Aim: Pediatric hypertension, a public health concern, is now commonly known worldwide to be an early risk factor for cardiovascular and renal morbidity and mortality. Early detection of hypertension is of the utmost importance to help reduce serious complications. Several distributions of country-specific blood pressure (BP) percentiles have been established worldwide. The aim of this study is to determine BP percentiles in healthy Turkish children aged 2 to 18 years. Materials and Methods: In this cross-sectional study, BP was measured in 4,984 randomly selected children and adolescents aged 2-17 years. The 50th, 90th and 95th percentile of BP percentiles were determined for gender, age and height with the use a polynomial regression model. BP percentiles at median height were compared with the US Fourth Report references. Results: The normative values of systolic blood pressure (SBP) and diastolic blood pressure (DBP) increased with age for both genders and varied by gender. At median height, the age-specific differences at the 90th percentile of SBP tended to be higher in boys than in girls at all ages. DBP values in girls were higher than in boys until the age of 9 years, after which boys demonstrated higher values compared to girls. Conclusion: The age and height specific reference BP values determined in this study is a novel reference for Turkish children and adolescents. Abstract: Depression in the elderly is associated with increased morbidity and mortality. The purpose of this study was to determine the prevalence and risk factors of depression among community-dwelling older population in an urban setting in Turkey. This cross-sectional study was conducted among 482 elderly individuals 65 years and over in an urban area. Cluster sampling method was used for sample size. Depression in the elderly had been diagnosed by a clinical interview and Geriatric Depression Scale. Data were collected by door-to-door survey. Chi square test was used for statistical analysis. P value, which was calculated by the results of chi square test and coefficient of phi (φ), below 0.05 was included in the analysis of logistic regression. Depression was significantly associated with female gender, being single or divorced, lower educational status, low income, unemployment, and lack of health insurance. However, logistic regression analysis revealed higher depression rates in the elderly with chronic obstructive pulmonary disease, psychiatric disease, cerebrovascular disease, low income and being dependent. Depression is common among community-dwelling older people in an urban area of Izmir, Turkey. Older adults living in community should be cautiously screened to prevent or manage depression. Abstract: The aim of this study is to determine the use of pre-hospital emergency medical services (EMS) in elderly people aged 65 years and over in Izmir, Turkey. In this descriptive study, older patients admitted to pre-hospital EMS of Izmir Province Health Directorate between 2004 and 2005 years was evaluated through the review of Emergency Call Registry Forms. The study data included socioeconomic characteristics, reasons of calling, distribution of calling times in the day, distribution of ambulance callers and preclinical diagnosis. A total of 34% of the subjects admitted to pre-hospital EMS were 65 years old and over. The rate of the use of ambulance services was 68.9/1000 population/year. The rate of pre-hospital EMS use for older persons living in urban areas was significantly higher than that of those living in rural areas. The most frequent pre-hospital EMS caller were persons in family (70.7%), and utilization of ambulance services was the highest in winter. Medically related incidents accounted for 89.1% of all emergency ambulance calls and cardiovascular diseases accounted for most common cause (32.6%) of calls. The utilization rate of pre-hospital EMS among older persons was approximately four times higher than that of the younger age groups. [Copyright &y& Elsevier]

