Objective: To describe African American and White women's perceptions of weight gain, physical activity, and nutrition during pregnancy and to explore differences in perceptions by race., Design: Qualitative interview study., Setting: Two Ob/Gyn clinics in South Carolina, USA., Participants: Thirty pregnant women (15 African American, 15 White) between 20 and 30 weeks gestation, equally represented across pre-pregnancy BMI categories (10 normal weight, 10 overweight, and 10 obese)., Findings: White women more frequently described intentions to meet weight gain, physical activity, and dietary guidelines in pregnancy than African American women. African American women were more concerned with inadequate weight gain while White women more commonly expressed concerns about excessive weight gain. More White women discussed the importance of physical activity for weight management. Regardless of race, few women described risks of excessive weight gain or benefits of physical activity as it relates to the baby's health. The primary cited barrier of healthy eating was the high cost of fresh produce., Key Conclusions and Implications for Practice: Several knowledge gaps as well as race differences were identified in women's perceptions and intentions toward weight gain, physical activity, and nutrition during pregnancy. Future interventions should seek to educate women about common misperceptions. This study examined associations of sleep and minutes spent in moderate-vigorous physical activity (MVPA) with C-reactive protein (CRP) and interleukin (IL)-6 among persons living with HIV. Cross-sectional analyses (n = 45) focused on associations of inflammatory outcomes (i.e., CRP and IL-6) with actigraph-derived sleep duration, latency, and efficiency; sleep onset; wake time; and wake-after-sleep-onset; as well as MVPA. Least square means for CRP and IL-6 by levels of sleep and MVPA were computed from general linear models. Individuals below the median of sleep duration, above the median for sleep onset, and below the median of MVPA minutes had higher CRP or IL-6 levels. Generally, individuals with both low MVPA and poor sleep characteristics had higher inflammation levels than those with more MVPA and worse sleep. Understanding the combined impact of multiple lifestyle/behavioral factors on inflammation could inform intervention strategies to reduce inflammation and therefore, chronic disease risk.

This investigation sought to determine the association of symptom distress with selected psychological factors in HIV-infected persons. Data from a randomized controlled trial were used; all subjects who completed baseline data collection were included (N = 99). Data packets included these questionnaires: the Perceived Stress Scale, HIV-related Symptom Distress Scale, and Profile of Mood State. Significant correlations were included in a final regression model. The Perceived Stress Scale, total mood disturbance (including the Profile of Mood State subscales), self-rated current health, and HIV status were independently associated with both frequency of symptoms and symptom distress. Symptom frequency, depression, anger, and fatigue retained significance in the final regression model. Findings from this study indicated significant associations of multiple psychological correlates, suggesting that symptom distress is a complex outcome with a multifactorial etiology. Psychological factors such as depression, anger, and fatigue contribute to the level of distress experienced with HIV-related symptoms., (Copyright © 2014 Association of Nurses in AIDS Care. Background: Churches are an appealing setting for implementing health-related behavior change programs., Purpose: The objective of the study was to examine the relationship between perceived environmental church support for physical activity (PA) and PA behaviors., Method: Black church members from South Carolina (n = 309) wore an Actigraph accelerometer prior to the initiation of an intervention. Relationships between moderate to vigorous PA (MVPA; counts ≥1,952), light PA (LPA; counts 100-1951), sedentary behavior (counts <100), and perceived environmental church support for PA (total, spoken informational, written informational, instrumental) were examined. Support × Gender interactions examined whether relationships differed by gender., Results: The mean age was 54.0 ± 12.3 years and mean body mass index was 32.9 ± 7.2 kg/m(2). On average, participants engaged in 14.4 ± 13.7 minutes/day of MVPA, 289.8 ± 82.4 minutes/day of LPA, and were sedentary 548.9 ± 102.2 minutes/day. Total, spoken informational, and written informational church support were significantly related to higher levels of LPA and lower levels of sedentary behavior in men but not women. The relationship between written informational support and MVPA approached significance for men but not women. Instrumental church support was not associated with PA behaviors., Conclusion: The church environment may have an important influence on Black church members' PA behavior, particularly men's, and thus should be targeted in interventions.

