Based on data on the annual incidence of malaria in Senegal in the period from 2000 to 2021, the article demonstrates the possibilities of predicting the annual dynamics of the epidemic using the SIR model and the CIR model. A modified SIR model with constant coefficients is constructed, and a description of the CIR balance model with stochastic parameters is given. The question of the accuracy of forecasting the annual statistical indicators of the epidemic when using these models is investigated. As numerical experiments show, the average error in predicting the annual number of sick people compared to actual statistical data when using the SIR model is quite large, while the CIR model generates more accurate forecasts when compared. Background and context: In Senegal, pediatric oncology patients arrive at the hospital at advanced stages of disease. Their large tumors, as well as the side effects of chemotherapy make children experience intense pain. In this low-resource setting, morphine supply was quite irregular. Doctors don´t prescribe morphine due to shortages; but few prescriptions also lead to limited orders. A vicious circle with only 1 victim: the patient. Hearing children in pain was agonizing for everyone: patients, caregivers and healthcare practitioners. Aim: This advocacy narrative illustrates how the fight for access to morphine in pediatric oncology has led to both positive and negative externalities. We will highlight ways in which this fight for morphine has provoked political tensions moving the issue forward, but has also affected the careers of health workers involved. Strategy/Tactics: Morphine is a cheap drug, yet it is extremely regulated by international laws. It takes political will to influence national morphine orders. This advocacy strategy was built on raising awareness and setting morphine shortage on the political agenda. Not providing morphine in oncology goes against international standards of care. But most importantly, letting patients suffer without "existing" relief is a breach of basic human right to live - and also die - in dignity. Program/Policy process: Conversations began within the pediatric oncology department. Focus groups highlighted caregivers' feelings of powerlessness before their suffering child. Interviews with key informants (doctor and nurses) were instrumental to a widely diffused Human Rights Watch report exploring the medical and political causes to morphine supply shortages, as well as its psychological repercussions on patient care. A BBC documentary was broadcasted soon after. Subsequently, meetings were held between the Ministry of Health, the National Supply Pharmacy and leading local oncologists. Outcomes: Morphine orders were multiplied by 10, leading to much improved pain management for patients. However, Senegal was portrayed negatively on the international scenes, much to the Health Minister´s dismay. The consequences were incurred by the health workers who contributed to the international publications/reports. What was learned: Health practices can inform policy just like policy can inform health practices. It is a cyclical process. Creating advocacy coalitions and rallying the help of the international community are effective strategies. However, in the political arena, health workers need more than commitment to human rights and quality care. Even in democratic republics, systems can penalise outspoken activists. We have learned that health care practitioners (especially working in public settings) who wish improvement for their patients must learn to be tactical and diplomatic. • Soil fertility and millet yields were higher in Homefield than in Outfield. • Inorganic fertilizer use efficiency was high in Homefield and low in Outfield. • Low fertilizer use efficiency was related to high bulk density and low soil P. • Striga hermontica infestation and late drought decreased the 1000-grain weight. • Manuring and millet rotation with peanut reduced Striga hermontica infestation. Much effort has been spent on formulating guidelines for inorganic fertilizer use in millet crops in Sub-sahelian farms. However, these guidelines do not take into account the diversity of manuring practices. In this study we analyzed over two years (2016–2017) the use efficiency of an inorganic NPK fertilizer as affected by the two most contrasted categories of organic manure strategies (OMS) found in millet fields of central Senegal. 19 farmers' fields were selected in a village typical of that region, 11 and 8 of which respectively corresponding to categories OMS1 and OMS2 as follows: OMS1, locally referred to as Homefields, were fields continuously cropped with millet over the last 15 years, having received organic manure regularly in the past, and manured again at the onset of the 2016 rainy season. OMS2 fields locally referred to as Outfields, were not manured in 2016 and were rarely manured in the past. Four of them were continuously cropped with millet and the others had followed a triennial millet-peanut-fallow rotation. In 2017, no manure was applied in any of the OMS1 or OMS2 fields. A pairwise treatments with and without the same inorganic fertilizer dose was applied in each field in both 2016 and 2017 cropping seasons. In 2016, the higher the manure application, the higher the use efficiency of the inorganic fertilizer applied. The use efficiency of the inorganic N was most closely related to soil bulk density and P availability. In 2017, with no new manure amendment, millet yield in OMS1 was about three times higher than in 2016. It was close to the water-limited yield, suggesting that the residual effect of the manure applied in 2016 was high. The use efficiency of the inorganic N was generally low under these conditions. In OMS2, millet yield and use efficiency of inorganic fertilizer remained low in both years. The crop rotation with peanuts did not enrich the soil compared to the millet returning every year, but it reduced Striga hermontica infestation and increased the millet 1000-grain weight. The methodological approach developed here may help in formulating guidelines to deal with the diversity of farming practices in Sub-sahelian villages. [ABSTRACT FROM AUTHOR]

