This paper provides a discourse analysis of 12 Swedish sexuality education texts intended for preschool practitioners and published between 1969 and 2021. Using Fairclough's framework, we identify three discourses about children's sexual play in relation to children's sexual agency in the texts: child sexuality as encouraged and entangled with adult sexuality; child sexuality as conditioned by what is perceived as normal or abnormal in children; and child sexuality as repressed. These three discourses mainly appear chronologically, but also overlap and connect with one another. When analysis begins from children's position and a theoretical understanding of children's and adults' agency as interdependent, it becomes possible to see how the child is construed as agentic, and how the adult role changes from encouraging sexual play to regulating and monitoring behaviour so that it does not occur. Over time, discourse on young children's sexual play has changed our understanding of both adults and children. Adults are increasingly construed as less knowledgeable in relation to young children's sexuality, and young children have become understood as more dangerous and in need of having their sexuality constrained and civilised. Purpose: For clients to understand social insurance decisions and processes, information from authorities needs to be comprehensible, and clients need sufficient individual abilities. These dimensions are captured by the concept social insurance literacy, which has been operationalized into a measure, the Social Insurance Literacy Questionnaire (SILQ). The aim of this study was to describe the development of the SILQ and evaluate its psychometric properties using Rasch measurement theory. Methods: The development of the SILQ included a Delphi study and cognitive interviews. A preliminary version, divided on four scales corresponding to the domains of the concept (obtaining information, understanding information, acting on information, and system comprehensibility) was psychometrically evaluated according to Rasch measurement theory, in a survey to a stratified random sample of people on sick leave (n = 1151) sent out in the fall of 2020. Results: Overall, the items in the final version of the SILQ demonstrated good fit to the Rasch model, and the response scale worked as intended. Unidimensionality was supported for all scales, but minor problems with local dependency was detected for three items. The person separation was 0.80 for the Obtain scale, 0.82 for the Understand scale, 0.68 for the Act scale, and 0.81 for the System scale. Corresponding ordinal alpha values were 0.91, 0.91, 0.86, and 0.91, respectively. Conclusion: This study is a first step toward exploring literacy in the social insurance field. Aim: To describe undergraduate nursing students' attitudes to learning during clinical practice in different semesters when using the conceptual learning model, Model for Improvements in Learning Outcomes (MILO) grounded in a caritative caring perspective. Background: With the intention to support interlinking between theory and praxis and offer understanding and structure to facilitate learning, MILO, theoretically grounded in hermeneutics and a caritative caring perspective based on ethical values, was implemented. MILO consists of four contextual concepts (peer learning, co‐clinical teachers, student‐centred and student‐active supervision) and four intrapersonal concepts (nursing, a reflective approach, a critical approach, quality and safety). Methods: A descriptive comparative quantitative study design was applied at a Swedish university, 3 hospitals and 13 municipalities in one county. Cross‐sectional data collected via a questionnaire developed to assess attitudes to learning related to MILO's contextual and intrapersonal concepts and their applications were used. Results: 209 students in semester 3, 4 and 6 participated in 6 different clinical practice courses. In comparison, intrapersonal concepts, that is, the student's own characteristics and abilities were viewed to be of greater value for learning than contextual, that is, organisational‐related concepts in all semesters. Understanding the needs of others and reflective learning were rated to be of major importance. Students in semester 3 valued the use of the applications the highest. To be supervised in pairs was rated the lowest in semester 6. Some of the concepts and their applications were to great extent not applied. Conclusions: In all semesters, fundamentals in caritative caring and characteristics and abilities related to the individual student were rated to be of greater importance for learning than environmental support. Providing students opportunities to develop independency seems essential. Use of a learning model such as MILO is dependent on a bearing of a caritative caring culture and a shared understanding between all involved in student learning during clinical practice. This article analyzes lay people's use of nutritionistic discourse in written correspondence with the Swedish Food Agency, an authority responsible for dietary advice. Examining 60 food related written digital messages, we apply a critical discourse analysis to parse the lexical items and grammar people use when constructing "food" in scientific terms. The findings show how message writers place nutrients at the discursive center. Message writers' grammatical constructions instrumentalize food and eating. This is reinforced by the message writers' frequent use of terms that indicate preciseness, such as numbers and amounts. Messages therefore emphasize the what, but not the how, of eating, implying a focus on food as subject to regulation and control. As such, eating is discursively reduced to an act of ingesting nutrients that can be decontextualized and managed in isolation--as entities to increase or avoid separately. These discursive features preclude the conceptualization of food choice and eating as subjective experiences of feelings, taste, and tradition. [ABSTRACT FROM AUTHOR]

