BACKGROUND: Although recommended for almost a decade, evidence for field effectiveness of vaccinating close adult contacts of newborn infants against pertussis ("cocooning” ) is lacking. We evaluated the impact of a government-funded cocoon program during a pertussis epidemic in New South Wales, Australia. METHODS: We matched all New South Wales laboratory-confirmed pertussis cases aged < 4 months with onset between April 1, 2009, to March 30, 2011 to controls from the state birth register by date of birth and area of residence. Parental vaccine receipt was by self-report, with a subset verified. Parents were considered “ immunized” if vaccinated ≥ 4 weeks before case symptom onset. The effectiveness of parental immunization (versus neither vaccinated) was quantified as (1 - odds ratio) X 100%. RESULTS: Case households had fewer immunized mothers (22% vs 32%) or fathers (20% vs 31%) but were more likely to include additional and older children. After adjustment, when both parents met our definition of immunized, risk of pertussis at< 4 months of age was reduced by 51% (95% confidence interval 10% to 73%). Maternal vaccination prepregnancy and an immunized father reduced the risk by 51% (95% confidence interval 0% to 76%). CONCLUSIONS: Timely parental pertussis boosters provided significant protection. Evidence of protection from maternal vaccination pre-pregnancy is biologically plausible, and more precise data on the magnitude and duration of this is important for future policy recommendations. Among older adults, the risk of hospitalization for pertussis increases with increasing age. Obesity and preexisting asthma also increase the likelihood of disease presentation. Prioritizing adults in these categories for pertussis vaccination may provide greater public health benefit.Background. There is limited information on the incidence, morbidity and risk factors for pertussis in adults, particularly those aged over 65 years.Methods. Population-based prospective cohort study of 263 094 adults aged over 45 years (mean 62.8 years) recruited in the Australian state of New South Wales (the 45 and Up Study) between 2006 and 2008, and followed by record-linkage to laboratory-confirmed pertussis notifications, hospitalizations, and death records. The incidence of pertussis notifications and hospitalizations and relative risk (RR) of pertussis according to various participant characteristics was estimated using proportional hazards models.Results. Over a total follow-up of 217 524 person-years, 205 adults had a pertussis notification and 12 were hospitalized; the incidence rate was 94 (95% confidence interval [CI], 82–108) and 5.5 (95% CI, 3.1–9.7) per 100 000 person-years, respectively. The incidence of a pertussis notification did not differ by age but hospitalization rates progressively increased (2.2, 8.5, and 13.5 per 100 000 person-years in age groups 45–64, 65–74, and 75+ years, respectively; Ptrend = .01). After adjusting for age, sex, and other factors, adults with a high body mass index (BMI; RR = 1.52; 95% CI, 1.06–2.19 for BMI 30+ kg/m2 vs BMI <25 kg/m2) and with preexisting asthma (RR = 1.64; 95% CI, 1.06–2.55 compared to those without asthma) were more likely to be notified.Conclusions. Adults older than 65 years are more likely to be hospitalized for pertussis than those aged 45–64 years. Obesity and preexisting asthma were associated with a higher likelihood of pertussis notification. Background: Australia's Aboriginal and Torres Strait Islander (Indigenous) populations have disproportionately high rates of adverse perinatal outcomes relative to other Australians. Poorer access to good quality maternal health care is a key driver of this disparity. The aim of this study was to describe patterns of delivery of maternity care and service gaps in primary care services in Australian Indigenous communities. Methods: We undertook a cross-sectional baseline audit for a quality improvement intervention. Medical records of 535 women from 34 Indigenous community health centres in five regions (Top End of Northern Territory 13, Central Australia 2, Far West New South Wales 6, Western Australia 9, and North Queensland 4) were audited. The main outcome measures included: adherence to recommended protocols and procedures in the antenatal and postnatal periods including: clinical, laboratory and ultrasound investigations; screening for gestational diabetes and Group B Streptococcus; brief intervention/advice on health-related behaviours and risks; and follow up of identified health problems. Results: The proportion of women presenting for their first antenatal visit in the first trimester ranged from 34% to 49% between regions; consequently, documentation of care early in pregnancy was poor. Overall, documentation of routine antenatal investigations and brief interventions/advice regarding health behaviours varied, and generally indicated that these services were underutilised. For example, 46% of known smokers received smoking cessation advice/counselling; 52% of all women received antenatal education and 51% had investigation for gestational diabetes. Overall, there was relatively good documentation of follow up of identified problems related to hypertension or diabetes, with over 70% of identified women being referred to a GP/Obstetrician. Conclusion: Participating services had both strengths and weaknesses in the delivery of maternal health care. Increasing access to evidence-based screening and health information (most notably around smoking cessation) were consistently identified as opportunities for improvement across services. The article discusses research on the assessment of maternal health care and medical services in several Indigenous primary health centres in various regions in Australia including North Queensland and Far West New South Wales. Variation in the documentation of routine antenatal investigations and health interventions is noted. Follow-up medical data of problems relating to hypertension or diabetes are described.

