Liquefied Petroleum Gas (LPG) is widely recognized as a safe option for household cooking. However, in settings like Nigeria, within sub-Saharan Africa, hazardous events related to LPG are not uncommon, often resulting from inadequate handling and utilization practices. Despite LPG's growing popularity for its sustainability and efficiency, safety concerns persist. This study introduces a smart system tailored to boost gas cylinder safety and efficiency. This system incorporates a compact and robust smart platform that integrates various smart components, provides real-time LPG level information, a load cell is available to continuously monitor LPG levels within the cylinder and issues alert upon reaching predefined thresholds. Furthermore, the system can detect gas leaks, promptly alerting users via a buzzer and LEDs. It is recommended for deployment in households, offices, and industrial environments, ensuring safety, convenience, and effectiveness in LPG usage. Introduction: Human Papillomavirus (HPV) infection is a risk factor for cervical cancer, the fourth most common cancer among women globally. Its burden is the highest in sub-Saharan Africa, with over 90% mortality. Interventions may fail without evidence-based data on stratified prevalence and risk factors among most at-risk women across Nigeria. Methods: A cross-sectional comparative study, with participants recruited from the Nigerian Institute of Medical Research's Clinics, NGO outreaches, a cancer screening centre and a university teaching hospital. Questionnaires were selfadministered. Trained medics performed sampling at healthcare facilities, and self-sampling was used at outreaches. Results: Nine hundred eighty-five study participants were recruited. About 37% and 27% of the women knew about HPV and its vaccines, respectively, but only 6% confirmed vaccination with HPV vaccines. HPV prevalence was highest among women with unknown marital status (35.9%), single women (33.8%), widowed/divorced/separated women (30.3%), and married/cohabiting women (19.6%). HPV infection was significantly higher among women who take alcohol (odds=1.7 [95% CI: 1.2-2.4]) and women who smoke (odds=2.6 [95% CI: 1.4 - 4.6]. HPV strains detected included HPV16 (1.3%), HPV18 (1.5%), Low Risk (0.2%) and Other High-Risk groups (19.7%). Conclusion: The inverse relationship between prevalence and education suggests interventions improving awareness and prevention would be impactful. Such interventions could also target HIV-positive women, women presenting with sexually-transmitted infections, who smoke and frequently drink alcohol. [ABSTRACT FROM AUTHOR]

Background: Retinal vascular occlusions are a common cause of visual impairment. Studies on retinal vascular occlusions in sub-Saharan Africa (SSA) have primarily been retrospective and on retinal vein occlusion (RVO) only. The aim of this study, therefore, was to determine the prevalence and pattern of retinal vascular occlusions and their systemic associations in SSA. Materials and Methods: This was a hospital-based, cross-sectional study involving all new patients presenting at the general ophthalmic and specialty retina clinics in four hospitals in Nigeria over a 1 year period. All the patients underwent a comprehensive eye examination. The demographic and clinical data of patients with retinal vascular occlusions were entered into an excel sheet and analyzed using the Statistical Package for the Social Sciences (SPSS) software version 22.0. Statistical significance was indicated by P < 0.05. Results: A total of 8614 new patients were seen, and a diagnosis of retinal vascular occlusion was made in 90 eyes of 81 patients giving a disease prevalence of 0.9%. Eighty-one eyes of 72 (88.9%) patients had RVO, while 9 eyes of 9 (11.1%) patients had retinal artery occlusion (RAO). The mean age of patients with RVO and RAO was 59.5 years and 52.4 years, respectively. Increasing age, hypertension, and diabetes were the significant associations with retinal vascular occlusion with P < 0.0001. Conclusion: Retinal vascular occlusions are an increasing cause of retinal disease in SSA and tend to occur at an earlier age. They are associated with hypertension, diabetes, and increasing age. Further studies will, however, be required to establish the demographic and clinical profile of patients with RAO in the region. [ABSTRACT FROM AUTHOR]

Vitamin A deficiencies is a becoming persistent among young children and a growing concern to parents in sub-Saharan Africa, especially in crisis-affected areas. Fermented cereal paste from maize, millets, and sorghum grains are significant food for young children. Thus, the study focuses on food fortification using orange-fleshed sweet potato (OFSP) as fortifier as studies have confirmed the presence of nutrients that can help meet the Vitamin A dietary requirement. The cereals were soaked ambient temperature (27 ± 1°C) for 72 hours and were blended with OFSP (90:10, 80:20, 70:30, 60:40, 50:50), and the formulated products were studied for Vitamin A, β-carotene, proximate composition, physicochemical, functional properties, and storage. Application of OFSP as forticant increased the Vitamin A (4.98-6.65 mg/100 g), β-carotene (0.10-0.17 mg/100 g) and the calorific value (222.03-301.75 kcal) of the gluten-free multi-grain cereal paste. The addition of OFSP also increased the ash content (1.41%-3.35%), crude fiber (2.56%-4.225%), carbohydrate (39.83%-48.35%), total