Adolescents in low- and middle-income countries face numerous developmental, sexual and reproductive health (SRHR) challenges, including exposure to multidimensional violence. Dealing with gender-based violence (GBV) is of great importance and health personnel are key players. The objective of this work was to study the knowledge and practices of health personnel on SRHR and gender-based violence in Guédiawaye, Kaolack and Kolda communities in Senegal. A descriptive and analytical cross-sectional study was conducted, which consisted of health professionals (general practitioners and specialists, nurses, and midwives) and community health workers (community relays, bajenu gox, matrons). All health facilities in the three communities were included. Data analysis consisted of univariate analysis and logistic regression modeling to investigate the factors associated with the knowledge and practice of health personnels. An alpha risk of 5% was taken. A total of 78 health professionals and 128 community actors were included in the study. More than half of the health personnel (56.3%) had good knowledge of policies, standards and protocols relating to sexual and reproductive health services for women (adolescents) and about 60% on conventions and laws. The level of knowledge was good among 51% of respondents and good practices among 54.9%. The factors associated with good knowledge were the municipality in which the profession was practiced, and the effects of training received in the social construction of gender. The factors associated with the practices were knowledge of policies, standards and protocols through training, training received in the provision of family planning services, and in medico-psychosocial management of cases of sexual violence. In Senegal, many adolescent victims of gender-based violence (GBV) do not receive care. The aim of this study was to analyse the care circuit for adolescent victims of GBV, taking gender differences into account. This was a qualitative case study. A thematic analysis of the data was carried out using Nvivo 12 software. The study showed that society attached less importance to the rape of boys. The study also showed that the main attitude of adolescents to GBV was silence, encouraged by under-reporting. The structural barriers to providing care were the insensitivity of health and judicial structures towards adolescents, as well as geographical and financial obstacles. Background: Ensuring access to removal services for implants and intrauterine devices (IUDs) is essential to realize informed choice and voluntary family planning. We document removal desires and experiences among women who received an implant or IUD from the public sector in 3 districts of Senegal., Methods: We conducted a phone survey of 1,868 implant and IUD users, 598 follow-up surveys with those who had ever asked a provider for a removal, and 24 in-depth interviews (IDIs) with women who had ever wanted an implant removal. We analyzed survey data descriptively and IDI data thematically., Results: Fifty-eight percent of implant users and 54% of IUD users reported having wanted a removal. Desired pregnancy and contraceptive-induced menstrual changes (CIMCs) were the main reasons for removal desires. Fifty-four percent of implant users and 55% of IUD users who asked a provider for a removal reported challenges accessing services, with over two-thirds noting long lines or wait times. Sixty-three percent of implant users and 73% of IUD users who saw a provider were satisfied with the outcome of their first interaction. Over 90% of participants had not been told about the removal cost at insertion. Almost all participants who had their method removed obtained a complete removal during their first clinical procedure. Around two-thirds of participants who obtained a removal did not take up another method at that time. IDIs confirmed the influence of CIMCs on removal desires and show some partner influence is common in removal decision making. Barriers include lack of available qualified providers and supplies. Provider interactions play an important role in satisfaction with removal services., Conclusion: Participants' experiences accessing removal services were generally positive. Background: The World Health Organization recommends recording vaccination status according to maternal recall in countries where administrative reporting systems are insufficiently reliable, as maternal recall in developing countries has been shown to be quite reliable compared with data from vaccination cards. This study aimed to investigate childhood vaccination coverage and its determinants according to the mothers' presentation of vaccination cards., Methods: The data come from the 2017 Senegalese Demographic and Health Survey, a nationally representative household survey of women aged 15-49 years, with a questionnaire focusing on children's health. This analysis was restricted to children aged 12-35 months (n = 4032) and it assessed vaccination coverage and associated sociodemographic factors with weighted multivariate logistic regressions. Stratified multivariate logistic regressions were also performed to investigate factors associated with routine childhood immunization uptake of the Bacillus Calmette-Guérin (BCG) vaccine, recommended for administration shortly after birth, as well as of the vaccines against yellow fever and measles (recommended at 9 months)., Results: Comparison of vaccination coverage estimates according to the vaccination card or parental recall resulted in a 5-10% difference in estimated coverage for the BCG, pentavalent, measles, and yellow fever vaccines, but a huge difference for the polio vaccine (93.0% with the card, 32.0% without it). Presentation of the vaccination card was correlated with mothers' attendance at health facilities (suggesting it serves as a concrete manifestation of a bond between mothers and the healthcare system) and their region of residence, but it was not correlated with usually strong predictors of childhood vaccination, such as maternal education level. Factors associated with vaccinations differed depending on whether they were administered shortly after birth or later on., Conclusions: Maternal recall was found to be quite reliable except for oral polio vaccination, which raises the possibility that complete immunization coverage rates could have been significantly underestimated due to potential confusion between injection and vaccination. Considering the ability to present vaccination cards as the materialization of a bond with the healthcare system, the decision path leading to vaccination among those who lack such a bond appears longer and more likely to be driven by supply-side effects.