Objective To evaluate the reliability and validity of the revised Turkish version of Mini Mental State Examination (rMMSE-T) in educated and uneducated community-dwelling elderly, to re-organize the present Turkish version of MMSE and to determine cut-off point of the revised test. Methods This cross-sectional and analytical study involved totally 490 elderly subjects selected by cluster sampling method. Receiver operating characteristic (ROC) analysis, kappa analysis and Cronbach's α coefficients were used for statistical analysis. Results Areas under ROC curve in educated and uneducated elderly were found as 0.953 and 0.907. Cut-off point of 22/23 of rMMSE-T in educated elderly had the highest sensitivity (90.9), specificity (97.0) and positive likelihood ratio (30.3), whereas cut-off point of 18/19 of the test in uneducated elderly had the highest sensitivity (82.7), specificity (92.3) and positive likelihood ratio (10.7). The Cronbach's α values of the rMMSE-T for educated and uneducated elderly were higher than 0.7 (sign of good internal consistency of the test). A significant correlations between intrarater and interrater test–retest in educated elderly subjects were observed (0.966 (p = 0.000); 0.855 (p = 0.000), respectively), and also in uneducated elderly (0.988 (p = 0.000); 0.934 (p = 0.000), respectively). Kappa value of the test in educated and uneducated elderly showed a perfect agreement interraters (1.000) and a substantial agreement in intraraters (1.000, 0.784; 0.826, 0.656, respectively). Conclusion rMMSE-T had a high reliability and validity. It will be more appropriate to use the revised test and the new cut-off point for the diagnosis and screening of dementia among community-dwelling Turkish elderly population. Abstract Background: Measles outbreaks seem to occur every 2- to 3-year intervals in Turkey. However, sero-epidemiological studies are limited. Knowing the prevalence of measles susceptibility as measured either by serologic markers of immunity or surveys of vaccination coverage is an important tool to assess the risk for measles outbreaks. Methods: In order to determine the seroprevalence of measles antibodies among a 1 to 29-year-old population in İzmir (Turkey) and to develop the best vaccination policy for measles, a total of 600 people aged from 1 to 29 were selected for the study with cluster sampling. The information on sociodemographic characteristics, vaccination status and measles history was gathered for each participant. Measles-specific IgG antibodies were screened qualitatively by using microenzyme immune assay for 595 subjects. Results: Of the 595 participants screened for the measles antibodies, 56 (9.4%) were seronegative. The proportion of the susceptible individuals in the age groups of 1–4, 5–9, 10–14, 15–19 and 20–29 was 20.0, 10.4, 6.0, 10.3 and 3.0%, respectively. The logistic regression analysis showed that none of the independent characteristics (sex, socioeconomic status, past measles history, vaccination status) with the exception of age group, was significantly associated with measles seronegativity. Conclusion: The optimal measles vaccination policy for Turkey may be to increase vaccination coverage above 90%, to conduct a catch-up campaign covering persons aged 1–19, regardless of previous vaccination status. Another factor to consider is to adopt a routine two-dose vaccination, giving the first dose at 12_15 months of age and the second dose at school entry. [ABSTRACT FROM AUTHOR]

Objective: To survey experience with the first-trimester combined test (FCT) for trisomy 21 (T21) in different risk score groups to determine the most useful clinical application of cell-free fetal DNA (cffDNA) screening.Methods: In a retrospective study, the records of FCT results obtained at a center in Turkey between January 2009 and January 2014 were reviewed. The FCT results and rates of uptake of invasive diagnostic testing were compared among different risk score groups.Results: FCT results were available for 4804 pregnancies; 276 (5.7%) had IDT results. Ten (72.7%) of 11 cases of T21 had a risk score of 1:300 or more. The IDT uptake rates were 54.5%, 51.9%, and 47.4% at risk scores of 1:100 or more, 1:200 or more, and 1:300 or more, respectively. In the group at intermediate risk (1:1001-1:3000), no pregnancy had an FCT result of both low pregnancy-associated plasma protein A and high free β-human chorionic gonadotropin, but 30 (3.9%) of 766 pregnancies had both advanced maternal age and high β-human chorionic gonadotropin.Conclusion: cffDNA screening should be used to optimize IDT uptake in pregnancies with a risk score of 1:101-1:1000. The selective power of the FCT diminishes beyond the 1:1001 score and cffDNA screening cannot yet be recommended routinely. [ABSTRACT FROM AUTHOR]

