Background: There is strong evidence on the efficacy of behavioral modification and treatment for reducing diabetes incidence and diabetes-related morbidity and mortality in persons with pre-diabetes and diabetes. But the extent to which the evidence has translated into gains in health in these population sub-groups in the US is unclear. Monitoring national diabetes-related mortality levels over time is important for evaluating the effectiveness of the US health system response to diabetes. Methods: We identified individuals with pre-diabetes and diabetes using Hemoglobin A1c. Two consecutive periods for investigating differences in mortality according to categories of glycemia were derived using nationally representative survey data on US adults ages 35-74 from subsequent rounds of the National Health and Nutrition Examination Survey (1988-1994 and 1999-2002). Age-standardized mortality rates were calculated for individuals with pre-diabetes and diabetes and proportional hazards models were used to assess change in the relative risks of dysglycemia (pre-diabetes and diabetes) adjusting for multiple confounding factors. Results: Age-standardized mortality rates in individuals with pre-diabetes and diabetes showed no statistically significant change between 1988-2001 and 1999-2006. In individuals with pre-diabetes, mortality rates were 11.19 and 14.02 deaths per 1,000 person-years in the early and later period, respectively. The corresponding values for individuals with diabetes were 20.34 and 20.82 deaths per 1,000 person-years. In contrast, the absolute level of mortality in the normo-glycemic population declined significantly between 1988-2001 and 1999-2006 (7.81 to 6.04; p for difference < 0.05). Adjusting for social and demographic variables, smoking and body mass index in a multivariate analysis, the hazard ratio of dysglycemia increased from 1.62 (95% CI: 1.36-1.93) in 1988-2001 to 2.36 (95% CI: 1.70-3.27) in 1999-2006 (p for difference < 0.05). Conclusions: We find no evidence of declines in excess mortality in persons with dysglycemia between 1988-2001 and 1999-2006, a result that was robust to adjustment for social and demographic variables, smoking and body mass index. In the context of long-term secular declines in mortality in the US population, our findings suggest that individuals with pre-diabetes and diabetes should be an important focus of future interventions aimed at improving population health in the US. Summary: Background: Type 2 diabetes is a major independent risk factor for cardiovascular disease. Accumulating evidence suggests that there is an association between cardiovascular risk and impaired fasting glucose. This study describes the prevalence of cardiovascular risk factors and examines the coronary heart disease (CHD) risk among US adults with impaired fasting glucose (IFG). Methods: This study is a cross-sectional analysis of NHANES 1999–2004. The study included 1925 individuals with normoglycemia, 838 with IFG, and 275 subjects with diabetes. Individuals 20–79 years old and free of CHD were included in this analysis. Ten-year CHD risk was assessed by Framingham Risk Score sheets. Results: Compared to normoglycemic individuals, subjects with IFG had higher rates of obesity, hypertension, high LDL-C, low HDL-C, and high triglycerides. Among male and female adults, respectively, the 10-year absolute risk of developing CHD increased from 8.78% (95% CI, 7.54–10.03%) and 3.13% (95% CI, 2.86–3.40%) for normoglycemic subjects to 9.11% (95% CI, 8.01–10.22%) and 6.09% (95% CI, 5.62–6.56%) for subjects with IFG, and further to 14.84% (95% CI, 12.95–16.72%) and 13.04% (95% CI, 11.15–14.94%) for subjects with diabetes. Compared to normoglycemic individuals, the 10-year odds ratio for CHD was 1.83 (95% CI, 1.62–2.08) for females and 1.09 (95% CI, 0.88–1.34) for males with impaired fasting glucose. Conclusions: Females with IFG had a significant 83% increased 10-year risk of developing CHD compared to subjects with normal glucose levels. While our results need replication, controlling IFG in females may reduce their CHD risk. [Copyright &y& Elsevier]

Background: Cross-sectional surveys report a higher prevalence of diagnosed type 2 diabetes mellitus (T2DM) in African Americans (AA) than European Americans (EA). We studied 5-year glycemic excursions among AA and EA in the Pathobiology of Prediabetes in A Biracial Cohort study, to assess ethnic disparities., Materials and Methods: Pathobiology of Prediabetes in A Biracial Cohort followed normoglycemic offspring of parents with T2DM for 5 years, with serial assessments of oral glucose tolerance test , anthropometry, body fat, insulin sensitivity and beta-cell function. The primary outcome was progression to prediabetes (impaired fasting glucose and/or impaired glucose tolerance). We further analyzed 5-year changes in fasting (FPG) and 2-hour plasma glucose (2hrPG)., Results: One hundred and one (52 AA, 49 EA) out of 343 subjects developed prediabetes during follow-up. The change in FPG ranged from -24 mg/dl to +38 mg/dl. The FPG remained stable (± 5 mg/dl from baseline) in 50% of EA and 46.8% of AA and the 2hrPG remained stable (± 25 mg/dl from baseline) in 73.7% of EA and 71.0 % of AA during follow-up. The proportions with change in FPG of 5mg/dl to >25 mg/dl and 2hrPG of 25 mg/dl to >50 mg/dl were similar in EA and AA offspring, as were the 10th - 90th percentiles of the distribution of 5-year changes in FPG and 2hrPG., Conclusions: During 5 years of follow-up, black and white offspring of parents with T2DM exhibited remarkable phenotypic concordance of glycemic trajectories. Objective: Statins are one of the most widely prescribed medications in the United States; however, there is a concern that they are associated with new-onset-diabetes (NOD) development. We sought to understand the risk of dysglycemia and NOD for a cohort of individuals that reflect real-world physician prescribing patterns.Methods: A retrospective cohort study was conducted among individuals with indications for statin use (n = 7064). To examine elevated glycosylated hemoglobin (>6.0%), logistic regression with inverse probability weighting was used to create balance between incident statin users and nonusers. To evaluate the risk of NOD development, Cox PH models with time varying statin use compared NOD diagnoses among statin users and nonusers.Results: A higher prevalence of elevated HbA1c (PD = 0.065; 95% CI: 0.002, 0.129, P = 0.045) occurred among nondiabetic incident users of statins. Additionally, statin users had a higher risk of developing NOD (AHR = 2.20; 95% CI: 1.35, 3.58, P = 0.002). Those taking statins for 2 years or longer (AHR = 3.33; 95% CI: 1.84, 6.01, P < 0.001) were at the greatest risk of developing NOD; no differences were observed by statin class or intensity of dose.Conclusion: As lifestyle programs like the Diabetes Prevention Program are promoted in primary care settings, we hope physicians will integrate and insurers support healthy lifestyle strategies as part of the optimal management of individuals at risk for both NOD and cardiovascular disease. The relationships between statin use and glycemic control should be evaluated in large cohort studies, medical record databases, and mechanistic investigations to inform clinical judgment and treatment. Background: In addition to hyperglycemia, hypoglycemia, and glycemic variability, reduced time in targeted blood glucose range (TIR) is associated with increased risk of death in critically ill patients. This relation between TIR and mortality may be confounded by diabetic status and antecedent glycemic control., Methods: This study retrospectively analyzed critically ill patients managed with the same IV insulin protocol at multiple centers. The percentage of TIR between 70 and 139 mg/dL was calculated. Patients with diabetic ketoacidosis, patients who had < 10 blood glucose readings, and patients with repeat admissions were excluded. The highest recorded glycosylated hemoglobin value in the preceding 3 months or up to 1 month following admission were used as a surrogate for the patient's preexisting glucose control. Stratified regression analyses were performed for 30-day mortality, with covariates of age, sex, TIR ≥ 80%, Acute Physiology Score, and Charlson Comorbidity Index., Results: A total of 9,028 patients, 53.2% of whom had diabetes, were studied. Median TIR was 84.1% for nondiabetic patients and 64.5% for patients with diabetes. Mortality was lower in those with TIR > 80% compared with those with TIR ≤ 80% (12.4% vs 19.2%; P < .001). TIR > 80% was independently associated with reduced mortality in nondiabetic patients (OR, 0.52; P < .001), patients with diabetes (OR, 0.69; P = .001), and patients with well-controlled disease (OR, 0.50; P < .001) but not in patients with poorly controlled disease (OR, 0.86; P = .40)., Conclusions: TIR was independently associated with mortality in critically ill patients, particularly those with good antecedent glucose control., (Copyright © 2019 The Authors. Background: In 2015, The US Preventive Services Task Force (USPSTF) recommended screening for prediabetes and undiagnosed diabetes (collectively called dysglycemia) among adults aged 40-70 years with overweight or obesity. The recommendation suggests that clinicians consider screening earlier in people who have other diabetes risk factors., Objective: To compare the performance of limited and expanded screening criteria recommended by the USPSTF for detecting dysglycemia among US adults., Design: Cross-sectional analysis of survey and laboratory data collected from nationally representative samples of the civilian, noninstitutionalized US adult population., Participants: A total of 3643 adults without diagnosed diabetes who underwent measurement of hemoglobin A1c (A1c), fasting plasma glucose (FPG), and 2-h plasma glucose (2-h PG)., Main Measures: Screening eligibility according to the limited criteria was based on age 40 to 70 years old and overweight/obesity. Screening eligibility according to the expanded criteria was determined by meeting the limited criteria or having ≥ 1 of the following risk factors: family history of diabetes, history of gestational diabetes or polycystic ovarian syndrome, and non-white race/ethnicity. Dysglycemia was defined by A1c ≥ 5.7%, FPG ≥ 100 mg/dL, and/or 2-h PG ≥ 140 mg/dL., Key Results: Among the US adult population without diagnosed diabetes, 49.7% had dysglycemia. Screening based on the limited criteria demonstrated a sensitivity of 47.3% (95% CI, 44.7-50.0%) and specificity of 71.4% (95% CI, 67.3-75.2%). The expanded criteria yielded higher sensitivity [76.8% (95% CI, 73.5-79.8%)] and lower specificity [33.8% (95% CI, 30.1-37.7%)]. Point estimates for the sensitivity of the limited criteria were lower in all minority groups and significantly different for Asians compared to non-Hispanic whites [29.9% (95% CI, 23.4-37.2%) vs. 49.8% (95% CI, 45.9-53.7%); P < .001]., Conclusions: Diabetes screening that follows the limited USPSTF criteria will identify approximately half of US adults with dysglycemia. Screening other high-risk subgroups defined in the USPSTF recommendation would improve detection of dysglycemia and may reduce associated racial/ethnic disparities.
