Colonoscopy is a commonly performed procedure in the United Kingdom and the gold standard for diagnosis and therapy in the gastrointestinal tract. Increased levels of pain during colonoscopy have been associated with reduced completion rates and difficulties in maintaining attendance for repeat procedures. Multiple factors play a role in causing discomfort intra-procedurally: patient factors, such as gender, anatomy and pre-procedure anxiety; operator factors, such as patient position and level of experience and other factors, such as bowel preparation and total procedure time. A literature search was performed to identify papers that explained how patient, operator and endoscopy factors influenced pain and discomfort in endoscopy. A further search then also identified papers describing solutions to pain and discomfort that have been explored. After review of the literature, key methods are selected and discussed in this paper. Solutions and aids that can resolve and improve pain and discomfort include endoscopic methods such as variable stiffness and ultrathin scopes. Operator improvements in techniques and ergonomics alongside the use of newer technologies such as propelled endoscopy, computer-assisted endoscopy and task distraction. To improve patient experience and outcomes, the investigation and research into improving techniques to reduce pain is crucial. This review aims to identify the modifiable and non-modifiable factors associated with intra-procedural discomfort during colonoscopy. Background Problems in intrapartum electronic fetal monitoring with cardiotocography (CTG) remain a major area of preventable harm. Poor understanding of the range of influences on safety may have hindered improvement. Taking an interdisciplinary perspective, we sought to characterise the everyday practice of CTG monitoring and the work systems within which it takes place, with the goal of identifying potential sources of risk. Methods Human factors/ergonomics (HF/E) experts and social scientists conducted 325 hours of observations and 23 interviews in three maternity units in the UK, focusing on how CTG tasks were undertaken, the influences on this work and the cultural and organisational features of work settings. HF/E analysis was based on the Systems Engineering Initiative for Patient Safety 2.0 model. Social science analysis was based on the constant comparative method. Results CTG monitoring can be understood as a complex sociotechnical activity, with tasks, people, tools and technology, and organisational and external factors all combining to affect safety. Fetal heart rate patterns need to be recorded and interpreted correctly. Systems are also required for seeking the opinions of others, determining whether the situation warrants concern, escalating concerns and mobilising response. These processes may be inadequately designed or function suboptimally, and may be further complicated by staffing issues, equipment and ergonomics issues, and competing and frequently changing clinical guidelines. Practice may also be affected by variable standards and workflows, variations in clinical competence, teamwork and situation awareness, and the ability to communicate concerns freely. Conclusions CTG monitoring is an inherently collective and sociotechnical practice. Improving it will require accounting for complex system interdependencies, rather than focusing solely on discrete factors such as individual technical proficiency in interpreting traces. BACKGROUND: The current in-service Royal Naval Fitness Test has two elements to test the aerobic endurance and muscular strength of Service Personnel through generic field-based tests and a short job task simulation. However, in 2017 the Royal Navy (RN) identified a requirement to update their in-service fitness test to align with international best practice. OBJECTIVE: The purpose of this study was to conduct an Objective Job Task Analysis on critical, physically demanding tasks that could be undertaken by RN sailors during sea deployments METHODS: Twenty-one training exercises were observed across a range of contexts; Type 23 Frigates, Type 45 Destroyers and at shore-based training facilities. A total of 203 RN personnel (age 27.8±7.1 years) were observed undertaking 36 job related tasks (e.g., 'firefighting', 'damage control', 'abandon ship' and 'casualty handling'). Tasks were evaluated by cardiovascular response, primary functional movements and contextual factors to aid a Military Judgement Panel in task list down-selection. RESULTS: 14 Criterion Tasks (e.g., 'firefighting', 'two-person magazine stowage' and 'casualty handling') were selected to progress to the next stage of the process. Five tasks were selected based on cardiovascular responses and a further nine tasks selected based on task ergonomics and other factors. CONCLUSION: This research has identified and quantified the most physically demanding, critical roles undertaken by RN sailors on sea deployments and will inform the development of the RN Physical Employment Standard. The relationship between isometric midthigh pull force-time characteristics and 2-km load-carrying performance in trained British army soldiers. J Strength Cond Res 38(2): 360-366, 2024--Load carriage is a fundamental military occupational task. As such, the aim of this study was to assess the relationship between isometric force-time characteristics and loaded march performance. Furthermore, this study aimed to investigate the relationship between isometric force-time characteristics and standing long jump (SLJ) performance. Thirty-nine, full-trained, male British Army infantry soldiers (age 3166.1 years; height 17667.3 cm; body mass 85.8611.5 kg) performed three isometric midthigh pull trials, three SLJ trials, and a 2-km loadedmarch carrying an external load of 25 kg. Both the isometricmidthigh pull test (intraclass correlation coefficient [ICC] 0.965) and SLJ (ICC 0.916) demonstrated excellent reliability. Relationships between all variables were assessed using Pearson's correlation coefficient. Isometric peak force (r520.059), relative peak force (r520.135), and rate of force development (r520.162) displayed a small correlation with loadedmarch time to completion. However, isometric relative peak force displayed a large relationship with SLJ performance (r50.545; p5,0.01). Our data demonstrate that isometric lower-limb strength measures account for, 2% of the total variance observed in 2-km loaded march performance. As such, the use of isometric lower-limb force-time characteristics as a proxy measure of load-carrying ability should be questioned. However, relative isometric strength seems to demonstrate