Background: Despite a global policy push toward the advancement of family- and community-based care, residential care for children and youth remains a relevant and highly utilized out-of-home care option in many countries, fulfilling functions of care and accommodation as well as education and treatment. Objective: As part of a larger project involving five European countries (Finland, Germany, Italy, Lithuania, and Spain), the objective was "to map" the context and content of residential care in each country, thereby building a foundation for meaningful comparisons and deepened understanding of each system's inherent logic. Within the context of global deinstitutionalization efforts, the study also aimed to understand factors that hinder or enhance the transformation of residential care. Method: Using an embedded multiple-case design, data was gathered by each country on its residential care macro context as well as salient variables related to three units of analysis--residential care system/program features, residential care training and personnel, characteristics of youth. Cross-case synthesis was used to summarize and compare cases across relevant dimensions. Results: The analysis highlighted areas of overlap and singularity, particularly with regard to utilization rates, concepts and methods, workforce professionalization, and characteristics of youth. Conclusions: Findings provide a more nuanced understanding of how residential care continues to be viewed and utilized in some countries, challenging the 'residential-care-as-a-last-resort-only' rhetoric that is currently dominating the discourse on residential care. It further provides an understanding of historical and sociocultural factors that need to be considered when trying to transform services for children, youth, and their families.

The aim of this study was to analyze the prevalence of mental health disorders in children living in residential care and their use of therapeutic services, including the relationship between these factors and social-family and intervention process variables, as well as the relationship among the disorders identified by professionals and the detection of disorders by means of a screening instrument. We studied a sample from the Autonomous Community of Extremadura in Spain made up of 264 children ages 6 to 18 years. Data collected from residential care workers on the children's characteristics and the existence of disorders and possible treatments were used, as well as the "Child Behavioral Checklist" as a screening instrument. In addition, the case social workers provided information on social-family and intervention process variables. Results show that 26% of the children in residential care were receiving or had received treatment for mental health disorders. Seventeen percent of the total presented mental disability, and in half of these cases, they were also being treated for mental disorder. The screening test, however, detected that cases with clinical range accounted for 44% of the children and that there was considerable bias in relation to referrals for treatment.

The authors carried out an assessment of social support networks with a sample of 884 Spanish adolescents aged 12 to 17. The main goal was to analyze the development of the figures of parents and peers as providers of social support in the two basic dimensions of emotional and instrumental support. In peers, they distinguished between the contexts from which they came (school, associations, and friends from the community) to observe whether there were different tendencies. The results of the MANOVA indicate significant effects on both dimensions for the factors age, provider, and interaction between the two. The authors conclude that the tendencies for decline in the provision of support from parents and the corresponding increase in support from peers during adolescence are different for each dimension, and also for the different types of friends, according to context. Parents maintain a good level of instrumental support, but not of emotional support. (Contains 3 tables and 2 figures.)

Transition to adulthood is an accelerated and early process for care leavers that requires intensive preparation, training and support from child welfare services. This study aimed to explore the perceived readiness for independent living of a group of care‐experienced young people preparing for leaving care in Spain and to compare it with the perceptions of their peers from the general population. A sample of 508 youth (50% women) aged 14–21 (M = 16.67; SD = 1.72) took part, of whom 279 were care experienced and 229 belonged to the general population in Spain. Participants' independent living skills, personal autonomy, self‐efficacy and sociodemographic characteristics were assessed through an online survey using standardized instruments. Care‐experienced young people displayed higher levels of life skills and autonomy in self‐care, daily living at home and employment domains, but not related to making daily arrangements in their community. However, their educational level and self‐efficacy levels were lower than in the comparison group. Work experience stood out as a significant predictor of care‐experienced young people's life skills level. These findings support the importance of assessing life skills as an outcome of leaving care preparation services and providing care‐experienced young people with real‐life experiences to develop their life skills. Money has become a key element in any kind of relationship, both within and outside national borders. However, different cultures may give money a distinct role in society. Hence, the current study investigated the relationship between cultural values and the love of money. A survey was carried out to collect data from 813 individuals in total, 613 Spanish and Portuguese and 200 U.S. Hispanic. The hypotheses were tested through structural equation modeling (SEM) with partial least squares (PLS). Despite several differences in the results of both samples, Iberian and North American Hispanic samples have in common the fact that long-term orientation and indulgence positively influence the love of money budget factor. This study presents new evidence and reinforces fresh insights for managers to better understand their staff worldwide. Furthermore, organizational policies and practices related to ethics and organizational behavior should be adequately implemented, according to the love of money perception of members of their teams. Abstract: Therapeutic residential care (TRC) is the name given to specialized children's homes for treating cases with severe emotional and behavioural problems that have been placed in residential care. A recent international review has revealed great diversity in the referral criteria of cases and in the models of intervention carried out. The goal of this study is to describe the population treated in these types of facilities in Spain and the therapeutic coverage given. The sample is made up of 215 young people in children's homes, of whom 93 are in TRC. The cases referred to TRC have been in residential care for less time but have gone through a greater number of placements. These young people also exhibit more problems of drug use, and there is a larger percentage of clinical‐range cases in the Child Behaviour Checklist scales of attention problems and aggressive behaviour. Nevertheless, the results of logistic regression indicate that the only variables that significantly increase the probability of being referred to TRC are drug use and changes of care placements. With regard to therapeutic care, there is a higher percentage in the TRC group receiving psychiatric care, and the sessions are also more frequent. [ABSTRACT FROM AUTHOR]

