Abstract

Objectives: The objective of this study was to determine time trends in the rate of Gram-positive cocci in 42 Dutch ICUs that continuously used or did not use selective oropharyngeal decontamination (SOD) or selective decontamination of the digestive tract (SDD) and ICUs that introduced SOD/SDD.Methods: The Dutch Surveillance System on Antibiotic Resistance was used to determine monthly rates of Staphylococcus aureus, Enterococcus faecalis and Enterococcus faecium isolates, including resistant phenotypes, in blood and respiratory tract specimens from 2008 to 2013. Per patient, the last isolate per species per month was selected, and cumulative rates per 100 beds per month were determined. Time trends were analysed by multilevel Poisson regression.Results: Eighteen ICUs used SOD/SDD (1296 months), 13 did not use SOD/SDD (936 months) and 11 introduced SOD/SDD (373 months before and 419 months after introduction). There was no significant increase in the rate of Gram-positive cocci in ICUs that used SOD/SDD. Introduction of SOD/SDD was associated with increased rates of S. aureus (β = 0.018, 95% CI 0.006-0.030) and E. faecalis isolates (β = 0.028, 95% CI 0.006-0.051) in respiratory tract specimens. Numbers of resistant phenotypes remained low, and an observed increase in E. faecium isolates (β = 0.033, 95% CI 0.012-0.054), 97% of which were resistant to ampicillin, in the absence of SOD/SDD disappeared after the introduction of SOD/SDD.Conclusions: In ICUs with a low endemicity of resistance, introduction of SOD/SDD was associated with increased rates of S. aureus and E. faecalis isolates, but not with resistant phenotypes. Abstract

Summary: Background: Previously, we assessed selective digestive tract decontamination (SDD) and selective oropharyngeal decontamination (SOD) on survival and prevention of bacteraemia in patients in intensive-care units. In this analysis, we aimed to assess effectiveness of these interventions for prevention of respiratory tract colonisation and bacteraemia with highly resistant microorganisms acquired in intensive-care units. Methods: We did an open-label, clustered group-randomised, crossover study in 13 intensive-care units in the Netherlands between May, 2004, and July, 2006. Participants admitted to intensive-care units with an expected duration of mechanical ventilation of more than 48 h or an expected stay of more than 72 h received SOD (topical tobramycin, colistin, and amphotericin B in the oropharynx), SDD (SOD antibiotics in the oropharynx and stomach plus 4 days'' intravenous cefotaxime), or standard care. The computer-randomised order of study regimens was applied by an independent clinical pharmacist who was masked to intensive-care-unit identity. We calculated crude odds ratios (95% CI) for rates of bacteraemia or respiratory tract colonisation with highly resistant microorganisms in patients who stayed in intensive-care units for more than 3 days (ie, acquired infection). This trial is registered at http://isrctn.org, number ISRCTN35176830. Findings: Data were available for 5927 (>99%) of 5939 patients, of whom 5463 (92%) were in intensive-care units for more than 3 days. 239 (13%) of 1837 patients in standard care acquired bacteraemia after 3 days, compared with 158 (9%) of 1758 in SOD (odds ratio 0·66, 95% CI 0·53–0·82), and 124 (7%) of 1868 in SDD (0·48, 0·38–0·60). Eight patients acquired bacteraemia with highly resistant microorganisms during SDD, compared with 18 patients (with 19 episodes) during standard care (0·41, 0·18–0·94; rate reduction [RR] 59%, absolute risk reduction [ARR] 0·6%) and 20 during SOD (0·37, 0·16–0·85; RR 63%, ARR 0·7%). Of the patients staying in intensive-care units for more than 3 days, we obtained endotracheal aspirate cultures for 881 (49%) patients receiving standard care, 886 (50%) receiving SOD, and 828 (44%) receiving SDD. 128 (15%) patients acquired respiratory tract colonisation with highly resistant microorganisms during standard care, compared with 74 (8%) during SDD (0·58, 0·43–0·78; RR 38%, ARR 5·5%) and 88 (10%) during SOD (0·65, 0·49–0·87; RR 32%, ARR 4·6%). Acquired respiratory tract colonisation with Gram-negative bacteria or cefotaxime-resistant and colistin-resistant pathogens was lowest during SDD. Abstract