a significant relationship with SLJ performance. As such, isometric testing may have utility in regard to assessing explosive strength, monitoring neuromuscular fatigue, and assessing training readiness in military populations. BACKGROUND: Work-related musculoskeletal disorders (WRMSDs) and ergonomic risk factors are very common in bakery workers. OBJECTIVE: The purpose of this study is to (1) assess the prevalence of musculoskeletal disorders among bakers because they use automated machines or traditional baking, and (2) to determine the strategies to prevent musculoskeletal disorders in bakers. METHODS: A systematic review of PubMed, Scopus, and Web of Science was conducted from the beginning to February 4, 2022, based on the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Mesh keywords and phrases were used to execute the search strategy. Information on MSDs and ergonomic risk factors in bakery workers was collected. Two reviewers worked independently on study selection, data extraction, and paper quality ranking. RESULTS: This study identified 14 papers from seven countries. Although the prevalence of MSDs in bakery workers has been studied, only a handful of them have been studied ergonomic risk factors, and the findings have been very limited. The association between different risk factors and MSDs seemed significant compared to many other occupational diseases. The traditional bread-baking system and lack of mechanization may increase the risk of MSDs in bakery workers. CONCLUSION: WRMSDs for bakery workers have been less studied than other occupational diseases. Our systematic review found several significant relations between the factors influencing the prevalence of MSDs. This study also showed the comparison of traditional and modern cooking systems with diseases of the upper limbs, shoulders, and back pain as possible fields for future research. Two major reports in the UK and USA have recently sanctioned as ethically acceptable genome editing of future generations for the treatment of serious rare inherited conditions. This marks an important turning point in the application of recombinant DNA techniques to humans. The central question this paper addresses is how did it became possible for human genetic engineering (HGE) of future generations to move from an illegitimate idea associated with eugenics in the 1980s to a concrete proposal sanctioned by scientists and bioethicists in 2020? The paper uses the concept of a regime of normativity to understand the co-evolution and mutual shaping of technology, imaginaries, norms and governance processes in debates about HGE in the USA and UK. It will be argued that interlinked discursive, institutional, political and technological changes have made proposals for the use of genome editing in the genetic engineering of future generations both "thinkable" and legitimate. Background: Electronic hand hygiene monitoring overcomes limitations associated with manual audit but acceptability to health workers varies and may depend on culture of the ward and the nature of the system. Objectives: Evaluate the acceptability of a new fifth type electronic monitoring system to frontline health workers in a National Health Service trust in the UK. Methods: Qualitative interviews with 11 informants following 12 months experience using an electronic monitoring system. Results: Informants recognised the importance of hand hygiene and embraced technology to improve adherence. Barriers to hand hygiene adherence included heavy workload, dealing with emergencies and ergonomic factors related to placement of alcohol dispensers. Opinions about the validity of the automated readings were conflicting. Some health workers thought they were accurate. Others reported problems associated with differences in the intelligence of the system and their own clinical decisions. Opinions about feedback were diverse. Some health workers thought the system increased personal accountability for hand hygiene. Others ignored feedback on suboptimal performance or ignored the data altogether. It was hard for health workers to understand why the system registered some instances of poor performance because feedback did not allow omissions in hand hygiene to be related to the context of care. Conclusion: Electronic monitoring can be very well tolerated despite some limitations. Further research needs to explore different reactions to feedback and how often clinical emergencies arise. Customer-facing train crew members have to follow strict procedures to guarantee that trains are safe and run on time. They are also responsible for revenue protection and customer care. Human factors and ergonomics research are instrumental to understand the safety-critical aspects and improve work. We bring user experience research and personas to describe how train crew perceive their routines and how new technology may impact them. We conducted 7 hours of interviews and 30 hours of shadowing observations with the train crew (N = 22) to provide an understanding of who are they and to define their experiences. We present the crew's current routines and created two personas to represent them. One is slightly reluctant to adopt the proposed technology, whereas the other is more accepting. Results indicate how such technology may affect crew work ergonomics and experiences, and suggest which valuable aspects should be maintained, for example the positive interactions with passengers. Practitioner summary: This study investigated the work routines of the customer-facing train crew. Interviews and shadowing were conducted with 22 crew from a large operator in the UK. In the late 1950s and early-to-mid-1960s, Glasgow led the world in the development of diagnostic obstetric ultrasound technology, the result of fortuitous collaboration between key individuals advancing the application of an industrial technology. Originally used to detect flaws in metal pressure vessels, the obstetrician Ian Donald, during his military service, reflected on how ultrasound could benefit his own field. Donald involved the engineer Tom Brown to tackle the technical challenges. Brown, in turn, employed a young graduating industrial designer, Dugald Cameron, to address the design, aesthetic and ergonomic aspects of these early engineering prototypes. While previous accounts of these developments have rightly celebrated the medical, technical engineering and imaging achievements of this innovative technology, the discussion of the role of the creative design expertise which addressed serious ergonomic, aesthetic and production manufacturing shortcomings of the first prototypes has been less coherent. This article