Autologous hematopoietic cell transplantation (AHCT) is a routinely used procedure in the treatment of adult patients presenting with multiple myeloma (MM), Hodgkin lymphoma (HL) and various subtypes of non-Hodgkin lymphoma (NHL) in upfront and relapsed/refractory settings. Successful hematopoietic progenitor cell mobilization (HPCM) and collection are the rate limiting first steps for application of AHCT. In 2015, almost 1700 AHCT procedures have been performed for MM, HL and NHL in Turkey. Although there are recently published consensus guidelines addressing critical issues regarding autologous HPCM, there is a tremendous heterogeneity in terms of mobilization strategies of transplant centers across the world. In order to pave the way to a more standardized HPCM approach in Turkey, Turkish Society of Apheresis (TSA) assembled a working group consisting of experts in the field. Although graft versus host disease (GVHD) is associated with a myriad of cutaneous signs, nail involvement is rarely mentioned in the literature. This study was conducted at the Department of Dermatology and The Bone Marrow Transplantation Unit of Hematology. All patients were examined clinically for presence of nail abnormalities. Severity of nail involvement was assessed as mild or severe. Of 28 patients diagnosed with chronic cutaneous GVHD, 14 had nail manifestations. Longitudinal ridging was the most frequently observed nail change on both the fingernails and the toenails. Severe nail changes such as ptergyium and onicoatrophy were noted in a few patients. As a result we could not find any relationship between nail changes and clinical severity but there was a relationship between nail changes and duration of disease. Allogenic hematopoietic cell transplantation (AlloHCT) is currently a curative cellular therapy approach in hematological malignancies of adults. Donor availability and eligibility of the recipient have been always the major steps on moving forward in alloHCT. Establishing a national registry is difficult task and present registries as partners of WMDA are serving worldwide on international platform. TURKOK is a new donor registry, established by Turkish Ministry of Health in 2015. The current grounds in donor availability/unrelated search is currently questioned by the rise in haplotype matched AlloHCT era. As a single active center since 2010, having performed more than 600 alloHCT, we analyzed the impact of TURKOK on our transplant kinetics and registry based outcome, hypothesizing faster kinetics for logistics and better survival using own citizens, who may have less disparity on HLA immunogenetics basis. One hundred and seventeen adult (17-72 years) patients who underwent AlloHCT using matched unrelated donors (MUD) at Florence Nightingale Hospital Hematopoietic Stem Cell Transplantation Center between 2015 and 2019 were included in this study. Medical records of patients were reviewed retrospectively. Transplantation was performed in cases with high or very high DRI. The analysis have been performed on intent to treat basis, having AlloHCT as landmark. Our alloHCT activity using MUD started in 2012, among 127 AlloHCT performed MUD were provided from German BM Registry (DKMS) (n=80), TURKOK (n=38), from other national registries (non-DKMS) (n=9). The donors differ significantly only on CMV seropositivity, in TURKOK more than 90%, whereas in others less than 55%. Matching rate (%) 9/10-10/10 was nearly the same for DKMS, TURKOK and non-DKMS, 60-40, 66.7-33.3 and 55.3-44.7, respectively. In 2015 only 20% of the MUD AlloHCT were facilitated by TURKOK in our center whereas in 2018 more than 55%. Mean duration (SD1) from start of the donor search to infusion of HSC for DKMS, TURKOK and non-DKMS was 176.2 (142.3), 92 days (40.4) and 123 (63.4), respectively. The overall survival analysis at 2 years showed no statistical difference on log-rank analysis (p=0.24) (Figure 1). Turkish national donor registry, TURKOK is serving faster as expected due to native donor candidates and facilitated logistics. The CMV donor positivity is clearly different in TURKOK, any impact on transplant outcome is still not clear. The HLA disparity is nearly the same on all three groups and survival analysis did not show any impact of national or international donor registry on transplant outcome. A letter to the editor is presented which was conducted on Turkish hematologists.

