Background: The Omicron variant BA.2 was the dominant variant in the COVID-19 outbreak in Shanghai since March 2022. We aim to investigate the characteristics of SARS-CoV-2 Omicron variant infection in pediatric liver-transplanted recipients. Methods: We conducted a single-center, prospective, observational, single-arm study. We enrolled pediatric liver-transplanted patients infected with the Omicron variant BA.2 from March 19th to October 1st, 2022 and analyzed their demographic, clinical, laboratory, and outcome data. The management of COVID-19 was conducted according to the 9th trial edition of the Chinese guideline. The immunosuppressive therapy was tailored considering the patients' infection developments and liver functions. Results: Five children were included. The primary diseases included Niemann-Pick disease, propionic acidemia, decompensated cirrhosis, biliary atresia, and Crigler-Najjar syndrome type I. All of the patients were onset with fever before or when getting RNA-positive results at the age of 3 (Range: 1–13) years. The infection duration was 29 (Range: 18–40) days. Three and two children were diagnosed with mild and moderate COVID-19 respectively. Two patients were tested RNA-positive within 14 days after having been tested negative. The immunosuppressants were paused or extenuated in four patients. Eight of all nine cohabitants were injected with at least two doses of inactivated SARS-CoV-2 vaccine. The disease courses were significantly longer than the patients (P < 0.05). Conclusions: Post-transplant immunosuppression slows down the virus clearance and increases the risk of relapse but does not affect symptom duration or infection severity in pediatric patients. Introduction: Cesarean scar defect (CSD) is a long‐term outcome of cesarean section (CS) and associated with numerous gynecological and obstetric problems. Previous studies indicate that infection may be a risk factor for CSD. Adjunctive azithromycin was shown to reduce the risk of postoperative infection in patients undergoing non‐elective primary cesarean delivery in labor or after the rupture of membranes compared with standard antibiotic prophylaxis. This study investigated the protective effect of adjunctive azithromycin in combination with single‐dose cephalosporin against CSD in women undergoing non‐elective cesarean delivery. Material and methods: A randomized, double‐blind, controlled clinical trial was conducted in a University hospital in Shanghai, China. A total of 242 women who underwent their first non‐elective CS were randomly assigned to receive 1500 mg cefuroxime sodium plus 500 mg intravenous azithromycin (n = 121; experimental group) or 1500 mg cefuroxime sodium plus a placebo (n = 121; placebo group). The primary outcome was CSD prevalence, as determined by transvaginal ultrasound and saline infusion sonohysterography within 6 months of delivery. Secondary outcomes were changes in infectious indicators (eg hypersensitive C‐reactive protein and procalcitonin), postoperative morbidity, and use of postoperative antibiotics. We also examined the operative procedure, pathogenic microorganism cultures, and fetal outcomes. Outcomes were compared between groups with the chi‐squared test, Fisher's exact test, or Student's t test. Results: Between May 2018 and May 2021, 121 women were randomized to each arm. Because the sonographic follow up was disrupted by the coronavirus disease 2019 pandemic and strict management policies, we merged the follow‐up time points (6 weeks and 6 months) into a single time period (6 weeks to 6 months); 104 and 108 women in the experimental and placebo groups, respectively, completed the first sonographic follow up. CSD was diagnosed by sonography in 34/104 (32.7%) and 50/108 (46.3%) patients in the experimental and placebo groups, respectively (relative risk 0.71, 95% confidence interval 0.50–0.99; p = 0.043). Characteristics of CSD and short‐term infection outcomes did not differ between groups. Conclusions: A single dose of intravenous 500 mg azithromycin adjunctive to single‐dose cefuroxime prophylaxis significantly reduced the incidence of CSD in women undergoing non‐elective CS. Urbanization alters the physicochemical environment on an unprecedented scale and strongly affects biodiversity. How urbanization affects the biodiversity of soil microbial communities, especially in large cities, however, is poorly known. We investigated soil microbial communities from 258 sites covering a variety of environmental gradients in the megacity of Shanghai, China, to determine the impact of urbanization on soil microbial biodiversity. Using the distance to city centre, urbanized land cover, and road density as three proxies to characterize the levels of urbanization, we revealed that increased urbanization was associated with slightly homogenized communities