relates, through key drawings, extracts from witness statements and discussions with Cameron himself, the key design decisions and features resulting in the Sundén and Diasonograph machines, the latter being the world's first commercial production-series obstetric ultrasound machine, deployed in Glasgow hospitals and beyond. The human desire for exploration, together with an increasingly demanding potential need for establishing a human colony away from Earth has stimulated the concept and early planning for Mars colonisation. Successful delivery of such a challenging and expensive technological feat requires a highly informed and supportive general society. Although the ergonomic constraints on astronauts living and working in space is well documented, no study has employed the use of ergonomics tools to explore colony and societal build on other worlds. We have employed Cognitive Work Analysis to derive an abstraction hierarchy for both colony establishment and maintenance with the vision of building a new Martian society. We find that our model will accommodate five major challenge themes collected from over 800 members from twenty two United Kingdom - based astronomical societies and scientific groups and propose that this study may have utility in guiding mission planners in the early colonisation phase and later stage expansion processes. Using the proposed architecture for the Mars One mission, we have performed a basic human factors analysis and propose a number of key design features which may enable long term habitation. The last 15 years has witnessed a surge in rail-related human factors research but little has examined the Track Worker role. Track work often takes place whilst trains are running under the protection of Lookouts who provide warning of approaching trains. Despite—or perhaps because of—these measures, there is a notable prevalence of incidents involving Lookouts, warranting a closer look at the nature of their task and the wider sociotechnical system (STS) to identify what may be learned for railway safety management. A systematic review of incident reports involving Lookouts was undertaken on Australian and UK rail incidents from 2006 to mid-2018 with a qualitative meta-analysis (n = 31) revealing nine factors corresponding with STS dimensions at the organisational, social, task and individual level. Much of the risk lay in the set up and implementation of Lookout working, and in Track Worker group dynamics—factors endemic to working whilst trains are running—rather than specific to the Lookout, with such factors arguably applicable across other Track Worker groups. Findings point to a need for more targeted research into the STS aspects of track working, as well as improving the training and/or procedures of those managing the Lookout protection system of work. It is well documented that exposure to chronic negative working conditions leads to stress. This subsequently impacts sickness absence and attrition, making it a key consideration for policy makers and academics alike. This study therefore seeks to investigate the influence of psycho-social working conditions on stress and related outcomes: sickness presenteeism, job satisfaction and turnover intentions in UK social workers. A cross-sectional survey was used, in addition to a single open-ended question designed to further investigate the sources of stress, to collect data from 1,333 registered social workers. Results demonstrate high levels of turnover intentions, presenteeism and low job satisfaction. Regression analyses found that the interaction between high demands, low levels of control and poor managerial support was related to social worker stress and related outcomes. Qualitative content analysis of the open-ended question corroborated and extended these findings, also demonstrating that poor ergonomic set-up of the work environment and a blame culture were adding to the experience of stress. Policy makers need to consider improvements in these working conditions or face losing a large proportion of the social work workforce. There is increasing demand for a systems approach within national healthcare guidelines to provide a systematic and sustainable framework for improvements in patient safety. Supported by this is the growing body of evidence within Human Factors/Ergonomics (HFE) healthcare literature for the inclusion of this approach in health service design, provision and evaluation. This paper considers the current interpretation of this within UK healthcare systems and the dichotomy which exists in the challenge to implement a systems approach. Three case studies, from primary and secondary care, present a systems approach, offering a novel perspective of primary care and blood sampling. These provide practical illustrations of how HFE methods have been used in collaboration with healthcare staff to understand the system for the purpose of professional education, design and safety of clinical activities. The paper concludes with the challenge for implementation and proposes five roles for systems HFE to support patient safety. Background:The European Disability Strategy (2010–2020) seeks to significantly raise the proportion of people with disabilities working in the open labour market. The ERGO WORK project is a collaboration of academic and industrial partners in six European countries, focused on understanding and tackling barriers to workplace inclusion for workers with disabilities. Methods:This study sought to explore the perceptions and needs of stakeholders in terms of workplace adaptation to the needs of employees with disabilities. An exploratory online survey was completed by 480 participants across six countries. Results:The analysis suggests that workplaces could be further improved to meet the needs of employees with considerable scope for training within companies to raise awareness about employees’ needs, employers’ obligations and workplace adaptation. Conclusions:This snapshot suggests there is still a gap between intent and reality in workplace inclusion and further strategies are needed to improve the opportunities for employees with disabilities. The paper argues that ergonomics may have a key role to play in tackling these challenges and adapting the workplace environment and job design to suit the needs of individual employees.Implications for rehabilitationThis study suggests there is considerable scope for workplace adaptation and improvements to meet the needs of employees with disabilities.Employers need and want further specialist practitioner guidance to facilitate workplace inclusion and support adaptation to individual needs.Organisations would benefit