnia, 7.7% for excessive daytime sleepiness, 3.8% to 54.6% for obstructive sleep apnea (OSA) diagnosis or high OSA risk, and 5.1% to 11.1% for sleep-disordered breathing. Specific to Asian Indians residing in countries outside India, this review advances PTSD-sleep literature by (a) suggesting substantial prevalence of sleep disturbances/disorders; (b) highlighting the need for culturally relevant sleep interventions; and (c) highlighting research gaps (e.g., no PTSD-focused research). Background: High-intensity focused ultrasound (HIFU) and monopolar radiofrequency (MRF) are common treatment modalities that have shown significant results in skin tightening. Nevertheless, the novel combination of these two treatments is new to the Malaysian landscape. Thus, this study aims to investigate the safety and efficacy of this novel combination treatment for the Malaysian population. Methods: This retrospective study included data on HIFU and MRF combination therapy for skin tightening collected from an aesthetic clinic in Johor Bahru, Malaysia from June 2018 to May 2021. Efficacy was assessed using the Global Aesthetic Improvement Scale (GAIS) and Glogau classification, while the safety of the treatment was analysed using pain scores and adverse events (AEs). Results: A total of 56 patients with a mean age of 47.7 years old (SD 10.00) were included in this study. The majority of the patients had Fitzpatrick skin types III and IV. Most of the patients were Chinese, followed by Malay, Indian and others. Most patients (96.4%) showed clinically significant improvement in skin tightening after treatment, with 15 patients scoring 1 (very much improved) and 39 scoring 2 (improved). All patients reported transient mild erythema, with no serious AEs, such as burn, swelling, numbness or muscle weakness. Among the patients, 80% reported a pain score of 5, while 10% reported pain scores of 4 and 6. Conclusion: Combining HIFU with MRF therapy improved GAIS scores by 96.4%, indicating a secure and efficient skin-tightening method. This study investigates how Malaysian Indian women perceived and internalised domestic violence in Malaysia. Domestic violence is linked to gender issues that have disproportionately affected women compared to men in society. In traditional societies, women were frequently socialised to be subordinate to men in terms of roles, responsibilities and positions. These setbacks have contributed to the domestic violence issue among women. Domestic violence is a prime example of exploitation and discrimination against women in various aspects of their lives. Women have typically been passive and resisting domestic violence experiences to protect their household, male partners, children and society. The data for this study were gathered through in-depth interviews with 24 Malaysian Indian women, who were recruited through the purposive sampling technique. The qualitative data was analysed with Nvivo11 software. Three themes were identified that reflect participants’ perceptions of domestic violence in Malaysia. First, participants saw domestic violence as a normalising aspect of marital life. Second, participants saw domestic violence as a serious problem that affects women and their rights. Third, participants saw domestic violence as a sign of male identity to protect women. This study offers gender perspectives, such as the impact of masculinities, the patriarchal system and unequal power relationships in the household. It also explains the complexities of women’s socialisation in traditional households, as well as enlightening various stakeholders in Malaysia on the perceptions of Malaysian Indian women on domestic violence. This paper traces the multi-dimensional aspects to South Indian coolie life in nineteenth and early twentieth century Malaya to interject into decades of scholarship that has trained on a homogenous focus on plantation fieldwork. In search of the nuances of diasporic diversity, I located three memoirs written by European planters of the Malayan plantocracy, set in rubber plantations and selected primarily for their reflections of South Indian coolie life from various socio-cultural angles. My reading of the three memoirs uncovered a host of various micro-narratives surrounding South Indians in the plantation, including references to cattle rearing, legacies of ancient ayurveda practices as well as the participatory communal network between coolie women as wage earners, wives, midwives and mothers. I conclude by arguing that such a host of eclectic tones can effectively bring other stories, to the surface and dismantle monotonous perceptions of South Indian coolie life in Malaya. X-chromosome short tandem repeats (X-STRs) are useful for human identification, especially in complex kinship