Educators who are collaborating globally identify the potential for student-to-student global interactions leading to deeper understanding of how the world works. This qualitative study explored the phenomenon of online global collaboration through interviews with geographically dispersed K-12 educators. The aim was to understand better online global collaborative practices: how K-12 educators leveraged personal beliefs and pedagogies within the school context; and the subsequent impact on educator pedagogical practices. Findings reveal how educators developed a Global Collaborator Mindset (GCM), identified as having attributes of openness, connection, autonomy and innovation. A model of the GCM is presented, along with implications and limitations.

Purpose: Language sample analysis (LSA) provides many benefits for assessing, identifying therapy goals, and monitoring the progress of children with language disorders. Despite these widely recognized advantages, previous surveys suggest the declining use of LSA by speech-language pathologists (SLPs). This study aimed to provide updates on clinical LSA use following the recent introduction of two new LSA protocols, namely, the Sampling Utterances and Grammatical Analysis Revised (SUGAR) protocol and the Computerized Language Analysis KIDEVAL program. Method: Survey data from SLPs practicing in the United States (N = 337) were used to examine rates of LSA use, methods, and protocols. Factors predicting LSA use and reported facilitators and barriers were also examined. Results: Results indicated that 60% of SLPs used LSA in the past year. LSA skill level, training, and serving preschool or elementary school children predicted LSA use, whereas workplace, caseload, and years of experience were not significant predictors. Most SLPs reported using self-designed LSA protocols (62%), followed by Systematic Analysis of Language Transcripts (23%) and SUGAR (12%) protocols. SLPs who did not use LSA reported limited time (74%), limited resources (59%), and limited expertise (41%) as barriers and identified additional training on LSA computer programs (52%) and access to automatic speech recognition programs (49%) as facilitators to their adoption of LSA. Conclusions: Reported rates of LSA use and methods were consistent with previous survey findings. This study's findings highlight the ongoing needs for more extensive preprofessional training in LSA. Although empirical work focused on smoking-drinking comorbidity among Latinx persons is growing, no work has explored the relation between alcohol use severity in terms of co-occurring smoking processes and mental health. Therefore, the present investigation aimed to explore the prevalence and role of alcohol use severity in relation to clinically significant tobacco and mental health problems among English-speaking Latinx adults who smoke cigarettes. Participants included 338 English-speaking Latinx adults who smoked cigarettes daily (Mage = 35.5 years; SD = 8.65; age range 18–61; 37.3% female). Results indicated that approximately 68% of male and 61% of female smokers scored above established clinical cutoffs for hazardous and harmful alcohol use and possible alcohol dependence. Moreover, alcohol use severity was associated with increased risk for cigarette dependence, perceived barriers for quitting, and more problematic symptoms when trying to quit. Alcohol use severity was also related to more severe anxiety and depressive symptoms. Overall, the current findings suggest that intervening to reduce alcohol use severity may be important to improving smoking cessation and mental health among Latinx persons who smoke. Purpose: Developmental language disorder (DLD) is an underidentified neurodevelopmental disorder that affects, on average, one out of 11 kindergarten-age children (ages 4-6 years). Children with DLD can face academic, behavioral, psychiatric, emotional, and social challenges. Universal screening is seen as an effective way for public school districts to increase DLD identification rates. However, little is known about factors impacting implementation of schoolbased universal screenings for DLD. We partnered with a large suburban school district in the Intermountain West region of the United States to gather detailed perspectives from school personnel regarding the barriers and facilitators to the implementation of universal screening in their district. Method: Using a two-phase mixed-methods design, we first conducted focus groups to identify potential barriers and facilitators to universal screening. We then used the qualitative data from the first phase to develop a 20-item survey to assess agreement with the focus group results among a wider group of district speech-language pathologists and kindergarten teachers from the school district. Results: Our survey showed moderate levels of agreement with our focus group results. In particular, school personnel showed high levels of support for universal screening for DLD, with interesting interplay across various factors: (a) the negative impact of unmanageable workload on personnel under both referral- and universal-based identification formats, (b) the preference for paraprofessionals to administer screenings, (c) the role that Response to Intervention programs may play in offsetting workloads associated with universal screenings, and (d) the need for increased awareness and education about child language development and impairment among general education teachers and the public. Conclusion: Recommendations for incorporating these factors into more useful and applicable collaborative research-based efforts are presented. Background: Although rates of cigarette smoking have declined in the United States (US), individuals experiencing affective vulnerabilities (e.g., anxiety, depression) have maintained high rates of smoking. Extant work has shown distress tolerance and anxiety sensitivity underpin both emotional disorders and smoking. Yet, limited work has evaluated the concurrent and simultaneous role of distress tolerance and anxiety sensitivity on smoking-related processes such as problems when attempting to quit smoking and perceived barriers for cessation. Methods: Participants included 155 (Mage = 29.59 years; SD = 7.49; 69% male) adults seeking treatment for smoking cessation and low distress tolerance. Results: Results indicated that only anxiety sensitivity was associated with problems when attempting to quit smoking. Distress tolerance and anxiety sensitivity were each associated with perceived barriers for cessation, with stronger relations for the impact of distress tolerance. Conclusions: The current findings help elucidate the individual effects of distress tolerance and anxiety sensitivity in one overarching smoking model, thereby refining our understanding of these transdiagnostic factors in terms of clinically-relevant smoking processes. Objective: The Latinx/Hispanic (hereafter, Latinx) population in the United States (US) experiences significant tobacco-related health disparities. Extant work suggests social determinants of health (SDoH) such as perceived discrimination is an individual differences factor for cigarette smoking behavior among Latinx individuals who smoke cigarettes. Other research has