of prokaryotes, total fungi, and arbuscular mycorrhizal fungi but not ectomycorrhizal fungi. The richness of soil prokaryotes and total fungi was weakly but positively related to urbanization as well. For the abundance of microbial phylotypes along urban gradients, we observed synchronous increases and decreases of many phylotypes at relatively high and low urbanization levels, respectively. Further, urbanization explained an independent part of microbial variances in richness and community composition, although the contribution of soil properties in explaining the variances was generally larger than that of urbanization. Together, this work provides evidence for the influences of urbanization on the biodiversity of soil microbes and highlights the importance of considering taxa and the level of urbanization to assess the impacts of urbanization on biodiversity. Based on the analysis of the temperature measurement data of the Shanghai Metro Line 15 cross passage freezing project, it was found that the gray silt layer of cross passage No. 2 outperforms that of cross passage No. 1 on the freezing effect, which is mainly attributed to the large loss of cooling capacity in the latter passage. Within the same stratum, the soil temperature at the duct piece is higher than that of the deep soil. When the soil freezes for 45 days, the temperatures of the sandy silt and gray silt layers of the same cross passage drop to −8.25°C and −6.91°C, respectively, indicating that the freezing effect of the sandy silt layer is better than that of the gray silt layer. Moreover, simulations were performed for deviation freezing pipes, nondeviation freezing pipes, and different freezing pipe diameters in the cross passage No. 1, respectively. It was found that the maximum difference of the closure completion time between the deviation and nondeviation freezing pipes is 6 days. Furthermore, for deviation freezing pipes and nondeviation freezing pipes at the center of the cross passage, the minimum difference in the freezing wall thickness reduces from 0.45 mm after 20 days of freezing to 0.06 mm after 45 days of freezing, indicating that the difference in the freezing wall thickness gradually weakens as freezing develops gradually. The deviation freezing pipe increases the spacing of freezing pipes in the deep soil. As the pipe spacing increases, the influence of the pipe diameter on the closure completion time of the freezing wall decreases. [ABSTRACT FROM AUTHOR]

Purpose: Given the lack of research on the left–right asymmetry of ovarian teratoma among Chinese patients, this study aimed to determine the lateral distribution and related clinical characteristics of Chinese ovarian teratoma patients treated at a single center. Methods: We conducted a cross-sectional study of surgical patients pathologically diagnosed with ovarian teratomas in the gynecology inpatient department of the International Peace Maternity and Child Health Hospital in Shanghai between July 2006 and July 2018. Results: Of the 4417 patients with ovarian teratoma, 3835 were finally analyzed. There were 2030 (53.24%) cases of right-sided benign ovarian teratoma versus 1783 (46.76%) cases of left-sided benign teratoma (P < 0.001). The recurrence rate of benign ovarian teratoma was 4.2%; recurrence occurred more often on the left side (left vs. right = 55 vs. 45%, P = 0.033). Compared with the right-sided ovarian teratoma patients, left-sided ones had significantly high recurrence risk (OR 1.430; 95% CI 1.03–1.99). The rate of ovarian torsion in patients with ovarian mature cystic teratomas (MCTs) during intrauterine pregnancy was 3.17 versus 1.72% in non-pregnant MCT patients (P = 0.049). For those MCT patients with intrauterine pregnancy, ovarian torsion occurs more often on the right side (left vs. right = 16.67 vs. 83.33%, P = 0.028). Conclusion: This study confirms a distinctive right-side dominance of benign ovarian teratomas. Compared with the right side, recurrent ovarian teratomas occur more often on the left side, requiring close follow-up. With the rapid and widespread urbanization, a large number of people pour into cities, which bring a series of urban diseases and directly threaten the sustainable development of the destination city. It is particularly important to reasonably measure the population carrying capacity to promote the sustainable development of cities. Therefore, based on Shanghai's municipal data from 1985 to 2017, this paper used the probability-satisfaction method to predict the urban population carrying capacity of Shanghai in 2020. Several important findings are derived: First, there is a general pattern that the urban population carrying capacity increases as the probability-satisfaction level decreases; second, the sensitive degrees of the population carrying capacity of different constraining