Therapeutic plasma exchange (TPE) is used to treat more than 60 diseases worldwide and has drawn growing interest. Little is known about the current situation of TPE activity in Turkey, so we developed a survey to obtain information about this timely topic. We collected data on TPE from 28 apheresis units throughout Turkey. We performed a total of 24,912 TPE procedures with 3203 patients over the past decade. Twenty years ago, the majority of procedures were performed for neurological and hematological disorders, and today, most TPE procedures are done for the same reasons. The only historical change has been an increase in TPE procedures in renal conditions. Currently, renal conditions were more frequently an indication for TPE than rheumatic conditions. Fresh frozen plasma was the most frequently used replacement fluid, followed by 5% albumin, used in 57.9% and 34.6% of procedures, respectively. The most frequently used anticoagulants in TPE were ACD-A and heparin/ACD-A, used with 1671 (52.2%) and 1164 (36.4%) patients, respectively. The frequency of adverse events (AEs) was 12.6%. The most common AEs were hypocalcemia-related symptoms, hypotension, and urticaria. We encountered no severe AEs that led to severe morbidity and mortality. Overall, more than two thirds of the patients showed improvement in the underlying disease. Here, we report on a nationwide survey on TPE activity in Turkey. We conclude that there has been a great increase in apheresis science, and the number of TPE procedures conducted in Turkey has increased steadily over time. Hematopoietic cell transplantation is an established treatment option with curative potential for a variety of clinical conditions. The last decade especially witnessed a remarkable increase in HCT activity in Turkey. In 2014, 696 pediatric and 2631 adult (total 3327) HCT were performed in Turkey. Corresponding transplant rates per 10 million inhabitants for autologous-HCT and allogeneic-HCT were 226 and 202, respectively. Total HCT procedures in Turkey increased 177% in the last 5 years and 791% in the last 14 years. This report focuses mainly on HCT activity of Turkey in 2014 based on the national HCT registry and presents a general picture of national HCT activity. [ABSTRACT FROM AUTHOR]

Turkish Bone Marrow Transplantation Registry (TBMTR) was established in 1995. Since 1992, data of adult lymphoma patients from fifteen national transplantation centers were collected and analyzed by TBMTR. A total of 437 adult lymphoma patients (185 Hodgkin's and 252 non-Hodgkin's lymphoma) undergoing autologous hematopoietic stem cell transplantation (HSCT) were registered in TBMTR from 1992 to 2002. Peripheral blood as stem cell source was used in 94% of the transplantations. Non-TBI (total body irradiation) conditioning regimens were administered in 88% of the cases. The 100-day transplantation-related mortality (TRM) was 11% in relapsed and 11% in primary refractory Hodgkin's lymphoma patients whilst TRM was found to be 9% and 30% in non-Hodgkin's lymphoma patients in first remission and in primary refractory cases, respectively. Infection was the most common cause of TRM. 10-year and 5-year survival rates were 50% and 49% in relapsed cases and primary refractory cases with Hodgkin's lymphoma, respectively; while in non-Hodgkin's lymphoma patients 10-year survival rate was 65% in cases in first remission, 7-year survival rate was 50% in sensitive relapse, 2-year survival rate was 0% in resistant relapse and 3-year survival was 24% in primary refractory cases. In conclusion, TBMTR results are comparable to EBMT and IBMTR results. Therefore, autologous HSCT may provide long-term survival in patients with Hodgkin's lymphoma as well as in patients with non-Hodgkin lymphoma in first remission and in sensitive relapse. Allogeneic hematopoietic stem cell transplantation (allo-HSCT) is a widely utilized treatment for various hematological diseases. While selection criteria for unrelated donors are well established, there is a lack of consistency and standardization in the selection of related donors. This study investigated the current approach of hematologists to the selection of related donors at Turkish HSCT centers. The study employed a cross-sectional survey design, distributing a self-administered questionnaire to 95 adult and pediatric transplantation centers in Türkiye to investigate their approaches to related donor selection for allo-HSCT. The questionnaire collected data on various topics including the center’s experience in performing allo-HSCT, patient groups treated, number of allo-HSCT procedures conducted between 2015 and 2021, preferences for related donors, considerations in related donor selection (such as sex and past pregnancies), guidelines utilized for related donor selection, upper age limit for related donors, and the use of specialized advanced analyses for elderly donors. The response rate to the survey was 38.9%. Variability was observed across centers in terms of sex consideration and the impact of past pregnancies on related female donor rejection. Different guidelines were employed for related donor selection, with the European Bone Marrow Transplantation guidelines being the most commonly used. Regarding the upper age limit for related donors, 8.1% of centers accepted an upper age limit of 55 years, 48.7% preferred an upper age limit of 65 years, and 43.2% selected related donors aged 65 and above. The lack of standardized guidelines for related donor selection in HSCT centers leads to variability in criteria and potential risks. Aim: To determine the unmet needs and challenges in management, diagnosis, treatment, follow-up and patient-physician communication in acute leukemia (AL). Materials & methods: The study was based on a modified Delphi approach. A questionnaire including the major potential obstacles was circulated twice among 13 hematologists. Results: The obstacles in AL management were limited access to the novel treatments and genetic tests, limited bed capacity, insufficient level of knowledge among allied health personnel, limited availability of psycho-oncological support and low levels of awareness in the population about the importance of stem cell donation. Conclusion: The challenges in the management of AL are critical to guide the efforts to improve the quality of healthcare delivery and the evidence-based decision making at treatment of AL patients.

