This year marks 30 years of close collaboration between a consortium of institutions, namely, the Northwest Public Health Research Center, Saint-Petersburg (NWPHRC); the Institute of Community Medicine (ICM) of UiT (The Arctic University of Norway, Tromsø); the National Institute of Occupational Health (NIOH), Oslo; the University Hospital of Northern Norway (UNN) at Tromsø; and McMaster University (MU), Hamilton, ON, Canada. During the early years of cooperation, Dr. Chashchin was the Director of the Scientific Laboratory of the North-West Public Health Centre Branch of the NWPHRC located in the town of Kirovks in the Murmansk Region. The primary focus of this long-standing collaboration was to assess and address issues that are important for maintaining the health of the population living in the border areas of Russia and Norway and included the reduction of occupational health risks among workers employed in mining, metallurgical and machine-building enterprises located in the north-western region of Norway and the adjoining Kola Peninsula in Russia. These industrial activities constituted essential components of the local industries. The ongoing Russian-Norwegian cooperation in the field of occupational medicine is an excellent example of the effective combination of intellectual potential and research technologies of multiple countries. It has resulted in the development of a scientifically based set of measures for practical implementation, contributing to the improvement of working conditions and preservation of the health of workers employed at enterprises where the joint research was carried out.

Background: Although predictors of contaminants in serum or whole blood are usually examined by chemical groups (e.g., POPs, toxic and/or essential elements; dietary sources), principal component analysis (PCA) permits consideration of both individual substances and combined variables., Objectives: Our study had two primary objectives: (i) Characterize the sources and predictors of a suite of eight PCBs, four organochlorine (OC) pesticides, five essential and five toxic elements in serum and/or whole blood of pregnant women recruited as part of the Mother-and-Child Contaminant Cohort Study conducted in Northern Norway (The MISA study); and (ii) determine the influence of personal and social characteristics on both dietary and contaminant factors., Methods: Recruitment and sampling started in May 2007 and continued for the next 31 months until December 2009. Blood/serum samples were collected during the 2nd trimester (mean: 18.2 weeks, range 9.0-36.0). A validated questionnaire was administered to obtain personal information. The samples were analysed by established laboratories employing verified methods and reference standards. PCA involved Varimax rotation, and significant predictors (p≤0.05) in linear regression models were included in the multivariable linear regression analysis., Results: When considering all the contaminants, three prominent PCA axes stood out with prominent loadings of: all POPs; arsenic, selenium and mercury; and cadmium and lead. Respectively, in the multivariate models the following were predictors: maternal age, parity and consumption of freshwater fish and land-based wild animals; marine fish; cigarette smoking, dietary PCA axes reflecting consumption of grains and cereals, and food items involving hunting. PCA of only the POPs separated them into two axes that, in terms of recently published findings, could be understood to reflect longitudinal trends and their relative contributions to summed POPs., Conclusions: The linear combinations of variables generated by PCA identified prominent dietary sources of OC groups and of prominent toxic elements and highlighted the importance of maternal characteristics., (Copyright © 2014 Elsevier GmbH. Background: Longitudinal monitoring studies of persistent organic pollutants (POPs) in human populations are important to better understand changes with time and age, and for future predictions., Objectives: We sought to describe serum POP time trends on an individual level, investigate age-period-cohort effects, and compare predicted polychlorinated biphenyl (PCB) concentrations to measured values., Methods: Serum was sampled in 1979, 1986, 1994, 2001, and 2007 from a cohort of 53 men in Northern Norway and analyzed for 41 POPs. Time period, age, and birth cohort effects were assessed by graphical analyses and mixed-effect models. We derived the predicted concentrations of four PCBs for each sampling year using the CoZMoMAN model., Results: The median decreases in summed serum POP concentrations (lipid-adjusted) in 1986, 1994, 2001, and 2007 relative to 1979 were -22%, -52%, -54%, and -68%, respectively. We observed substantial declines in all POP groups with the exception of chlordanes. Time period (reflected by sampling year) was the strongest descriptor of changes in PCB-153 concentrations. Predicted PCB-153 concentrations were consistent with measured concentrations in the study population., Conclusions: Our results suggest substantial intraindividual declines in serum concentrations of legacy POPs from 1979 to 2007 in men from Northern Norway. These changes are consistent with reduced environmental exposure during these 30 years and highlight the relation between historic emissions and POP concentrations measured in humans. Observed data and interpretations are supported by estimates from the CoZMoMAN emission-based model. A longitudinal decrease in concentrations with age was evident for all birth cohorts. Overall, our findings support the relevance of age-period-cohort effects to human biomonitoring of environmental contaminants.

