Aims: To assess the feasibility of implementing and sustaining the WHO guidelines for inpatient management of severe malnutrition in under-resourced rural South African hospitals, and to identify any constraints. Intervention: Three 2-day training workshops were held in 1998, followed by monthly 1 -day visits for 5 months, ending in March 1 999, in two rural district hospitals with limited resources in Eastern Cape Province, South Africa. Methods: A 12-month observational study was conducted from April 2000 to April 2001 in Mary Theresa and Sipetu hospitals (Eastern Cape Province, South Africa), including 1011 child-hours of observation on the wards, medical record reviews, interviews with carers and staff, and inventories of essential supplies. All admissions (n = 193) for severe malnutrition to the two hospitals were studied. The main outcomes were the extent to which the 10 steps for routine care of severely malnourished children were implemented, proficiency of performance and constraining factors. Results: The hospitals made the changes required in clinical and dietary management, but the tasks were not always performed fully or with sufficient care. Play and stimulation and an effective system of follow-up were not implemented. Doctors' poor knowledge, nurses' inattentiveness and insufficient interaction with carers were constraints to optimal management. The underlying factors were inadequate undergraduate training, understaffing, high doctor turnover and low morale. Conclusions: Guidelines for severe malnutrition are largely feasible but training workshops are insufficient to achieve optimal management as staff turnover and an unsupportive health system erode the gains made and doctors treat cases without having being trained. Background: It is three years since the government of South Africa began implementing a PMTCT programme. Over this period of time attempts have been made to scale up this programme across all provinces under routine health service conditions., Objectives: To report on the uptake and performance of South Africa's national pilot programme for preventing mother to child HIV transmission (PMTCT) and to identify health system constraints to optimal coverage., Methods: Routine programme data were collected from antenatal records and delivery registers at the pilot sites and interviews were conducted with health workers on site and with provincial programme managers., Results: Routine PMTCT programme data were collected from all 18 pilot sites for the period January to December 2002. During this period, of 84406 women attending the sites for first antenatal visits, 47267 (56%) agreed to an HIV test. 14340 (30%) of the women tested were HIV positive and of these 7853 (55%) were dispensed nevirapine. 7950 (99%) of infants born to women identified as being HIV positive received nevirapine syrup. 58% (4196/7237) of HIV positive women expressed an intention to exclusively formula feed, and 42% (3041/7237) intended to exclusively breastfeed. 1907 infants were due for 12 month HIV testing between January and December 2002, of these 949 (50%) infants were tested., Conclusion: Programme effectiveness was limited by the low rate of HIV test acceptance, poor delivery of nevirapine to mothers and inability to track mother-infant pairs postnatally for 12-month HIV testing of infants. Infant feeding intentions of mothers suggest inadequate counselling and possible negative effects of the provision of free formula milk. The poor performance of the main components of this programme will seriously reduce its operational effectiveness. There is a need for greater integration of VCT within antenatal care, a review of the current policy of providing free formula milk and an alternative model for mother-infant follow up.

Background: WHO case-management guidelines for severe malnutrition aim to improve the quality of hospital care and reduce mortality. We aimed to assess whether these guidelines are feasible and effective in under-resourced hospitals., Methods: All children admitted with a diagnosis of severe malnutrition to two rural hospitals in Eastern Cape Province from April, 2000 to April, 2001, were studied and their case-fatality rates were compared with the rates in a period before guidelines were implemented (March, 1997 to February, 1998). Quality of care was assessed by observation of medical and nursing practices, review of medical records, and interviews with carers and staff. A mortality audit was used to identify cause of death and avoidable contributory factors., Findings: At Mary Theresa Hospital, case-fatality rates fell from 46% before implementation to 21% after implementation. At Sipetu Hospital, the rates fell from 25% preimplementation to 18% during 2000, but then rose to 38% during 2001, when inexperienced doctors who were not trained in the treatment of malnutrition were deployed. This rise coincided with less frequent prescribing of potassium (13% vs 77%, p<0.0001), antibiotics with gram-negative cover (15% vs 46%, p=0.0003), and vitamin A (76% vs 91%, p=0.018). Most deaths were attributed to sepsis. For the two hospitals combined, 50% of deaths in 2000-01 were due to doctor error and 28% to nurse error. Weaknesses within the health system--especially doctor training, and nurse supervision and support--compromised quality of care., Interpretation: Quality of care improved with implementation of the WHO guidelines and case-fatality rates fell. Although major changes in medical and nursing practice were achieved in these under-resourced hospitals, not all tasks were done with adequate care and errors led to unnecessary deaths.

Objective: To improve the clinical management of severely malnourished children in rural hospitals in South Africa., Study Design: A pre- and post-intervention descriptive study in three stages: assessment of the clinical management of severely malnourished children, planning and implementing an action plan to improve quality of care, and monitoring and evaluating targeted activities. A participatory approach was used to involve district and hospital nutrition teams in all stages of the research., Setting: Two rural first-referral level hospitals (Mary Theresa and Sipetu) in Mount Frere District, Eastern Cape Province., Main Measure: A retrospective record review of all admissions for severe malnutrition to obtain patient characteristics and case fatality rates, a detailed review of randomly selected cases to illustrate general case management, structured observations in the paediatric wards to assess adequacy of resources for care of malnourished children, and in-depth interviews and focus group discussions with nursing and medical staff to identify barriers to improved quality of care., Results: Before the study, case fatality rates were 50% and 28% in Mary Theresa and Sipetu hospitals, respectively. Information from case studies, observations, interviews, and focus group discussions revealed many inadequacies in knowledge, resources, and practices. The hospital nutrition team developed and implemented an action plan to improve the quality of care and developed tools for monitoring its implementation and evaluating its impact. In the 12-month period immediately after implementation, case fatality rates fell by approximately 25% in both hospitals., Conclusion: Participatory research led to the formation of a hospital nutrition team, which identified shortcomings in the clinical management of severely malnourished children and took action to improve quality of care. These actions were associated with a reduction in case fatality rates.