from training to raise awareness about potential solutions and approaches that would support more widespread employment of people with disabilities. A questionnaire survey of drivers (n= 903) was conducted covering musculoskeletal symptoms, the vehicle seat, access to specific vehicle features, ingress/egress, driving performance and driving behaviours. Significantly, more discomfort was reported by older drivers (aged 65+) in the hips/thighs/buttocks and knees. Older drivers reported more difficulty parallel parking (p≤ 0.01), driving on a foggy day (p≤ 0.01), and turning their head and body to reverse (p≤ 0.001). They also reported that their reactions were slower than they used to be (p≤ 0.01). Dissatisfaction was found by all drivers with adjusting the headrest (height and distance), seat belt height and opening/closing the boot. There is a growing population of older people globally, and the number of older drivers is showing a parallel increase. Clearly, efforts are needed to ensure car design of the future is more inclusive of older drivers. Purpose Due to today’s volatile business environment companies have started to establish a better understanding of the total risk/benefit-balance concerning manufacturing location decisions of their component supply. The focus is now much more on comprehensive and strategic supply chain issues rather than simply relying on piece-part cost analysis. This has led to an emerging trend called re-shoring. The purpose of this paper is to understand the primary motivation behind the re-shoring strategy in the UK and investigate the factors that influence this decision from Indian industries perspectives.Design/methodology/approach The analysis of the paper is based on interviews conducted in the UK and India (state of Tamil Nadu) in various industries including automotive, industrial goods, textile, and marine. For this purpose an interview framework based on key enablers identified from the literature, being information technology solutions, manufacturing equipment and human factors. This provided an assessment of the capability of the companies for being responsive to western demand.Findings The findings indicate that re-shoring to the UK is the result of inadequacy in responsiveness and long production lead times of the Indian suppliers. The outcome of this paper indicates that the top factors behind this inadequacy in responsiveness are logistics and transportation, electricity shortage, excessive paperwork and working attitude.Originality/value This paper aims to fill the gap in the re-shoring literature by providing a clear picture behind the reason for re-shoring in the UK and identify the drivers behind this shortcoming in the component supply from India. Background:Robotic equipment can greatly add to the ergonomics of a surgical procedure and pre-operative simulation can provide risk-free training of the surgeon leading to precision surgery and less trauma to the patient. Freehand®is a second-generation robotic camera-holding device, which has recently become available to laparoscopic surgeons. It is controlled by the operator selecting a direction using head movement followed by activation with a foot pedal. The purpose of this study was to assess the rate of skill acquisition in the use of the FreeHand®robotic laparoscope holder by a group of laparoscopic surgeons by enrolling them into a programme of training modules at The ICENI Centre, Colchester Hospital University, UK. Material and methods:Twenty surgical registrars performed a series of exercises, escalating in difficulty, to test their skill in controlling the FreeHand®robot. Subjective and objective assessments were evaluated by an observer and by tracking analysis software created for this trial. Results:The observed number of head movements showed a Percentage Performance Score (PPS) of 98% by the end of the third repetition of all exercises, the mean Total Head Movements Score (HMS) reached a plateau of performance at 72%. Fifty per cent of the participants selected ‘Effective control of movements without difficulty’ in the subjective evaluation by the end of the third repetition of exercises, while 35% selected ‘competent intuitive movements’. Conclusion:The FreeHand®robotic laparoscope holder is a useful device, which is easy to operate and requires a very short course of training to achieve competence in its use. Construction is a heavy manual industry where working into later life can be a challenge. An interview study was conducted to explore workers' understanding of their health at work and ways of making their jobs easier, safer or more comfortable. Using purposive sampling, 80 trades' workers were selected from construction sites in the UK. The Nordic Musculoskeletal Questionnaire and Work Ability Index were used to explore aches and pains and reducing strain on the body. A high prevalence of symptoms was reported and ratings of work ability were high. Workers were aware of the physical demands of their work and had over 250 ideas around health and wellbeing e.g. rucksacks for tools, bespoke benches, adapting PPE, and higher cost solutions e.g. mechanical lifting aids. Over the past decade there has been significant pressure to minimise emissions and safety risks related to commercial driving. This pressure to meet the triple bottom line of cost, environment, and society has often resulted in the rapid application of vehicle technologies designed to mitigate undesired effects. Often the cognitive and behavioural effects of technologies on the commercial driver have not received in-depth analysis to determine comprehensive viability. As such, this paper aims to identify a timescale for implementation for future technologies for UK road freight, and likely associated human factors issues, improving upon the currently employed 'trial-and-error' approach to implementation which may carry high economic, environmental, safety-related risk. Thought experiments are carried out to broadly explore these future systems. Furthermore, this work aims to examine whether technology alone will be enough to meet future CO2 reduction targets, and assess the role of behavioural and systems interventions for future research. Medication safety and errors are a major concern in care homes. In addition to the identification of incidents, there is a need for a comprehensive system description to avoid the danger of introducing interventions that have unintended consequences and are therefore unsustainable. The aim of this study was to explore the impact and uniqueness of Work Domain Analysis (WDA) to facilitate an in-depth understanding of medication safety problems within the care