The objective of this study is to compare meconium and maternal serum as biomarkers of fetal exposure to organochlorines (OCs). A subset of 40 meconium samples and complementary maternal sera from the Northern Norway Mother-and-Child Contaminant Cohort Study (MISA) were selected. Meconium samples were collected at the earliest opportunity (median 9.0 hours postpartum, range 0-61) and maternal serum in the 2nd trimester (median 19.0 gestational weeks, range 13-34) and analysed for OC contaminants selected from the Arctic Monitoring and Assessment Programme's (AMAP) suite of OCs and selected hydroxylated metabolites. Eight compounds with detection frequencies ≥70% in both media (criterion for inclusion) were included in the statistical analyses. Median concentration ratios for p,p'-DDE, HCB, trans-nonachlor and cis-nonachlor favoured meconium, and PCB 138 and 153 and OH-PCB 146 and 172 were higher in maternal serum. All inter-media correlations were significant (Spearman's rho) for wet-weight concentrations and improved when concentrations in a small subset of 15 meconium and serum samples were both lipid-adjusted; only OH-PCB 146 slightly favoured maternal serum. Multivariable linear regression modelling confirmed that maternal serum was the most consistent predictor of meconium concentrations, with gestational age and time of meconium sampling improving the models. Although more challenging to analyse, the lipid-adjusted OC concentration in meconium is viewed as a sensitive and informative fetal exposure index when taking into account the gestational age and its postpartum sampling time.

Dermal application of personal care products (PCPs) is considered an important human exposure route for siloxanes. Their presence as minor or major constituents in many personal care products (PCPs) and cosmetics is of concern for human exposure. The aim of this study was to quantify cyclic volatile methylsiloxanes (cVMS) in blood plasma of pregnant and postmenopausal women, and to investigate possible links to self-reported use of PCPs for the latter group. Participants were recruited from two studies, namely the Norwegian Women and Cancer Study (NOWAC) and the North Norwegian Mother-and-child Study (MISA). For the NOWAC cohort, 94 plasma samples from postmenopausal women were analyzed (blood drawn in 2005) and information about PCP use and breast implants was derived from a self-administered questionnaire. In the MISA study, the collection of the plasma samples (blood drawn in 2009) constituted a re-sampling because the original serum vacutainers used were contaminated with cVMS. D4 (octamethylcyclotetrasiloxane) was the dominant compound in plasma for both cohorts. For the NOWAC samples, more than 85% of the women had D4 concentrations above the LOQ (2.74 ng/mL), while the detection frequency was only 18% for the MISA participants. The highest cVMS plasma concentrations were observed for D4: 12.7 ng/mL (NOWAC) and 2.69 ng/mL (MISA). For the other cVMS, decamethylcyclopentasiloxane (D5) and dodecamethylcyclohexasiloxane (D6) concentrations were below the detection limit in most samples. There was no significant correlation between the concentrations of D4 and the reported total body cream use. Background: The foetal effects of occupational exposure to organic solvents in pregnancy are still unclear. Our aim was to study the risk of non-chromosomal congenital anomalies at birth in a well-defined population of singletons born to women employed as painters and spoolers in early pregnancy, compared to women in non-hazardous occupations., Method: The study population for this prospective cohort study was singleton newborns delivered to working mothers in the industrial community of Mončegorsk in the period 1973-2005. Occupational information and characteristics of the women and their newborns was obtained from the local population-based birth register., Results: The 597 women employed as painters, painter-plasterers or spoolers had 712 singleton births, whereof 31 (4.4%) were perinatally diagnosed with 37 malformations. Among the 10 561 newborns in the group classified as non-exposed, 397 (3.9%) had one or more malformations. The overall prevalence in the exposed group was 520/10 000 births [95% confidence limits (CL): 476, 564], and 436/10 000 births (95% CL: 396, 476) in the unexposed. Adjusted for young maternal age, smoking during pregnancy, maternal congenital malformation and year of birth, the odds ratio (OR) was 1.24 (95% CL: 0.85, 1.82); for multiple anomalies it was 1.54 (95% CL: 0.66, 3.59).The largest organ-system specific difference in prevalence between the two groups was observed for malformations of the circulatory system: 112/10 000 (95% CL: 35, 190) in the exposed group, and 42/10 000 (95% CL: 29, 54) in the unexposed, with an adjusted OR of 2.03 (95% CL: 0.85, 4.84). The adjusted ORs for malformations of the genital organs and musculoskeletal system were 2.24 (95% CI: 0.95, 5.31) and 1.12 (95% CI: (0.62, 2.02), respectively., Conclusion: There appeared to be a higher risk of malformations of the circulatory system and genital organs at birth among newborns to women in occupations with organic solvent exposure during early pregnancy (predominantly employed as painters). However, the findings were not statistically conclusive. Considering that these two categories of malformations are not readily diagnosed perinatally, the difference in prevalence between the exposed and unexposed may have been underestimated.