Over the last two decades, the inclusion of patient voices and public values in the field of health care through deliberation has become increasingly emphasized, by patients as well as policy-makers. This is achieved not only through individual patient participation but also through patient interest organizations. Geographical representation within national interest organizations is especially important in a decentralized, multilevel policy field such as Swedish health care, allowing representation from all regions to be present in national advocacy. Using Pitkin's conceptualization of political representation, this study aims to characterize the shaping of representation among Swedish federative patient organizations, in a time of professionalization and centralization of civil society. The results show that patient organization representation has functioning mechanisms for all studied aspects of representation; however, the nature of the substantive representation seems to contain a challenge from a democratic perspective. This leads us to a discussion about management, rooted in democratic ideals but simultaneously strongly characterized by more managerial ideals, and the contradiction of democracy and actionable management. Background: While increasingly discussed in somatic care, the concept of patient participation remains unsettled in psychiatric care, potentially impeding person‐centred experiences. Objective: To describe outpatient psychiatric care patients' conceptualization of patient participation. Design: An exploratory survey. Setting and participants: Patients in four psychiatric outpatient care units. Variables: Patients conceptualized patient participation by completing a semi‐structured questionnaire, including optional attributes and free text. Data were analysed using statistics for ordinal data and content analysis for free text. Results: In total, 137 patients (69% of potential respondents) completed the questionnaire. The discrete items were favoured for conceptualizing patient participation, indicating a primary connotation that participation means being listened to, being in a reciprocal dialogue, learning about one's health care and managing one's symptoms. Additional free‐text responses acknowledged the attributes previously recognized, and provided supplementary notions, including that patient participation is about mutual respect and shared trust. Discussion: What patient participation is and how it can be facilitated needs to be agreed in order to enable preference‐based patient participation. Patients in outpatient psychiatric care conceptualize participation in terms of both sharing of and sharing in, including taking part in joint and solo activities, such as a reciprocal dialogue and managing symptoms by yourself. Conclusion: While being a patient in psychiatric care has been associated with a lack of voice, an increased understanding of patient participation enables person‐centred care, with the benefits of collaboration, co‐production and enhanced quality of care. The study examines how students in Grade 9 handle conceptual knowledge through the concept of continuity and change in the context of large‐scale assessment. The research questions address (a) what strategies the students use when they use the two parts of the concept concomitantly and (b) any gender‐related differences regarding these strategies. A total of 100 student responses on a specific item in the national test in history are examined. The method of concept analysis is applied to identify the strategies of the students from the two groups. The analytical framework is based on previous research regarding the concept. The results show that just over half of the students use the concept as expected. Boys are overrepresented among the students that struggle with the concept. Any correlation between the identified strategies is discussed in addition to the implications the results may have on equity and education. [ABSTRACT FROM AUTHOR]

The aim of the article is to understand ageing as a process of lifelong learning through a whole variety of experiences, to consider what ageing in a biographical perspective means, and to investigate the process of biographical ageing within identity formation. The method of the project was based on the employment of in-depth narrative interviews with older Polish migrants now living in Sweden. The author argues that ageing is a natural process of the life cycle and is socially and culturally constructed and that consequently everyone experiences this process individually. One of the paper's conclusions is that we do not age according to the numbers of years we live, but due to our life experience and the social-historical contexts in which we have lived. Furthermore, we age due to the biography we re/ construct while we are ageing during our life