Introduction . Today, in Sub-Saharan Africa, vaccine-preventable diseases still contribute heavily to high child mortality. Maintaining high coverage rates for childhood vaccines and reducing related social inequalities are public health priorities in Senegal. Our aim was to investigate the determinants of childhood vaccination, including sociodemographic factors and previous vaccine-related decision-making. Methods . Data come from the 2016 Senegalese Demographic and Health Survey, a nationally representative household survey targeting women aged 15-49, with a questionnaire focusing on health and reproductive issues, including their children's health. We restricted the analysis to children aged 12-23 months (n = 1,143). We used bivariate and multivariate analyses for investigating the determinants of several childhood vaccinations (Bacillus Calmette-Guérin, pentavalent, polio, measles and yellow fever vaccines), including sociodemographic factors and previous shots. Results . We identified two main sociodemographic predictors of childhood vaccination in Senegal: the mother's education level, which was strongly and positively correlated to every vaccination considered, except from the BCG vaccination, and the region of residence, with higher vaccination coverage rates in the Centre and West of Senegal. Moreover, previous shots were also strongly predictive of subsequent shots. Conclusion . The positive impact of mother's education on child vaccination illustrates the wide-ranging benefits of educating girls, while the regional variability of immunization rates requires more research to be better understood. Previous shots are probably a proxy variable for unobservable factors strongly correlated to vaccinations, but beyond this 'proxy effect', they may also have their own specific effect on following shots. We believe this topic deserves further research.

Context: Abortion is highly restricted by law in Senegal. Although women seek care for abortion complications, no national estimate of abortion incidence exists., Methods: Data on postabortion care and abortion in Senegal were collected in 2013 using surveys of a nationally representative sample of 168 health facilities that provide postabortion care and of 110 professionals knowledgeable about abortion service provision. Indirect estimation techniques were applied to the data to estimate the incidence of induced abortion in the country. Abortion rates and ratios were calculated for the nation and separately for the Dakar region and the rest of the country. The distribution of pregnancies by planning status and by outcome was estimated., Results: In 2012, an estimated 51,500 induced abortions were performed in Senegal, and 16,700 (32%) resulted in complications that were treated at health facilities. The estimated abortion rate was 17 per 1,000 women aged 15-44 and the abortion ratio was 10 per 100 live births. The rate was higher in Dakar (21 per 1,000) than in the rest of the country (16 per 1,000). Poor women were far more likely to experience abortion complications, and less likely to receive treatment for complications, than nonpoor women. About 31% of pregnancies were unintended, and 24% of unintended pregnancies (8% of all pregnancies) ended in abortion., Conclusions: Unsafe abortion exacts a heavy toll on women in Senegal. Reducing the barriers to effective contraceptive use and ensuring access to postabortion care without the risk of legal consequences may reduce the incidence of and complications from unsafe abortion.

The Rift Valley fever (RVF), which first appeared in Kenya in 1912, is an anthropozoonosis widespread in tropical areas. In Senegal, it is particularly felt in the Ferlo area where a strong presence of ponds shared by humans, cattle and vectors is noted. As part of the studies carried out on the environmental factors which favour its start and propagation, the focus of this paper is put on the decision making process to evaluate the impacts, the interactions and to make RVF monitoring easier. The present paper proposes a model based on data mining techniques and dedicated to trade experts. This model integrates all the involved data and the results of the analyses made on the characteristics of the surrounding ponds. This approach presents some advantage in revealing the relationship between environmental factors and RVF transmission vectors for space-time epidemiology monitoring purpose.