home system and identify the potential benefits of WDA to design safety interventions to improve medication safety. A comprehensive, systematic and contextual overview of the care home medication system was developed for the first time. The novel use of the abstraction hierarchy (AH) to analyse medication errors revealed the value of the AH to guide a comprehensive analysis of errors and generate system improvement recommendations that took into account the contextual information of the wider system. This study used a cognitive engineering method, Work Domain Analysis, to map the care home medication system and analyse medication errors. Ensuring patients are comfortable in bed is key to effective palliative care, but when moving and positioning patients in bed, health professionals face an occupational risk of injury. The turning and positioning (TAP) system is a new method of moving patients in bed, that evidence has shown to reduce the risk of injury to caregivers. Providing the correct bed surface is another aspect of bed care essential to the comfort of the palliative patient, and to aid wound prevention and treatment. It is important to take a patient-centred approach when considering the most appropriate bed surface patients. This international study aimed to understand, from the perspective of surgeons, their experience of performing minimal access surgery (MAS), to explore causes of discomfort while operating and the impact of poor ergonomics on surgeon welfare and career longevity across different specialties and techniques. A quantitative online survey was conducted in Germany, the UK and the USA from March to April 2019. The survey comprised 17 questions across four categories: demographics, intraoperative discomfort, effects on performance and anticipated consequences. In total, 462 surgeons completed the survey. Overall, 402 (87.0%) surgeons reported experiencing discomfort while operating at least 'sometimes'. The peak professional performance age was perceived to be 45-49 years by 30.7% of surgeons, 50-54 by 26.4% and older than 55 by 10.1%. 86 (18.6%) surgeons felt it likely they would consider early retirement, of whom 83 were experiencing discomfort. Our findings highlight the continued unmet needs of surgeons performing MAS, with the overwhelming majority experiencing discomfort, frequently in the back, neck and shoulders, and many likely to consider early retirement consequently. Innovative solutions are needed to alleviate this physical burden and, in turn, prevent economic and societal impacts on healthcare systems resulting from MAS limiting surgeon longevity., (© 2022. The article presents a pilot study to evaluate the applicability of human reliability assessment (HRA) principles in simple surgical skill acquisition in medical students, particularly in Great Britain, as of September 2014. Also cited are the Technique for Human Error Rate Prediction (THERP) as an HRA model, behavioural theory, and the Intercollegiate Surgical Curriculum Programme.

This article outlines how adolescents are currently overlooked as a specific user group of medical devices and positions the contribution that ergonomics (human factors) can make in mitigating this issue. Details are provided of the current barriers to adolescent inclusion in medical device design research. The discussion then provides guidance and suggested strategies for researchers, clinical staff and medical device developers about how to overcome difficulties such as ethical considerations and gaining access to this specific population. This is tackled through discussion of: informed consent, assent, gatekeepers, confidentiality, appropriateness of topics and proxies. Correct bicycle fit is important, and kinematic instability must be addressed to reduce the risk of knee injury. A change in Q Factor (horizontal distance between crank arms) has been shown to decrease metabolic cost. The combined effect upon gross mechanical efficiency (GME) and knee variability is unclear, however; there is no known simple method to predict self-selected Q Factor (SSQ). The SSQ was hypothesized to provide the greatest GME and least variability at the knee. Ten trained cyclists completed bouts of submaximal cycling at a range of Q Factors. The effectiveness of hanging and stepping tasks as predictors of SSQ was also tested. Measured SSQ (142 mm) provided the best combination of GME and knee stability compared with other Q Factors, and could be accurately predicted using a simple hanging task. Touchpads in vehicles offer a range of potential benefits over existing input devices, such as touchscreens. This article describes a study aiming to establish where a touchpad should be located within a right-hand drive vehicle. Sixteen participants (50:50 right/left handed) drove three routes in a right-hand drive simulator while following a lead vehicle at a perceived safe distance. At specific points, participants were asked to carry out three tasks of varying complexity using the touchpad. For each of the routes travelled, the touchpad was positioned in one of the three locations: in the centre console; in the door armrest and in the steering wheel. Differences in the performance and preferences of right-handed people vs. left-handed people were found. Right-handed people rated the door armrest location highly and made few glances towards this location while driving. In contrast, left-handed drivers were more positive towards the centre console location. The steering wheel location required a particularly high-visual demand. It is concluded that, for right-hand drive vehicles, a touchpad should be located in both the centre console and the door armrest to suit the diverse needs of the driver population. The aim of this study was to assess and improve how recyclers (individuals carrying out the task of recycling) make use of visual cues to carryout recycling tasks in relation to ‘recycling chimneys’ (repositories for recycled waste). An initial task analysis was conducted through an activity sampling study and an eye tracking experiment using a mobile eye tracker to capture fixations of recyclers during recycling tasks. Following data collection using the eye tracker, a set of recommendations for improving information representation were then identified using the widely researched skills, rules, knowledge framework, and for a comparative study to assess the performance of improved interfaces for recycling chimneys based on Ecological Interface Design principles. This study describes an eco-ergonomics-based approach to improve the design of interfaces for recycling chimneys. Data on health and lifestyle from the Naval service cohort