course. Ageing, from a biographical perspective, is a learning process in which people re/ construct their lives with social, educational and cultural contexts and from which they draw from their experiences relating to social practices and historical events. In this essay, David Karlander examines what happens when concepts developed by scholars of language circulate and become embedded in policies and law. In exploring how the distinction between a "language" and a "dialect" became encoded in the European Charter for Regional or Minority Languages (ECRML), Karlander examines the consequences when applied to the status and state support of minority languages in Sweden. What counts as a language, he demonstrates, is not simply an "academic" matter. The term woodland key habitat (WKH) was launched in Sweden in 1990. Definitions for the concept have changed over the years, and today the WKH concept and its application are issues of debate in Sweden. Consequently, the Swedish Forestry Agency (SFA) initiated a collaborative process including forest stakeholders with the purpose to clarify the application and develop the inventory methodology of WKH. We have studied, by means of interviews and observations, participant perceptions of how endogenous and exogenous factors affect the collaborative process. During our research, we identified three game changers: the pause in WKH registration in northwestern Sweden that caused several participants to drop out of the process; budget allocations for new nationwide WKH inventories that put the process on hold; and formal instructions from the government that came nine months later and essentially re-initiated the collaborative process. Altogether, this not only affected the participants' abilities, understanding and willingness to participate, but also the overall legitimacy of the process – indicating the difficulty of conducting policy development in collaborative form, especially when it is highly politicized since it impact on the participants' anticipation of the process and its end results. The aim of this study was to bring to light the meanings attributed to four concepts denoting problematic drinking—"alcoholism," "alcohol dependence," "alcohol misuse," and "risky drinking"—and to investigate whether individuals' preferences for using these concepts vary depending on their demographic characteristics, their stakeholder group membership, and their own drinking habits. These questions were examined by means of a vignette experiment, in which Swedish social workers, health care staff, journalists, researchers, and the general public (n = 863) read a series of 10 randomly constructed fictitious cases (vignettes; n = 8,630) and made judgments about which of the four concepts best portrayed the case descriptions. Results based on multilevel multinomial logistic regression showed that alcoholism was the concept chosen for the "heaviest" and most problematic drinking, whereas risky drinking was most often employed to describe moderate drinking habits that resulted in few negative consequences. The concepts alcohol misuse and alcohol dependence were positioned between alcoholism and risky drinking, and while misuse was linked to drinking large quantities and to more negative consequences, dependence was more frequently used to describe vignettes that involved depictions of craving. Moreover, the respondents' conceptual choices were affected by their own gender, age, drinking habits, and stakeholder group membership. Taken together, the study's findings reveal both consensus and variation in the respondents' choice of concepts, indicating that while these concepts may to some extent be employed for making meaningful distinctions between different types of problematic drinking, there is substantial variation in the way the concepts are employed. Aims The aims of this study were two-fold: to develop the concept analysis by Allvin et al. from lung recipients' perspective of their post-transplant recovery process and, to identify the recovery trajectories including critical junctions in the post-transplant recovery process after lung transplantation., Background Lung transplantation is an established treatment for patients with end-stage lung disease. The recovery process after lung transplantation is very demanding. Today, patients are expected to play an active role in their own recovery but require ongoing support during the process. Design A deductive, retrospective interview study using directed content analysis and Allvin′s recovery concept analysis. Method Fifteen adult lung transplant recipients who were due their 12-month follow-up were consecutively included and interviewed during 2015. Patients who were medically unstable or had difficulties speaking Swedish were excluded from this multi-centre study. Findings Allvin's concept analysis is partly applicable to the context of lung transplantation. The recipients' experience of the post-transplant recovery process could be confirmed in the main dimensions of the concept analysis, while several sub-dimensions were contradictory and were excluded. Six new sub-dimensions emerged; symptom management, adjusting to physical restraints, achieving an optimum level of psychological well-being, emotional transition, social adaptation and reconstructing daily occupation. Conclusion The concept analysis by Allvin et al. was possible to expand to fit the lung transplantation context and a