Aim: To explore the links between antenatal care (ANC) non-attendance and economic welfare. METHOD AND SUBJECT: This was a cross-sectional, descriptive and analytical study of women aged 15 to 49 years living in Senegal in 2005. Data were from the Demography and Health Survey using a two-stage random sampling procedure. Participants were classed by quintile using an economic well-being score based on housing characteristics and ownership of sustainable goods. The quality of ANC was determined from the number of visits, the qualification of the person delivering care, and content (counseling, weight, height and blood pressure measurements). Logistic regression was used for data analysis., Results: A total of 6927 women were surveyed. Mean age was 28.15 years (±2.6); 20.3% were primiparous; 61.2% resided in rural areas; 70.0% had received no education. Each of the first four economic quintiles included about 20% (19.2% to 21.5%) of the participants while 16.9% were in the fifth (richest) quintile. A total of 457 women (6.6%) did not undergo any ANC visit. ANC non-attendance increased with parity, decreased with education level and was higher in rural areas than in urban areas, OR=7.2 (95% CI [5.1-10.1]). It decreased with increasing economic well-being: OR=0.6 [0.47-0.75] 2nd quintile vs. 1st, OR=0.02 [0.01-0.05] 5th quintile vs. 1st, p<0.05 overall., Conclusion: Economic welfare plays a major role in determining use of ANC. The only way to solve health problems is to reduce inequalities. Background: In 2009, the first national long-lasting insecticide-treated net (LLIN) distribution campaign in Senegal resulted in the distribution of 2.2 million LLINs in two phases to children aged 6-59 months. Door-to-door teams visited all households to administer vitamin A and mebendazole, and to give a coupon to redeem later for an LLIN., Methods: A nationwide community-based two-stage cluster survey was conducted, with clusters selected within regions by probability proportional to size sampling, followed by GPS-assisted mapping, simple random selection of households in each cluster, and administration of a questionnaire using personal digital assistants (PDAs). The questionnaire followed the Malaria Indicator Survey format, with rosters of household members and bed nets, and questions on campaign participation., Results: There were 3,280 households in 112 clusters representing 33,993 people. Most (92.1%) guardians of eligible children had heard about the campaign, the primary sources being health workers (33.7%), neighbours (26.2%), and radio (22.0%). Of eligible children, 82.4% received mebendazole, 83.8% received vitamin A, and 75.4% received LLINs. Almost all (91.4%) LLINs received during the campaign remained in the household; of those not remaining, 74.4% had been given away and none were reported sold. At least one insecticide-treated net (ITN) was present in 82.3% of all households, 89.2% of households with a child < 5 years and 57.5% of households without a child < 5 years. Just over half (52.4%) of ITNs had been received during the campaign. Considering possible indicators of universal coverage, 39.8% of households owned at least one ITN per two people, 21.6% owned at least one ITN per sleeping space and 34.7% of the general population slept under an ITN the night before the survey. In addition, 45.6% of children < 5 years, and 49.2% of pregnant women had slept under an ITN., Conclusions: The nationwide integrated LLIN distribution campaign allowed household ITN ownership of one or more ITNs to surpass the RBM target of 80% set for 2010, though additional distribution strategies are needed to reach populations missed by the targeted campaign and to reach the universal coverage targets of one ITN per sleeping space and 80% of the population using an ITN.

In order to increase the effectiveness and the efficiency of diagnosis and treatment procedures of the most common diseases, algorithmic pathways have been developed and applied within the implementation of the "Bamako Initiative Strategy" at the health post level in Senegal. Among the many expected advantages of these pathways, was a reduction of abusive prescriptions of parental drugs. A study comparing the trend of the prescriptions 2 years before (1991) and 2 years after (1996) the implementation of the Bamako Initiative strategy, was conducted using 810 randomly selected patients in 7 out of the 19 health posts of the Tambacounda Health district, one of the 45 health districts of Senegal. In 1991, all the oral drugs prescribed (sirup, pills, powder) represented 54.5% against 45.5% for parental drugs (intramuscular and intravenous drugs). These proportions had risen to respectively 78.1% and 21.9% in 1996. The difference thus observed is statistically significant with a khi2 test of 47.76 and p<10(-7). This result then suggests that the use of pathways had highly reduced the prescription of parental drugs among patients treated in the health posts of the Tambacounda health district in Senegal.