BACKGROUND: Rotavirus vaccine has been funded for infants under the Australian National Immunisation Program since 2007, with Rotarix vaccine used in New South Wales, Australia, from that time. In 2017, New South Wales experienced a large outbreak of rotavirus gastroenteritis. We examined epidemiology, genotypic profiles, and vaccine effectiveness (VE) among cases. METHODS: Laboratory-confirmed cases of rotavirus notified in New South Wales between January 1, 2010 and December 31, 2017 were analyzed. VE was estimated in children via a case-control analysis. Specimens from a sample of hospitalized case patients were genotyped and analyzed. RESULTS: In 2017, 2319 rotavirus cases were reported, representing a 3.1-fold increase on the 2016 notification rate. The highest rate was among children aged,2 years. For notified cases in 2017, 2-dose VE estimates were 88.4%, 83.7%, and 78.7% in those aged 6 to 11 months, 1 to 3 years, and 4 to 9 years, respectively. VE was significantly reduced from 89.5% within 1 year of vaccination to 77.0% at 5 to 10 years postvaccination. Equinelike G3P[8] (48%) and G8P[8] (23%) were identified as the most common genotypes in case patients aged $6 months. CONCLUSIONS: Rotarix is highly effective at preventing laboratory-confirmed rotavirus in Australia, especially in infants aged 6 to 11 months. Reduced VE in older age groups and over time suggests waning protection, possibly related to the absence of subclinical immune boosting from continuously circulating virus. G8 genotypes have not been common in Australia, and their emergence, along with equinelike G3P[8], may be related to vaccine-induced selective pressure; however, further strain-specific VE studies are needed. Objective: To assess changes in diagnostic practice and vaccine schedules for pertussis, we used culture-confirmation and clinical severity to compare pertussis cases at a single Australian tertiary pediatric hospital during relevant periods., Study Design: We replicated the case ascertainment methods of a study reporting a 2-year epidemic period 1997-1999 (whole cell pertussis vaccine with 18-month booster, only culture available) to conduct a retrospective cross-sectional observational study over a 6-year period 2007-2012 (acellular pertussis vaccine, no 18-month booster, polymerase chain reaction and culture available). Cases were compared from case note review 2007-2012 (including prevalence of comorbidities) and published data 1997-1999., Results: During 2007-2012, average annual hospitalizations in those aged < 6 months increased 2.3-fold (32.0 vs 14.0) and in those aged > 6 months by 5.1-fold (17.7 vs 3.5). Limited to culture-positive hospitalizations, there was no increase in those aged < 6 months (14.0 vs 14.5) contrasted with a 4.6-fold increase in those aged > 6 months (2.3 vs 0.5), despite increased annual culture requests (488 vs 188). In 2007-2012, significant comorbidities were documented in 41/72 (57%) hospitalized children aged ≥ 12 months vs 38/225 (17%) <12 months (OR 6.5, 95% CI 3.7-11.7)., Conclusions: Increased cases of culture-positive hospitalized pertussis were limited to fully immunized children > 6 months of age, consistent with schedule changes. Background: The availability of new pneumococcal conjugate vaccines covering a broader range of serotypes, has seen many countries introduce these into their national immunisation program. When transitioning from 7-valent to 13-valent pneumococcal conjugate vaccines, Australia is one of a small number of countries that included a supplementary dose of the 13-valent pneumococcal conjugate vaccine to offer protection against additional serotypes to an expanded age group of children. An evaluation of the implementation and uptake of the 13-valent pneumococcal conjugate vaccine supplementary dose was undertaken in two local health districts (LHDs) in New South Wales, Australia., Methods: A self-administered postal survey of immunisation providers in the Northern New South Wales and Mid North Coast LHDs. Trends in vaccine ordering were examined. Coverage was assessed using data from the Australian Childhood Immunisation Register (ACIR)., Results: Of the 177 surveys sent, 125 were returned (70%). Almost all providers (96%) were aware of the 13vPCV supplementary dose program though took an opportunistic approach to program promotion and parental reminders. Supplementary doses of 13vPCV were ordered for 37% of the eligible cohort, mostly in the program's first six months. Coverage as recorded on the ACIR was 27%, though was lower in older children and those not due for scheduled childhood vaccines. Of the children who received the 13vPCV supplementary dose, 3% received it at the same time as vaccines due at 12-months of age, and 44% at the time of those due at 18-months of age., Conclusion: Despite the high awareness of the program, reported coverage was lower than that for other PCV supplementary dose programs in Australia and internationally. This may be influenced by providers' largely opportunistic approach to implementation, under-reporting to the ACIR or vaccine uptake. Lessons learned from this evaluation are relevant for future time-limited childhood vaccination programs. Prior to commencement, providers should be informed about the importance of catch-up/supplementary vaccination for their patients and their active role in promoting this. They should also receive program information before parents. An understanding of parental reasons for non-receipt of time-limited childhood vaccines and evaluation of the effect of aligning supplementary (or catch up) vaccination programs with the NIP schedule would be useful to inform future programs.