factors vary. The sensitive degrees of the city's GDP, fiscal revenues and paved road areas are lower than those of other constraining factors; third, currently the number of medical practitioners, the paved road areas and the volume of waste emission are the three most important constraining factors in Shanghai. Fourth, results of the multifactor analysis reveal that when the probability-satisfaction level is equal to the ideal level, the overall population carrying capacity of Shanghai is between 17.55 million and 23.52 million; when the probability-satisfaction level research the acceptable level, the overall population carrying capacity of Shanghai is between 20.35 million and 30.12 million people. Therefore, by 2020, the Shanghai government needs to formulate well-considering population management plan according to actual resources conditions in order to achieve balanced and sustainable urban development. The majority of research on residential architecture of Shanghai has focused on modern Shikumen. While in Shanghai, there are more than seventy representative residential buildings of Ming and Qing Dynasties, which demonstrate great achievements in technology and art; few efforts have been made on them. Via literature research and on-site investigation, this article contributes to the investigation of timber frames of tingtang of Ming and Qing residences in Shanghai. It studies the characteristics and influential mechanism from three aspects. First, it reviews the history of residences development in Shanghai. Second, it interprets the establishment of the timber frame style. Through-jointed frame in ancient Wu Area was influenced by the space need and the Central Plains' raised-beam frame. It finally evolved into "zhengtie raised-beam frame + biantie through-jointed frame" style by Xiangshan Bang carpenters. Third, the article discusses the regional characteristics of being material economic and flexible collocation, which reflects the local customs. The study clarifies the importance of social background influence on timber frames of tingtang of Ming and Qing residences in Shanghai. It is one of the series of architectural studies in Jiangnan region, and appeals for more attention to the protection and maintenance of traditional architectures in Shanghai. [ABSTRACT FROM AUTHOR]

The magnetic resonance sounding (MRS) method can noninvasively, directly, and quantitatively detect groundwater and forecast the potential for a water-induced disaster during tunnel construction. However, due to the spatial limitations of underground engineering, the signal-to-noise ratio (SNR) of the receiving signal is low (<1), which severely limits the reliability of the disaster warning. Thus, the question of how to design the meter-scale antenna and improve the SNR has become the bottleneck that restricts the successful application of the underground MRS method. To address this problem, we comprehensively consider the factors of detection depth, signal amplitude, and antenna size and weight using theoretical calculations and model simulations in this paper. We determine that an 8-turn transmitting antenna and a 30-turn receiving antenna with both side lengths of 6 m constitute an effective combination for achieving reliable detection of large water-bearing structures. Meanwhile, we propose a front-end matching circuit for a high-inductance antenna to solve the problem of attenuation of the signal amplitude due to high impedance and successfully increase the SNR from 1.12 to 5.13. Finally, in the tunnel of the Shanghai–Kunming high-speed railway of China, we perform the field measurement of an underground river and validate the practicability of the proposed antenna and matching circuit. The travel behavior of passengers from the transportation hub within the city area is critical for travel demand analysis, security monitoring, and supporting traffic facilities designing. However, the traditional methods used to study the travel behavior of the passengers inside the city are time and labor consuming. The records of the cellular communication provide a potential huge data source for this study to follow the movement of passengers. This study focuses on the passengers' travel behavior of the Hongqiao transportation hub in Shanghai, China, utilizing the mobile phone data. First, a systematic and novel method is presented to extract the trip information from the mobile phone data. Several key travel characteristics of passengers, including passengers traveling inside the city and between cities, are analyzed and compared. The results show that the proposed method is effective to obtain the travel trajectories of mobile phone users. Besides, the travel behavior of incity passengers and external passengers are quite different. Then, the correlation analysis of the passengers' travel