suggested sensitivity to internal cues, referred to as anxiety sensitivity, is related to smoking among Latinx adults, but this work has not explored whether anxiety sensitivity may moderate the association between perceived discrimination and smoking behavior. Method: Therefore, the present investigation sought to explore the main and interactive association of perceived discrimination and anxiety sensitivity in relation to cigarettes smoked per day, severity of problems experienced when quitting, and perceived barriers for smoking cessation among 338 English-speaking Latinx individuals living in the US (Mage = 35.5 years; SD = 8.65; age range 18–61; 37.3% female) who smoke cigarettes. Results: Results supported statistically significant main effects for perceived discrimination and anxiety sensitivity in relation to increased severity of problems experienced when quitting and perceived barriers for smoking cessation. These associations were evident after adjusting for a sociodemographic covariates. Conclusion: Overall, the present investigation suggests that both perceived discrimination and anxiety sensitivity are important constructs relevant to understanding smoking processes among Latinx adults who smoke cigarettes and should be integrated in theoretical models of smoking among this population. This article explores the relationship between the locations of neuro-oncology clinical trials in the United States and social vulnerability. The authors used data from ClinicalTrials.gov and the Centers for Disease Control and Prevention's Social Vulnerability Index (SVI) to analyze the trial locations in terms of socioeconomic, household, ethnoracial, and housing/transportation factors. The findings reveal that trial locations in the Midwest and Northwest states have lower socioeconomic and household vulnerability but similar or greater vulnerability in terms of ethnoracial and housing/transportation factors. On the other hand, trial facilities in Southern states, including Texas and Louisiana, are located in counties with greater vulnerability across all SVI themes. The authors suggest that these findings highlight the need for tailored interventions to engage socially vulnerable populations in clinical trials, particularly in areas with higher relative Black/Latino populations. Background: The COVID-19 pandemic produced unprecedented demands and rapidly changing evidence and practices within critical care settings. The purpose of this study was to identify factors and strategies that hindered and facilitated effective implementation of new critical care practices and policies in response to the pandemic. Methods: We used a cross-sectional, qualitative study design to conduct semi-structured in-depth interviews with critical care leaders across the United States. The interviews were audio-taped and professionally transcribed verbatim. Guided by the Consolidated Framework for Implementation Research (CFIR), three qualitative researchers used rapid analysis methods to develop relevant codes and identify salient themes. Results: Among the 17 hospitals that agreed to participate in this study, 31 clinical leaders were interviewed. The CFIR-driven rapid analysis of the interview transcripts generated 12 major themes, which included six implementation facilitators (i.e., factors that promoted the implementation of new critical care practices) and six implementation barriers (i.e., factors that hindered the implementation of new critical care practices). These themes spanned the five CFIR domains (Intervention Characteristics, Outer Setting, Inner Setting, Characteristics of Individuals, and Process) and 11 distinct CFIR constructs. Salient facilitators to implementation efforts included staff resilience, commitment, and innovation, which were supported through collaborative feedback and decision-making mechanisms between leadership and frontline staff. Major identified barriers included lack of access to reliable and transferable information, available resources, uncollaborative leadership and communication styles. Conclusions: Through applying the CFIR to organize and synthesize our qualitative data, this study revealed important insights into implementation determinants that influenced the uptake of new critical care practices during COVID-19. As the pandemic continues to burden critical care units, clinical leaders should consider emulating the effective change management strategies identified. The cultivation of streamlined, engaging, and collaborative leadership and communication mechanisms not only supported implementation of new care practices across sites, but it also helped reduce salient implementation barriers, particularly resource and staffing shortages. Background: Smoking-related health disparities are prevalent in the Latinx community in the United States (US). Although Social Determinants of Health (SDoH) are known to contribute to a myriad of health problems among the Latinx population, there is relatively limited work expressly aimed at elucidating SDoH among Latinx smokers. Perceived racial/ethnic discrimination is one SDoH construct that may be especially relevant to understanding smoking-related cognition for Latinx smokers in the US. Smoking outcome expectancies, reflecting beliefs about the consequences of smoking, are established cognitive processes that have been linked to the maintenance and relapse of smoking. The purpose of the present work is to investigate perceived racial/ethnic discrimination in relation to smoking outcome expectancies amongst Latinx smokers in the US. Method: Participants included 338 English-speaking Latinx adult daily cigarette smokers from the US (Mage = 35.5 years; SD = 8.65; age range 18–61; 37.3% female). Results: Results indicated that perceived racial/ethnic discrimination was related to greater levels of negative reinforcement, positive reinforcement, and appetite-weight control expectancies, but not negative consequences expectancies. Discussion: Overall, the current findings provide novel insight into the relationship between perceived racial/ethnic discrimination and smoking outcome expectancies amongst English-speaking Latinx cigarette users in the US. Importance: Unhealthful dietary patterns, low levels of physical activity, and high sedentary time increase the risk of cardiovascular disease.Objective: To synthesize the evidence on benefits and harms of behavioral counseling interventions to promote a healthy diet and physical activity in adults without known cardiovascular disease (CVD) risk factors to inform a US Preventive Services Task Force recommendation.Data Sources: MEDLINE, PsycINFO, and the Cochrane Central Register of Controlled Trials through February 2021, with ongoing surveillance through February 2022.Study Selection: Randomized clinical trials (RCTs) of behavioral counseling interventions targeting improved diet, increased physical activity, or decreased sedentary time among adults without known elevated blood pressure, elevated lipid levels, or impaired fasting glucose.Data Extraction and Synthesis: Independent data abstraction and study quality rating and random effects meta-analysis.Main Outcomes and Measures: CVD events, CVD risk factors, diet and physical activity measures, and harms.Results: One-hundred thirteen RCTs were included (N = 129 993). Three RCTs reported CVD-related outcomes: 1 study (n = 47 179) found no significant differences between groups on any CVD outcome at up to 13.4 years of follow-up; a combined analysis of the other 2 RCTs (n = 1203) found a statistically significant association of the intervention with nonfatal CVD events (hazard ratio, 0.27 [95% CI, 0.08 to 0.88]) and fatal CVD events (hazard ratio, 0.31 [95% CI, 0.11 to 0.93]) at 4 years. Diet and physical activity behavioral counseling interventions were associated with small, statistically significant reductions in continuous measures of blood pressure (systolic mean difference, -0.8 [95% CI, -1.3 to -0.3]; 23 RCTs [n = 57 079]; diastolic mean difference, -0.4 [95% CI, -0.8 to -0.0]; 24 RCTs [n = 57 148]), low-density lipoprotein cholesterol level (mean difference, 2.2 mg/dL [95% CI, -3.8 to -0.6]; 15 RCTs [n = 6350]), adiposity-related outcomes (body mass index mean difference, -0.3 [95% CI, -0.5 to -0.1]; 27 RCTs [n = 59 239]), dietary outcomes, and physical activity at 6 months to 1.5 years of follow-up vs control conditions. There was no evidence of greater harm among intervention vs control groups.Conclusions and Relevance: Healthy diet and physical activity behavioral counseling interventions for persons without a known risk of CVD were associated with small but statistically significant benefits across a variety of important intermediate health outcomes and small to moderate effects on dietary and physical activity behaviors. As the global pandemic and acts of racial violence continue to impact employees and students across the United States, five members of a university arts-based research group called the Creativity Collaborative came together to support each other. The Creativity Collaborative's guiding principles of Critical Media Literacy, construction of knowledge, Expressive Arts, and Trauma-Informed Pedagogy led them to find creative ways to sustain themselves and each other through communal visual journaling via Zoom discussions. Members of the Creativity Collaborative then noticed that their work within the group enhanced their work with students. From these insights, the Creativity Collaborative makes suggestions for how higher education leaders can care and support colleagues and students during times of crisis. [ABSTRACT FROM AUTHOR]

Objective: Latino patients are overrepresented among cases of coronavirus disease 2019 (COVID‐19) and are at an increased risk of severe disease. Prevalence of COVID‐19 in Latinos with rheumatic diseases is poorly reported. This study was undertaken to characterize COVID‐19 clinical features and outcomes in Latino patients with rheumatic diseases. Methods: We conducted a retrospective study of Latino patients with rheumatic diseases from an existing observational cohort in the Washington, DC area. Patients seen between April 1, 2020 and October 15, 2020 were analyzed in this study. We reviewed demographic characteristics, body mass index (BMI), comorbidities, and use of immunomodulatory therapies. An exploratory classification and regression tree (CART) analysis along with logistic regression analyses were performed to identify risk factors for COVID‐19 and rheumatic disease flare. Results: Of 178 Latino patients with rheumatic diseases, 32 (18%) were identified as having COVID‐19, and the incidence rate of infection was found to be 3‐fold higher than in the general Latino population. No patients required intensive care unit–level care. A CART analysis and multivariable logistic regression analysis identified a BMI of >30.35 as a risk factor for COVID‐19 (odds ratio [OR] 3.37 [95% confidence interval (95% CI) 1.5–7.7]; P = 0.004). COVID‐19 positivity was a risk factor for rheumatic disease flare (OR 4.57 [95% CI 1.2–17.4]; P = 0.02). Conclusion: Our findings indicate that Latino patients with rheumatic diseases have a higher rate of COVID‐19 compared with the general Latino population. Obesity is a risk factor for COVID‐19, and COVID‐19 is a risk factor for rheumatic disease flare. Black women in the United States have experienced substantial improvements in health during the last century, yet health disparities persist. These health disparities are in large part a reflection of the inequalities experienced by Black women on a host of social and economic measures. In this paper, we examine the structural contributors to social and economic conditions that create the landscape for persistent health inequities among Black women. Demographic measures related to the health status and health (in)equity of Black women are reviewed. Current rates of specific physical and mental health outcomes are examined in more depth, including maternal mortality and chronic conditions associated with maternal morbidity. Cardiovascular disease (CVD), including hypertensive disorders of pregnancy (HDP) and peripartum cardiomyopathy, is a leading cause of pregnancy-related death in the United States. Women who are African American or American Indian/Alaskan Native, have HDP, are medically underserved, are older, or are obese have a major risk for the onset and/or progression of CVD during and after pregnancy. Paradoxically, women with no preexisting chronic conditions or risk factors also experience significant pregnancy-related cardiovascular (CV) complications. The question remains whether substantial physiologic stress on the CV system during pregnancy reflected in hemodynamic, hematological, and metabolic changes uncovers subclinical prepregnancy CVD in these otherwise healthy women. Equally important and similarly understudied is the concept that women's long-term CV health could be detrimentally affected by adverse pregnancy outcomes, such as preeclampsia, gestational hypertension, and diabetes, and preterm birth. Thus, a critical life span perspective in the assessment of women's CV risk factors is needed to help women and health care providers recognize and appreciate not only optimal CV health but also risk factors present before, during, and after pregnancy. In this review article, we highlight new advancements in understanding adverse, pregnancy-related CV conditions and will discuss promising strategies or interventions for their prevention, diagnosis, and treatment. We assess whether media professionals construct stories in ways that enhance or diminish women's legitimacy as agents of change. Our analysis of 269 broadcast news stories sampled between 1970 and 2012 examines depictions of activists in five social movements—Women's Rights, Gay Rights, Immigrant Rights, Occupy Wall Street, and Tea Party—at the intersection of gender, age, and race. We find that broadcast news coverage diminishes the legitimacy of women and older activists and activists of color by underrepresenting them, presenting opponents' claims at higher rates, and reinforcing dominant cultural narratives about political authority. Although they share many common qualities in design and operation, mesonetworks across the United States were established independently and organically over the last several decades. In numerous instances, the unique ways each network matured and developed new protocols has led to important lessons learned. These experiences have been