Background: Surveillance for intussusception (IS) has been recommended in countries using rotavirus vaccine, but can be resource intensive. There is little data about the relative severity of rotavirus vaccine-associated IS compared with other IS cases. We collected detailed clinical data on all cases to evaluate the validity of ICD coding for IS in routinely collected data and case severity., Methods: Hospitalizations and emergency department presentations coded as IS in infants aged <12 months from July 1, 2007, to June 30, 2010, were classified using Brighton criteria by case note review. We used self-controlled case series analysis to estimate IS risk after vaccination for all and only Brighton level 1 cases., Results: Of 179 unique episodes coded as IS, 110 (61%) met Brighton level 1 criteria; self-controlled case series analysis found a relative incidence of IS in days 1-7 after the first dose of RV1 of 11.1 (95% confidence interval: 2.6-48.0). When all coded episodes of IS were included, relative incidence was 4.0 (95% confidence interval: 1.3-12.7). The proportion of Brighton 1 cases requiring surgery was 39% for those within 21 days of vaccine receipt and 34% for others (P = 0.67)., Conclusions: Using ICD-coded cases without individual confirmation yielded a lower point estimate of risk for IS post rotavirus vaccination; however, the risk remained statistically compatible with that for chart confirmed cases only. Analysis using healthcare databases to evaluate risk of IS if conducted without case confirmation may be insufficient to confirm a low-level risk. IS episodes after vaccination were not more severe.

The pertussis epidemic experienced in NSW in 2008-2009 was likely to be in part due to changes in diagnostic practice since 2007, which amplified disease notifications. We used population-based seroepidemiology as a less biased means of interpreting age-specific pertussis infection patterns in NSW from three serosurveys undertaken in 1997-98 (during an epidemic), 2002 (post-epidemic) and 2007 (inter-epidemic), using a standardised pertussis toxin IgG enzyme-linked immunosorbent assay (ELISA). There was a decrease in the proportion of high anti-pertussis toxin IgG titres (>62.5ELISAUnits/mL) across all age groups in the 2007 serosurvey compared to the previous two serosurveys. In the 2007 serosurvey, the proportion of undetectable (<5ELISAUnits/mL) anti-pertussis toxin IgG titres increased in many age groups. The seroepidemiological profiles of the three serosurveys demonstrate fluctuating immunity profiles related to changes in vaccination schedules.

Over the past decade the number of recommended and funded vaccines for adolescents has increased, becoming a substantial part of the National Immunisation Program in Australia. In response, NSW has implemented disease-specific vaccination campaigns for both children and adolescents and more recently established a routine high school-based vaccination program to administer vaccines to this often hard to reach group. This paper outlines the history of school-based vaccination in NSW from its commencement in 1971 to coverage from early disease-specific programs, and describes the implementation of the current program of routine vaccination. Substantial coverage has been achieved across the age spectrum 5-17 years, highlighting the effectiveness of the school-based vaccination program in reaching large numbers of adolescents.

In 2008 and 2009 increased numbers of pertussis notifications were reported in NSW. During the epidemic period, the pertussis notification rate was 2.7 times higher than the previous 5-year average. Rates of pertussis notifications and hospitalisations were highest among infants aged less than 1 year across all years studied. Compared to previous years, the notification rate for children aged 1-4 years increased dramatically and was particularly striking for children aged 3 years with notifications exceeding those for infants in 2009. Changes in testing practices during the epidemic period, including a significant increase in the use of polymerase chain reaction, may account for some of the relative increase in size of the 2008-2009 outbreak compared with previous outbreak years.

Vaccine effectiveness in the first large-scale use of adolescent pertussis vaccine in Australia was evaluated by the screening method. Vaccine effectiveness was 78.0% (95% CI: 60.7-87.6%) for all study cases (n = 167), increasing to 85.4% (95% CI: 83.0-87.5%) for laboratory-confirmed cases (n = 155). Effectiveness should be comparable in settings with similar programs, such as the United States and Canada.