study of occupational stress form the background to a discussion of employee health and the sustainability of a healthy workforce in relation to secular changes – in particular, the increasing prevalence of obesity. One thousand two hundred and forty-one Naval personnel returned questionnaires in 2007 and 2011 and reported their body mass index (BMI), alcohol consumption and smoking, general health and health conditions such as back pain and high blood pressure on both occasions. Health risk due to high BMI and waist circumference in 2011 was predicted by low participation in exercise in 2007. Those performing physically demanding work in 2007 were more likely to be in the ‘no risk’ category in 2011. High BMI in 2007 was associated with the development of back pain, musculoskeletal conditions and high blood pressure in 2011. The findings are discussed in relation to two aspects of sustainability: the sustainability of current ergonomics practice in relation to health and safety; and the role of ergonomics in sustaining a healthy and productive workforce. Practitioner Summary:Evidence is presented for the benefits of exercise in sustaining employee health and for an association between a high BMI and a number of common health complaints that have traditionally been tackled through workplace ergonomics. This is the first phase of an ergonomics study of sustainability in a rail organisation, particularly environmental sustainability. The main emphasis has been on the use of a qualitative approach to carry out in-depth consultation with those in influential and policy setting roles in the organisation, collecting and analysing perceptions on sustainability policy and related business processes. The study identified factors affecting implementation of policy on sustainability and these have been developed to produce a list of requirements for implementing the policy. The findings are valuable in understanding the range of attitudes, aspirations and perceived constraints, from the perspective of those in senior roles in the company, and development of a sustainability strategy for a rail infrastructure owner. There is need for wider consultation, both within the organisation and externally, to validate and refine the understanding of barriers to the implementation of the policy. The role of ergonomics in supporting the work on sustainability is discussed. The participatory process within four case study organisations with a target population of high mileage business drivers is described. The aim was to work with drivers and their managers to co-develop intervention activities to raise awareness of musculoskeletal health in drivers, including use of the car as a mobile office and manual handling from the car. Train-the-trainer sessions were delivered in each organisation, along with the co-production of training materials. The effectiveness of these activities were evaluated using three sources of data: post-intervention questionnaires, interviews with organisation ‘champions’ and observations from the research team's diaries. The approach raised management awareness of the risks to drivers and was successful in affecting change, and as such, participatory research should consider the early stages of a project as part of any intervention activities. The research team also reflect on conducting applied longitudinal research in the field. Abstract: A study of a small sample of construction fatal accidents was used to pilot a potential method of analysis for the UK Health and Safety Executive (HSE) Construction Division. The work was conducted in the context of a Governmental Inquiry into construction fatalities in 2009, but has been developed further since. The sample of 26 accidents (28 fatalities) was drawn from the 211 fatal accidents in the years ending 2006–2008, to be broadly illustrative of the range of accident characteristics. The accidents were analysed on the basis of available inspectorate reports and structured interviews with the investigating inspectors. A standard method of classification on four levels was developed, based on the Human Factors Analysis and Classification System (HFACS) classification of errors and task level factors, with additional categories covering the organisational and regulatory/market levels of the system. The results showed a concentration of underlying factors associated with inadequacies in planning and risk assessment, competence assurance, hardware design, purchase and installation, and contracting strategy. These findings were partially validated by comparing them with another sample of 50 accidents analysed earlier by the HSE. This paper describes the development and testing of the investigation and classification method and how it is being further developed since the initial study. A correctly fitting bra is essential for good health; this study investigates the use of professional bra fitting criteria to establish best-fit in an underwire bra commonly sold in the UK. A comparison was made between women's bra size as measured by the traditional bra fitting method with their recommended bra size based on professional bra fitting criteria. Forty-five female participants were recruited; their mode self-reported bra size was 34DD. Participants were measured in their own bra using the traditional bra-fitting method to establish their ‘traditional size’. A ‘best-fit’ bra size was recorded for participants based on professional bra fitting criteria. Significant differences were found between traditional and best-fit cup and band sizes (p < 0.001); the traditional method of bra fitting overestimated band size and underestimated cup size. As band size increased the traditional method also became more inaccurate (p < 0.001). It is recommended that women are educated in assessing their own bra fit using professional bra fitting criteria and less emphasis placed on determining absolute bra size. Practitioner Summary: This is the first study to investigate using professional bra fitting criteria to establish best-fit in an underwired bra commonly sold in the UK. This study determined how backrest inclination and the frequency of vibration influence the perception and discomfort of vibration applied parallel to the back (vertical vibration when sitting upright, horizontal vibration when recumbent). Subjects experienced backrest vibration at frequencies in the range 2.5 to 25 Hz at vibration magnitudes up to 24 dB above threshold. Absolute thresholds, equivalent comfort contours, and the principal locations for feeling vibration were determined with four backrest inclinations: 0° (upright), 30°, 60° and 90° (recumbent). With all backrest inclinations, acceleration