Objective. General practitioners (GPs) are the first-line physicians who are consulted for upper digestive symptoms. Persons with symptoms may, however, prefer to buy acid inhibitors or antacids in drugstores or pharmacies and bypass a GP. The aim of this work was to study users, reasons for use, and utilization patterns of over-the-counter (OTC) acid inhibitors and antacids in The Netherlands. We also studied factors that were associated with the substitution of OTC acid inhibitors or antacid use for consultation with a GP. Material and methods. From July 2005 to January 2006, persons buying OTC acid inhibitors or antacids in 12 pharmacies and 4 drugstores were asked to complete a questionnaire. A total of 82/160 (51%) questionnaires were returned. Results. Heartburn was the main symptom for buying an acid inhibitor or antacid. Seventy-one (87%) participants substituted OTC drug use for a GP consultation. The most commonly reported reason was the belief that symptoms were not serious enough to seek medical care. Exploratory analyses showed that substitution was less common in participants with comorbidity, a history of upper gastrointestinal disorder, use of an acid inhibitor or antacid previously prescribed by a physician, alarm symptoms (such as pain and nausea), and with being symptomatic for >4 days/week. Conclusions. Although the reasons for substitution of OTC acid inhibitor or antacid use for a GP consultation in The Netherlands do not suggest an a priori increased risk of an underlying serious disorder, it may be advisable for staff in drugstores and pharmacies to provide users with information on appropriate use and when to consult a GP. Many different cultures all over the world have used the ANISETTE STAR to diminish children's stomach ache, however countries like France, Netherlands, Spain and USA are imposing severe restrictions for its commercialization due to the new knowledge of its complex chemical composition as we can see in several official documents. According to the observation of different clinical and laboratory reports, a strong relation has been found between the consumption of ANISETTE STAR and the appearance of digestive-neurological disease. However, not everything is against this plant, since there's much research that demonstrates its utility to counteract the effects of baldness or the production of insecticides, what we urgently recommend is the regulation of its use. Abstract

Objective: Although ample evidence indicates that child health is compromised by early adversity (e.g., abuse and poverty), less is known about the contribution of parenting in low-stress contexts to child health, especially in infancy. This longitudinal study extends previous research on early adversity to ask the question: Does quality of parental care predict infant health in a low-risk community sample?, Method: Participants were 187 healthy mothers and their full-term infants (86 girls) from the Netherlands, followed from birth to age 1. Home observations of mothers' behavior were conducted during a naturalistic task (bathing session) when infants were 5 weeks old. Trained researchers interviewed mothers about the infants' health and prescribed antibiotic use every month for 12 months. Infant health problems were categorized into 4 domains according to the International Classification of Primary Care to capture a range of outcomes: respiratory, digestive, skin, and general illnesses and symptoms., Results: Controlling for health-related covariates (e.g., maternal smoking and breastfeeding), maternal sensitivity predicted reduced rates of infant respiratory symptoms and skin conditions and marginally lower prescribed antibiotic use over the first year. Maternal behavior was unrelated to infant digestive and general illnesses., Conclusion: Even in the absence of adversity, quality of maternal care may have implications for the development of physical health, beginning as early as the first year of life. That such findings emerge in a low-risk sample helps rule out potential confounders and underscores the importance of parenting for physical and psychological health outcomes.

The article reports that gluten-intolerant people might one day be able to eat wheat-based bread and pasta without getting ill. Frits Koning and his colleagues at Leiden University Medical Center in the Netherlands have developed two tests to screen wheat for different types of gluten, the gum-like proteins that aggravate gut inflammation in people with coeliac disease. Coeliac disease affects around 1 in 200 people in the west, where it is the most common food intolerance.

The article reports on the creation of the programmable capsule Intelligent Pill (iPill) by Philips Electronics in the Netherlands. The capsule can be taken with food or water, target specific troubled body parts and dispense its medicine. iPill is being tested in animals in the U.S. and could aid in the treatment of digestive tract diseases such as Crohn's disease and colitis. One-third of the size of iPill consists of medicine and two-thirds are microprocessor, battery and antenna.