The aims of this cohort study were to evaluate the association of malignant lymphoproliferative disorders in patients with chronic viral hepatitis and to compare the results with those in individuals with non-alcoholic fatty liver disease. A total of 3,873 patients with chronic liver disease who were seen consecutively in the Liver Disease Outpatient Clinic between January 2001 and July 2007 were assessed retrospectively. The frequency of malignant lymphoproliferative disorders including non-Hodgkin's lymphoma, Hodgkin's lymphoma, and chronic lymphocytic leukemia in these patients was investigated. Of the total, 1,999 patients had chronic hepatitis B infection (male/female: 1,226/773, mean age: 45.1 ± 13.2 years), 978 had chronic hepatitis C infection (male/female: 437/541, mean age: 53.8 ± 13.7 years), and the remaining 896 had non-alcoholic fatty liver disease (male/female: 450/446, mean age: 50.8 ± 11.2 years). A malignant lymphoproliferative disorder was identified in 13 patients (male/female: 9/4, mean age: 52.8 ± 16.8 years) with chronic viral hepatitis, while no case of malignant lymphoproliferative disorder was identified in individuals with non-alcoholic fatty liver disease (P = 0.048). Among the patients with malignant lymphoproliferative disorders, seven had chronic hepatitis B infection and six had chronic hepatitis C infection; 11 had non-Hodgkin's lymphoma and two had chronic lymphocytic leukemia. All non-Hodgkin's lymphoma cases were B-cell lymphoma. Based on the data obtained in this investigation, the association with malignant lymphoproliferative disorders in chronic viral hepatitis seems to be high as compared to that occurring in individuals with non-alcoholic fatty liver disease., (Copyright © 2011 Wiley-Liss, Inc.)

The 2nd International Congress on Leukemia, Lymphoma and Myeloma (ICLLM) is a novel initiative of the Turkish Society of Hematology. The first congress was organized in Fethiye in May, 2007. After the success and positive impressions of the participants, the THD decided to organize the ICLLM every other year. The major focus was hematological malignancies, not only LLM, but also myeloproliferative disorders and myelodysplastic syndromes. The target audience was hematologists, oncologists and those involved in stem cell transplantation. The congress was held over 2 and a half days and attended by more than 350 participants from 25 different countries. The scientific program was shaped by 12 distinguished international speakers, who co-chaired and designed the sessions. The majority of the hematologists are caring for malignant patients and the Congress was a great opportunity for participants to get the latest update before the American Society of Clinical Oncology and American Society of Hematology 2009 official meeting. The rising interest and encouraging increase in the number of participants geared us to announce the organization of 3rd ICLLM in May 2011 in Istanbul.

Data collection in the field of medicine is extremely important for patient safety. It is viewed as a quality issue in several countries around the world. In order to achieve this goal, nationwide apheresis registries were planned. In 2003, World Apheresis Association (WAA) decided to establish a world-wide apheresis registry. Although some of the apheresis centers in our country sent data to this registry, we believe that we need our own Turkish Registry. For this purpose, we launched new web based apheresis software for donor and therapeutic apheresis procedures. This paper summarizes the characteristics of this new software.