scenarios. Since forensic statistical parameters vary among populations and the X-STRs population data for the diverse population of Peninsular Malaysia's are unavailable, this attempt for Indians (n = 201) appears forensically relevant to support the 12 X-STRs markers' evidential value for human identification in Malaysia. The Qiagen Investigator® Argus X-12 QS kit showed that DXS10135 was the most polymorphic locus with high genetic diversity, polymorphism information richness, heterozygosity, and exclusion power. Based on allele frequencies, the strength of discrimination and mean exclusion chance (MEC Krüger , MEC Kishida , MEC Desmarais , and MEC DesmaraisDuo) values for the Malaysian Indians were ≥0.999997790686228. As for haplotype frequencies, the overall discrimination power and mean exclusion probability (MEC Krüger , MEC Kishida , MEC Desmarais , and MEC DesmaraisDuo) were ≥0.9999984801951. The genetic distance, neighbor-joining phylogenetic tree, and principal component analysis also supported the evidential value of the 12 X-STRs markers for forensic practical caseworks in Malaysia. Introduction: Hereditary hemochromatosis (HH) is an autosomal recessive disorder that causes accumulation of iron in circulating blood and organs. The disease is associated with H63D, S65C and C282Y variants of the haemochromatosis (HFE) gene and, if not treated can cause organ damage and may prove fatal. The main objectives of the present survey were to screen these genetic variants and establish risk profiles for developing HH in Malays, Chinese and Indians. Methods: A total of two hundred and twenty-two unrelated and healthy individuals together representing Malay, Chinese and Indian ethnicities in Malaysia were scored for the H63D, S65C and C282Y variants using a polymerase chain reaction-restriction fragment length polymorphism technique. Results: There are clear differences in H63D, S65C and C282Y allele and genotype frequency distributions between Malays, Chinese and Indians. In particular, H63D is more common in Chinese (5.19%) and Indians (7.29%), while S65C is more common in Malays (1.03%) and Chinese (1.04%). In addition, a susceptibility genotype for HH (the compound heterozygote for C282Y and H63D) was only detected in Indians (0.02%). Conclusion: Overall, our study is the first to provide data on the prevalence of H63D, S65C, and C282Y genetic variants and HH risk profiles for Malays, Chinese and Indians. [ABSTRACT FROM AUTHOR]

We describe the clinical features of two sisters with Parkinson's disease (PD) of Indian descent living in Malaysia. Both were homozygous for the known PINK1 mutation p.Leu489Pro (c.1466T>C). The proband, who has been followed up by us over a span of 35 years, had a fairly "classic" clinical presentation for PARK-PINK1, including young onset, a clear response to dopamine replacement therapy, and development of troublesome motor fluctuations and dyskinesias. Her dyskinesias improved substantially with clozapine treatment, which to our knowledge has not specifically been reported for PARK-PINK1; this treatment has been sustained for nearly a decade. The clinical phenotype of the older sister was more akin to later-onset "idiopathic" PD; however, her brain MRI showed abnormal signal in the posterior limb of the internal capsules and the hypothalamus. Our report contributes to the scarce literature on monogenic PD in the Malaysian / Indian population, and further supports the pathogenicity of the PINK1 p.Leu489Pro variant. [ABSTRACT FROM AUTHOR]

This article brings together a compilation of poems, short stories and novels written by Malaysian Indian writers from 1940 to 2018. These verse and prose forms collected here were all written in English within the last 80 years. In fact, in the contemporary Malaysian literary scene, the number of works by Malaysian Indian writers writing in English, has increased compared to the previous volumes which were primarily dominated by writers such as Cecil Rajendra, K.S. Maniam and M. Shanmughalingam. It is interesting to note that the interest of writing poetry and prose among the younger Malaysian Indian writers is on the rise with publications from younger poets and writers such as Paul GnanaSelvam and Cheryl Ann Fernando. In Malaysia, poetry and prose written by Malaysian Indian writers in English is diversifying with many promising new talents. Departing from the Malaysian Indian diaspora, the emerging themes of these lesser-known writers are more liberal. As such, the main aim of this article is to map out the cartography of Malaysian Indian works in these three genres; poem, short story and novel, and identify the main thematic concerns in the writings. Malaysia, just like all the other developed countries, is steadily moving towards becoming an