The annual Eastern Canadian Gastrointestinal Cancer Consensus Conference 2023 was held in Quebec City, Quebec 2–4 February 2023. The purpose of the conference was to develop consensus statements on emerging and evolving treatment paradigms. Participants included Canadian medical oncologists, radiation oncologists, pathologists and surgical oncologists from across Ontario, Quebec, and the Atlantic provinces. Consensus statements were developed following rapid review presentations and discussion of available literature. The recommendations proposed here represent the consensus opinions of physicians involved in the care of patients with gastrointestinal malignancies who participated in this meeting. Considering the key role of health care providers in integrating assistive technologies into clinical settings (e.g., in/outpatient rehabilitation) and home, this study explored the care providers' perspectives on benefits, barriers and facilitators to the implementation of the Cognitive Orthosis for coOking (COOK) for adults with traumatic brain injury (TBI) within clinical contexts and homes. Using a qualitative descriptive approach, semi-structured individual interviews and focus groups were carried out with experienced care providers of adults with TBI (n = 30) in Ontario-Canada. Qualitative analysis based on the Miles et al approach was used. According to the participants, COOK could potentially be used with individuals with cognitive impairments (TBI and non-TBI) to increase safety and independence in meal preparation and support healthcare providers. However, limited access to funding, clients' lack of motivation/knowledge, and the severity of their cognitive and motor impairments were perceived as potential barriers. Facilitators to the use of COOK include training sessions, availability of private/provincial financing, and comprehensive assessments by a clinical team prior to use. Health care providers' perspectives will help develop implementation strategies to facilitate the adoption of COOK within homes and clinical contexts for individuals with TBI and improve the next version of this technology. COOK shows a high potential for increasing independence and safety during meal preparation with its sensor-based monitoring of the environment and cognitive-based assistance, for adults with TBI. Comprehensive clinical assessments to identify individuals' therapeutic goals, clinical characteristics, and living environments are necessary to facilitate the deployment of COOK. A farm owner was found unresponsive with crushing head injuries on his property in rural Ontario. His small farm tractor was found 60 meters away down a small incline with the engine running and transmission in neutral. The owner's son alleged that when the parking brake was engaged (with the engine running and transmission in neutral), this tractor's parking brake would "pop out," allowing the tractor to move. Field tests were conducted on the tractor to attempt to duplicate the scenario and to determine if the alleged sequence of events was plausible. Components of the parking brake and one exemplar were assessed with specialized metrology to determine whether they were within the manufacturing specifications on the blueprint. A 3-D CAD model of fit was created, and several variances were identified between the parts and the factory drawing. The results of the analysis concurred with the scenario that these variances led to the disengagement of the parking brake and operator fatality. [ABSTRACT FROM AUTHOR]

Physiological correlates of seasonal growth patterns were measured in lake trout Salvelinus namaycush from two populations with contrasting diets (zooplankton-dominated diet in Louisa Lake; fish-dominated diet in Opeongo Lake). Fish in Opeongo Lake grew faster and were in better condition than fish in Louisa Lake. The most prominent biochemical difference between populations was higher citrate synthase (CS) and cytochrome c oxidase activity in the white muscle of fish from Opeongo Lake, indicating greater sustained swimming activity in this lake. In contrast, lactate dehydrogenase (LDH) activity in white muscle, an indicator of capacity for burst swimming, was similar between lakes. Nucleoside diphosphate kinase (NDPK) activity in white muscle, an indicator of protein synthesis, was higher in Opeongo Lake than in Louisa Lake but only in the autumn. In both lakes, protein concentration and therefore nutritional status increased as the growing season progressed from spring to summer to autumn. Biochemical indicators of growth and activity showed similar seasonal patterns in the two lakes with the spring characterized by high NDPK, high CS and high LDH activities ( i.e. high levels of protein synthesis in association with high aerobic and anaerobic activities). Variations in fish condition, pyloric caeca (PC) mass and tissue metal concentrations were examined in two consecutive summers in yellow perch collected along two polymetallic gradients in Canada, Sudbury (ON) and Rouyn-Noranda (QC). The potential of PC mass variations to help interpret food web-mediated changes in condition was also evaluated. The lack of relationships between PC weight and condition, even in reference lakes, indicated that factors other than feeding rate influenced fish condition. Inter-annual variations in fish condition (12–58%), PC weight (14–20%) and tissue metal concentrations (30–550%) were often of greater magnitude than differences between reference and contaminated fish sampled in the same year. The apparent lack of correspondence between feeding rate and condition and the large inter-annual variation in examined metrics highlight the need to better understand the ecological underpinnings that influence metal toxicity in field situations.
