To examine whether maternal caffeine consumption is associated with the risk of spontaneous abortion, we analyzed data from a population-based prospective study. The study population comprised 575 women delivering singleton livebirths and 75 women who had spontaneous abortions. The subjects were predominantly white, middle-class women enrolled before pregnancy. Study participants were traced to delivery of a liveborn, singleton infant or a spontaneous abortion. Of the 71 women who did not experience nausea, 29.6% had a spontaneous abortion, compared with 7.2% of 514 women who did experience nausea. Maternal caffeine consumption before pregnancy, or in women without nausea, did not increase the risk of spontaneous abortion, whereas maternal caffeine consumption during the first trimester after nausea started might increase risk of spontaneous abortion (risk ratio = 5.4, 95% confidence interval = 2.0-14.6 for caffeine consumption > or = 300 mg per day compared with < 20 mg per day). These results suggest that maternal caffeine consumption during pregnancy may influence fetal viability in women with nausea.

Preterm delivery, low birthweight, and intrauterine growth retardation are common and recalcitrant problems in many countries. Although much remains to be learned, we know particularly little about the relationships between preconceptional and early pregnancy exposures and these and other reproductive outcomes. The study presented here was primarily designed to investigate the relationships between nutritional exposures measured before and during pregnancy and reproductive outcomes. This paper describes methods used to recruit the required 1000 preconceptional women from the collaborating health maintenance organisation (HMO) and the retention of participants in this time-intensive study. The results presented demonstrate that an adequate completion rate (66.3%), and a remarkably representative sample of women that poses few threats to the study's validity, can be obtained by population-based recruitment of women from an HMO.

Location of body fat stores, as indicated by waist-to-hip circumference ratio (WHR), affects a variety of metabolic processes in women, and some of these changes could affect fetal growth during pregnancy. We tested the hypothesis that WHR affects fetal growth among 702 participants of the Diana Project, a prospective study designed to identify preconceptual exposures related to reproductive outcomes. We tested the effect of maternal WHR on the outcomes of infant birthweight, length, and head circumference in regressional models that included 16 variables such as maternal body mass index, duration of gestation, and pregnancy weight gain previously related to birthweight. Maternal WHR was related to each measure of newborn size. A 0.1-unit increase in WHR predicts a 120-gm greater birthweight, a 0.2-inch greater length, and a 0.3-cm greater head circumference. We conclude that WHR is related to fetal growth and that the effect of WHR on fetal growth may be mediated by metabolic alterations associated with a preponderance of central body fat stores or to other factors closely aligned with WHR. The common finding of an independent effect of pregnancy BMI on birthweight may be largely attributable to maternal WHR.

Infant feeding was examined in 492 children in a population-based survey conducted in a low-income, urban county of St Paul, Minn. Of 41 Southeast Asian infants who were foreign born, 93% (38) had been breast-fed compared with 10% (12) of Southeast Asian infants born in the United States (n = 116). Among non-Southeast Asian infants, 73% (173) of whites (n = 237), 63% (27) of blacks (n = 43) and 65% (36) of other ethnic groups (n = 55) had been breast-fed. Among the non-Southeast Asian infants, the initiation of breast-feeding was associated with higher parental education and with being married. Ethnic group, level of poverty, and participation in the Supplemental Food Program for Women, Infants, and Children during pregnancy did not appear to influence the initiation of breast-feeding. The findings indicate a higher incidence of breast-feeding than in previous surveys of low-income black and white women; however, this may reflect the higher educational level of the non-Southeast Asian study population. In contrast, the sharp decline in the incidence of breast-feeding among Southeast Asian infants who were born in the United States compared with those who were foreign born indicates the need for public health approaches to strengthen traditional breast-feeding practices.

Prepregnancy weight status and weight gain during pregnancy are major independent variables associated with infant birth weight. This study quantitated the influence of weight gain on birth weight and identified rates and total amounts of weight gain related to the birth of healthy-sized infants to healthy low-income women who entered pregnancy underweight, at normal weight, overweight, or obese. Data used in the study were obtained from randomly sampled prenatal health records from Maternal and Infant Care (MIC) projects in Cleveland and Minneapolis. Subsamples of healthy mothers who delivered healthy-sized infants were identified from each sample, and rates and total amounts of weight gain by prepregnancy weight status group were calculated. There were 384 healthy mother and healthy-sized infant pairs in the Cleveland subsample and 75 such pairs in the Minneapolis sample. Multiple regression analysis revealed that the influence of prenatal weight gain and birth weight varied depending on prepregnancy weight status. Prenatal weight gains related to the birth of healthy-sized infants (newborns with birth weights of 3,000 to 4,500 gm) to healthy mothers in the Cleveland MIC sample averaged 33 lb for underweight, 32 lb for normal weight, 29 lb for overweight, and 19 lb for obese women. Except for obese women, rates and total amount of weight gain associated with the birth of healthy-sized infants were equivalent for the two samples.

A nutrition assessment survey was undertaken among 566 preschool children from randomly selected low income households located within an urban county. The purpose of the survey was to determine the prevalence of selected nutrition problems and to identify relationships among ethnic and economic variables and nutritional status. Underweight was identified in 1.8%, overweight in 15.4%, and short stature in 13.1% of children. Southeast Asian children had a higher prevalence of short stature than the other ethnic groups. Low hemoglobin values were identified in 4.3%, low hematocrit in 6.9%, and elevated erythrocyte protoporphyrin in 6.0% of children. The prevalence of elevated EP was significantly higher among Southeast Asian children than non-Southeast Asians and likely was due to iron deficiency. Results of the survey suggest that Southeast Asian children constitute the group at highest nutritional risk in the low income areas surveyed.