Objectives: The Sino-Nasal Outcome Test (SNOT)-22 is a widely used health-related quality of life questionnaire. This study aimed to conduct a translation and validation study of the SNOT-22 in the Turkish language., Design: We achieved a convenient translation and cultural adaptation process to translate the original SNOT-22 into the Turkish language (observational prospective cohort study)., Setting: This study was conducted in a single tertiary-level university hospital., Participants: We performed reliability, validity and responsiveness analyses in 313 participants. One hundred eighty-nine of the participants were the chronic rhinosinusitis (CRS) group, and 79 healthy volunteers enrolled in the control group. Twenty-nine participants who underwent endoscopic sinus surgery for CRS were evaluated for pre-postoperative responsiveness analysis., Main Outcome Measures: Reliability (internal consistency and test-retest reproducibility), validity, responsiveness, sensitivity and specificity., Results: The internal consistency coefficient (Cronbach's alpha) was 0.909 in the CRS test group, 0.955 in the CRS retest group, and 0.916 in the control group. The reliability value (Pearson correlation coefficient) of the test-retest group was 0.901. The validity was assessed by the independent sample t-test between the CRS and control groups and resulted in a significant difference (P < 0.001). Responsiveness was interpreted using the paired t-test between pre- and post-medical and pre- and post-surgical treatment groups; statistical analysis found significance in both groups (P < 0.001). When using the SNOT-22 cut-off value of 33.5, the sensitivity and specificity of the Turkish version of the SNOT-22 was 54.5% and 75.9%, respectively (95% CI, area under the curve (AUC): 0.69, range 0.624â€"0.756, P = 0.000)., Conclusions: The authors suggest that the Turkish SNOT-22 is a valid, reliable, reproducible and responsive questionnaire., (© 2019 John Wiley & Sons Ltd.)

Objective: To determine and compare the prevalence of and risk factors for home accidents in the elderly in two different districts., Method: This cross-sectional study involves 497 older participants ages 65 and older. Sociodemographic characteristics of the elderly and characteristics of the houses in two different regions are evaluated. Data are analyzed by chi-square and t test., Results: The prevalence of home accidents was 39.3% in the low socioeconomic region and 13.1% in the high socioeconomic region within the past 6 months (p = .000). The most common type of home accident was fall (61.8%), followed by cut or piercing (22.0%). Home accidents are associated with being female and increasing age in the higher socioeconomic region, whereas being female, being unmarried, and living in a squatter house are associated with home accidents in the low socioeconomic region., Discussion: The prevalence of home accidents in the elderly population who live in low socioeconomic region was higher.

Introduction: Sexual dysfunction in females is an important public health problem worldwide. It is suggested that sexual problems among women are more common than the number of diagnosed female sexual dysfunction (FSD) cases indicates., Aim: To determine the frequency and causes of sexual problems among premenopausal and married women who attend primary healthcare facilities., Methods: This study was conducted at the Mother and Child Health and Family Planning Center. All women who attended this center during a 3-month period were included in the study. Sexual problems were evaluated via questionnaire and a standardized scale known as the Golombok Rust Inventory of Sexual Satisfaction (GRISS). MAIN OUTCOME MEASURE. A cross-sectional study., Results: Although a total of 422 women aged 19-51 years were eligible for inclusion in the study, the participation rate was 27%. Nearly two-thirds of the women were aged 20-34 years, and of this group, 84.3% were unemployed. According to self-reports, 15.7% (18) of the women had sexual problems, whereas the prevalence of sexual dysfunction using GRISS was 26.1%. Vaginismus (41.7%), infrequent intercourse (39.1%), and nonsensuality (38.3%) were the most common complaints of the women with sexual problems. The rate of sexual dissatisfaction was found to be 7%. Sexual problems among women who had a long-term marriage (more than 11 years) and who were sexually inexperienced at the time of their marriage were significantly higher (P = 0.036, P = 0.034, respectively). It was found that discussing sexual problems with husbands and healthcare professionals did not reduce sexual problems., Conclusions: According to GRISS, nearly one-quarter of the women were suffering from sexual problems. The most common sexual problem was vaginismus, followed by infrequent intercourse. It is suggested that inadequate knowledge and the attitudes of spouses and health workers in primary healthcare settings are the important causes of FSD in this population.

Environmental tobacco smoke is an important public health problem. The objective of this study was to evaluate the effect of passive smoking on lower respiratory tract infections (LRTIs) in children aged 2-12 years. A case-control study was conducted on matched-pair design. One-hundred and fifty children with LRTIs and 150 healthy children were included in the study. Data were collected through questionnaire and urine samples for the determination of cotinine levels, and were analysed by McNemar chi-square, paired t-test and Pearson correlation tests. The prevalence of parental self-reported, indoor smoking was 71.3% in children with LRTI and 72.0% in healthy children. Employing 30 ng mg(-1), the cut-off level of urinary cotinine/creatinine as commonly accepted, 87.3% of the children with LRTIs and 84.7% of healthy children were found to be passive smokers (p = 0.61, odds ratio (OR) = 0.93; confidence interval (CI) = 0.34-2.53). If 60 ng mg(-1) of urinary cotinine/creatinine was accepted as a cut-off level, it was observed that the rates of passive smoking were 76.7% and 50.7%, respectively (p = 0.000, OR = 4.72; 95% CI = 2.62-8.52). Dose-dependent exposure to environmental tobacco smoke was found to be associated with the incidence of LRTI.