aged population. The United Nations sets those above 60 years as older persons, Doris, et. al. (1) reclassified into 4 categories. Chan et. al. (2) discloses that 50% of Malaysian elderly are inactive. This paper narrows into the elderly Malaysian Indian population. An online survey was conducted and the results show that changes in lifestyle and living arrangements have caused challenges and also caused changes in the social support system while the socioeconomic status (gender, education, income, strata) has an impact on the wellbeing of elderly Malaysian Indians. In general, gang can be described as a group of friends with identifiable leadership with certain internal organization that engage in various illegal activities such as crimes and drug trafficking. Although voluminous studies have been conducted in the West regarding gangs and gangsterism but very few studies were actually explored this issue in Malaysia. With this in mind, this study sought to provide some evidence for reasons behind decisions to join gangs as there is a lack of local qualitative research on the phenomenon of gangsterism. This lack is largely due to the vulnerability of people coming forward to share their experiences. The specific aim of this study was to explore demography and sociological factors behind gangsterism among Malaysian Indians. This study utilised a qualitative research design (semistructured interview) based on the inductive approach. Ex-gangsters and police personnel were interviewed individually. Thematic analysis was used to extract main themes and sub-themes. Three demography factors were identified: age, gender, and educational level. Four sociological factors were generated: poor parenting style, family members who were also gangsters, gang culture in the neighborhood, and financial difficulties. Two new factors emerged from the informants: social power and to experience gangster lifestyles. The outcome of this research is useful for proactive crime prevention in the form of evidence-based knowledge about demography and sociological factors behind gangsterism. One way of gauging identity construction in a community is by studying how the community organises itself into associations, the causes these associations fight for and the stands they take on issues pertaining to their communities' interests. The English-educated Indian elite in Malaysia began making efforts in the last quarter of the 19th century to organise themselves into socioeconomic and quasipolitical associations. However, no cohesive ethnic identity was visible even among this English-educated middle class until the mid-forties. The Indians at large remained as fragmented as ever. Ironically, they did not even have a truly national organisation with which they could identify themselves as an ethnic community. This situation began to change with the formation of the Malayan Indian Congress (MIC) in August 1946. The MIC was the first full-fledged political party of the Indian community in Malaysia, and it was formed to defend Indian interests. True to this objective, the MIC played a crucial role in defending the rights of the Indian community during the period soon after the Second World War. With that, the MIC inadvertently paved the way for the transition of Indian identity from being an immigrant community to being rightful citizens of Malaysia. An analysis of the MIC's role in and stance on various issues that affected the community from the time of its inception through the midfifties is imperative for understanding identity construction among Indians in Malaysia. An attempt is made here to study the various phases that the MIC went through during its political struggle as the representative of the Indian community and its influence on the transition of Indian identity from an immigrant community to citizens. This article examines the experience of political, social and cultural rights for Malaysia's ethnic minorities. Using a survey method, our research finds that while Malaysia has made substantial economic progress, lack of attention to political, social and cultural rights for ethnic minorities in Malaysia has meant that ethnic minorities have become increasingly dissatisfied with their experience of citizenship. Experiences of institutional and social discrimination within Malaysia's ethnically differentiated model of citizenship are common. Such experiences can lead to low levels of confidence in Malaysia's national institutions and have the capacity to undermine the kind of political support that is necessary for good governance and national stability. This article examines the strategies employed by the ruling coalition, Barisan Nasional (or National Front), and the opposition alliance, Pakatan Rakyat (People’s Alliance), in the 13th