trajectories is provided to research the availability of the comprehensive area. Moreover, the results of the correlation analysis further indicate that the comprehensive area of the Hongqiao hub plays a relatively important role in passengers' daily travel. Introduction: The aim of this study was to explore the prevalence and predictors of cesarean scar defect (CSD) at 6 weeks postpartum in Shanghai, China.Material and Methods: Women scheduled to receive a cesarean section (CS) were recruited from a university hospital. Surgery-related factors, pregnancy complications, routine examinations, perioperative medications, and physical signs were collected and transvaginal ultrasonography was performed to assess the presence of a CSD at 6 weeks postpartum. Multivariate logistic regression was applied to identify the predictors for CSD.Results: A CSD was detected in 223 of 514 women (43.4%, 95% CI 39.1%-47.7%) by transvaginal ultrasonography. Of women with normal temperature (T < 37.5°C, CSD prevalence 33.9%, 95% CI 28.4%-39.5%), women with postpartum fever (T ≥ 38°C, CSD 44.1%, 95% CI 31.0%-57.1%), and women who were subfebrile in the postpartum (37.5 ≤ T ≤ 38.0°C, CSD 58.3%, 95% CI 50.9%-65.7%), the latter two had significantly increased risk for CSD (adjusted odds ratio [aOR] 2.7, 95% CI 1.3-5.2 and aOR 3.3, 95% CI 2.1-5.3, respectively). In comparison to single-dose antibiotic administration (CSD 49.0%, 95% CI 43.8%-54.3%), multi-dose antibiotic administration (CSD 31.1%, 95% CI 23.8%-38.3%) had a protective effect (aOR 0.4, 95% CI 0.3-0.7). Postpartum fever, intrapartum infection, emergency CS, and cervical dilation ≥ 3 cm were found to be the main predictors of multi-dose antibiotics management. Low platelet count postpartum (defined as ≤ 150 × 109 /L) and high fibrinogen pre-CS (defined as ≥ 4.5 g/L) increased CSD risk (aOR 2.0, 95% CI 1.1-3.6 and 1.7, 95% CI 1.1-2.5, respectively).Conclusions: The prevalence of CSD in the Chinese population is high enough to be a concern. Postmenstrual spotting and chronic pelvic pain after cesarean delivery are associated with the presence of niches. Levonorgestrel intrauterine system (52 mg) and hysteroscopic niche resection have been shown to relieve niche-related symptoms at 6 months after the intervention. This trial aimed to compare the effectiveness of 52-mg levonorgestrel intrauterine system with that of hysteroscopic niche resection in reducing niche-related postmenstrual spotting. This randomized, open-label, controlled trial was conducted at a medical center in Shanghai, China. Women with symptoms of postmenstrual spotting after cesarean delivery, with a niche depth of at least 2 mm and residual myometrium of at least 2.2 mm on magnetic resonance imaging, and no intention to conceive within the next year were randomly assigned to receive treatment with 52-mg levonorgestrel intrauterine system or hysteroscopic niche resection. The primary outcome was the reduction in postmenstrual spotting at 6 months after randomization, defined as the percentage of women with a reduction of at least 50% in spotting days relative to baseline. Efficacy and safety were assessed using intention-to-treat analysis. Between September 2019 and January 2022, 208 women were randomized into the levonorgestrel intrauterine system group (N=104) or the hysteroscopic niche resection group (N=104). At the 6-month follow-up, a 50% reduction in spotting had occurred in 78.4% (80/102) of women in the levonorgestrel intrauterine system group and in 73.1% (76/104) of women in the hysteroscopic niche resection group (relative risk, 1.07 [95% confidence interval, 0.92–1.25]; P =.370). Spotting decreased over time (P trend =.001), with a stronger reduction observed in the levonorgestrel intrauterine system group (P =.001). There was also a significant interaction between time and treatment (P =.007). From 9 months onward, a more significant reduction in spotting was observed in the levonorgestrel intrauterine system group than in the hysteroscopic niche resection group (9 months, 89.2% vs 72.1%; relative risk, 1.24 [95% confidence interval, 1.08–1.42]; 12 months, 90.2% vs 70.2%; relative risk, 1.29 [95% confidence interval, 1.12–1.48]). Moreover, compared with the hysteroscopic niche resection group, the levonorgestrel intrauterine system group had significantly fewer postmenstrual spotting days and total bleeding days from 6 months onward (all P <.001), and less pelvic pain from 3 months onward (all P <.010). No intervention-related complications were reported in any group. During follow-up, 11 (10.8%) women reported hormone-related side effects, and 2 women (2.0%) in the levonorgestrel intrauterine system group had spontaneous partial expulsion. Meanwhile, 3 unintended pregnancies were reported in the hysteroscopic niche resection group. In women with niche-related