Background/purpose: High-dose-rate (HDR) electronic brachytherapy (EBT) has been shown to be effective for non-melanoma skin cancer (NMSC) in Caucasian patients. However, its efficacy remains unknown in Asian patients. To analyze the clinical outcome of HDR EBT for NMSC in a Taiwanese medical center.Methods: Medical records over a 5-year period between January 2015 to December 2019 were retrospectively analyzed.Results: Forty-seven patients with 54 NMSC including 42 basal cell carcinomas (BCCs) and 12 squamous cell carcinomas (SCCs) were treated with HDR EBT. The average age was 73.8 years. The mean radiation dose was 45.3 Gy (40-80 Gy). Mean follow-up duration was 33.1 months. Adequate local control was achieved in 50 lesions (92.6%). Grade 1 acute skin toxicity was noted in 63.0% of lesions, while no tumors had Grade 4 acute toxicity. No ulceration was observed six months after completion of treatment. At the last follow-up visit, all lesions were rated to have "fair" to "excellent" cosmetic outcomes.Conclusion: HDR EBT provides adequate clinical outcomes and cosmetic results for NMSC in Asian patients. Betel quid (BQ) products, with or without tobacco, have been classified by the International Agency for Research on Cancer (IARC) as group I human carcinogens that are associated with an elevated risk of oral potentially malignant disorders (OPMDs) and cancers of the oral cavity and pharynx. There are estimated 600 million BQ users worldwide. In Taiwan alone there are 2 million habitual users (approximately 10% of the population). Oral and pharyngeal cancers result from interactions between genes and environmental factors (BQ exposure). Cytochrome p450 (CYP) families are implicated in the metabolic activation of BQ- and areca nut-specific nitrosamines. In this review, we summarize the current knowledge base regarding CYP genetic variants and related oral disorders. In clinical applications, we focus on cancers of the oral cavity and pharynx and OPMDs associated with CYP gene polymorphisms, including CYP1A1, CYP2A6, CYP2E1, and CYP26B1. Our discussion of CYP polymorphisms provides insight into the importance of screening tests in OPMDs patients for the prevention of oral and pharyngeal cancers. Future studies will establish a strong foundation for the development of chemoprevention strategies, polymorphism-based clinical diagnostic tools (e.g., specific single-nucleotide polymorphism (SNP) "barcodes"), and effective treatments for BQ-related oral disorders.

To facilitate making a diagnosis of cholestatic liver disease in Taiwan, we have established reference ranges for serum gamma-glutamyl transpeptidase (gamma-GT) activity and bile acids in normal Taiwanese infants. The serum level of gamma-GT activity was assayed in 90 normal Taiwanese infants and children aged between 2 days and 2 years old. These data were analyzed in twenty-three 0-3-month-old, twenty-four 4-6-month-old, and forty-three 7-24-month-old infants. The mean values of serum gamma-GT activity were 47.4 +/- 26.6, 21.5 +/- 7.3, and 14.0 +/- 3.2 IU/L for the respective age groups. The highest reference gamma-GT values were 99.5, 35.8, and 20.3 IU/L for the respective age groups. The mean values of serum gamma-GT activity were highest in infants younger than 3 months, and these gradually decreased to the adult level with age (p < 0.01). Serum bile acids were measured in 24 premature and 56 full-term infants. The mean values of serum bile acids were 34.5 +/- 34.5 mumol/L in preterm infants and 18.7 +/- 21.9 mumol/L in full-term babies. The bile acid levels of preterm infants were higher than those in full-term babies (p < 0.01).