The objective of this study was to investigate changes in the levels of toxic and essential elements in maternal blood during pregnancy and subsequently. A subset of 211 pregnant women from the North Norwegian Mother-and-Child Study was included. Blood samples were collected during the 2(nd) trimester (P1) and postpartum (day 3, P2; and 6 weeks, P3) in different regions of northern Norway, and were analyzed for a suite of 10 selected elements. The latter feature three general but distinct concentration patterns across the three collection periods, namely: progressive increase [Group 1: As, Cd (non-smokers only), Mn, Pb and Zn]; V-shaped, with a minimum at P2 [Group 2: Cd (smokers), Hg, Mo and Se]; and downward V, with a maximum at P2 [Group 3: Co and Cu]. These trends are interpreted in the context of underlying metabolic, hematological and physiological changes that occur in mothers, as well the biochemistry and accumulation preferences of these elements within the whole blood compartment and breast milk. Implications for biomonitoring strategies are discussed. In a multivariable analysis of the P1 data, fish consumption was a robust positive predictor for Hg (p < 0.02), As (p < 0.01) and Se (p ≤ 0.001) blood concentrations, multivitamin intake for Se (p < 0.001), and parity for Co (p < 0.002); age reached significance only for Hg (p < 0.001). An inverse association was observed between parity and Hg (p < 0.05). For the other elements, predictor patterns were not evident.

The objective was to explore how perinatal mortality relates to birthweight, gestational age and optimal perinatal survival weight for two Arctic populations employing an existing and a newly established birth registry. A medical birth registry for all births in Murmansk County of North-West Russia became operational on 1st January 2006. Its primary function is to provide useful information for health care officials pertinent to improving perinatal care. The cohort studied consisted of 17,302 births in 2006-07 (Murmansk County) and 16,006 in 2004-06 (Northern Norway). Birthweight probability density functions were analysed, and logistic regression models were employed to calculate gestational-age-specific mortality ratios. The perinatal mortality rate was 10.7/1000 in Murmansk County and 5.7/1000 in Northern Norway. Murmansk County had a higher proportion of preterm deliveries (8.7%) compared to Northern Norway (6.6%). The odds ratio (OR) of risk of mortality (Northern Norway as the reference group) was higher for all gestational ages in Murmansk County, but the largest risk difference occurred among term deliveries (OR 2.45, 95% confidence interval 1.45, 4.14) which hardly changed on adjustment for maternal age, parity and gestation. Proportionately, more babies were born near (± 500 g) the optimal perinatal survival weight in Murmansk County (67.2%) than in Northern Norway (47.6%). The observed perinatal mortality was higher in Murmansk County at all birthweight strata and at gestational ages between weeks 25 and 42, but the adjusted risk difference was most significant for term deliveries., (© 2011 Blackwell Publishing Ltd.)