shared in informal ways among various network operators over the years to promote reliable operation. As existing networks begin to introduce new sensors and technologies, and as new networks come online, there is a common need for guidance on best practices. This paper aims to formally provide recommendations to improve and harmonize the various aspects of operating a "mesonet," including siting, sensors, maintenance, quality assurance, and data processing. The ability of mesoscale environmental monitoring networks to collect spatially unbiased observations and to detect mesoscale environmental phenomena is radically determined by the spatial configuration of the network. However, there is lack of an objective, practical, and amenable method for guiding the spatial configuration of multifunctional, long-term mesoscale networks. The objective of this study is to present and demonstrate the application of a new method based on computational geometry that identifies the optimal location of future monitoring stations by finding the largest unmonitored area of the network. The computation of the method is first illustrated using the spatial distribution of the Kansas Mesonet as a case-study scenario and is then applied to several statewide and nationwide mesoscale networks in the United States. The proposed geometric method was effective to generate a long-term road map with the location of future monitoring stations. The geometric method seamlessly integrated with georeferenced data to identify the largest unmonitored areas with frequent occurrence of wildland fires and severe drought and to identify underrepresented soil types. Spatially dense statewide mesoscale networks with >120 stations across the studied U.S. states resulted in largest unmonitored areas of about 602 km2, whereas nationwide networks had largest unmonitored areas of 5002–6002 km2. The proposed method based on the geometric arrangement of network stations can be used by scientists, network managers, and state climatologists to improve the spatial representability of existing networks, better plan the allocation of limited resources, and increase the preparedness potential of mesoscale networks. Obesity prevalence is high in Native American (NA) adults, and there is a critical need to establish and implement evidence-based social, behavioral, and policy interventions that are theoretically informed. The use of multilevel, multicomponent (MLMC) interventions has been shown to be an effective strategy for comprehensive health behavior change; however, there is little guidance available in the literature to facilitate implementation in this underserved and understudied population. To decrease obesity and related comorbidities in NA adults, an MLMC intervention called OPREVENT (Obesity Prevention and Evaluation of InterVention Effectiveness in NaTive North Americans) was implemented in 5 rural NA communities to modify the food-purchasing environment, improve diet, and increase physical activity (PA). Five NA communities across the Upper Midwest and Southwest United States were randomly assigned to Immediate (n  = 3) or Delayed (n  = 2) Intervention. OPREVENT was implemented in Immediate Intervention community food stores, worksites, schools, and media over 1 y. A community-randomized controlled trial was used to evaluate intervention impact in adults at the individual and institutional levels, with individual-level data being collected on diet, PA, and psychosocial variables at baseline and follow-up; and institutional-level data being collected on food stores, worksites, and schools, media, and process measures. The OPREVENT intervention was one of the first MLMC obesity interventions in this population and provides evidence-based practices for future program development. BACKGROUND: Beginning in 2017, successfully passing the four-part pre-enlistment Occupational Physical Assessment Test (OPAT) became a requirement for all U.S. Army recruits. To ensure the test accurately identifies individuals who are qualified for their job, it was necessary to examine classification errors. OBJECTIVE: The objectives were to 1) determine the accuracy of OPAT cut-scores for combat arms Soldiers, and 2) determine which events contribute to the individuals that are misclassified as passing or failing the OPAT METHODS: A total of 741 trainees were tested on the OPAT within two weeks of entering their initial entry training. At the end of their training, trainees were tested on simulations of the most physically demanding tasks of their job. RESULTS: There was a high classification concordance (76.5%) between success on the OPAT and job task simulations. False positives (6.7%) were misclassified because they did not perform as well on the strength-dominant task simulations. While the interval aerobic run was the greatest contributor to false negatives (16.8%), previous studies indicated high performance on this event as a potential key indicator of injury and attrition risk. In the wake of collective traumas and acts of terrorism, media bring real graphic images and videos to TV, computer, and smartphone screens. Many people consume this coverage, but who they are and why they do so is poorly understood. Using a mixed-methods design, we examined predictors of and motivations for viewing graphic media among individuals who watched a beheading video created by the terrorist group Islamic State of Iraq and Syria (ISIS). A representative national sample of U.S. residents (N = 3,294) reported whether they viewed a video and why (or why not) via an anonymous survey administered during a 3-year longitudinal study. Accounting for population weights, about 20% of the sample reported watching at least part of a beheading video, and about 5% reported watching an entire video. Increased likelihood of watching a video was associated with demographics (male, unemployed, and Christian), frequency of typical TV watching, and both prior lifetime exposure to violence and fear of future terrorism. Watching at least part of a beheading video was prospectively associated with fear of future negative events and global distress approximately 2 years after the beheading videos went viral. The most common reasons respondents reported for watching a beheading video were information seeking and curiosity. Results suggest attentional vigilance: Preexisting fear and history of violent victimization appear to draw individuals to graphic coverage of violence. However, viewing this coverage may contribute to subsequent fear and distress over time, likely assisting terrorists in achieving their goals. [ABSTRACT FROM AUTHOR]

The article surveys the history of sport museums and halls of fame and discusses the ways they provide insight into the development and nature of contemporary sport, with special focus on the United States, where the sports hall of fame is a peculiar institution. America's sports heritage movement continues to flourish and show further signs of expansion. Commercialism and sponsorship, hero-worship and religion, amateurism and professionalism are but a few among the plethora of social phenomena and institutions involved in the heritage of sport. They will continue to reflect the place and values of sport in society in a unique manner.