postmenstrual spotting, the levonorgestrel intrauterine system was not more effective than hysteroscopic niche resection in reducing the number of spotting days by at least 50% at 6 months. However, the levonorgestrel intrauterine system was superior in reducing spotting from 9 months onward, and it reduced the absolute number of spotting days from 6 months onward and pelvic pain from 3 months onward. This article aims to examine the actual severity of criminal sanctions on identity theft imposed by Shanghai courts, which entails an analysis of the effects of eight factors on the probability of imprisonment, probation and the length of imprisonment/detention in 507 Shanghai courts' judgements. Since the Supreme People's Court promulgated the Judicial Interpretation on sanctioning identity theft crimes in 2017, the severity of criminal punishment substantially increased. Judges seemed to have considerable discretion to deviate from the sentencing criteria set by the Criminal Code and the 2017 Judicial Interpretation. Judges' discretion indicates a gap between formal criminal law and its enforcement. A more coordinated legal framework of personal information protection that contains criminal law, administrative law and civil law might be more effective in fighting identity theft. As a medical course approach of posing contextaulized questions that are based upon "real life" clinical problems, case-based learning (CBL) is used to stimulate and underpin the acquisition of knowledge, skills, and attitudes by clinical cases. Good effects were acquired in the practice of CBL teaching in English applied by College of Stomatology, Shanghai Jiao Tong University. Responses from both students and teachers reflected that this course approach won their high acceptability, and was worthy of use in selected stomatological courses. To determine whether hysteroscopic niche resection (HNR) and expectant management are suitable in women with fertility desire and a niche with a residual myometrium thickness (RMT) ≥ 2.5 mm. This retrospective cohort study was conducted at International Peace Maternity and Child Health Hospital, School of Medicine, Shanghai Jiaotong University, Shanghai, China between September 2016 and December 2021. We reported the fertility outcomes between women (with fertility desire and a niche with RMT ≥ 2.5 mm) who received HNR or expectant management. We studied 166 women, of whom 72 accepted HNR and 94 accepted expectant management. The HNR group included more symptomatic women, in terms of postmenstrual spotting or infertility. No differences were found regarding niche measures before treatment. The live birth rate was comparable in both groups (HNR versus expectant management as 55.5% versus 45.7%, risk ratio = 1.48, 95% Cl 0.80–2.75, p = 0.21). The pregnancy rate was higher in HNR group than that in expectant management group (n = 72.2% versus n = 56.4%, risk ratio = 2.01, 95% CI 1.04–3.88, p = 0.04). In a subgroup of women with infertility before entry in the study, HNR resulted in a significant higher live birth rate (p = 0.04) and pregnancy rate (p = 0.01). In women with infertility with a symptomatic niche with RMT ≥ 2.5 mm, HNR may be superior to expectant management. This retrospective cohort biased selection against a randomized study, our results still need to be validated in the future with larger clinical multicenter randomized controlled trials. Objective: To evaluate fertility intensions among couples in Shanghai under the novel coronavirus infection (COVID‐19) pandemic against the backdrop of persistently low fertility. Methods: A cross‐sectional study was carried out using data from studies conducted before the COVID‐19 pandemic. Data were collected regarding sociodemographic characteristics, history of reproduction and gynecology, fertility intention before and after the COVID‐19 pandemic, female psychological state, and the impact of the COVID‐19 pandemic on daily life. Results: Under the influence of COVID‐19, 296/447 (66.2%) participants did not change their original fertility intention to have children, while 151/447 (33.8%) of participants were affected by the outbreak. Participants who believed in government and hospital control policies were less likely to change their intention to become pregnant (P < 10–3, P < 10–3). In contrast, concerns about the impact of COVID‐19 on female and fetal health led participants to cancel their original pregnancy plans (P < 10−3). Conclusion: Three in ten couples of childbearing age, who originally expressed their intention of becoming pregnant, canceled their pregnancy plans after the COVID‐19 outbreak. The COVID‐19 outbreak has brought new challenges to people's physical and mental health. Effective policies and measures can help to improve people's fertility intentions with respect to having children.
