Background: The number of individuals living with multiple (≥2) long term conditions (MLTCs) is a growing global challenge. People with MLTCs experience reduced life expectancy, complex healthcare needs, higher healthcare utilisation, increased burden of treatment, poorer quality of life and higher mortality. Evolving technologies including artificial intelligence (AI) could address some of these challenges by enabling more preventive and better integrated care, however, they may also exacerbate inequities. Objective: We aim to deliver an equity focused, action-ready plan for transforming MLTC prevention and care, co-designed by people with lived experience of MLTCs and delivered through an Innovation Hub: SysteMatic. Design: Our Hub is being co-designed by people with lived experience of MLTCs, practitioners, academics and industry partners in Liverpool and Glasgow, UK. This work builds on research into mental-physical health interdependence across the life-course, and on mobilisation of large-scale quantitative data and technology validation in health and care systems serving deprived populations in Glasgow and Liverpool. We work with 3 population segments: 1) Children & Families: facing psychosocial and environmental challenges with lifetime impacts; 2). Working Life: people with poorly integrated mental, physical and social care; and 3) Pre-Frailty: older people with MLTCs. We aim to understand their experiences and in parallel look at routinely collected health data on people with MLTCs to help us identify targets for intervention. We are co-identifying opportunities for systems transformation with our patient partners, healthcare professionals and through discussion with companies and public-sector organisations. We are co-defining 3/5/7-year MLTC innovation/transition targets and sustainable learning approaches. Discussion: SysteMatic will deliver an actionable MLTC Innovation Hub strategic plan, with investment from the UK National Health Service, civic health and care partners, universities, and industry, enabling feedback of well-translated, patient and public prioritised problems into the engineering, physical, health and social sciences to underpin future equitable innovation delivery. Introduction: Age‐related macular degeneration (AMD) is the most common cause of irreversible visual impairment in the United Kingdom. It has a wide‐ranging detrimental impact on daily living, including impairment of functional ability and quality of life. Assistive technology designed to overcome this impairment includes wearable electronic vision enhancement systems (wEVES). This scoping review assesses the usefulness of these systems for people with AMD. Methods: Four databases (Cumulative Index to Nursing and Allied Health Literature, PubMed, Web of Science and Cochrane CENTRAL) were searched to identify papers that investigated image enhancement with a head‐mounted electronic device on a sample population that included people with AMD. Results: Thirty‐two papers were included: 18 studied the clinical and functional benefits of wEVES, 11 investigated use and usability and 3 discussed sickness and adverse effects. Conclusions: Wearable electronic vision enhancement systems provide hands‐free magnification and image enhancement producing significant improvements in acuity, contrast sensitivity and aspects of laboratory‐simulated daily activity. Adverse effects were infrequent, minor and spontaneously resolved with the removal of the device. However, when symptoms arose, they sometimes persisted with continued device usage. There are multi‐factorial influences and a diversity of user opinions on promotors to successful device use. These factors are not exclusively driven by visual improvement and incorporate other issues including device weight, ease of use and inconspicuous design. There is insufficient evidence of any cost–benefit analysis for wEVES. However, it has been shown that a user's decision to make a purchase evolves over time, with their estimates of cost falling below the retail price of the devices. Additional research is needed to understand the specific and distinct benefits of wEVES for people with AMD. Further patient‐centred research should assess the benefits of wEVES in user‐led activities when directly compared with alternative coping strategies, allowing professionals and users to make better prescribing and purchasing decisions. Background: The prevalence of Parkinson disease (PD) is becoming an increasing concern owing to the aging population in the United Kingdom. Wearable devices have the potential to improve the clinical care of patients with PD while reducing health care costs. Consequently, exploring the features of these wearable devices is important to identify the limitations and further areas of investigation of how wearable devices are currently used in clinical care in the United Kingdom. Objective: In this scoping review, we aimed to explore the features of wearable devices used for PD in hospitals in the United Kingdom. Methods: A scoping review of the current research was undertaken and reported according to the PRISMA-ScR (Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews) guidelines. The literature search was undertaken on June 6, 2022, and publications were obtained from MEDLINE or PubMed, Embase, and the Cochrane Library. Eligible publications were initially screened by their