The U.S. Bureau of Labor Statistics Census of Fatal Occupational Injuries (CFOI) collects information on union status for workers fatally injured on the job. Understanding how unions and collective bargaining agreements may affect workplace safety is an important area of research for policymakers, public health officials, employers, workers, and unions. This article provides background on the CFOI program and describes how the program collected union information from 2011 to 2013. It further describes the methods used as part of a special effort in Massachusetts to determine what union information was available in administrative documents. In addition, the article describes methods that may enable other CFOI state agents to generate more robust data and presents Massachusetts data by union and nonunion status for 2011-13. A systematic review was conducted to identify and analyze articles and databases that reported on physical characteristics (height, weight, and body mass index) and/or Army Physical Fitness Test (APFT) scores of male infantrymen or men in infantry units.Materials and Methods: The National Library of Medicine's PubMed and the Defense Technical Information Center were searched using specific keywords. Reference lists of obtained articles, specific author searches, contact with authors, and secondary analysis of available databases enhanced the search. Studies and databases were selected if they involved infantry soldiers or soldiers in infantry units and provided a quantitative assessment of at least one physical characteristic or APFT measure. Average values for each measure were obtained, plotted by the year of data collection, and fitted to linear regression models.Results: Thirty-one articles and five available databases met the review criteria encompassing years 1976-2015. Regression analysis suggested a small temporal increase in height for infantry soldiers (2%) and soldiers in infantry units (1%). Body weight and body mass index increased over the period (9-15%) in both groups. APFT performance of infantry soldiers was generally higher than that of infantry units. There was little change in APFT performance over the period for infantry soldiers. For soldiers in infantry units, push-up performance changed little over time, whereas sit-up performance increased (8%) and two-mile run performance decreased (8%).Conclusion: Over the surveyed period, body weight and body mass index increased in US Army infantry soldiers and soldier in infantry units. Infantry soldier performance on the APFT appears to have been maintained over the period. Returning to the community after incarceration is a particularly vulnerable time with significantly increased risk of death in the first 2 weeks. The elevated risk of death persists as long as 2 years, with cardiovascular disease (CVD) among the leading causes. African-Americans, especially African-American men, have higher rates of incarceration and community supervision (e.g., probation and parole) and an earlier onset of hypertension compared to Whites. Few studies have objectively assessed the cardiovascular health profile of criminal justice involved individuals. This study is designed to determine the cardiovascular health profile among men in community corrections and/or transitional housing, identify the prevalence of key CVD risk factors, and assess if risk varies by race/ethnicity. We recruited 100 adult men (mean age = 42.7, SD = 11.35, 60% White, 40% non-Hispanic White) with a history of incarceration in jail or prison of ≥ 6 months during their most recent incarceration and enrolled in a community corrections program. Using the American Heart Association's Life's Simple 7™ (LS-7), measures of each of the LS-7 components (body mass index, blood pressure, lipids, blood glucose, smoking, diet, and physical activity) were obtained, and LS-7 scores were generated for each measure using AHA-defined categories of poor (1 point), intermediate (2 points), and ideal (3 points) and summed to yield a total score ranging from poor for all (7 points) to ideal for all (21 points). Mann-Whitney U tests were performed to assess differences in LS-7 scores (poor, intermediate, ideal) by race/ethnicity. Additionally, an independent samples t test was conducted for race/ethnicity and LS-7 total score. Mann-Whitney U tests for LS-7 categories and race/ethnicity indicated a greater number of non-Whites had poor blood pressure (p < .01) and diet (p < .05) as compared to Whites. The independent samples t test demonstrated significantly lower LS-7 scores for non-Whites compared to Whites. To our knowledge, this is the first study to evaluate cardiovascular health among individuals with a history of incarceration using the LS-7 metric, which included objective measures for four of the seven LS-7 metrics. Non-Whites, which included African-Americans, Hispanics, and American Indians, were more likely than Whites to fall into the poor category for both diet and blood pressure and had significantly lower total LS-7 scores than Whites, indicating they have worse scores across all seven of the LS-7 measures. Similar to what is found among non-incarcerated samples, non-Whites with incarceration histories are at elevated risk for cardiovascular events relative to their White peers. [ABSTRACT FROM AUTHOR]

Cigarette smoking remains the leading preventable cause of death and disability in the United States and frequently co-occurs with anxiety and depressive symptoms. A novel and integrative, theory-driven approach to address the heterogeneity of mood-related symptoms associated with cigarette use is to focus on transdiagnostic processes, such as distress tolerance, that underpin both mood-related symptoms and smoking behavior. The current study sought to develop and examine the feasibility, acceptability, and initial efficacy of a digitally delivered integrated personalized feedback intervention (PFI) that addresses smoking-distress tolerance relations. Participants included 121 adults (71.1 % male; M age = 29.33 years, SD = 7.52) who smoked cigarettes daily and reported low distress tolerance. The study randomized participants to the Active PFI (feedback on distress tolerance and smoking) or the Control PFI (feedback on smoking only). Results indicated feasibility and acceptability demonstrated by the ability to retain participants through the 1-month follow-up (98.2 % retention rate) and positive feedback from participants, including satisfaction regarding the Active PFI. The Active PFI (vs. Control PFI) was also a statistically significant predictor of change in motivation and intention to quit smoking and