Background: The voice of foster families is a valuable tool in the development and improvement of foster family services. Regularly evaluating the satisfaction of foster carers can facilitate the early identification of a range of problems that might pose a risk to the placement. Method: This article reports the experience of 200 Spanish foster families (kinship and non-kinship) with foster services in relation to motivation for becoming foster carers, sources of stress and reward, satisfaction with the services and needs. Semi-structured interviews were performed. The aims of the research are principally of a descriptive character, so each group of variables was examined using frequency analysis. Results: The foster carers interviewed demonstrated a high degree of satisfaction with the foster programs, although some areas seem more problematic, such as financial compensation, information provided about the fostered child, contact with the birth family and the sensitivity of professionals. Conclusions: This study reveals several differences with regard to international literature, that are related to particularities of the Spanish child care system. The results may be extremely useful for the implementation of policy changes which could contribute to raised levels of satisfaction for the foster carers, and increased effectiveness of the programs. There is an international consensus a bout the need to avoid prolonged stays in residential care and a preference for family-based interventions for those children who have been separated from their families. Nevertheless, Spanish statistics show a high number of children entering residential placements and little development of non-kinship family foster programmes. This article analyses the factors which influence the intensive use of residential placements and the reasons which lead to many children spending long periods of their lives in residential facilities. These questions are investigated through a study of 238 children aged twelve and under in residential foster-care, who had spent at least a third of their lives in that situation. The case information was obtained through collaboration with social workers who supplied data about the profile of the children, their families, their care histories and prospective outcomes. Certain characteristics of the children (age, physical and psychological problems) and the family of origin (serious psycho-social problems) seem to explain the long stays of these children in residential care. The analysis of the historical development of child protection in Spain and Italy shows remarkable common aspects. There has been a strong tradition of using residential care by means of large institutions, mostly run by catholic religious organizations, in both countries, and both have had to face the challenge of changing the tendency of this institutional care to a family based care. On the other hand, both countries share a Mediterranean culture where the family has been the main provider of personal and social wellbeing, although the welfare state has achieved a high level of development with large health, education, and social services coverage in the last decades. The current financial crisis of these countries is jeopardizing these recently attained advances and again the importance of family support is becoming crucial. The article reviews the historical development of out-of-home care in both countries and compares foster family and residential care, as well as the main research contributions to these topics in both countries. Abstract: Children and adolescents who live in out of home care in the child protection system are considered to be vulnerable to manifesting mental health disorders as well as other types of difficulties. This risk is greater in the case of children who display any type of disability. The aim of this study is to profile the state of health and well-being of a group of children presenting intellectual disability who live in residential care in a Spanish autonomous community and to compare these results with their non-disabled peers. Two questionnaires filled in by the caseworker and key residential worker, respectively, were used and a screening test (CBCL) was administered to assess the children's emotional and behavioral status. The results indicate that there are more vulnerability factors in this group of children comparing to their peers; in particular, a greater likelihood of having parents with a history of mental health and alcoholism and greater frequency of intellectual disability in their mothers. Likewise, they had a greater probability of having suffered physical abuse. The screening revealed the existence of greater alterations on the scale of social and thought problems, as well as a greater probability of being referred for treatment and greater use of psychotropic medication. The development of molecular methods is becoming a promising field in the analysis of parasite fauna in wildlife species. This is especially useful in the case of parasite species where developmental larval stages are difficult to assess using standard methods. In this study we screened for the presence of parasitic nematodes infecting lacertid lizards from the Iberian Peninsula and Balearic islands using nematode-specific 18S ribosomal RNA gene primers on host tissue samples. Sequencing of positive samples revealed the presence of different genera of nematodes. The detection of Strongyloides, a monoxenous genus reported for the first time in Podarcis lilfordi is probably the result of the amplification of larval stages present in the host circulatory system. Two spirurid nematodes, Synhimantus and a new unidentified clade, were also found, suggesting that reptiles might be paratenic hosts of several spirurid species. This study demonstrates the benefits of using specific molecular markers on tissue samples to identify infecting larval stages of nematodes, otherwise difficult to assess using traditional screening methods. ABSTRACT In this paper, we analyse reunification processes from family foster care, both kinship and non-kinship, and the variables associated with them in a Spanish sample. Data collection was carried out after a review of child protection and foster care files, and those responsible for the cases were also consulted. The cases studied were closed, which made it possible to assess the final outcome of the fostering. We found that only one-fifth of the children return to their birth family after the fostering, and that those in non-kinship are less likely to do so. Seven factors were associated with family reunification processes: age of the child, kinship care with family co-operation, drug dependence in parents, neglect due to alcoholism, foster care under voluntary arrangement with visits, short-term fostering with family co-operation and parental impossibility due to imprisonment. The contribution of these data takes on particular importance given that the Spanish child protection system keeps no official statistics on the development or outcome of its interventions and records even less information on the factors associated with such outcomes. Abstract: Kinship care represents 46% of all out-of-home care placements in Spain and 80% of all foster care placements. Therefore, the role of fostering relatives, most of whom (about 70%) are grandparents, is particularly relevant in the Spanish child care system. In 1998 we carried out a descriptive study on profiles