The objective of this study was to investigate changes in the levels of common organochlorines (OCs) and lipids in maternal serum during and after pregnancy. A subset of 50 pregnant women from the North Norwegian Mother-and-Child Study was included in this study. Blood samples were collected during the 2(nd) trimester and postpartum (Day 3 and 6 weeks) in different regions of Northern Norway, and were analyzed for the Arctic Monitoring and Assessment Programme (AMAP) suite of OC contaminants. During the gestational period, both lipids and wet-weight OC levels peaked at birth and were the lowest at 6 weeks postpartum. When the OC concentrations were lipid-adjusted, this peaking was no longer evident. Wet-weight concentrations of OCs appear to be driven by the physiological lipid profiles and are interpreted to constitute biomarkers of lipidemia. It is suggested that this observation may have implications for the biomonitoring of individuals at risk of Type 2 diabetes. Both age and parity were strong predictors for the OCs measured, but no consistent association with body mass index (BMI) was evident. Independent of lipid-adjustment, all compounds were positively and significantly correlated with each other (within and across the three collection time periods). The peaking of OCs during pregnancy suggests that the period spanning the last weeks of the 3(rd) trimester and the early postpartum days constitutes an optimum sampling window purely from the analytical perspective.

Objectives: This paper describes a comprehensive assessment of the association of concentrations of essential and toxic elements in maternal and neonatal body fluids and the placenta as predictors of birth weight and newborn body mass index (BMIC) for deliveries in northern Norway and Russia., Study Design: A prospective cross-sectional study of delivering women and their outcomes from different locations in Russian and Norwegian arctic and sub-arctic areas., Methods: Life-style information, blood, urine and placenta specimens were collected for 50 consecutive mother-infant pairs from hospital delivery departments in a total of six communities located in Finnmark, Norway, or the western arctic/subarctic regions of Russia. Questionnaire information was collected by individual interviews performed by trained health personnel. Pregnancy outcomes were verified by consulting medical records. Cadmium, copper, iron (as ferritin), nickel, lead, selenium and zinc were measured in maternal blood, serum or maternal urine: and in cord blood, or neonatal urine and placental tissue. Univariate and multivariate linear regression analysis and ANOVA were employed to explore associations between these clinical chemistry outcomes and birth weight and BMIC., Results: A number of significant relationships were evident between: placental and maternal blood cadmium (p < 0.005); cord and maternal blood lead (p < 0.001); placental and maternal blood lead (p < 0.001); placental and cord-blood lead (p < 0.001); placental and maternal serum, or blood, selenium (p < 0.001); and placental and maternal serum copper (p < 0.001). Maternal body mass index (BMI), maternal age, placental lead, or maternal blood lead, and smoking were retained as predictors of birth weight and BMIC in the multivariate modelling. Birth weights in both countries were normally distributed., Conclusions: Maternal age and BMI as positive predictors of birth weight, and cigarette smoking and lead exposure as negative determinants, are discussed in terms of established evidence and recognized confounders, including maternal genetic factors, socio-economic status, socio-political change, life-style issues, prenatal care and nutrition. It is recommended that future work in societies undergoing socio-economic transition might best focus on preventive measures to improve neonatal health and development.

Research is described that constitutes an extension of an earlier paper (J. Environ. Monit., 2001, 3, 177-184), in which concentrations were measured in 263 human placentas of 11 essential elements (P, Ca, Mg, Cu, S, Na, Fe, Zn, K, Se, Mn) and 5 toxic elements (Ba, Sr, Pb, Ni, Cd). The additional data considered derive from earlier visits to 4 of the original 6 communities and 3 others, all but one of which are located in northern Norway and neighbouring areas of Russia. This more than doubled the number of placental samples available (263 to 571). Unfortunately, the personal, life-style and morphometric information obtained for the first study group was not available for the additional mothers. Country differences were evident for all elements except Ba, Fe and Zn; Cd, Cu, Mn, Na, Se, Ni, Pb, Sr and S were higher and K, P, Ca and Mg were lower in Russia (p < 0.03). Not unexpectedly, the highest median lead concentration was observed for the largest city in the western arctic region of Russia, namely Murmansk. Similarly, the higher median nickel level observed for Russia reflects the established observation that urinary nickel concentrations are higher in the Russian than in the Norwegian communities. Even though sampling was performed at different times of the year and before and after a 3-year interval in four centres, inter-collection differences were of relatively small magnitude and appear not to be linked to seasonal or temporal changes. Principal component analysis (PCA) confirmed the prominence of Factor 1, which grouped those metals that are known to form insoluble phosphate complexes and whose concentrations showed a dependence on gestational age and maternal smoking in the earlier study. It is concluded that PCA is a powerful statistical tool for exploring and identifying fundamental pathways and processes involved in governing the inorganic elemental composition of placental tissue. It also has the potential of identifying study limitations and quality assurance shortfalls. Further our findings show promise that placental concentrations of toxic elements may serve as an index of exposure and of nutritional intake for selected essential micro-elements.