willingness to use adaptive coping strategies from baseline to 1-month follow-up. For individuals with low distress tolerance who smoke cigarettes, this study's findings suggest that the current intervention may be a first-step to aid in increasing motivation/intention to quit smoking and willingness to use adaptive coping strategies. • Distress tolerance underpins emotional disturbances and smoking behavior. • Lower tolerance for distress is associated with increased smoking behavior. • The Active PFI evidenced greater motivation and intention to quit smoking. • Computer-delivered PFI's addressing distress tolerance may have clinical utility. Abstract: Drought events occurring under warmer temperatures (i.e. “hotter droughts”) have resulted in widespread tree mortality across the globe, and may result in biome‐level vegetation shifts to alternate vegetation types if there is a failure of trees to regenerate. We investigated how overstorey trees, understorey vegetation, and local climatic and edaphic conditions interact to influence tree regeneration, a key prerequisite for resilience, in a region that has experienced severe overstorey tree mortality due to hotter droughts and beetle infestations. We used detailed field observations from 142 sites that spanned a broad range of environmental conditions to evaluate the effects of climate and recent tree mortality on tree regeneration dynamics in the spatially extensive piñon (Pinus edulis)‐juniper (Juniperus osteosperma, Juniperus monosperma) woodland vegetation type of the southwestern USA. We used a structural equation modelling framework to identify how tree mortality and local climatic and edaphic conditions affect piñon and juniper regeneration and electivity analyses to quantify the species‐specific associations of tree juveniles with overstorey trees and understorey shrubs. Piñon regeneration appears to be strongly dependent upon advanced regeneration, (i.e. the survival of juvenile trees that established prior to the mortality event), the survival of adult seed‐bearing trees (inferred from basal area of surviving trees) and the facilitative effects of overstorey trees for providing favourable microsites for seedling establishment. Model results suggest that local edaphoclimatic conditions directly affected piñon and juniper regeneration, such that stands with hotter, drier local climatic conditions and lower soil available water capacity had limited tree regeneration following large‐scale dieback. Synthesis. We identify four indicators of resilience to hotter drought conditions: (1) abundant advance regeneration of tree seedlings; (2) sufficient canopy cover for survival of emergent seedlings and existing regeneration; (3) sufficient seed source from surviving trees with high reproductive output; (4) areas with cooler and wetter local climates and greater soil available water capacity. Importance: Unhealthful dietary patterns, low levels of physical activity, and high sedentary time increase the risk of cardiovascular disease.Objective: To systematically review the evidence on the benefits and harms of behavioral counseling for the primary prevention of cardiovascular disease in adults without known cardiovascular risk factors to inform the US Preventive Services Task Force.Data Sources: MEDLINE, PubMed, Cochrane Central Register of Controlled Trials, and PsycINFO for studies published in the English language between January 1, 2013, and May 25, 2016, and ongoing surveillance in targeted publications through March 24, 2017. Studies included in the previous review were reevaluated for inclusion.Study Selection: Randomized clinical trials of behavioral interventions targeting improved diet, increased physical activity, decreased sedentary time, or a combination of these among adults without known hypertension, dyslipidemia, diabetes, or impaired fasting glucose.Data Extraction and Synthesis: Independent critical appraisal and data abstraction by 2 reviewers.Main Outcomes and Measures: Cardiometabolic health and intermediate outcomes, behavioral outcomes, and harms related to interventions.Results: Eighty-eight studies (N = 121 190) in 145 publications were included. There was no consistent benefit of the interventions on all-cause or cardiovascular mortality or morbidity (4 trials [n = 51 356]) or health-related quality of life (10 trials [n = 52 423]). There was evidence of small, statistically significant between-group mean differences for systolic blood pressure (-1.26 mm Hg [95% CI, -1.77 to -0.75]; 22 trials [n = 57 953]), diastolic blood pressure (-0.49 mm Hg [95% CI, -0.82 to -0.16]; 23 trials [n = 58 022]), low-density lipoprotein cholesterol level (-2.58 mg/dL [95% CI, -4.30 to -0.85]; 13 trials [n = 5554]), total cholesterol level (-2.85 mg/dL [95% CI, -4.95 to -0.75]; 19 trials [n = 9325]), and body mass index (-0.41 [95% CI, -0.62 to -0.19]; 20 trials [n = 55 059]) at 6 to 12 months as well as small-to-modest associations with dietary and physical activity behaviors. There was no evidence of greater incidence of serious adverse events, injuries, or falls in intervention vs control participants.Conclusions and Relevance: Diet and physical activity behavioral interventions for adults not at high risk for cardiovascular disease result in consistent modest benefits across a variety of important intermediate health outcomes across 6 to 12 months, including blood pressure, low-density lipoprotein and total cholesterol levels, and adiposity, with evidence of a dose-response effect, with higher-intensity interventions conferring greater improvements. Binge drinking is a serious public health problem among American Indian adolescents, yet few theoretical models specific to this population and type of problematic drinking have been tested. The White Mountain Apache Tribe has begun surveillance of binge drinking and a related line of research to inform tailored prevention efforts. The goal of this paper is to use structural equation modeling to understand the relationships between different individual, family, peer, and cultural factors that predict or protect against binge drinking behavior among Apache adolescents ages 10-19 years old. A cross-sectional case-control study was completed with N = 68 Apache adolescents who required medical attention due to a recent binge event (past 90 days) and N = 55 controls with no lifetime history of binge drinking. The hypothesized model was estimated with Mplus using the WLSMV robust least squares estimator. In the final model, stressful life events were related to family functioning and peer relationships. In turn, family functioning affected