Background: Nurse engagement and retention are critical issues affecting the quality of healthcare. Although the work environment is known to affect nursing performance and patient outcomes, little is known regarding the impact of this environment on nurse burnout and future work intention, especially in non-Western settings. Purpose: This study explored the extent to which Taiwanese nurses' perceptions of their practice environment mediates burnout levels and intention-to-stay. Methods: This descriptive, correlational study, conducted between September and December 2020, recruited nurses employed at a regional teaching hospital located in a relatively rural region in Taiwan. The participants completed standardized questionnaires, including a demographic information form, the 31-item Practice Environment Scale of the Nursing Work Index (PES-NWI), the 22-item Maslach Burnout Inventory (MBI-HSS), and the 6-item Intention-to-stay (ITS) scale. The mediating effect among the variables related to intention-to-stay was examined using Model 4 of the PROCESS macro and tested using the bootstrapping method (repeated 5,000 times) with a p value < .05. Results: The mean age of the 376 participants was 34.3 years (SD = 8.1). Nearly one-third (30.3%) scored > 27 on the emotional exhaustion subscale of the MBI-HSS, indicating they were "highly burned-out". Also, nearly one-third (30.6%) were uncertain about their intention to stay with their current employer. Burnout was associated with intention-to-stay (β = -.088, p < .001), and holding a positive perception toward their practice environment was shown to mediate the relationship between burnout and intention-to-stay (β = -.015; p < .001). Conclusions/Implications for Practice: Targeted interventions and technical support that promote a collaborative climate and psychological empowerment in clinical practice may alleviate burnout and intention-to-leave in nurses and improve workplace quality. Background: Musculoskeletal pain in people with type 2 diabetes is a common issue even to this day. The study aimed to explore the 10-year cumulative incidence of musculoskeletal pain, the mean number of doctor visits for musculoskeletal pain, and the mean number of doctor visits for musculoskeletal pain by location in people with type 2 diabetes, compared with respective values for people without diabetes.Methods: The study utilized a population-based retrospective cohort study design. The subjects were randomly obtained from the Taiwan National Health Insurance Research Database. The diabetic group included 6586 people with type 2 diabetes aged 18-50 years, while the non-diabetic group consisted of 32,930 age- and sex-matched people. Based on the medical records of individuals with musculoskeletal pain in the two groups from 2001 to 2010, the 10-year cumulative incidence of musculoskeletal pain, the mean number of doctor visits for musculoskeletal pain, and the mean number of doctor visits for musculoskeletal pain by location were calculated and compared, with the aim of identifying differences between the two groups.Results: Showed that people in the diabetic group had a higher 10-year cumulative incidence of and a higher mean number of doctor visits for musculoskeletal pain than the non-diabetic group (p < 0.05). The relative risk (RR) of the 10-year cumulative incidence of musculoskeletal pain in the two groups was the highest (RR = 1.39) for people between 30 and 39 years of age. The mean number of doctor visits for musculoskeletal pain by location was significantly different between the two groups. However, the mean number of doctor visits for limb pain registered the largest difference between the two groups.Conclusion: People with type 2 diabetes aged 18-50 years had a higher 10-year cumulative incidence of and a higher mean number of doctor visits for musculoskeletal pain than the non-diabetic group. Musculoskeletal pain might directly or indirectly interfere with or decrease the physical activity levels of people with diabetes. Background: Despite the burden of schizophrenia and bipolar disorder in the Chinese population, country-specific data to guide practitioners regarding antipsychotic therapy are lacking. The primary aim of this systematic review was to examine evidence of the efficacy, effectiveness, and safety of olanzapine in Chinese populations. Methods: A systematic literature search was conducted using databases covering international and Chinese core journals using search terms related to schizophrenia and bipolar disorder, specified countries (People's Republic of China, Hong Kong, Taiwan), and olanzapine treatment. Following initial screening, inclusion and exclusion criteria were applied to the search results to identify relevant studies from which data were extracted. Results: A total of 489 publications were retrieved and 61 studies were identified for inclusion. Most studies were related to schizophrenia (n=54), with six studies related to bipolar disorder and one study related to both conditions. The quality of study methods and reporting in international journals was noticeably better than in Chinese language journals. Most studies included relatively small patient