The authors comment on an article that reports on a decrease in the presenting CD4 T cell count among patients with human immunodeficiency virus (HIV) in a large academic center in the U.S. They find that the health care system is less capable of early detection of the disease despite significant advances in antiretroviral therapy and improved services for HIV-infected persons. It is also speculated whether universal HIV testing would be enough to improve early detection and entry to care.