of kinship care in Spain, including children, birth parents, fostering relatives and patterns of process and intervention, with a sample of 568 cases. This paper presents a follow-up study of a part of this group who were adults in 2006, aimed at assessing their transition to adulthood. A subset of 143 young adults was eligible for this follow-up study, which consists of a personal interview and questionnaires. We were able to assess a sample of 67 young adults in this way, and collect some relevant information on another 56 cases. The results show that only a small part of the sample present serious problems of social exclusion (9%), whilst 70% have found employment or are in higher education and with a relatively stable life, even though, in general, their jobs are of a low-skilled nature. We also discuss other important aspects, such as the frequent loss (through death) of grandparent foster carers and the lack of help and support from the authorities for their kinship carers perceived by the young people themselves. Abstract: This article presents data from the first nationwide study in this area, with the aim of analyzing its level of establishment and in order to make comparisons with the situation in other countries. We analyzed a sample of 649 cases, covering both kinship and non-kinship foster care. With the help of child welfare staff we analyzed the cases and considered variables in relation to the profiles of the children, their birth families and the foster caregivers, as well as some basic data on processes and outcomes. In general, the results indicate substantial differences with respect to other countries, especially in view of the high levels of stability and permanence of children in foster care. However, non-kinship care is scarcely established (15%, compared to 85% for kinship care), and the article analyzes some of the possible reasons for this. Kinship care differs from non-kinship care in many aspects, including some as important as greater rates of family reunification and a lower rate of placement disruptions. Due to the fact that non-kinship care frequently becomes a long-term placement, and even in many cases ending with adoption by caregivers, they tend to take on no more than a single experience of foster care. This paper presents a study carried out in Spain that followed up 260 young people, the majority of them adults, who had been in residential care for significant periods. The follow-up takes place between 1 and 9 years after participants had left residential care, and assesses their level of social and work integration, as well as considering the incidence of problems related to marginalization and social exclusion. Furthermore, it analyses the reasons for the child being taken into care, the socio-family context, length of stay, changes in placement and other features or incidents in relation to the residential care. The data show that close to 15% have serious problems (drug dependence, delinquency, etc.), 25% receive help from the social services and the rest manage independently to a greater or lesser extent. The aim of this study is to identify the variables related to stress and burnout syndrome among child residential care workers, as well as the most problematic areas of their work. A random sample of 257 professionals working in the Residential Child Care sector in Spain was studied by means of the TBQ questionnaire (Teachers' Burnout Questionnaire), adapted to child care workers. The results enabled us to detect the principal sources of job dissatisfaction, as well as revealing a greater incidence of stress than of burnout. Adverse family conditions, abuse and neglect during childhood present important risk factors for the appearance of emotional and behavioral problems. The main aim of this paper is to describe the presence of these kinds of disorders in children in residential child care and to explore individual, socio-family and care process factors associated with the use of mental health services. The sample consisted of 1216 children 6–18 years old in residential care in several Spanish regions. Information about emotional and behavioral problems was gathered according to two criteria: receiving some kind of treatment services and/or being identified as within the clinical range in the Child Behavior Checklist (CBCL). Results showed that 49% of cases were receiving some kind of mental health treatment and 61% were identified as within the clinical range in some of the broad band scales of the CBCL. In terms of agreement between referral to treatment and CBCL scores, results showed that four out of ten cases identified as within the clinical range were not receiving any kind of treatment. Several factors related to the type of problems detected in the CBCL, personal variables, and child care arrangements are associated with greater use of mental health services. Objective: To evaluate the safety of COVID-19 vaccines in patients with multiple sclerosis (MS) by assessing their impact on serum neurofilament light chain (sNfL) levels as a marker of neuroaxonal damage., Methods: Single-center observational longitudinal study including patients with MS who consecutively received their initial vaccination against SARS-CoV-2 at Hospital Universitario Ramón y Cajal, following the first national immunization program in Spain. Serum samples were collected at baseline and after receiving the second dose of the vaccine. sNfL levels were quantified using the single molecule array (SIMOA) technique. Adverse events, including clinical or radiological reactivation of the disease, were recorded., Results: Fifty-two patients were included (median age, 39.7 years [range, 22.5-63.3]; 71.2% female). After SARS-CoV-2 vaccination, no increased inflammatory activity, either determined by the presence of relapses and/or new MRI lesions and/or high sNfL levels, was detected. Accordingly, there was no difference between median sNfL levels before and after vaccination (5.39 vs. 5.76 pg/ml, p=0.6). Despite this, when looking at baseline patient characteristics before vaccination, younger age associated with disease activity after vaccination (OR 0.87, 95% CI: 0.77-0.98, p=0.022). Background: Residential child care is a very complex measure and has been the subject of heated debate in many countries. However, there is a paucity of studies that examine quality assessments of these services, much less that have asked the children and young people receiving those services about their own evaluations., Method: This study interviewed 209 young people between 11 and 20 years of age, in 21 residential facilities, about their degree of satisfaction with the care they receive. These residential facilities are differentiated into three types of programs that will be compared: preparation for adult life (PAL), therapeutic care for behavioral problems (TRC) and general programs for children and young people without any specific profile (GRC). The instrument used is an interview that includes both quantitative and qualitative assessments., Results: The results show a trend toward positive average scores on practically all aspects, highlighting the support from as well as the connection they feel with the educators who care for them. On the other hand, young people in TRC displayed lower scores on almost all aspects evaluated while those in PAL were the most positive., Conclusions: The main implications for the practice will be discussed from these youths’ perspective.

