The deterioration in masticatory function often seen in older adults rules out the consumption of hard foods, while a common decline in autonomic nervous response often observed in older adults may result in maladjustment to environmental stresses. The purpose of this study was to investigate whether autonomic nervous activity in older adults increased during the mastication of soft foods compared to at rest. The participants comprised 9 volunteer nursing home residents aged 78 to 100 years. All consumed soft rice as a grain dish and coarsely chopped food as side dishes. The coefficient of variation of R-R intervals (CVRR) and high-frequency (HF) power on electrocardiograph (ECG) served as indices of parasympathetic nervous activity, while the low-frequency (LF)/HF ratio served as an index of sympathetic nervous activity. Total power, reflecting the level of autonomic nervous activity, was also measured. We measured autonomic nervous activity while nursing home residents were eating lunch. HF was 3,457.4±3,037.5 ms 2 at rest and 4,085.1±3,697.7 ms 2 during mastication. The CVRR was 26.8±16.9% at rest and 26.1±16.1% during mastication. No significant difference was observed in HF or CVRR. On the other hand, the LF/HF ratio was 0.77±0.24 ms 2 at rest and 31.0±3.77 ms 2 during mastication (p=0.011). A significant difference was observed in the LF/HF ratio between the resting and masticating conditions. Total power was 6,244.1±5,752.5 ms 2 at rest and 8,417.7±7,266.4 ms 2 during mastication (p=0.038). A significant difference was also observed in total power between resting and masticating conditions. These results suggest that mastication of soft foods stimulates autonomic nervous activity in older adults. In conclusion, autonomic nervous activity increased during chewing soft foods compared to that during the resting level in older adults.

Objective: The aim of this study is to clarify the association between area-based social capital and suicide rate among municipalities of Tokyo., Methods: The study areas were 20 administrative municipalities of Tokyo. Social capital (i.e., organizational membership, social trust, fairness, helpfulness, and confidence in organizations) was measured based on data from a previous survey (response rate 28 %). Gender-specific age-adjusted suicide rates averaged over 5 years, sociodemographic, and other area characteristics were obtained from relevant national statistics. Multiple linear regression analysis of suicide rates was applied on each social capital variable, adjusting for the other area characteristics., Results: There was no significant crude Pearson's correlation between any social capital variable and suicide rate. Multiple regression analyses revealed a significant negative association between social trust and suicide rate for men (p = 0.04)., Conclusions: While based on only limited evidence from a cross-sectional ecological study, area-based social trust may be associated with decreased suicide rates for men in Tokyo.

On the day of the disaster, 641 victims were seen at St. Luke's International Hospital. Among those, five victims arrived with cardiopulmonary or respiratory arrest with marked miosis and extremely low serum cholinesterase values; two died and three recovered completely. In addition to these five critical patients, 106 patients, including four pregnant women, were hospitalized with symptoms of mild to moderate exposure. Other victims had only mild symptoms and were released after 6 hours of observation. Major signs and symptoms in victims were miosis, headache, dyspnea, nausea, ocular pain, blurred vision, vomiting, coughing, muscle weakness, and agitation. Almost all patients showed miosis and related symptoms such as headache, blurred vision, or visual darkness. Although these physical signs and symptoms disappeared within a few weeks, psychologic problems associated with posttraumatic stress disorder persisted longer. Also, secondary contamination of the house staff occurred, with some sort of physical abnormality in more than 20%.

We developed a new questionnaire in the surgical area based on a core quality of life (QOL) questionnaire for patients with gastrointestinal cancer. In this study, we investigated the validity and reliability of a QOL questionnaire (Tokyo Yamabuki Forum Version) for patients with colorectal cancer. The questionnaire was composed of 17 items including 5 scales (basic sensory scale, psychological scale, physiological scale, defection-related scale and active scale) and a face scale as an global scale. The time needed to answer questionnaires was expected to be around 7 minutes and the questionnaires should basically be answered by the patients themselves everyday in the hospital. The study was performed in 10 hospitals in the Tokyo area, and 394 samples collected from 21 patients with rectal and colonic cancers were analyzed. A number of respondents failed to answer the question "Do you feel your foods tasty?", so we judged this item inappropriate and deleted it from the analysis. Fifteen items, including 5 scales showed satisfactory internal consistency and construct validity in correlation and factor analyses. Performance status showed a low correlation between each item, each scale and the global scale, while SDS and STAI showed an inordinately negative correlation with the fundamental and physical scales. Especially, SDS revealed an extremely close correlation with the active scale, and STAI showed an excessive correlation with the psychological scale. In the time course of QOL under chemotherapy, reductions (aggravations) were observed in both the total score of 15 items and global scale within one week postoperatively, but after that recovered to preoperative levels at 2 weeks postoperatively. A tendency to QOL improvement was observed 2 weeks after starting chemotherapy or chemoimmunotherapy. QOL of 13 patients was measured over 3 months, and the longest term was 8 months. The results suggested that this QOL questionnaire has sufficient reliability and validity to be usable for patients with colorectal cancer in the surgical area and that this model is applicable for long-term QOL surveys and frequent measurement.
