Background: Childhood obesity is a public health concern that disproportionately affects populations from low socioeconomic status (SES) and minority groups. Evaluation of social and health risk factors of preschool children living along the Texas-Mexico border provides feedback to design health interventions. Methods: South Texas Early Prevention Study-PreK (STEPS-PreK) is a cluster randomized trial designed to assess the effect of the Bienestar coordinated school health program on children's health outcomes. Family characteristics, dietary intake, fitness, and anthropometric data were collected from 1277 preschool students enrolled in 28 preschools. Results: The response rate was 67%. Overall, 57% of families lived in poverty. The mean age of students was 4.7 years, 95% were Hispanic, and 51% were male. The average serving of fruits and vegetables per day were 1 and 1/3, respectively. Of these, students consumed 39.7% of fruits and 18.9% of vegetables. Obesity prevalence for boys was 19.2% and for girls 16.8%. Nearly one-half reported some form of food insecurity. Conclusions: Children living in low-income areas are affected by high levels of social and health risk factors. It is these families who should be targeted with early-age and culturally appropriate health programs.

The South Texas Early Prevention Study-PreK, a cluster randomized trial, explored the Bienestar Coordinated School Health Program's influence on health variables in South Texas children. Factors associated with food insecurity (FI) were analyzed. Income-to-poverty ratio, income, and food assistance (FA) program participation were all associated with FI. Forty percent of families (257/643) reporting FI did not utilize FA. Of those families with FI, but who reported no FA participation, 31.8% were FA eligible. Families experiencing FI may underutilize FA programs, despite eligibility. Curtailing obstacles preventing FA program participation and matching those who are eligible with FA programs are crucial. Student-centered learning is an approach in education focusing on the needs of the students, rather than those of others involved in the educational process, such as teachers and administrators (Blumberg, 2009). This approach has many implications for the design of curriculum, course content, and interactivity of courses (O'Neil & McMahon, 2005). Major tenets of student-centered learning include understanding of the material, active learning on the student part and increased responsibilities on the student part, increased instructor responsibilities on creating an environment that facilitates the learning process, assessment process is intergraded with feedback providing, and both parties (the instructor and the students) share some of the decision making responsibilities (Blumberg, 2009; Lea, Stephenson, & Troy, 2003). This paper presents a novel Lasso Logistic Regression model based on feature-based time series data to determine disease severity and when to administer drugs or escalate intervention procedures in patients with coronavirus disease 2019 (COVID-19). Advanced features were extracted from highly enriched and time series vital sign data of hospitalized COVID-19 patients, including oxygen saturation readings, and with a combination of patient demographic and comorbidity information, as inputs into the dynamic feature-based classification model. Such dynamic combinations brought deep insights to guide clinical decision-making of complex COVID-19 cases, including prognosis prediction, timing of drug administration, admission to intensive care units, and application of intervention procedures like ventilation and intubation. The COVID-19 patient classification model was developed utilizing 900 hospitalized COVID-19 patients in a leading multi-hospital system in Texas, United States. By providing mortality prediction based on time-series physiologic data, demographics, and clinical records of individual COVID-19 patients, the dynamic feature-based classification model can be used to improve efficacy of the COVID-19 patient treatment, prioritize medical resources, and reduce casualties. The uniqueness of our model is that it is based on just the first 24 hours of vital sign data such that clinical interventions can be decided early and applied effectively. Background: Popliteal artery entrapment syndrome (PAES) is an increasingly encountered disorder that typically presents as claudication in young and active individuals. However, despite the increased recognition, accurate preoperative diagnosis can be difficult. The objective of this study was to describe the surgical assessment and outcomes of patients treated for PAES., Methods: Retrospective case series of all patients managed surgically for a diagnosis of PAES at the San Antonio Military Medical Center from 2005 to 2013., Results: Over 8 years, PAES was surgically treated in 25 consecutive limbs of 15 patients (mean age, 35; range, 21-49) in a military tertiary medical center. Type III was the most common variant (n = 13, 52%), followed by type VI (n = 7, 28%). Most patients presented with class I or II ischemia (88%), with anterolateral symptoms (56%), and were referred by orthopedics (66%). Diagnostic work-up included stress ankle-brachial indices, magnetic resonance imaging (MRI) and provocative angiography. Sixty-three percent of limbs with negative MRI demonstrated findings consistent with either type III or V PAES. Tendon release was used in those with types III and V, whereas liberal myectomy was used in those with types I, II, or VI. Two patients required revascularization. At a median follow-up of 126 days (range, 25 days-7 years), 83% of patients with type III demonstrated partial resolution of symptoms. Only 27% of patients without an identifiable muscle slip had clinical improvement., Conclusions: Despite modern imaging, open surgical exploration remains the definitive diagnostic modality for PAES. Patients with a muscular or tendinous slip identified intraoperatively have the best clinical outcomes. Those with no identifiable muscle slip (functional entrapment) are less likely to demonstrate clinical improvement. Further evaluation on outcomes in the management in PAES is warranted., (Published by Elsevier Inc.)