general election held in Malaysia in May 2013. It argues that while the opposition used the right strategy for the 2013 campaign, it lost because it could not overcome the three biggest hurdles for opposition politics in Malaysia: East Malaysia, the rural Malay votes and a biased electoral system. The Malays, Chinese and Indian community in Malaysia have been homogenized since British colonialism. The existence of Indian Muslims' identity caused a new paradigm shift in Malaysia involving the racial discussion. This paper traces the difference in Indian Muslims' identities from Indian and the Hindus. I argued that Indian Muslims share Islam as their religion and faith, while maintaining a Malay way of life and custom in their daily practices. In Malaysia, the Indian Muslim community struggled to place their future in terms of social, economic allocation and political justification among the other communities. However, the strength of ethnic politics clearly charted out their involvement in the political base and moved them to fight for their cause and rights. This article analyses the status of Indian Muslims in Malaysia from a historical perspective and its definition as a minority ethnic group. It also highlights the political reality of the Indian Muslims, particularly as a smaller and relatively insignificant minority group in comparison with the numerically larger Malays as well as the non-Muslim Chinese and Hindu Indians. It describes the social interactions of the various ethnic groups in Malaysia and the Indian Muslims as a minority fighting for their identity and survival. It discusses the “position” of the Indian Muslims with particular reference to Article 152 of the Malaysian Constitution which states that a Malay person is defined as one speaking the Malay language, practicing Malay customs, and following the religion of Islam. The great contributions of Indian Muslims are also elaborated. This inquiry highlights the reality facing the Indian Muslims in Malaysia today: they have no political power but remain a marginalized minority in the midst of Malay political domination and Chinese economic hegemony. Indeed they are facing the grim prospect of permanent bifurcation of their identity—some are slowly but surely being assimilated into the Malay cultural milieu, mainly through marriage and for political expediency, on the one hand and others stubbornly resist this cultural absorption, and resiliently retain and preserve their ethnic traditions and purity. Building regions often creates socio-economic imbalances that exclude low-income earners from the benefits of development. In Malaysia, an example of this is the territorial and cultural displacement of Indian plantation workers by rapid economic growth and urbanisation. Since 1980, nearly 300,000 Indians have been forced to move from the long-term relative security of their plantation estate communities, and have relocated into urban squatter settlements, when plantation land has been subdivided for housing and industrial estates. This article examines the loss of income, social role and cultural identity which accompanied the loss of physical place. A population-based cross-sectional study was conducted in all states of Malaysia with the aim to determine the prevalence of obesity among Malaysians aged fifteen years and above and factors associated. A stratified two-stage cluster sampling design with proportional allocation was used. Trained interviewers using a standardized protocol obtained the weight and height measurements and other relevant information. Subjects with a body mass index ≥ 30 kg/m² were labelled as obese. The results show that the overall national prevalence of obesity among Malaysians aged 15 years old and above was 11.7% (95% CI = 11.1 - 12.4%). The prevalence of obesity was significantly higher in females (13.8%) as compared to 9.6% in males (p< 0.0001). Prevalence of obesity was highest amongst the Malays (13.6%) and Indians (13.5%) followed by the indigenous group of "Sarawak Bumiputra" (10.8%) and the Chinese (8.5%). The indigenous group of "Sabah Bumiputra" had the lowest prevalence of 7.3%. These differences are statistically significant (p< 0.0001). Logistic regression analysis results show that there was a significant association between obesity and age, gender, ethnicity urban/rural status and smoking status. The prevalence of obesity amongst those aged ≥ 18 years old has markedly increased by 280% since the last National Health and Morbidity Survey in 1996. Conclusion: The overall prevalence of obesity in Malaysia is very high as compared to 1996. There is an urgent need for a comprehensive integrated population-based intervention program to ameliorate the growing problem of obesity in Malaysians. [ABSTRACT FROM AUTHOR]