Vaccines and genetic resistance offer potential future alternatives to the exclusive use of anthelmintics to control gastrointestinal nematodes (GIN). Here, a Teladorsagia circumcincta prototype vaccine was administered to two sheep breeds which differ in their relative levels of resistance to infection with GIN. Vaccination of the more susceptible Canaria Sheep (CS) breed induced significant reductions in worm length and numbers of worm eggs in utero (EIU) when compared to control CS sheep. In the more resistant Canaria Hair Breed (CHB), although vaccination induced a reduction in all parasitological parameters analysed, differences between vaccinated and control sheep were not statistically significant. Such interactions between sheep breed and vaccination may allow better integrated control of GIN in future.

A zoonotic, opportunistic out-break of tropical rat mite Ornithonyssus bacoti [Acari: Macronyssidae; Ornithonyssus bacoti (Hirst)] in an animal facility, is described. Immunocompetent mice [Mus musculus (Linnaeus)] and rat [Rattus norvegicus (Berkenhout)] strains in a conventional health status facility suffered from scratching and allopecia and staff members suffered from pruritic, erythemato-papular lesions, presumed to be allergic in origin. O. bacoti was identified and treatment with a 0.1% ivermectin solution led to its complete erradication. Safety assessment revealed no signs of acute toxicity in any animal strain. Following this inexpensive strategy, 7 wk after the initial dose, samples were negative for the presence of acari. At the time of this report, 26 months after diagnosis, O. bacoti remains undetected.