peer relationships and adolescent impulsivity, which were both associated with greater risk of binge drinking. The path between peer relationships and having engaged in binge drinking was statistically significant for those expressing lower cultural identity, but not for those reporting higher cultural identity. Findings suggest preventive interventions should emphasize teaching coping skills to manage life stressors and handle impulsivity, strengthening families, and changing peer dynamics with social network-based approaches as well as social skill training. Currently, 2.2 million individuals are incarcerated, and more than 11 million have been released from U.S. correctional facilities. Individuals with a history of incarceration are more likely to be of racial and ethnic minority populations, poor, and have higher rates of cardiovascular risk factors, especially smoking and hypertension. Cardiovascular disease is a leading cause of death among incarcerated individuals, and those recently released have a higher risk of being hospitalized and dying of cardiovascular disease compared with the general population, even after accounting for differences in racial identity and socioeconomic status. In this review, the authors: 1) present information on the cardiovascular health of justice-involved populations, and unique prevention and care conditions in correctional facilities; 2) identify knowledge gaps; and 3) propose promising areas for research to improve the cardiovascular health of this population. In an effort to maintain the global competitiveness of the United States, ensuring a strong Science, Technology, Engineering and Mathematics (STEM) workforce is essential. The purpose of this study was to identify high school courses that serve as predictors of success in college level gatekeeper courses, which in turn led to the successful completion of STEM degrees. Using a purposive sample of 893 students who had declared a STEM major between the fall of 2006 and the spring of 2008, data were collected on students' high school grades, college grades, national test scores, grade point average, gender, and ethnicity. Using analysis of variance, correlations, multiple discriminant function analysis, and multiple regression models we found that high school calculus, physics, and chemistry (respectively) were predictors of success in STEM gatekeeper college courses. Then using those courses, we constructed a predictive model of STEM degree completion. The implications of this study highlight and reinforce the importance of providing rigorous mathematics and science courses at the high school level, as well as provide some evidence of a potential mediated model of the relationship between high school performance, college performance, and graduating with a STEM degree. The United States’ War on Drugs encompasses a body of legislation characterized by punitive approaches to drug control. These policies have resulted in escalating incarceration rates and have extracted a particularly harsh toll on low-income people of color. This article argues that education on the War on Drugs is essential for effective practice with people affected by addictions and is also an important social justice issue. It is thus the responsibility of educators to incorporate a critical examination of drug policy into course work. Strategies for addressing drug policy in social work education are provided, including the use of guest speakers and multimedia presentations, case study assignments that prompt students to examine the intersection of policy and practice, and international drug policy comparisons. Increasing disparities in out-of-school suspension and dropout rates have led a number of school districts to develop alternative models of education to include alternative learning centres (ALCs). Using an exploratory mixed methods design, this study explores dimensions of social inclusion among ALCs, located in the southeastern region of the USA. In the first phase, case-study analyses across two sites contextualised student experiences from the perspective of students, teachers, and assistant principals. In the second phase, data explored student outcomes among a sample of 593 students across 28 ALCs in the school district. Qualitative findings revealed varying dimensions of social inclusion through access, success through empowerment, and participation and engagement. Quantitative findings revealed a majority of students transitioned back to the traditional school environment. There were no clear student or school advantages associated with student transitions; however, high school students were more likely to transition back to the traditional school environment than middle school students. AimsStereotactic body radiotherapy (SBRT) for oligometastases is increasingly used with few evidenced-based guidelines. We conducted a survey to determine patient selection and follow-up practice patterns. Materials and methodsSeven institutions from US, Canada, Europe, and Australia that recommend SBRT for oligometastases participated in a 72-item survey. Levels of agreement were categorized as strong (6–7 common responses), moderate (4–5), low (2–3), or no agreement. ResultsThere was strong agreement for recommending SBRT for eradication of all detectable oligometastases with most members limiting the number of metastases to five (range 2–5) and three within a single organ (range 2–5). There was moderate agreement for recommending SBRT as consolidative therapy after systemic therapy. There was strong agreement for requiring adequate performance status and no concurrent chemotherapy. Additional areas of strong agreement included staging evaluations, primary diagnosis, target sites, and follow-up recommendations. Several differences emerged, including the use of SBRT for sarcoma oligometastases, treatment response evaluation, and which imaging should be performed during follow-up. ConclusionSignificant commonalities and variations exist for patient selection and follow-up recommendations for SBRT for oligometastases. The financial crisis and recession of 2007-09 left deep scars on the U.S. economy. Total factor productivity, a key source of long-run output growth, declined sharply during the crisis and has remained below its precrisis level. Tight credit conditions may have contributed to productivity's decline. During the crisis, widespread fear and uncertainty drove lenders to raise interest rates and lend more cautiously. As a result, firms faced reduced access to credit, potentially preventing them from investing in innovation. Redmond and Van Zandweghe examine the