titles and abstracts. Publications that passed the initial screening underwent a full review. The study characteristics were extracted from the final publications, and the evidence was synthesized using a narrative approach. Any queries were reviewed by the first and second authors. Results: Of the 4543 publications identified, 39 (0.86%) publications underwent a full review, and 20 (0.44%) publications were included in the scoping review. Most studies (11/20, 55%) were conducted at the Newcastle upon Tyne Hospitals NHS Foundation Trust, with sample sizes ranging from 10 to 418. Most study participants were male individuals with a mean age ranging from 57.7 to 78.0 years. The AX3 was the most popular device brand used, and it was commercially manufactured by Axivity. Common wearable device types included body-worn sensors, inertial measurement units, and smartwatches that used accelerometers and gyroscopes to measure the clinical features of PD. Most wearable device primary measures involved the measured gait, bradykinesia, and dyskinesia. The most common wearable device placements were the lumbar region, head, and wrist. Furthermore, 65% (13/20) of the studies used artificial intelligence or machine learning to support PD data analysis. Conclusions: This study demonstrated that wearable devices could help provide a more detailed analysis of PD symptoms during the assessment phase and personalize treatment. Using machine learning, wearable devices could differentiate PD from other neurodegenerative diseases. The identified evidence gaps include the lack of analysis of wearable device cybersecurity and data management. The lack of cost-effectiveness analysis and large-scale participation in studies resulted in uncertainty regarding the feasibility of the widespread use of wearable devices. The uncertainty around the identified research gaps was further exacerbated by the lack of medical regulation of wearable devices for PD, particularly in the United Kingdom where regulations were changing due to the political landscape. Background: Remote measurement technologies, such as smartphones and wearable devices, can improve treatment outcomes for depression through enhanced illness characterization and monitoring. However, little is known about digital outcomes that are clinically meaningful to patients and clinicians. Moreover, if these technologies are to be successfully implemented within treatment, stakeholders' views on the barriers to and facilitators of their implementation in treatment must be considered. Objective: This study aims to identify clinically meaningful targets for digital health research in depression and explore attitudes toward their implementation in psychological services. Methods: A grounded theory approach was used on qualitative data from 3 focus groups of patients with a current diagnosis of depression and clinicians with >6 months of experience with delivering psychotherapy (N=22). Results: Emerging themes on clinical targets fell into the following two main categories: promoters and markers of change. The former are behaviors that participants engage in to promote mental health, and the latter signal a change in mood. These themes were further subdivided into external changes (changes in behavior) or internal changes (changes in thoughts or feelings) and mapped with potential digital sensors. The following six implementation acceptability themes emerged: technology-related factors, information and data management, emotional support, cognitive support, increased self-awareness, and clinical utility. Conclusions: The promoters versus markers of change differentiation have implications for a causal model of digital phenotyping in depression, which this paper presents. Internal versus external subdivisions are helpful in determining which factors are more susceptible to being measured by using active versus passive methods. The implications for implementation within psychotherapy are discussed with regard to treatment effectiveness, service provision, and patient and clinician experience. [ABSTRACT FROM AUTHOR]

In an effort to provide timely clinical input for people with Parkinson's disease (PD) in the face of increasing demand and resource limitation in our UK based service, we introduced remote management in place of clinic appointment, including the use of the Parkinson's KinetiGraph (PKG™), a wrist-worn device that provides a continuous measure of movement. We evaluated our reporting methods and findings, the nature of unmet need we identified, our treatment recommendations and the degree of their implementation in our patients whose feedback guided our service developments. Our evaluation highlighted opportunities and challenges associated with incorporating digital data into care traditionally delivered via in-person contact. [ABSTRACT FROM AUTHOR]