Background: The SenseWear™ Armband (SWA) (BodyMedia, Inc. Pittsburgh, PA) is a physical activity and lifestyle monitor that objectively and accurately measures free-living energy balance and sleep and includes software for self-monitoring of daily energy expenditure and energy intake. The real-time feedback of the SWA can improve individual self-monitoring and, therefore, enhance weight loss outcomes., Methods: We recruited 197 sedentary overweight or obese adults (age, 46.8 ± 10.8 y; body mass index (BMI), 33.3 ± 5.2 kg/m(2); 81% women, 32% African-American) from the greater Columbia, South Carolina area. Participants were randomized into 1 of 4 groups, a self-directed weight loss program via an evidence-based weight loss manual (Standard Care, n = 50), a group-based behavioral weight loss program (GWL, n = 49), the armband alone (SWA-alone, n = 49), or the GWL plus the armband (GWL+SWA, n = 49), during the 9-month intervention. The primary outcome was change in body weight and waist circumference. A mixed-model repeated-measures analysis compared change in the intervention groups to the standard care group on weight and waist circumference status after adjusting for age, sex, race, education, energy expenditure, and recruitment wave., Results: Body weight was available for 62% of participants at 9 months (52% standard care, 70% intervention). There was significant weight loss in all 3 intervention groups (GWL, 1.86 kg, P = 0.05; SWA-alone, 3.55 kg, P = 0.0002; GWL+SWA, 6.59 kg, P < 0.0001) but not in the Standard Care group (0.89 kg, P = 0.39) at month 9. Only the GWL+SWA group achieved significant weight loss at month 9 compared to the Standard Care group (P = 0.04). Significant waist circumference reductions were achieved in all 4 groups at month 9 (Standard Care, 3.49 cm, P = 0.0004; GWL, 2.42 cm, P = 0.008; SWA-alone, 3.59 cm, P < 0.0001; GWL+SWA, 6.77 cm, P < 0.0001), but no intervention group had significantly reduced waist circumference compared to the Standard Care group., Conclusions: Continuous self-monitoring from wearable technology with real-time feedback may be particularly useful to enhance lifestyle changes that promote weight loss in sedentary overweight or obese adults. This strategy, combined with a group-based behavioral intervention, may yield optimal weight loss.

Background: Few prospective studies have simultaneously investigated the relationship between physical activity, cardiorespiratory fitness (CRF), and the development of hypertension in initially normotensive individuals. In the Aerobics Center Longitudinal Study (ACLS), we examined this association among initially healthy normotensive men., Methods: Participants were 16,601 men aged 20-82 years who completed a baseline examination during 1970-2002 and were followed for hypertension incidence. Physical activity was self-reported and CRF was quantified from the duration of a maximal treadmill test., Results: A total of 2,346 men reported hypertension during a mean 18 years of follow-up. Event rates per 10,000 man-years adjusted for age and examination year were 86.2, 76.6, and 66.7 across physical activity groups of sedentary, walker/jogger/runner (WJR), and sport/fitness, respectively, and 89.8, 78.4, and 64.6 for low, middle, and high CRF, respectively (trend P < 0.0001). These associations persisted after further adjustment for body mass index (BMI), smoking, alcohol intake, resting systolic blood pressure, baseline health status, family history of diseases, and survey response patterns., Conclusion: Both physical activity and CRF are associated with lower risk of developing hypertension in a graded fashion. These findings provide a basis for health professionals to emphasize the importance of participating in regular physical activity to improve fitness for the primary prevention of hypertension in men.

This prospective study evaluated regular physical activity and self-reported physician-diagnosed osteoarthritis of the knee and/or hip joints among 16,961 people, ages 20-87, examined at the Cooper Clinic between 1970 and 1995. Among those aged 50 years and older, osteoarthritis incidence was higher among women (7.0 per 1000 person-years) than among men (4.9 per 1000 person-years, P = 0.001), while among those under 50 years of age, osteoarthritis incidence was similar between men (2.6) and women (2.7). High levels of physical activity (running 20 or more miles per week) were associated with osteoarthritis among men under age 50 after controlling for body mass index, smoking, and use of alcohol or caffeine (hazard ratio = 2.4, 95% CI: 1.5, 3.9), while no relationship was suggested among women or older men. These findings support the conclusion that high levels of physical activity may be a risk factor for symptomatic osteoarthritis among men under age 50.

Background: The ideal screening tool for worksite health screening programs has not yet been identified, and the effectiveness of screening programs in predicting future chronic disease risk factors status is not well understood. This study compared two commonly used screening tools, health-related questionnaires and measurement of physiological parameters, in terms of their ability to predict future chronic disease risk factor status in an employee population., Methods: Data were collected over a 10-year period from Liberty Corporation employees (N = 723). Baseline evaluation included the administration of questionnaires and measurement of physiological data. Follow-up evaluation measured physiological data only. Regression analyses identified the statistically significant predictors of future risk factor status using 1) baseline questionnaire data; 2) baseline physiological data; and 3) both questionnaire and physiological data as predictor variables., Results: Although both screening methods were able to predict future risk factor status, R 2 values were higher in models including baseline physiological measures than models including questionnaire data only. Adding questionnaire data to physiological data provided little or no additional predictive ability., Discussion: These results suggest that physiological data, particularly baseline measures of a risk factor, are more predictive of future risk factor status than questionnaire data.

This paper uses data from an employee health promotion project for government employees to examine initial health practices and their relationship to social and demographic variables. It then uses data collected one year later to examine changes in health behaviors and to try to explain what types of people are most likely to undertake health behavior changes in a year, within the context of a worksite health promotion project. Most people in this sample of employees do make positive changes in health habits in at least one of the following areas: smoking, seatbelt usage, diet, exercise, alcohol usage. While a variety of different social and demographic variables are important in explaining initial differences in health practices, these same variables along with measures of personal efficacy and job stress are poor predictors of whether people change their health behavior over a year. Future research might usefully focus on more detailed collection of qualitative data to help understand what factors motivate people to change health behavior. Future survey approaches may then incorporate broader and more diverse categories of explanatory variables into regression models.
