Background: The trochanteric stabilizing plate (TSP) is used as an adjunct to the sliding hip screw (SHS) in unstable trochanteric and subtrochanteric fractures. We wanted to describe the choice of implant for trochanteric fractures with a focus on the TSP in Norway. Methods: A total of 20,902 fractures from the Norwegian Hip Fracture Register treated surgically in 43 hospitals from 2011 to 2017 were included. Logistic regression analyses were performed to detect factors potentially influencing implant choice. Results: The mean age was 83 years, and 15,137 (72%) were women. An SHS was used in 13,273 (63%) fractures, of them 4407 (33%) with a TSP. Fracture classification was the most important determinant of TSP. In cases where an SHS was used, the odds ratio (OR) for using a TSP was 14 for AO/OTA 31A2 fractures and 71 for AO/OTA 31A3 and subtrochanteric fractures, compared to AO/OTA 31A1 fractures. The probability of receiving a TSP was higher in urban, academic, and high-volume hospitals (OR 1.2 to 1.3) and lower in Central and Northern Norway (OR 0.3 to 0.7). The use of an intramedullary nail (IMN) (n = 7629 (36%)) was also to a degree decided by fracture classification (OR 1.8 to 5.3). However, hospital factors, with OR 0.1 to 0.4 for IMN in academic, urban, and high-volume hospitals and OR 1.5 to 2.6 outside South-Eastern Norway (all p < 0.001), were also important. Conclusions: Fracture classification was the main determinant for TSP use. Any additional benefit from a TSP on postoperative fracture stability or clinical outcome needs to be clarified. Introduction: The objective of this study was to identify indications and predictors for subsequent surgeries in the same hip and to evaluate life expectancy following screw fixation of undisplaced femoral neck fractures (FNF). The study further aimed to determine the necessary follow-up time for future studies aiming to evaluate the treatment of such fractures.Materials and Methods: This is a single-center retrospective cohort study with prospectively collected data including skeletally mature patients with undisplaced FNFs operated between 2005 and 2013. Gender, age at fracture, American Society of Anesthesiologists score, smoking status and excess use of alcohol were retrieved from electronical medical records. Further, complications leading to all consecutive reoperations were registered along with time from primary operation to all reoperations, type of procedure during subsequent surgeries and time of death.Results: 383 patients with a median (range) follow-up of 77 (23-125) months were identified. Within 1, 2 and 5 years from primary surgery, 8%, 17% and 21% respectively, had at least one subsequent surgery in the same hip. 10% of the patients underwent salvage arthroplasty, however, in long time survivors; conversion to arthroplasty was estimated in one out of four. Posterior tilt of the femoral head was a predictor for new surgeries due to instability of the bone-implant construct, but not for later avascular necrosis. For patients 70 years or older, the one-year mortality in men was 32% with an expected survival of approx. 2.5 years, compared to 17% and 5.5 years in women.Conclusions: Screw fixation of undisplaced femoral neck fractures appears to be a safe procedure in particular in the absence of a posterior tilt of the femoral head. Conversion to arthroplasty was estimated to occur in one out of four of long time survivors. Men have a particularly poor medical prognosis and should receive careful medical attention. In order to capture 80% of reoperations, clinical studies and register studies must have a follow-up time of at least two years. [ABSTRACT FROM AUTHOR]

Summary: We estimated the cost-effectiveness of hemiarthroplasty compared to internal fixation for elderly patients with displaced femoral neck fractures. Over 2 years, patients treated with hemiarthroplasty gained more quality-adjusted life years than patients treated with internal fixation. In addition, costs for hemiarthroplasty were lower. Hemiarthroplasty was thus cost effective. Introduction: Estimating the cost utility of hemiarthroplasty compared to internal fixation in the treatment of displaced femoral neck fractures in the elderly. Methods: A cost-utility analysis (CUA) was conducted alongside a clinical randomized controlled trial at a university hospital in Norway; 166 patients, 124 (75%) women with a mean age of 82 years were randomized to either internal fixation ( n = 86) or hemiarthroplasty ( n = 80). Patients were followed up at 4, 12, and 24 months. Health-related quality of life was assessed with the EQ-5D, and in combination with time used to calculate patients' quality-adjusted life years (QALYs). Resource use was identified, quantified, and valued for direct and indirect hospital costs and for societal costs. Results were expressed in incremental cost-effectiveness ratios. Results: Over the 2-year period, patients treated with hemiarthroplasty gained 0.15-0.20 more QALYs than patients treated with internal fixation. For the hemiarthroplasty group, the direct hospital costs, total hospital costs, and total costs were non-significantly less costly compared with the internal fixation group, with an incremental cost of €2,731 ( p = 0.81), €2,474 ( p = 0.80), and €14,160 ( p = 0.07), respectively. Thus, hemiarthroplasty was the dominant treatment. Sensitivity analyses by bootstrapping supported these findings. Conclusion: Hemiarthroplasty was a cost-effective treatment.