solid content (55.20%-60.87%), and water absorption capacity (112.20%-137.49%) of the formulated cereal samples. The fortified fermented paste was objectively stable throughout on the shelf from the storage studies. The study deduced that addition of orange-fleshed sweet potato to fermented mixed cereal paste as a fortifier can help increase the nutritional quality of the complementary food. Linkage to care (LTC) and initiation of antiretroviral therapy (ART) are key components in the longitudinal care cascade for people living with HIV. Many strategies to optimize these stages of HIV care have been implemented, though there is a paucity of analyses comparing the outcomes of these efforts in low- and middle-income countries. We conducted a systematic review of studies assessing interventions along all stages of the HIV care continuum published between 2008 and 2020. A comprehensive search strategy reviewed five electronic databases to capture studies assessing HIV testing, LTC, ART initiation, ART adherence, and viral suppression. Of the 388 articles that met the inclusion criteria, 78 described interventions for improving LTC/ART initiation. Efforts focused on empowering patients through integrative approaches generally yielded more substantive results compared to provider-initiated non-adaptive LTC interventions or cash incentives. Specifically, tailoring care and incorporating ART initiation into existing infrastructures, such as maternal clinics, had a high impact across settings. Moreover, strategies such as home-based HIV counseling and testing (HBHCT) appear to be most effective when implemented in tandem with other approaches including motivational counseling and point-of-care CD4 testing. Purpose: To determine the burden of retinal diseases and the degree of visual impairment associated with each disease, amongst Nigerians. Patients and Methods: This was a hospital-based multicenter, prospective, cross-sectional, non-comparative study conducted from January to December 2018. Data was obtained from consecutive patients with a retinal diagnosis presenting at the general ophthalmic and specialty retina clinics in four hospitals (three public, and one private teaching eye department) in Nigeria. Biodata, visual acuity and refraction, intraocular pressure, findings on dilated retinal examination, diagnosis and systemic diseases were noted. Degree of monocular and bilateral visual loss associated with each diagnosed retinal disease was summarized and p value was calculated using chi-square test. P < 0.05 was considered significant. Results: Eight hundred seventy-six of 8614 patients had a retinal diagnosis; establishing a hospital-based retinal disease prevalence of 9.8%. Male:female ratio was 1.1:1. The mean age of study patients was 49.97 (standard deviation 17.64 years). Mean symptom duration was 21.63 months (standard deviation 41.94). The mean intraocular pressure was 13.87 mmHg. Forty-three different retinal diseases were diagnosed. The most common was retinal complications of diabetes, i.e., diabetic retinopathy (DR) alone, diabetic macular edema (DME) alone and a combination of DR and DME, which accounted for 13.7%, 5.6% and 9.3%, respectively (contributed 28.6% of the entire diagnosis). This was followed by retinal detachment (RD), in 219 eyes (15.4%), dry age-related macular degeneration (AMD) in 124 eyes (8.7%). Nearly half of the eyes were blind or severely visually impaired. Blindness occurred in 34.1% of eyes; severe visual impairment in 8.2% of eyes and 29.7% had normal vision. There were 469 patients who had systemic diseases. The common systemic diseases were hypertension in 169 patients (19.3% of the total number of patients), hypertension and diabetes in 156 patients (18%), and diabetes alone in 98 patients (11.1%). Sickle cell disease was present in 1.5%. Conclusion: There is need to invest in infrastructure, local training and development of systems for early detection and treatment of several retinal diseases in sub-Saharan Africa; DR and DME having the largest burden. Debt stock for most of African countries rose increasingly since early 1970s. The phenomenal increasing may have been triggered by the oil shocks in 1973 and 1979. Additionally, since there are two sides to a lending deal, uncontrolled lending by international financial institutions was also a major factor, which propelled overborrowing by developing countries. At the meantime, debt service repayments remained at low levels until late 70s. Sharp increase in international interest rates also posed a major challenge. Ill prepared for the shock and having obligations to payback, external debt stock increased very rapidly. Against this background, this article adopts a simple quantitative technique to analyse the trends in debt burden indicators and growth in selected African countries. It recommends that external debt should only be sourced for 'high priority projects' which are well appraised and having direct positive net present value. [ABSTRACT FROM AUTHOR]