thresholds and equivalent comfort contours were similar and increased with increasing frequency at 6 dB per octave (i.e. velocity constant). It is concluded that backrest inclination has little effect on the frequency dependence of thresholds and equivalent comfort contours for vibration applied along the back, and that the W d frequency weighting in current standards is appropriate for evaluating z-axis vibration of the back at all backrest inclinations. Statement of Relevance: To minimise the vibration discomfort of seated people, it is necessary to understand how discomfort varies with backrest inclination. It is concluded that the vibration on backrests can be measured using a pad between the backrest and the back, so that it reclines with the backrest, and the measured vibration evaluated without correcting for the backrest inclination. Little has been written about wartime ergonomics and the role this played in prompting the need for a society dedicated to ergonomics within the UK, namely the formation of the Ergonomics Research Society (ERS) in early 1950. This article aims to fill this gap in our understanding of the history of ergonomics in the UK and provide further details of the types of research undertaken by wartime research groups and committees such as the Institute of Aviation Medicine, Medical Research Council Applied Psychology Unit and the Flying Personnel Research Committee. In addition, the role of societal developments such as wartime links with the USA, the post-war drive to increase productivity and collaboration with industry and the recommendations of government committees in stimulating the work of the ERS are described in detail. This article also offers some reflection on present-day ergonomics in the UK and how this contrasts with the past. Statement of Relevance: This article will provide practitioners with a historical perspective on the development of ergonomics from its roots in the Second World War. Professional artists seem to develop a preference for certain types of pencils and paper (Akyuz 1995). While theaesthetic outcome may be the obvious reason for this preference, haptic feedback may also play a role in selecting media (Prytherch and Jerrard 2001, 2003). This suggestion implies that artists may be sensitive to vibrotactile signals resulting from the interaction between pencil and paper during drawing. Tribological tests showed that H-range leads generate more resistance than B-range pencils when writing on a standard 80 g/m 2 paper. Moreover, a constant stimulus 2IFC procedure showed that Fine Arts students found it easier to discriminate between H-range and HB than B-range and an HB. This suggests that the human haptic system may be sensitive to the vibrotactile signals generated by the differences in resistance resulting from the interaction of leads with the paper's textured surface. Implications for pencil manufacturing and simulations of virtual writing are discussed. Statement of Relevance: The study assesses the tactile sensitivity of Fine Artists to the physical properties of the interaction between different types of lead and a standard printing paper. In cognitive ergonomics, laboratory experimentation using computer-based simulations (microworlds) has played a significant role in understanding human decision making and reasoning. In this paper, we describe the design and deployment of a social work microworld (BRIGIT), which simulates the electronic recording systems now widely implemented in UK children's services. BRIGIT provides a fabricated but realistic social work environment, enabling the fine structure of professional information processing to be studied in response to experimental manipulations, such as time pressure, etc. A preliminary experiment is described here, which shows BRIGIT to provide a convincing psychological experience and a useful research tool. As well as demonstrating its face and external validity, the results highlight BRIGIT's utility as a way of probing ‘practice culture’ and for examining different patterns of professional sense making. We conclude that the microworld paradigm provides a valuable and innovative approach for researching social work practice. A range of possible applications are discussed, not only in fundamental research, but as a practical tool for use in the workplace, such as for supporting staff selection. There is also obvious potential for ‘design research’ aimed directly at improving the usability and effectiveness of electronic systems, which is important given the widely reported problems of current ICT systems in social work. Firefighting is known to be demanding, but low retirement age in this field means the capacity of the older worker to fight fires is less understood. In the Royal Fleet Auxiliary (RFA), firefighting is a critical secondary task that all personnel must be capable of. Heart rate (HR), work ability index and subjective work demand were obtained from 48 RFA personnel (18-58 years) during compulsory training. Measures of stature, mass, waist circumference (WC) and self-reported regular physical exercise were taken. The aim was to determine if cardiovascular responses were affected by age. Both cardiovascular and self-reported work demand scores were high but there was no evidence of any age-related increase in cardiovascular responses. Participation in extra-mural exercise and WC accounted for significant variance in both age-corrected HR and HR recovery. Results suggest that, in this sample, self-reported exercise and WC are more important determinants of HR response to fighting fires than age. Some limitations of the study are briefly discussed. Statement of Relevance: There is renewed interest in the work capacity of older people, particularly in demanding tasks such as firefighting. The findings suggest that factors such as self-reported regular exercise and measures of overweight/obesity are more important determinants of cardiovascular responses to high physical demands than age in firefighters up to the age of 58 years. In order to determine how the interior of a car should sound, automotive manufacturers often rely on obtaining data from individual evaluations of vehicle sounds. Company identity could play a role in these appraisals, particularly when individuals are comparing cars from opposite ends of the performance spectrum. This research addressed the question: does company identity influence the evaluation of automotive sounds belonging to cars of a similar performance level and from the same market segment? Participants listened to car sounds from two competing manufacturers, together with control sounds. Before listening to