new contextual definition of post-transplant recovery after solid organ transplantation was developed. Previous research on how students’ acceptance of emotionally charged theories relates to their understanding is based on the measurement of acceptance and understanding as two separate variables. As an alternative, the present study takes a qualitative approach with the aim of exploring what 24 upper-secondary school students accept when they come to understand the concept of gender and how to justify gender-related statements. The results show how the students accept ideas about social structures, power, and emancipation, as well as ideas about essences, individual independence, and a natural order. As for their justifications, they accept ideals of equality and objectivity. By assuming that the ideas have positive connotations for the students, it becomes possible to understand them as engaged in negotiating norms that are brought to the fore in the interviews. Due to Swedish history, to date there has been a common understanding of the meaning of volunteering in Sweden. However, it seems as if the meaning of volunteering is changing in Sweden, at least in some atypical hybrid organizations. However, this change presupposes that there is a conception of volunteering that has been institutionalized by tradition. Hence, to understand this change, one has to capture the institutionalized meaning of volunteering. In the academic debate there is sometimes an implied opposition between conceptual meaning and existential meaning. It is argued that this divide is unfortunate, the existential aspect presupposes the conceptual aspect of meaning. The study of the meaning of volunteering is an attempt to uncover existential meaning in everyday life. AIM - To analyse the Swedish drug question by examining dominant concepts used to portray the problem in the years 1839-2011. Theoretically, we understand these concepts as ideological tools that shape the political initiatives and administrative efforts to deal with the problem. The study is based on two kinds of source material: articles in medical journals from the years 1839-1964 and public reports on vagrancy, the alcohol problem, mental health and the drug problem from the years 1882-2011. FINDINGS - During the nineteenth century and the first part of the twentieth century the drug problem remained an individual problem handled by doctors. When the Swedish drug problem was established as a political question from the 1960s on, it also came to disengage itself from the medical frame of understanding. Medically oriented descriptions of 'dependence' and 'addiction' have appeared adequate or attractive when, for example, the socially motivated coercive treatment solution has been discredited (as in the 1970s), when there has been a desire to connect with an internationally accepted terminology (as in the 1990s) or when a new organisational model with a stronger professional support has been on the agenda (as in the 2010s). But otherwise the social problem description has called for concepts that have more or less explicitly dissociated themselves from speculations in physiological or psychological predispositions for substance abuse. This article describes the results of a nationwide questionnaire study of 3229 Swedish upper secondary school teachers’ conceptual understanding of sustainable development in relation to their subject discipline and teaching experience. Previous research has shown that teachers have difficulties understanding the complex concept of sustainable development. According to the Swedish curriculum, all teachers in all subjects should integrate a holistic perspective of sustainable development including economic, ecological, and social dimensions. This study shows that teachers differ in their understanding of the concept mostly according to their subject traditions. Social science teachers emphasize social dimensions and science teachers’ ecological dimensions, respectively. Teachers are aware of the relevance of the three dimensions to various degrees, but do not generally have a holistic understanding. The greatest uncertainty in teachers’ understanding is related to the economic dimension. Science and social science teachers are critical of incorporating economic growth into the concept of sustainable development while language, vocational, and esthetical–practical teachers are not. No experience-bound differences of the teachers’ understanding could be found, but recently qualified teachers consider their understanding of sustainable development to be poorer in comparison with more experienced teachers’ self-evaluation. In this paper, we report a case study of a 16-year-old Swedish upper secondary student's developing understanding of key concept areas studied in his upper secondary school chemistry course. This study illustrates how the thinking of an individual learner, Jesper, evolves over a school year in response to formal instruction in a particular educational context. Jesper presented a range of ideas, some of which matched intended teaching whilst others were quite inconsistent with canonical chemistry. Of particular interest, research data suggest that his initial alternative conceptions influenced his thinking about subsequent teaching of chemistry subject matter, illustrating how students' alternative conceptions interact with formal instruction. Our findings support the claims of some researchers