OBJECTIVE • To determine the prevalence and describe possible trigger factors of urinary incontinence (UI) among adult women in a Nigerian community. PATIENTS AND METHODS • The present study comprised a household community survey conducted among 5001 women aged ⩾ 18 years. Cluster multistage sampling was used to select eligible respondents. Information was obtained by an interviewer who administered a structured questionnaire on sociodemographics, obstetric and gynaecological characteristics, leakage of urine and trigger factors. RESULTS • The mean ± SD age of the women was 33.2 ± 14.7 years. The proportion of women currently experiencing leakage was 2.8% (95% CI, 2.6-3.0). The types of incontinence reported by women currently leaking included stress incontinence (2.3%), urge incontinence (1%) and mixed (0.6%). • Severe incontinence was reported in 0.5% of women, 0.1% had moderate incontinence, whereas 2.2% had mild incontinence. • Women with history of vaginal delivery only were approximately two-fold more likely (95% CI OR, 1.11-3.02), and those who delivered by other modes over four-fold more likely (95% CI OR, 1.96-9.27), than nulliparous women to report currently leaking urine. CONCLUSIONS • The present study shows a prevalence of UI comparable to other settings, and the commonest type is stress is found to be UI. • The mode of delivery is a significant correlate of UI amongst Nigerian women. Hybrid Renewable Energy Systems (HRESs) have been touted as an appropriate way for supplying electricity to remote and off-grid areas in developing countries, especially in sub-Saharan Africa (SSA), where rural electrification challenges are the most pronounced. This study proposes a two-step methodology for optimizing and analyzing a stand-alone photovoltaic/wind/battery/diesel hybrid system to meet the electricity needs of Fanisua, an off-grid and remote village of northern Nigeria. In the first step, the MATLAB environment was used to run simulations and optimize the system via the genetic algorithm. Then, techno-economic and emissions analysis was carried out in the second step to compare the proposed system to the existing traditional modes of rural electrification in sub-Saharan Africa, namely, the grid-extension and diesel generator. The break-even distance parameter was adopted in the comparison with grid-extension. Besides, the hypothetical project of replacing the diesel generator by the optimal system was analyzed using the Simple Payback Period (SPP) and Net Present Value (NPV) parameters. The resulting optimal design architecture included an 89.271-kW photovoltaic array, a 100.31-W diesel generator, and 148 batteries with a total annualized cost (TAC) and cost of energy (COE) of USD 43,807 and USD 0.25/kWh, respectively. The break-even distance found was 16.2 km, while the NPV and SPP of the hypothetical project were USD 572,382 and 2.8 years, respectively. The savings in carbon dioxide (CO2) emissions of the proposed system compared to the grid extension and the diesel generator were found to be 85,401.08 kg/year and 122,062.85 kg/year, respectively. Malaria remains the major cause of disease morbidity and mortality in sub-Saharan Africa with complex immune responses associated with disease outcomes. Symptoms associated with severe malaria have generally shown chemokine upregulation but little is known of responses to uncomplicated malaria. Eight villages in central Cameroon of 1045 volunteers were screened. Among these, malaria-positive individuals with some healthy controls were selected for chemokine analysis using Enzyme-Linked Immunosorbent Assay (ELISA) kits. Depressed serum levels of CXCL5 and raised CCL28 were observed in malarial positives when compared with healthy controls. The mean concentration of CXCL11 was higher in symptomatic than asymptomatic group, while CCL28 was lower in symptomatic individuals. Lower chemokine levels were associated with symptoms of uncomplicated malaria except for CXCL11 which was upregulated among fever-positive group. The mean CXCL5 level was higher in malaria sole infection than co-infections with HIV and Loa loa . Also, there was a raised mean level of malaria + HIV co-infection for CXCL9. This study hypothesises a situation where depressed chemokines in the face of clinical presentations could indicate an attempt by the immune system in preventing a progression process from uncomplicated to complicated outcomes with CXCL11 being identified as possible biomarker for malarial fever. Abstract: Objective: To describe the perceived causes of urinary incontinence (UI) and factors associated with awareness of causes of UI among women in the community. Study design: Secondary analysis of data extracted from the Ibadan Urinary Incontinence Household Survey (IUIHS), a multi-stage community survey conducted among 5001 women in Nigeria. Results: The mean age was 34.8 years (SD=14.2). The majority had at least secondary education and were currently married. Within this population, 13% had ever leaked urine. On their perception of possible aetiological factors of UI, 20.5% mentioned pelvic floor or bladder-related causes such as stress incontinence triggers and bladder problems; 14.6% mentioned uncontrollable factors such as medical comorbidity, age and prior surgery; 8.8% attributed the cause to being female; and 6.8% mentioned sex-related factors. Multiple logistic regression analysis revealed significantly lower odds of awareness of the aetiology of urinary incontinence among women aged less than 30 years, those with lower level of education, rural women, those with five or more children and women without history of urine leakage. Conclusions: The study shows a low level of awareness of possible cause of UI amongst women surveyed. We recommend health education and mobilization of women on the right aetiological factors of UI.
