Dear Editor, Spider venom dermonecrosis is very rare in northern Italy, with only one case described in the literature.1 We report a case of cutaneous loxoscelism2 caused by a violin spider's bite that occurred in Bolonia, in the middle of the Po Valley. At the follow-up visit, the patient had developed fever, unilateral axillary and cervical lymphadenopathy, and a worsening pain in the arm. The standard therapies of cutaneous loxoscelism aim to reduce the local inflammation, prevent infections, manage possible systemic reactions, and achieve prompt wound healing.5 We added paracetamol IV (1000 mg) two times a day for 5 days and betamethasone IV 4 mg/ml once a day for 5 days to the patient's existent therapy.