Concentrations in human placenta of 11 essential elements (P, Ca, Mg, Cu, S, Na, Fe, Zn, K, Se, Mn) and 5 toxic elements (Ba, Sr, Pb, Ni, Cd) are compared for each of two arctic communities in eastern Norway and western Russia, and for another in each country located at more southerly latitudes. All but Mg, Fe, P and K were present in higher concentrations in the Russian study group. The observed inter-element correlations are reflected by the four major factors identified in a principal component analysis. The total variation explained was 67.3%, of which more than half (35.3%) was contributed by Factor 1. P, Ca, Mg, Ba, Sr, Pb, and Ni were major contributors to this factor. The placental concentrations of these elements depended strongly on gestational age, increasing from about week 35 and peaking near weeks 39 and 40, and exhibited skewed frequency distributions and a dependence on maternal smoking. The gestational-dependent mineralization of the placenta is interpreted to reflect the deposition of metal phosphates coinciding with smoking-induced tissue damage. The loadings of the remaining three factors are reviewed in the context of common uptake mechanisms, similar biochemistries and unique transport pathways. The inter-element relationships and grouping of the elements observed should constitute a scientific base for the use of placenta composition in environmental monitoring and epidemiological studies.

Background: Delivering women and their newborns in the Kola Peninsula of Russia and the neighboring arctic area of Norway were studied to explore relationships between maternal cadmium and lead status and birth weight as a pregnancy outcome., Methods: Life-style information, maternal blood and cord blood specimens were collected from 50 consecutive mother-infant pairs from hospital delivery departments in three Russian and three Norwegian communities. Pregnancy outcomes were verified by consulting medical records. Lead and cadmium were determined in the blood samples by electrothermal atomic absorption spectrometry., Results: The median blood-cadmium concentration for the Russian mothers was 2.2 nmol/L (n = 148) versus 1.8 nmol/L in the Norwegian group (n = 114, p = 0.55). A weak association was observed between maternal cadmium and amount smoked (r = 0.30, p<0.001); no correlation was found between maternal blood cadmium and birth weight. The corresponding maternal lead values were 0.14 (Russia) and 0.06 micromol/L (Norway), p<0.001. The latter lead concentration constitutes one of the lowest adult population values reported to date. Maternal and cord blood lead levels were strongly correlated (r = 0.88, p<0.001). In a multivariate linear regression model, maternal blood lead was recognized as a negative explanatory variable (p<0.05) for birth weight and child's body mass index (BMIC), with or without adjustment for gestational age. A similar association was suggested by ANOVA-analysis of maternal blood lead by quartiles., Conclusion: Maternal blood-lead level as an environmental factor is an apparent predictor of low birth weight and BMIC. It reduced substantially the contribution of a country factor in explaining the observed differences in birth weight.

Background: This project is part of an assessment of the impact of environmental factors on human health in the Kola Peninsula of Russia and the neighboring arctic area of Norway. Pregnant women and their newborns were studied to explore a relationship between maternal status of essential metals and birth weight., Methods: Life-style information and serum specimens were collected from at least 50 consecutive mother-infant pairs from hospital delivery departments in three Russian and three Norwegian communities (N=151 and 167, respectively). Pregnancy outcomes were verified by consulting medical records. Copper, selenium and zinc in serum were determined by atomic absorption spectrometry and ferritin by an automated analyzer method., Results: Mean birth weight and child's body mass index (BMIC) were significantly lower in the Russian group (p<0.001), with or without adjustment for gestational age. Copper, iron (as ferritin) and selenium serum concentrations were in the normal range, while zinc levels in both countries were mostly below the lower limit (10.8 micromol/L) of reported reference intervals. A positive correlation between zinc and birth weight or BMIC was only observed for concentrations exceeding 10.8 micromol/L. Analysis by quartiles showed that maternal urinary creatinine and birth weight were negatively correlated (p=0.001). The influence of the different elements on BMIC, grouped by quartiles, was significantly positive only for selenium (p=0.03) and ferritin (p=0.02), while there was no significant relationship for copper or zinc. Adjustment of birth weight and BMIC for gestational age did not alter substantially the various associations indicated., Conclusions: With the exception of zinc, the mineral status of delivering women in arctic and sub-arctic regions of Norway and western Russia appears to be adequate. The significantly lower BMIC for the Russian group suggests the likely occurrence of nutritional deficiencies during pregnancy in Russia. However, the significant contribution of a country factor in the predictive model implies that the maternal serum trace-element concentrations explored in this study are incomplete indicators of fetal undernourishment.