Introduction: Toxic epidermal necrolysis is a severe disease often leading to death or to mucosal, particularly ocular, after effects. The principle drugs responsible are antibacterial sulfonamides, anti-epileptics, non-steroid anti-inflammatories, allopurinol and chlormezanone. We report a series of 38 cases of toxic epidermal necrolysis, observed in Dakar, imputable to thiacetazone and lethal in 60 percent of cases., Patients and Methods: Our study was retrospective. Diagnosis of toxic epidermal necrolysis was made in patients presenting more than 30 p. 100 of the epidermis of their total body surface stripped off, multi-orificial mucosal damage and epidermal necrosis revealed on histological examination. Drug imputability was established on classical criteria. Treatment was composed of reanimation and antibiotics., Results: Among the 38 cases of toxic epidermal necrolysis counted, 24 were imputable to thiacetazone. All the patients presented typical clinical features, confirmed histologically. Evolution was lethal in 60 p. 100 of cases. The causes of death were frequently hypovolemic shock during the first week and septic shock during the second. The deceased were generally aged over 50, had more than 50 p. 100 of total epidermis stripped off, presented evolving tuberculosis at the time of the accident and HIV infection at the AIDS stage. After effects were vaginal synechia and 2 cases of blindness., Comments: Our series is exceptional in that a) the drug responsible: thiacetazone, an economic tuberculostatic of minor efficacy, was systematically introduced after 2 months of intensive treatment with 4 major anti-tuberculosis agents; b) the 60 percent mortality rate, two-fold greater that that usually observed. Other than the known elements of poor prognosis in our patients, the treatment conditions of this dermatological emergency explain this high rate of mortality.

National immunization and control programs have not brought about a significant decline in tuberculosis, which remains a real public health concern in our regions. Prevention in the working environment should be part of an overall prevention program for the general population. Nevertheless, companies should play a leading role because they have the necessary structure and assets. We analyzed the epidemiology of tuberculosis in the working environment using demographic data on employees in Senegal, current medicolegal data on tuberculosis in Senegal and data on prevention of tuberculosis. Our analysis led to a proposed strategy for controlling tuberculosis spread and its prevention in the working environment in Senegal.

Objectives: To document the level of HIV infection in Senegal and also to review evidence of the impact of efforts in prevention, developed by the National AIDS Control Programme and the Civil Society, on the level of the HIV epidemic., Methods: Research, compilation and critical review of all relevant data on HIV and sexually transmission diseases (STDs) epidemiology, sexual behaviour, and the efforts in prevention developed in Senegal., Results: From 1989 to 1996, the levels of HIV infection estimated in four sentinel urban regions remained stable at around 1.2% in the population of pregnant women, and at 3% in male STD patients. It had increased to 19% in female sex workers. A strong political and community commitment led to an early response to the HIV/AIDS epidemic that has been extended since 1986. Blood transfusion safety was established at the start of the HIV epidemic. The level of knowledge of preventive practices relating to HIV/AIDS among the general population exceeded 90% in the early 1990s. From 1991 to 1996, a 30% to 66% decrease of the STD prevalence rates was observed in pregnant women and sex workers in Dakar. In 1997, 33% of men aged 15-49 years in Dakar reported having had sex with non-regular partners. Among them 67% reported condom use., Conclusions: It is not possible to know what the course of the HIV epidemic in Senegal would have taken in the absence of efforts at prevention. Certainly, several factors that pre-dated the occurrence of AIDS in Senegal laid the groundwork for a positive response. However, data from a number of sources do reveal the successfulness of efforts in prevention. From available data, Senegal can rightfully claim to have contained the spread of HIV by intervening early and comprehensively to increase knowledge and awareness of HIV/AIDS and to promote safe sexual behaviour.

This study examines contraceptive use among clients at the three clinics providing family planning services in Dakar, Senegal in early 1983. Most clients first became interested in family planning following the birth of a child, and most are interested in spacing future pregnancies, although one-third state that they want no more children. The clinic itself was found to be an important determinant of the type of contraceptive used, with only the government-operated clinic providing a balance between IUDs, oral contraceptives, and barrier methods. Nearly half of the clients interviewed said that a lack of knowledge about contraception is the reason for the low contraceptive prevalence rates among Senegalese women; another frequently cited reason was the opposition of the husband. Most clients reported the broadcast media to be the best means of providing family planning information to potential acceptors.