Episodes of expansion and isolation in geographic range over space and time, during which parasites have the opportunity to expand their host range, are linked to the development of host-parasite mosaic assemblages and parasite diversification. In this study, we investigated whether island colonization events lead to host range oscillations in a taxon of host-specific parasitic nematodes of the genus Spauligodon in the Canary Islands. We further investigated whether range oscillations also resulted in shifts in host breadth (i.e., specialization), as expected for parasites on islands. Parasite phylogeny and divergence time estimates were inferred from molecular data with Bayesian methods. Host divergence times were set as calibration priors after a priori evaluation with a global-fit method of which individual host-parasite associations likely represent cospeciation links. Parasite colonization history was reconstructed, followed by an estimation of oscillation events and specificity level. The results indicate the presence of four Spauligodon clades in the Canary Islands, which originated from at least three different colonization events. We found evidence of host range oscillations to truly novel hosts, which in one case led to higher diversification. Contemporary host-parasite associations show strong host specificity, suggesting that changes in host breadth were limited to the shift period. Lineages with more frequent and wider taxonomic host range oscillations prior to the initial colonization event showed wider range oscillations during colonization and diversification within the archipelago. Child welfare professionals regularly make crucial decisions that have a significant impact on children and their families. The present study presents the Judgments and Decision Processes in Context model (JUDPIC) and uses it to examine the relationships between three independent domains: case characteristic (mother's wish with regard to removal), practitioner characteristic (child welfare attitudes), and protective system context (four countries: Israel, the Netherlands, Northern Ireland and Spain); and three dependent factors: substantiation of maltreatment, risk assessment, and intervention recommendation. The sample consisted of 828 practitioners from four countries. Participants were presented with a vignette of a case of alleged child maltreatment and were asked to determine whether maltreatment was substantiated, assess risk and recommend an intervention using structured instruments. Participants' child welfare attitudes were assessed. The case characteristic of mother's wish with regard to removal had no impact on judgments and decisions. In contrast, practitioners' child welfare attitudes were associated with substantiation, risk assessments and recommendations. There were significant country differences on most measures. The findings support most of the predictions derived from the JUDPIC model. The significant differences between practitioners from different countries underscore the importance of context in child protection decision making. A survey of the gastrointestinal helminth communities of a population of Chalcides sexlineatus Steindachner, a small skink endemic to Gran Canaria island (Canary Archipelago, Spain), was conducted to determine the prevalence, abundance and species diversity of intestinal parasites in these reptiles. Only three parasite species were found, one cestode, Oochoristica agamae Baylis, 1919 and two nematodes, Parapharyngodon micipsae (Seurat, 1917) and Pharyngodonidae gen. sp. Helminth infracommunities of C. sexlineatus showed low values of abundance and species richness and diversity, being more similar to the helminth community of Tarentola boettgeri boettgeri (Steindachner) rather than those of Gallotia stehlini (Schenkel), both syntopic with the sampled host.