The two objectives of this study were to compare urinary nickel excretion in pregnant women and their newborns living in the Murmansk and Arkhangelsk Counties of Russia with that in comparable Norwegian populations living in Finnmark and the city of Bergen and to assess the influence on pregnancy outcome of different risk variables, specifically urinary nickel concentrations and questionnaire-based anamnestic information. Life-style information and urine samples were collected from 50 consecutive mother-infant pairs from hospital delivery departments in three Russian and three Norwegian communities. Pregnancy outcomes were verified from medical records. Urinary nickel excretion was significantly higher in the Russian communities, independent of the presence of a nickel refinery as a local environmental source. The birth weight and mean body mass index of the newborn children (BMIC) were significantly lower (p < 0.001) in the Russian groups, with or without adjustment for gestational age. A multivariate linear regression analysis indicated that maternal urinary nickel concentration had no impact on birth weight. The maternal body mass index (BMI) and maternal height were positive explanatory variables; maternal urinary creatinine is suggested as a weak negative factor. Smoking was shown to be a strong negative predictor only in the Norwegian group among whom there was a significantly higher smoking frequency (p = 0.005). The significant contribution of a country factor in the predictive model is interpreted to indicate that a number of important risk factors for low birth weight were not identified.

Unlabelled: As part of the Arctic Monitoring and Assessment Programme (AMAP), we assessed pregnancy outcome among Sami and Norwegian populations of Finnmark County in Norway and Russians living in the Kola Peninsula of Russia using body mass index of the newborn child (BMIC) as the main indicator; concentrations of essential and toxic elements in biological fluids of delivering women and their children served as additional sources of information. At the hospitals of Hammerfest and Kirkenes in the period November 1993-June 1994 a total of 107 consecutive women gave birth to a child, of whom 15 regarded themselves as Sami. The Russian group (N = 151) delivered their children in the same period. The Sami women were significantly older than the Russian group (28.5 versus 25.1 years, p = 0.04). The mean birth weight was significantly lower in the Sami group compared to non-Sami Norwegians (p = 0.01), but was of comparable magnitude to that recorded in Russia (p = 0.4). For BMIC, the Sami and non-Sami Norwegian results were similar (p = 0.2); both were significantly higher than in Russia (p < 0.001). The essential elements copper, zinc, selenium and iron (as ferritin) in serum showed no differences between the groups, although relatively low levels of serum zinc were documented in all populations studied. Blood cadmium concentrations were strongly related to smoking frequency. Blood lead and urinary nickel levels were significantly higher for the Russian mothers, but did not reach levels of medical concern., Conclusion: No ethnic differences in concentrations of essential elements in biological fluids, nor of cadmium and mercury, were observed. However, national differences for lead and nickel were evident. Because of small sample size, these conclusions need confirmation. The similar BMIC values observed for the non-Sami Norwegian and Sami newborns, compared to the Russian group, suggest that BMIC may serve as a good indicator of the nutritional status and possibly also the general health condition of neonates.

Recently, considerable attention has been allotted to the possible health effects of the industrial pollution in the regions adjoining the Norwegian-Russian border. As a measure of general health, the status of trace elements and some clinical-chemical parameters in serum and blood that reflect general health was assessed for 50 consecutive obstetric patients presenting at delivery departments of local hospitals in the Kola Peninsula, Arkhangelsk and selected centres in Norway. A written questionnaire and consent form were completed by each participant. Specimens of maternal and cord blood were collected. Corresponding samples were analyzed for copper, zinc, selenium, iron and ferritin. We found a lower birth weight in the Russian centres. The measurement of trace elements showed a higher iron status for the Russian females and relatively low serum zinc levels were observed in both the Russian and the Norwegian communities. It may be concluded that with the exception of zinc, the mineral status in the Norwegian and Russian arctic centres considered appears to be satisfactory.