populations and were of short duration. The efficacy of olanzapine in Chinese populations was confirmed by multiple comparative and noncomparative studies that found statistically significant reductions in symptom measures in studies conducted for ≥6 weeks (schizophrenia) or ≥3 weeks (bipolar disorder). Findings related to effectiveness (treatment discontinuation, quality of life, and neurocognitive improvements) were generally consistent with those observed in non-Chinese populations. No new safety signals specific for Chinese populations were raised for olanzapine. Conclusion: Chinese and non-Chinese populations with schizophrenia or bipolar disorder treated with olanzapine display broadly similar responses. Differences between these populations, especially in relation to the relative efficacy of olanzapine versus other antipsychotics, may warrant further investigation via studies incorporating both populations. Taiwan has one of the highest suicide rates in the world, especially among its elderly. The epidemiologic characteristics and trends of the surging elderly suicide rates from 1993 to 2003 are described, with a special emphasis on the risk groups, the methods used in suicide, and their geographical variations. Data on annual mortality for persons over 65 years of age with external cause-of-death codes E950--E959 were obtained from the Death Certification data file provided by the Department of Health and used in the analysis. The suicide rate for each 5-year age group over 65 years old increased during the 11-year period. Elderly males had the highest suicide rate and experienced an increased rate of 49% during the decade. The average suicide ratio between elderly males and females was about 2:1. Never-married males had the highest age-adjusted as well as sex- and marital status-specific suicide rates, and showed an increased rate of 66% over the 11-year period. Geographical variations in suicide rates were significant, with the lest urbanized eastern Taiwan having a higher rate than other regions. Hanging, strangulation, and suffocation were the most used methods for committing suicide by the elderly, but their use had decreased from 63% to 54% during the decade; but jumping off a building and drowning increased significantly. Variation in suicide rate among months was not significant. With the increase in the elderly suicide rate, more governmental and societal interventions are needed to alleviate this social and human problem. [ABSTRACT FROM AUTHOR]

The dispersed dyestuff (Blue S-3RF) obtained from a textile dyeing finish factory located in central Taiwan was decomposed by a photocatalytic process. The parameter such as the concentration of dispersed Blue S-3RF was investigated to find out the effect on dispersed Blue S-3RF decomposition. Experimental results indicate that the ultraviolet light only cannot provide enough oxidation potential to destroy the structure of dispersed Blue S-3RF, while the reduction of color presented by VIS500 and American Dye Manufacture Index (ADMI) are less than 20%. As the photocatalytic process equipped with a TiO2-coated stainless steel net is applied, it is understood that the reduction of VIS500 and ADMI can be enhanced to 99.4 and 96.7% after a 6-h photoreaction. Also, based on the spectrum of Fourier transform infrared spectrometer (FTIR), the functional groups leading the appearance of color can be effectively oxidized by the photocatalytic process. Background: Polymorphisms within intron 7 of the tryptophan hydroxylase (TPH1) gene were found to be associated with alcohol dependence in different ethnic groups, including the aboriginal Bunun group in Taiwan. This study aimed to identify genetic variants at the TPH1 locus and to examine their associations with alcoholism. We hypothesized that the polymorphism of TPH1 gene is functional and influences the human circadian rhythm to contribute to the pathophysiology of alcohol dependence. Methods: DNA from the Taiwanese Han and Bunun was subjected to sequence for screening genetic variation in the coding and promoter regions of the TPH1 locus. Polymorphisms among individuals with alcohol dependence and control subjects in two ethnic groups in Taiwan were investigated. Results: Three variants in the TPH1 promoter region were identified, and the markers are in complete linkage disequilibrium in both populations. Positive associations at both allelic and genotypic levels were obtained between case and control groups in the Bunun. Expression studies demonstrated that the variants indeed affected reporter gene activity in human choriocarcinoma and colon adenocarcinoma cell lines. Conclusions: Polymorphisms in the promoter region may influence the function of the TPH1 gene and further influence the proclivity of alcohol dependence in one ethnic group in Taiwan. A letter focusing on a study that analyzes demographic changes in the suicide rate among the elderly in Taiwan between 1994 and 2003 is presented. Older males who had no relatives, who were living alone, widowed or divorced had the highest risk of suicide in Taiwan and elsewhere in the world. The embedding factors that shaped this geriatric phenomena were explored.