Objective: To determine and compare the prevalence and risk factors of elder abuse in elderly population in two different districts in Izmir, Turkey., Methods: This cross-sectional study involved 497 subjects selected by cluster sampling., Results: The prevalence of physical and financial abuse among the elderly in the district of low socioeconomic status was 1.5% and 2.5%, respectively, while among the elderly in the district of high socioeconomic status, it was 2% and 0.3%, respectively. However, the prevalence of elder neglect in the two districts was 27.4% and 11.2%, respectively. Prevalence of neglect was associated with infrequent contact with relatives, little or no income, and fewer years of education among the elderly in the low socioeconomic district. In the high socioeconomic district, neglect was associated with fewer years in education, poor health status and having chronic status., Conclusion: The prevalence of abuse among the elderly living in the two different districts was low. However, nearly one-fifth of elderly people were exposed to neglect.

Study Objective: Adolescent pregnancy is considered a high risk for both the mother and infant. The aim of this study was to determine obstetric and neonatal outcomes and risk factors in adolescent pregnant women and to compare perinatal outcomes among the teen age groups and between adolescent and adult women., Design: Retrospective study including adolescent pregnant women and adult women., Setting: A public maternal hospital., Main Outcome Measures: This retrospective cohort study included 945 teenagers who gave birth at year 2004 in a maternity hospital in Izmir. Dependent variables included perinatal and maternal outcomes. Independent variables were miscellanous socio-demographic characteristics and obstetric complications. Chi-square, Fisher exact test, and t-tests were used for the comparison of the adolescent group and adult women., Results: Overall, adolescents accounted for 11.8% of all deliveries in hospital. 99.7% of teenagers were unemployed, and 59.8% of those had no health insurance. 81.5% of the pregnant adolescents were nulliparous. 27.5% of teenagers gave birth by cesarean delivery. The rates of preterm birth and low birthweight of teen mothers were 18.2% and 12.1%, respectively. Twenty-eight percent of women had some obstetric and neonatal complications. The rate of preterm delivery was higher in adolescent mothers; however, cesarean delivery rate was higher in adult women (P = 0.000, P = 0.0002 respectively). Absence of health insurance, less education, and non-official marriage were significantly higher in pregnant women aged 17 years or younger compared with women aged 18-19 years (for all, P = 0.000). However, there was no statistically significant difference between the adolescent age groups regarding perinatal complications., Conclusion: The rates of pregnancy and the rates of adverse obstetric and neonatal outcomes were considerably higher in teenagers.

The aim of this study is to determine the prevalence of and risk factors of dementia in the elderly with a low socio-economical status. The study is a cross-sectional and analytic study. Two hundred and four persons aged 65 years and above were selected by using the cluster sampling method. The presence of dementia was determined by Standardized Mini Mental State Examination (SMMSE). Twenty-three and 18 cut-off points were accepted as the existence of dementia in educated and uneducated older individuals, respectively. Chi-square test, Student's t-test, and logistic regression analysis were used for data analysis. The mean age of older subjects was 70.8+/-6.45 (S.D.); 71.5+/-5.9 in males and 70.3+/-6.8 in females (p=0.19). Ninety-one percent of older people had never attended school, 54.6% were living below the poverty level, and 97% of the residences were in squatter settlements. Dementia was determined in 46 older subjects (22.9%). Dementia was found significantly higher in females, in elderly subjects aged 80 years and above, in older subjects who were in a bad or very bad self-health status and in those who had a chronic disease, did not have any occupation, had a lower monthly income, and with a depressive score eight and above (p<0.05). In our study, the prevalence of dementia in older people was higher, in agreement with many reported studies. We assumed that this was due to the fact that the ratio of the uneducated, poor, and advanced aged subjects in this study was considerably higher. The different prevalence rates in studies demonstrate that the adoption of SMMSE for the study population, the validity-reliability analyses of tests and the standardization of diagnostic criteria are necessary.