each sound, participants were presented with the correct company identity for that sound, the incorrect identity or were given no information about the identity of the sound. The results showed that company identity did not influence appraisals of high performance cars belonging to different manufacturers. These results have positive implications for methodologies employed to capture the perceptions of individuals. Statement of Relevance: A challenge in automotive design is to set appropriate targets for vehicle sounds, relying on understanding subjective reactions of individuals to such sounds. This paper assesses the role of company identity in influencing these subjective reactions and will guide sound evaluation studies, in which the manufacturer is often apparent. Abstract: Musculoskeletal disorders are one of leading causes of work related ill health and sickness absence. Those drive as part of their job may be at particular risk, with evidence suggesting that prolonged exposure to driving is associated with increased absence from work due to low back pain. Business drivers often work away from a traditional office environment. Such mobile working may pose greater risks to occupational health due to increased ergonomic risks, for example working from the car, longer working hours and a lack of concern amongst drivers about health and safety. It has been suggested that occupational health practices have not adapted to meet the needs of peripatetic workers. The current study explored how occupational health services are delivered to business drivers. Semi-structured interviews were carried out with a sample of 31 stakeholders in 4 organisations. Respondents included, health and safety professionals, occupational health nurses, fleet managers and high mileage business drivers. The interviews were transcribed and analysed using ‘Template Analysis’. The data revealed that, within these organisations, the provision of occupational health services was often fragmented and drivers and other key stakeholders were often unaware of the existing systems within their organisations. The peripatetic nature of business drivers meant that they were difficult for occupational health teams to reach. The paper concludes by presenting recommendations for occupational health professionals and researchers engaged with improving the health of peripatetic workers, namely that occupational health policies should be integrated in company strategy and widely disseminated to drivers and those with responsibility for managing their occupational health provision. Rail signalling in the UK has seen a move from mechanical lever frame boxes to entry and exit signalling, and on through to situating signallers within a visual display unit-based workstation environment. These developments have taken place in tandem with changes such as making the signallers more remote from their area of control and the introduction of automation. These changes have implications not only at a cognitive level for factors such as workload and situation awareness, but also at an organizational level, such as the shift away from traditional career progression, and the resulting implications for training and the development of expertise. Understanding the implications of the signalling interface and the design implementation of automation is critical in facilitating more effective performance, safety, and signaller well-being, as well as informing the design of future rail control systems. Bainbridge articulated a set of ironies of automation — unintended consequences of introducing automation that may not be beneficial to the overall system effectiveness. The work presented in this article uses a structured observation approach to examine behavioural indicators of the impact of automation, either as a successful tool to support signalling or as a source of some or all of the ironies noted by Bainbridge. The work was conducted over a period of 2 years, to investigate the effect of levels of automation on rail signallers’ activity and workload as part of the EPSRC Rail Research UK B6 programme. Abstract: In this paper we first outline a framework which aims to capture some of the social and organisational aspects of human factors integration (HFI) which have been outlined by previous research. The framework was partly used to design a set of interview questions that were used with a case study of a human factors team working with the UK defence industry. The findings from the case study revealed a number of barriers which accord with previous research in the domain of HFI (e.g., attitudes and perceptions towards HF), as well as providing insights into the improvement strategies used by the HF team in order to improve HFI. These included attempts to build relationships and establish a working rapport with other groups in the company, as well as other activities aimed at addressing the organisational culture within the company as a whole (e.g., attempts to raise the profile of HF within the company). We use the framework for social and organisational aspects of HFI to discuss our findings alongside other research on group behaviour and boundary management within large organisations. The conclusions of the paper point to the utility of the framework as a means of planning HFI improvement strategies which can help to overcome some of the social and organisational barriers to HFI. Low-hours solo glider pilots have a high risk of accidents compared to more experienced pilots. Numerous taxonomies for causal accident analysis have been produced for powered aviation but none of these is suitable for gliding, so a new taxonomy was required. A human factors taxonomy specifically for glider operations was developed and used to analyse all UK gliding accidents from 2002 to 2006 for their overall causes as well as factors specific to low hours pilots. Fifty-nine categories of pilot-related accident causation emerged, which were formed into progressively larger categories until four overall human factors groups were arrived at: 'judgement'; 'handling'; 'strategy'; 'attention'. 'Handling' accounted for a significantly higher proportion of injuries than other categories. Inexperienced pilots had considerably more accidents in all categories except 'strategy'. Approach control (path judgement, airbrake and speed handling) as well as landing flare misjudgement were chiefly responsible for the high accident rate in early solo glider pilots. Statement of Relevance: This paper uses extant accident data to produce a taxonomy of underlying human factors causes to analyse gliding accidents and identify the specific causes associated with low hours pilots.