AKT1 (V-akt murine thymoma viral oncogene homolog 1) is a protein kinase isoform of AKT. Five single-nucleotide polymorphisms, rs3803300, rs1130214, rs3730358, rs2498799 and rs2494732, at the genomic region of AKT1 have been reported to be significantly associated with schizophrenia. We tested for the presence of these five single-nucleotide polymorphisms in a Taiwanese population by genotyping 218 co-affected schizophrenia families. Both single locus and haplotypes analyses showed no association of these single-nucleotide polymorphisms with schizophrenia. These findings fail to support AKT1 as a susceptibility gene for schizophrenia in the Taiwanese population. [ABSTRACT FROM AUTHOR]

A 64-item Chinese food frequency questionnaire (FFQ) combined with open questions on types of staples and cooking oil most frequently consumed was designed for a prospective study in Taiwan to appraise participants' usual intake. We examined its reproducibility and validity. The form was administered three times at three-month intervals by face-to-face interview to 83 senior college students majoring in nutrition, in order to recall their usual dietary intake over the past six months. They also completed five-day (includes both weekends) diet records (DR) after each interview. Averaged intake levels of most nutrients assessed by the three FFQs were slightly higher than those of 15-day DR. Intraclass correlation coefficients for nutrient intakes assessed by three FFQs three months apart ranged from 0.37 for saturated fat to 0.82 for alcohol (average: 0.52). Averaged Pearson correlation coefficients between the unadjusted and energy-adjusted nutrient intakes measured by DR and by the third FFQ (which asked about diet during the six months encompassing the diet records) were 0.40 and 0.35, respectively, not including vitamins A and C. These correlations were higher (average: 0.47) after adjusting for the daily variation of the diet records. On average, 50% subjects were correctly classified into the same tertiles by both methods; 11% of the subjects were misclassified to extreme categories. These data indicate that this FFQ for Chinese-speaking people in Taiwan is reproducible and provides a useful measure of intake for many nutrients over a six-month period. [ABSTRACT FROM AUTHOR]

Pharmacogenomics is the study of drug treatment responses in subgroups of patients on the basis of their genetic variants or genetic expression information. In 2006, the of Taiwan launched a methadone maintenance treatment program, which is usually prescribed for patients with heroin dependence. In this study, 366 patients who had taken methadone continually in the previous 7 days were examined. Data from the Treatment Outcomes Profile (TOP), Severity of Dependence Scale (SDS), Clinical Opioid Withdrawal Scale (COWS), and Treatment Emergent Symptoms Scale (TESS) were obtained from the patients' report records. The genes CYP2B6, CYP3A4, and CYP2C19 ― which encode the liver cytochrome P-450 (CYP) enzymes that are involved in the metabolism of methadone ― were selected and genotyped in this cohort. We found that single nucleotide polymorphisms (SNPs) on the CYP2B6 gene were associated with the plasma S-methadone concentration. The SNPs on the CYP3A4 gene were associated with withdrawal symptoms and side effects. The SNPs on the CYP2C19 gene were associated with the methadone dose. The SNPs in the genes (e.g., UGT2B7) encoding the morphine phase II metabolic enzyme were associated with the withdrawal symptom scores. In pharmacodynamic genes, the SNPs on the OPRM1 gene were associated with the side effects of insomnia and change in libido. We conclude that SNP markers may be useful in the future for adjusting methadone dosage and for reducing adverse reactions. [ABSTRACT FROM AUTHOR]