Parasite taxonomy traditionally relies on morphometric and life-cycle characteristics which may not reflect complex phylogenetic relationships. However, genetic analyses can reveal cryptic species within morphologically described parasite taxa. We analysed the phylogenetic variation within the nematode Spauligodon atlanticus Astasio-Arbiza, Zapatero-Ramos, Ojeda-Rosas & Solera-Puertas, 1987, a parasite of the Canarian lizard genus Gallotia Boulenger, inferring the origin of their current association. We also attempted to determine its relationship with other Spauligodon spp. Three different markers, mitochondrial COI plus nuclear 18S and 28S ribosomal RNA, were used to estimate the evolutionary relationships between these nematodes. S. atlanticus was found to be paraphyletic, suggesting that Gallotia spp. were colonised by two independent lineages of Spauligodon. Additional analyses of other Spauligodon spp. are required for a more complete interpretation of the evolution of this genus from the Canarian archipelago and its closest taxa. Our results emphasise the importance of extensive sampling and phylogenetic studies at the intrageneric level, and highlight the limitations of a morphologically based taxonomy in these parasites.

Breakdown of foster care has been defined as the situation in which one of the involved parties terminates the intervention before having achieved the goals established for the case plan. This work presents a study carried out with a Spanish sample of 318 closed cases of children who were placed in foster homes and kinship care. The data were collected through the exhaustive review of the child protection and foster placement files, complemented with interviews of the welfare workers in charge of each case. The rate of breakdown of the entire sample was 26.1%, although it was significantly different in kinship care (19.7%) and foster care (31.2%). The results of this study indicate that the variables related to breakdown depend on the placement modality, either in foster care or kinship care. In the first case, the variables related to the child's characteristics are noteworthy, especially behavior and academic problems, with special relevance in the 9-12-year-old group, and in children who were previously in residential care. In contrast, in kinship care, the parents' problems (prison, mental health) and having some measure of guardianship are the most important. The fact of undergoing foster placement after having lived in various residential homes is transcendental. Lastly, the availability of economic resources and even the foster carers' studies seem to be related to foster breakdown.

The present study aims to assess the impact of the Spanish young offenders law (LO/2000). Recidivism and its associated risk factors were used as indicators of impact. Data were collected from young offenders' reports opened after 2001 and closed before 2005. The final sample consisted of 382 young offenders (327 males and 55 females). Results indicated that 70% had not re-offended in an average period of 1.6 years. Most of the youngsters with fewer risk factors, usually start their criminal careers with less serious offences and the interventions seemed to be fairly effective. In the cases of young offenders with a higher number of risk factors, the interventions (custodial and non-custodial) were less effective, as recidivism rates were higher. Interventions must focus on family and community contexts in order to achieve adequate social integration of young delinquents.

Giardia duodenalis (syn. Giardia lamblia, Giardia intestinalis) is a wide-spread intestinal protozoa of both humans and animals. Although giardiosis in goat is commonly asymptomatic, young kids may bear an enteric disease associated with persistent diarrhoea and delayed weight gain. In the present study we have analysed the occurrence of Giardia in 315 young goat kids (2-6 months old) from Gran Canaria Island (Spain) through visualization of faecal cysts. The identification of genotypes of G. duodenalis among the farms was attained by nested PCR of the triophosphate isomerase (TPI) and single PCR of beta-giardin genes and subsequent sequencing. Positive samples were found in 42.2% of the animals and 95.5% of the farms. Goat faecal specimens were positive for only livestock-associated G. duodenalis assemblage E genotype for both TPI and beta-giardin genes. The genetic analysis of these two loci revealed the presence of different haplotypes among the farms included in the survey and high homology with homologous genes from cattle and sheep. Altogether, the data presented here provide additional information to the prevalence and genetic characterization of Giardia isolates. The absence of assemblages A and B in this study suggests that zoonotic transmission of Giardia from goats could be of low epidemiological significance, although these findings should be validated in studies including other geographical areas, age groups and larger number of samples.

This study compares the susceptibility to Haemonchus contortus infection in two breeds of sheep endemic to the Canary Islands, the Canaria Hair Breed sheep and the Canaria sheep. Sheep were experimentally infected with 20,000 larvae of H. contortus and animals killed on days 7 and 28 post-infection. No difference between sheep breeds were detected in immature worm counts at days 7 or 28 post-infection. However, in comparison to the Canaria sheep breed, the Canaria Hair Breed sheep showed lower mean faecal egg counts, lower adult worm counts, lower number of eggs in utero and female worm stunting. Overall, these data suggest that the Canaria Hair Breed sheep has a greater resistance to H. contortus infection than Canaria sheep, and that this resistance may act at the level of the adult parasite.