that alternative conceptions may be stable and tenacious in the context of instruction. Jesper's rich conceptualisation of matter at submicroscopic scales drew upon intuitions about the world that led to teaching being misinterpreted to develop further alternative conceptions. Yet his intuitive thinking also offered clear potential links with canonical scientific concepts that could have been harnessed to channel his developing thinking. These findings support the argument that identifying students' intuitive thinking and how it develops in different instructional contexts can support the development of more effective science pedagogy. Abstract: Background: The drug and alcohol fields are characterised by continuous contestations of key concepts and the competing uses of concepts by various actors, in different geographies and over time. This study investigates the political processes leading to legislation enabling coercive treatment of (non-psychotic) drug users in Finland and Sweden from the 1950s until the early 1980s. The drug treatment policies are analysed through conceptual changes and innovations. Methods: The article analyses conceptual discussions in public reports in Finland and Sweden, focusing on the work preceding the first legislations where both alcohol and drug treatment were included (in Finland 1961, in Sweden 1982). Theories from conceptual history are applied. Results: The Finnish and Swedish discussions carry arguments from two periods of the Nordic welfare state: in an early development stage and a fragile situation in Finland, and in a more mature and affluent time in Sweden. The paternalistic arguments vary over time and between countries. Still, in both countries and time periods, the view of the drug problem as a youth issue, as particularly enslaving and on society's obligation to protect drug using individuals from damaging their future give enough motivation for coercive treatment. The conceptual work included avoidance of certain terms but in other cases, a broadening of their meaning, to adopt them to the political goals. Conclusion: Close analyses of conceptual history can reveal new features of drug policy struggles and show how central concepts in drugs and alcohol field are continuously contested. Aim. This article describes the development of literature-based models for bachelor degree essays in Swedish undergraduate nursing education. Students' experiences in a course with literature-based models for bachelor degree essays are discussed. Background. The ever-growing body of nursing research and specialized and complex health care practices make great demands on nursing education in terms of preparing students to be both skilled practitioners and users of research. Teaching to help students understand evidence-based practice is a challenge for nursing education. Methods. Action research was used to generate knowledge of and practical solutions to problems in everyday locations. Results. Six models were developed: concept analysis, contributing to evidence-based nursing by means of quantitative research, contributing to evidence-based nursing by means of qualitative research, discourse analysis, analysis of narratives, and literature review. Conclusions. Action research was found to be a relevant procedure for changing ways of working with literature-based, bachelor degree essays. Background: Information technology (IT) plays a key role in public health care management because it could improve quality, efficiency, and patient care. Researchers and practitioners repeatedly contend that a health care organization's information systems strategy should be aligned with its objectives and strategies, a notion commonly known as IT alignment. Purpose: Actor-related IT alignment issues in health care institutions were explored in this study. More specifically, it explores the possibility of moving beyond the current IT alignment perspective and, in so doing, explores whether IT alignment-as currently conceptualized in the dominant body of research-is sufficient for attaining improved quality, efficiency, and patient care in health care organizations. Methods: The findings are based on a qualitative and longitudinal study of six health care organizations in the Stockholm metropolitan area. The empirical data were gathered over the 2005-2011 period from interviews, a focus group, observations, and archival material. Findings: The data suggest recurrent misalignments between IT strategy and organizational strategy and operations due to the failure to deconstruct the IT artifact and to the existence of various levels of IT maturity. Conclusions: A more complex picture of IT alignment in health care that goes beyond the current perspective is being offered by this study. It argues that the previously common way of handling IT as a single artifact and applying one IT strategy to the entire organizational system is obsolete. Managerial Implications: The article suggests that considerable benefits can be gained by assessing IT maturity and its impact on IT alignment. The article focuses on the importance of organizational consistency in health care organizations within and across a country. Key components of organizational coherence for a successful quality improvement (QI) in health care program include people, processes and organizational perspective. A study on QI in health care programs in Great Britain, U.S. and Switzerland is discussed.

