The influence of variables that might affect the accuracy of pulse oximetry (SpO2) recordings in critically ill patients is not well established. We sought to describe the relationship between paired SpO2/SaO2 (oxygen saturation via arterial blood gas analysis) in adult intensive care unit (ICU) patients and to describe the diagnostic performance of SpO2 in detecting low SaO2 and PaO2. A paired SpO2/SaO2 measurement was obtained from 404 adults in ICU. Measurements were used to calculate bias, precision, and limits of agreement. Associations between bias and variables including vasopressor and inotrope use, capillary refill time, hand temperature, pulse pressure, body temperature, oximeter model, and skin colour were estimated. There was no overall statistically significant bias in paired SpO2/SaO2 measurements; observed limits of agreement were +/-4.4%. However, body temperature, oximeter model, and skin colour, were statistically significantly associated with the degree of bias. SpO2 <89% had a sensitivity of 3/7 (42.9%; 95% confidence intervals, CI, 9.9% to 81.6%) and a specificity of 344/384 (89.6%; 95% CI 86.1% to 92.5%) for detecting SaO2 <89%. The absence of statistically significant bias in paired SpO2/SaO2 in adult ICU patients provides support for the use of pulse oximetry to titrate oxygen therapy. However, SpO2 recordings alone should be used cautiously when SaO2 recordings of 4.4% higher or lower than the observed SpO2 would be of concern. Shape, size and orientation measurements of quartz grains sampled along two transects that cross zones of increasing metamorphic grade in the Otago Schist, New Zealand, reveal the role of quartz in the progressive development of metamorphic foliation. Sedimentary compaction and diagenesis contributed little to the formation of a shape-preferred orientation (SPO) within the analysed samples. Metamorphic foliation was initiated at sub-greenschist facies conditions as part of a composite S1-bedding structure parallel to the axial planes of tight to isoclinal F1 folds. An important component of this foliation is a pronounced quartz SPO that formed dominantly by the effect of dissolution–precipitation creep on detrital grains in association with F1 strain. With increasing grade, the following trends are evident from the SPO data: (i) a progressive increase in the aspect ratio of grains in sections parallel to lineation, and the development of blade-shaped grains; (ii) the early development of a strong shape preferred orientation so that blade lengths define the linear aspect of the foliation (lineation) and the intermediate axes of the blades define a partial girdle about the lineation; (iii) a slight thinning and reduction in volume of grains in the one transect; and (iv) an actual increase in thickness and volume in the survivor grains of the second transect. The highest-grade samples, within the chlorite zone of the greenschist facies, record segregation into quartz- and mica-rich layers. This segregation resulted largely from F2 crenulation and marks a key change in the distribution, deformation and SPO of the quartz grains. The contribution of quartz SPO to defining the foliation lessens as the previously discrete and aligned detrital quartz grains are replaced by aggregates and layers of dynamically recrystallized quartz grains of reduced aspect ratio and reduced alignment. Pressure solution now affects the margins of quartz-rich layers rather than individual grains. In higher-grade samples, therefore, the rock structure is characterized increasingly by segregation layering parallel to a foliation defined predominantly by mica SPO. [ABSTRACT FROM AUTHOR]

Honey from New Zealand and Saudi Arabia at concentrations approximating 20% (v/v) inhibit the growth of H. pylori in vitro. The anti-H. pylori effect involves both hydrogen peroxide- and non-peroxide-mediated killing mechanisms. This study was designed to determine whether the anti-H. pylori activity of honey differed regionally (honey from Texas, Iowa, and New Zealand) and to determine whether this activity was due to the presence of hydrogen peroxide. Broth dilution susceptibility tests were performed using solutions of honey prepared in BHI broth ranging in concentration from 5 to 35% (v/v) in 5% increments. Control solutions containing glucose, fructose, and combined glucose/fructose solutions in ratios of 1:1.23 were also prepared. Paired catalase controls were included in all tests. Twenty-eight clinical isolates of H. pylori were tested. Growth was determined on the basis of a plus/minus grading score. All of the solutions containing either fructose, glucose, glucose and fructose combinations, or honey were equally effective in inhibiting the growth of H. pylori. All of the isolates were inhibited by solutions containing 15% (w/v) carbohydrate. Honey solutions, with or without catalase, inhibited 24/28 isolates at a concentration of 10%, and 28/28 isolates at a concentration of 15%. In conclusion, regional differences in honey activity against H. pylori were not detected, nor was the effect of killing related to the presence of hydrogen peroxide in the honey samples. Osmotic effects were shown to be the most important parameter for killing H. pylori as all carbohydrate solutions > or = 15% (v/v) inhibited 100% of the H. pylori. [ABSTRACT FROM AUTHOR]

Objectives: The aim of this project was to evaluate the perceptions, knowledge and attitudes regarding generic medicines., Methods: A cross-sectional study, with self administered questionnaires, was conducted to survey consumers visiting pharmacies in four regions of Auckland (North Shore, Waitakere, Central Auckland and South Auckland). Through stratified random sampling, approximately 10% of pharmacies from each region were selected, which turn out to be 30 pharmacies. Every alternate customer coming to the pharmacy, who was eligible to participate in the study, was asked by the researchers to complete the questionnaire., Results: A total of 441 questionnaires were included in the analysis. Different response rates were obtained in different regions of Auckland. Of all respondents, 51.6% had previous knowledge of generic medicines. Pharmacists were the main source of information regarding generic medicines followed by doctors and media. A higher level of education had a direct relationship with having correct knowledge of generics (P = .002). Attitude of participants toward the use of generic medicines was determined by their knowledge of generics, whether it was recommended by a pharmacist and their type of illness. Participants were more prepared to change to a generic for a minor illness (79%) than for a major illness (58.7%). Those who had better knowledge were more likely than those with poor knowledge to say they would to use a generic in major illness (P = .001) as well as minor illness (P < .0001). Previous positive experiences with generics also determined consumers' willingness to use generics., Conclusion: Many consumers have misconceptions regarding generic medicines. Having knowledge about generics and the advice by doctors and pharmacists are key indicators to improve the quality use of generic medicines.