Purpose: To present the radiological and clinical follow-up results of 75 bone marrow transplant patients who underwent fluoroscopy-guided tunneled catheter placement between June 2001 and June 2004., Materials and Methods: Tunneled catheters were placed in 75 bone marrow transplant patients with fluoroscopic guidance. The left subclavian vein was used in 67 patients, whereas the right side was used in 9., Results: The first attempt of catheter insertion was failed in 3 patients who then underwent contralateral catheter placement. No complications were noted during or immediately after the procedures. Late complications included 8 cases of infection, 2 cases of fibrin sheath formation, and 1 case of persistent hiccups, which began at the time of catheter insertion. Inadvertent catheter removal was noted in 4 cases., Conclusion: Fluoroscopy-guided central venous catheterization should be preferred over the anatomical landmark technique due to its higher technical success rate, shorter procedure time, and lower complication rate. When placing a central venous catheter, multiple factors should be considered, such as catheter type, number of lumens, duration and frequency of pertinent treatments, and patient needs. The procedural and early post-procedural complications were mostly related to the placement technique; however, the late complications could have been prevented by nurse care and patient education.

Background: The number of therapeutic procedures is increasing steadily year by year with growing collaboration of departments other than Hematology. In the aim to demonstrate our single center activity we analyzed our data since four years., Methods: Between years 1998 and 2001, 658 therapeutic plasma exchange (TPE) procedures were performed on 158 patients. Median age and male/female ratio were 37 (range, 15-87) and 80/78, respectively. Main indications were myastenia gravis (n=55, 34%), TTP (n=13, 8.5%), post ABO mismatched allogeneic hematopoietic cell transplantation aregeneratoric anemia (n=6, 7.5%), progressive systemic sclerosis (n=10, 6.5%), multiple myeloma (n=10, 6.5%), Gullian Barre Syndrome (n=9, 5.9%), multiple sclerosis (n=7, 4.6%), Waldenström Macroglobulinemia (n=5, 3.4%), polymyositis (n=4, 2.7%), sepsis and disseminated intravascular coagulation (n=4, 2.7%). Departments who referred the majority of the patients for TPE were neurology (n=199), hematology (n=153), immunology (n=78), intensive care unit (n=78) and thorax surgery (n=51)., Results: The median TPE procedure per patient was 4 (range, 1-50). All the procedures were performed on continuous flow cell separators and median plasma volume processed per cycle was 2471 ml (range 436-5000). The replacement fluids used were 3% hydroxyethylstarch (HES) (24%), 5% albumin (35%), fresh frozen plasma (25%), and HES and albumin (16%). HES was tolerated well even as a sole replacement fluid with acceptable minor side effects. In three patients with progressing hypoalbuminemia HES was replaced or combined with 5% albumin. Close monitoring of serum albumin and fibrinogen levels after repeated procedures is mandatory., Conclusion: In our four years of TPE experience we have increased our collaboration with other departments. 3% HES+/-5% Albumin is a feasible, well tolerated and cost effective replacement fluid combination especially for short-term plasma exchange therapy.

The aim of this study was to evaluate the clinical manifestations of cutaneous graft-versus-host disease (GVHD) developed after allogeneic haematopoietic cell transplantation. In all, 67 patients were evaluated: 49 patients developed acute GVHD, 17 patients developed de novo chronic GVHD and 29 developed secondary chronic (15 limited, 14 progressive) GVHD following acute cutaneous GVHD. Of the 46 patients with chronic GVHD, lichenoid lesions were observed in 32 and sclerodermoid lesions were observed in 12. In four patients with sclerodermoid cutaneous GVHD, these lesions occurred after a lichenoid phase. Oral lesions were present in 61% of the patients and six of them had only oral mucosal involvement without any skin lesions. Nail lesions were observed in 31% of the patients. During the follow-up period 15 patients with GVHD died and in 7 of them the cause of death was related to chronic GVHD. In conclusion, GVHD has a wide spectrum of cutaneous manifestations, which can be used as an important tool for the early diagnosis of the disease.