relationship between credit conditions and total factor productivity and find the financial crisis altered their usual relationship. During normal times, productivity growth fluctuates over the business cycle largely unaffected by credit conditions. But during the crisis, distressed credit markets significantly dampened productivity growth, leaving total factor productivity on a lower trajectory as the economy began to recover. Smoking during pregnancy is associated with poor birth outcomes. The Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) is a public nutritional assistance program for low-income pregnant women and their children up to age five. This study examined differences in smoking behavior among women enrolled in the Kansas WIC program. A secondary analysis was conducted using the Pregnancy Nutrition Surveillance System dataset of enrolled women between 2005 and 2011. Geographic residency status was obtained through application of the Census tract-based rural-urban commuting area codes. Chi square tests of association were used to assess differences. Multi-variable binary logistic regression was used to assess maternal characteristics and smoking 3 months prior to pregnancy. Total sample size averaged 21,650 women for years 2005 through 2011. Low-income, rural pregnant women smoked at significantly higher rates before, during, and after pregnancy. High smoking rates have remained unchanged since 2008. The following characteristics were associated with reduced odds of smoking 3 months prior to pregnancy: being 17 years old or younger, Hispanic, a high school graduate, urban location, normal body mass index, no live births prior to current pregnancy, and using multi-vitamins. Results from this study indicate that the WIC population in rural areas may have different needs regarding smoking cessation programming than the urban WIC population. Purpose: Co-occurring attention-deficit/hyperactivity disorder (ADHD) and communication disorders represent a frequently encountered challenge for school-based practitioners. The purpose of the present study was to examine in more detail the clinical phenomenology of co-occurring ADHD and language impairments (LIs). Method: Measures of nonword repetition, sentence recall, and tense marking were collected from 57 seven- to nine-year-old children. The performances of children with ADHD+LI status were compared with those of children with specific language impairment (SLI) and children with typical development (TD). Results: ADHD status had no independent detrimental impact on the affected children's LIs (SLI = ADHD+LI < TD). A modest positive correlation was found between the severity of children's ADHD symptoms and their sentence recall performance, suggesting a tendency for affected children who had higher levels of ADHD symptoms to perform better than those children with lower levels. Conclusion: These outcomes are difficult to reconcile with attention-deficit/information-processing accounts of the core deficits associated with SLI. Background and Objectives: Ultrasound guidance has become standard of care in hospital medicine for invasive bedside procedures, especially central venous catheter placement. Despite ultrasound-guided bedside procedures having a high degree of success, only a few hospitalists perform them. This is because these are usually performed by radiologists or in the setting of trainee-run procedure teams. We sought to determine the impact of a non-trainee driven , hospitalist-run procedure service relative to time from consult to procedure. Methods: The University of Alabama at Birmingham Hospital (UAB), Department of Hospital Medicine, trained 8 non-trainee hospitalist physicians (from existing staff) to implement the ultrasound-guided procedure service. This study examines consult to procedure completion time since the implementation of the procedure service (2014 to 2020). Univariate analyses are used to analyze pre-implementation (2012-2014), pilot (2014-2016), and post-implementation data (2016-2018 initial, and 2018-2020 sustained). Results: Results suggest a 50% reduction in time from consult to procedure completion when compared with the period before implementation of the nontrainee hospitalist procedure service. Conclusions: A hospitalist procedure service, which does not include trainees, results in less time lag from consult to procedure completion time, which could increase patient satisfaction and improve throughput. As such, this study has wide generalizability to community hospitals and other nonacademic medical centers that may not have trainees. [ABSTRACT FROM AUTHOR]

This article examines the extent to which electoral selection based on candidate quality alone can account for the pattern of re-election rates in the US Senate. In the absence of officeholder benefits, electoral selection is simulated using observed dropout rates from 1946 to 2010. This provides a benchmark for the re-election rate that would be generated by incumbent quality advantage alone. The simulation delivers a re-election rate that is almost identical to the observed rate prior to 1980, at around 78 per cent. In the later subsample, quality-based selection generates a re-election rate that is seven percentage points lower than observed. The divergence in the re-election rates in the later subsample is consistent with the findings of vote margin studies that indicate rising incumbency advantage due to officeholder benefits. In addition, it is found here that the quality-based selection first-term re-election rate is significantly lower than the observed first-term re-election rate. BACKGROUND: Military culture and workplace are areas of interest for researchers across disciplines. However, few publications on military culture exist. OBJECTIVE: The purpose of this article is to introduce general concepts regarding the structure and culture of the United States Military and discuss how this creates challenges for reintegrating into the civilian world. METHOD: Topics that will be covered in this article include an overview of the Department of Defense (DoD) and Department of Veterans Affairs (VA), socialization to military culture, the unique features of the military as a workplace, the cultural experiences of military personnel reintegrating back into the community, and the challenges faced by military members and their spouses. RESULTS: The provided information on military culture will expand military cultural competency so that civilian employers can enhance their ability to create supportive workplaces for veterans and military spouses during times of transition and reintegration.