Aim The aim of the present study was to describe preceptors' expectations and experiences of participating in group supervision (GS). Background The challenging role of preceptors and their need for support is well known. Therefore, a collaborative project was carried out, providing GS to preceptors to strengthen them in their role. Method Data were collected from 48 preceptors by means of study-specific forms and field notes, and analysed using qualitative content analysis. Results Both positive and negative expectations and experiences of group supervision were found, but the positive experiences exceeded the expectations. The group in itself had a significant meaning for the preceptors; their pedagogical and personal competence increased and they became aware of their role as 'bridge-builders'. Conclusions Using GS to strengthen preceptors in their role was found to be successful. The findings provided new arguments for this model as a reflection tool. Implications for nursing management Strengthened preceptors can have an impact on nursing students' learning, and thereby also on future nurses' competence, which is a great concern of nurse managers. By using nurse lecturers as group leaders, the collaboration between the clinical placements and the nursing faculties can be improved. [ABSTRACT FROM AUTHOR]

Sexual orientation has been conceptualized by Ross and Paul (1992) as fixed in terms of sex object (exclusively heterosexual or homosexual) versus fluid (bisexuals, for whom sex of partner may be unimportant). The authors of the present article investigated characteristics of 1,913 young men and women with fixed and fluid sexual orientation recruited in an Internet-based study of sexual behavior in Sweden. One half of the respondents were students at a major Swedish university. Data indicated that women were twice as likely as men to report a fluid orientation. There were major differences between men and women, with maximal fluidity in the 25 to 34 age range, fluid women being more likely to be urban and not religious, and fluid men reporting higher education levels. Higher sexual partner numbers were associated with fluidity in women but fixedness in men. For both, living away from the family was associated with fluidity. These data suggest that fluidity is more prevalent in women, and that it may be associated not with an unformed sexual preference but with an expanding one in the midtwenties. Conceptualizing sexual orientation as fixed or fluid may be a useful alternative approach to conceptualization of sexual orientation as homosexual, heterosexual, or bisexual. A study of how Swedish students understand the concept of complex numbers was performed. A questionnaire was issued reflecting the student view of own perception. Obtained answers show a variety of concept images describing how students adopt the concept of complex numbers. These concept images are classified into four categories in order to clarify the learning situation. Furthermore, this study also revealed a variety of misconceptions regarding this concept, and most of the misconceptions were also possible to refer to the classification system. In addition, results from an identification test show that students have difficulties discerning the basic property of complex numbers, i.e. that any number is a complex number. This paper analyses recent experiences at the School of Social Work, Lund University, the first in Sweden to use an innovative pedagogical approach for integrating social service users with social work students in the classroom (an EU-funded project). The concept of gift exchange was used to make sense of the approach. Gift exchange is based on the norm of reciprocity and the principles of gift economy—to give, to receive and to return. As one of the most significant integrative relations, this exchange of gifts and courtesies serves the purpose of keeping balance in the system. The focus in this project was on the relationship between two groups of students: service user students—that is, social service users in the role of students on the course—and social work students. The experience of undertaking this project raised many questions, such as what promotes and what hinders mutual learning between two parties in the classroom. Findings show the relevance of such concepts as the gift economy and the importance of reciprocity in teaching contexts. They played an important role in the process of creating a positive learning climate and understanding the integration of social relations between the two groups. [ABSTRACT FROM AUTHOR]

The article discusses a study focusing on individual placement needs, problems/issues, and implementation of ideas for the health care sector. It notes that in the study based on a Swedish council county improvement program, health care departments and primary health care centers in the Kalmar County Council were invited to apply for money to accomplish improvement projects. The study found that the 5 types of improvement projects include organizational process, evidence and quality, competence development, process technology and proactive patient work.

This article explores the meaning of the business concept in the context of a professional service (knowledge-intensive) company. It is based on a study of a Swedish computer consultancy company (CCC), covering a wide set of aspects (Alvesson, 1995), only a few of which are addressed here. The study is grounded in a cultural approach to management, that is, it focuses on the level of shared meanings, ideas, and symbolism, which are viewed as significant elements in how companies--as socioeconomic phenomena--function. The article then aims to make an empirical and theoretical contribution in three respects: (1) developing the understanding of the business concept, (2) illuminating certain aspects of the management of professional service companies, and (3) illustrating how a cultural approach to business and organizations also provides insights about themes conventionally understood in analytic, objective, and rationalistic ways. While the first aspect refers to certain core aspects of "macro management" and organizational functioning, the second involves a specific type of organizations, and the third is mainly a matter of a certain form of understanding--the use of culture as a metaphor for organizations.