The COVID-19 pandemic has wreaked havoc globally and still persists even after a year of its initial outbreak. Several reasons can be considered: people are in close contact with each other, i.e., at a short range (1 m), and the healthcare system is not sufficiently developed or does not have enough facilities to manage and fight the pandemic, even in developed countries such as the USA and the U.K. and countries in Europe. There is a great need in healthcare for remote monitoring of COVID-19 symptoms. In the past year, a number of IoT-based devices and wearables have been introduced by researchers, providing good results in terms of high accuracy in diagnosing patients in the prodromal phase and in monitoring the symptoms of patients, i.e., respiratory rate, heart rate, temperature, etc. In this systematic review, we analyzed these wearables and their need in the healthcare system. The research was conducted using three databases: IEEE Xplore®, Web of Science®, and PubMed Central®, between December 2019 and June 2021. This article was based on the PRISMA guidelines. Initially, 1100 articles were identified while searching the scientific literature regarding this topic. After screening, ultimately, 70 articles were fully evaluated and included in this review. These articles were divided into two categories. The first one belongs to the on-body sensors (wearables), their types and positions, and the use of AI technology with ehealth wearables in different scenarios from screening to contact tracing. In the second category, we discuss the problems and solutions with respect to utilizing these wearables globally. This systematic review provides an extensive overview of wearable systems for the remote management and automated assessment of COVID-19, taking into account the reliability and acceptability of the implemented technologies. Background: In the absence of a vaccine or effective treatment for COVID-19, countries have adopted nonpharmaceutical interventions (NPIs) such as social distancing and full lockdown. An objective and quantitative means of passively monitoring the impact and response of these interventions at a local level is needed., Objective: We aim to explore the utility of the recently developed open-source mobile health platform Remote Assessment of Disease and Relapse (RADAR)-base as a toolbox to rapidly test the effect and response to NPIs intended to limit the spread of COVID-19., Methods: We analyzed data extracted from smartphone and wearable devices, and managed by the RADAR-base from 1062 participants recruited in Italy, Spain, Denmark, the United Kingdom, and the Netherlands. We derived nine features on a daily basis including time spent at home, maximum distance travelled from home, the maximum number of Bluetooth-enabled nearby devices (as a proxy for physical distancing), step count, average heart rate, sleep duration, bedtime, phone unlock duration, and social app use duration. We performed Kruskal-Wallis tests followed by post hoc Dunn tests to assess differences in these features among baseline, prelockdown, and during lockdown periods. We also studied behavioral differences by age, gender, BMI, and educational background., Results: We were able to quantify expected changes in time spent at home, distance travelled, and the number of nearby Bluetooth-enabled devices between prelockdown and during lockdown periods (P<.001 for all five countries). We saw reduced sociality as measured through mobility features and increased virtual sociality through phone use. People were more active on their phones (P<.001 for Italy, Spain, and the United Kingdom), spending more time using social media apps (P<.001 for Italy, Spain, the United Kingdom, and the Netherlands), particularly around major news events. Furthermore, participants had a lower heart rate (P<.001 for Italy and Spain; P=.02 for Denmark), went to bed later (P<.001 for Italy, Spain, the United Kingdom, and the Netherlands), and slept more (P<.001 for Italy, Spain, and the United Kingdom). We also found that young people had longer homestay than older people during the lockdown and fewer daily steps. Although there was no significant difference between the high and low BMI groups in time spent at home, the low BMI group walked more., Conclusions: RADAR-base, a freely deployable data collection platform leveraging data from wearables and mobile technologies, can be used to rapidly quantify and provide a holistic view of behavioral changes in response to public health interventions as a result of infectious outbreaks such as COVID-19. In large-scale cohort studies, sedentary behavior has been routinely measured using self-reports or devices that apply a count-based threshold. We employed a gold standard postural allocation technique using thigh inclination and acceleration to capture free-living sedentary behavior. Participants aged 46.8 (standard deviation (SD), 0.7) years (n = 5,346) from the 1970 British Cohort Study (United Kingdom) were fitted with a waterproofed thigh-mounted accelerometer device (activPAL3 micro; PAL Technologies Ltd., Glasgow, United Kingdom) worn continuously over 7 days; data were collected in 2016-2018. Usable data were retrieved from 83.0% of the devices fitted, with 79.6% of the sample recording at least 6 full days of wear (at least 10 waking hours). Total daily sitting time (average times were 9.5 (SD, 2.0) hours/day for men and 9.0 (SD, 2.0) hours/day for women) accounted for 59.4% and 57.3% of waking hours in men and women, respectively; 73.8% of sample participants recorded ≥8 hours/day of sitting. Sitting in prolonged bouts of 60 continuous minutes or more accounted for 25.3% and 24.4% of total daily sitting in men and women, respectively. In mutually adjusted models, male sex, underweight, obesity, education, poor self-rated health, television-viewing time, and having a sedentary occupation were associated with higher device-measured sitting times.
