The current research examined the effect of unethical and ethical acts on memory. We hypothesized that, compared with ethical acts, unethical acts, which threaten people's self-image, would be more likely to dampen the general memory process. Three studies provided supports for the hypothesis. We found in Study 1 that an unethical act led to worse performance in an incidental memory test among Chinese participants. In Study 2, to test the generalizability of the obtained results in a different culture, we recruited participants in the United States and replicated the findings from Study 1. In Study 3, we further tested whether the influence of unethical or ethical acts on memory would be different when the acts were self-related versus other-related, with self-related unethical acts being more likely to threaten people's self-image. We found that self-related but not other-related unethical acts elicited the dampening effect on memory. Finally, we showed through a mini meta-analysis of the obtained results that both ethical and unethical acts led to worse memory performance than neutral acts, and the unethical acts triggered a stronger dampening effect on memory than did ethical acts. Background: The association between serum ferritin and bone mineral density (BMD) is still controversial. This study aims to investigate the association of serum ferritin level with BMD in US adults. Methods: We conducted a cross-sectional study consisting of 8445 participants from National Health and Nutrition Examination Survey. Serum ferritin and lumbar spine BMD were used as independent variables and dependent variables, respectively. We evaluated the association between serum ferritin and lumbar spine BMD through a weighted multivariable linear regression model. Subgroup and interaction analysis was also performed in this study. Results: After adjusting for other confounding factors, serum ferritin was negatively correlated with lumbar spine BMD [β = − 0.090, 95% CI (− 0.135, − 0.045)]. Further subgroup analysis found that the strongest negative association mainly exists in females aged over 45 years [β = − 0.169, 95% CI (− 0.259, − 0.079)], and this association is not significant in other groups. Conclusions: The results found that the association between serum ferritin and lumber spine BMD differed by gender and age. Increased level of serum ferritin may indicate a higher risk of osteoporosis or osteopenia in females aged over 45 years. [ABSTRACT FROM AUTHOR]

Background: To estimate the incidence, risk factors, and impact on mortality and functional outcomes for early coagulopathy after the return of spontaneous circulation (ROSC) in pediatric out-of-hospital cardiac arrest (OHCA) patients. Methods: A post hoc analysis of the Therapeutic Hypothermia after Pediatric Cardiac Arrest Out-of-Hospital (THAPCA-OH) trial was conducted. Early coagulopathy was defined as presence of at least one of the following coagulation abnormalities upon admission: international standard ratio (INR), platelets, and age-adjusted activated partial thromboplastin time (APTT) within 6 h after OHCA and before therapeutic hypothermia initiation. The outcomes included 28-day mortality and functional prognosis. Multivariable logistic regression models were used to explore risk factors and association between early coagulopathy and outcomes. Results: Of the 227 patients included, 152 (67%) were male and the median age was 2.3 years [interquartile range (IQR), 0.7–8.6 years]. The overall 28-day mortality was 63%. The incidence of early coagulopathy was 46%. Lower age, longer duration of chest compression, lower temperature, and higher white blood cell (WBC) upon admission increased the risk of early coagulopathy. Early coagulopathy [OR, 2.20 (95% CI, 1.12–4.39), P = 0.023] was independently associated with 28-day mortality after adjusting for confounders. Conclusions: Early coagulopathy occurred in almost half of pediatric patients with OHCA. Lower age, longer duration of chest compression, lower temperature, and higher WBC increased the risk. Word-of-mouth (WOM) marketing has recently attracted a great deal of attention from marketers, especially since the emergence of social media. Three field experiments demonstrate the effects of consumer mind-set (i.e., competition versus collaboration) on narrowcasting and broadcasting intentions and actual behaviors. The results show that a collaborative mind-set promotes narrowcasting, whereas a competitive mind-set encourages broadcasting. Further, self-affirmation moderates the relationship between consumer mind-set and broadcasting. Background: Breast cancer is a heterogeneous disease, divided into subtypes based on the expression of estrogen receptor (ER), progesterone receptor (PR), and human epidermal growth factor receptor 2 (HER2). Subtypes have different biology and prognosis, with accumulating evidence of different risk factors. The purpose of this study was to compare breast cancer risk factors across tumor subtypes in a large, diverse mammography population. Methods: Women aged 40–84 without a history of breast cancer with a screening mammogram at three United States health systems from 2006 to 2015 were included. Risk factor questionnaires were completed at mammogram visit, supplemented by electronic health records. Invasive tumor subtype was defined by immunohistochemistry as ER/PR+HER2−, ER/PR+HER2+, ER, and PR−HER2+, or triple‐negative breast cancer (TNBC). Cox proportional hazards models were run for each subtype. Associations of race, reproductive history, prior breast problems, family history, breast density, and body mass index (BMI) were assessed. The association of tumor subtypes with screen detection and interval cancer was assessed using logistic regression among invasive cases. Results: The study population included 198,278 women with a median of 6.5 years of follow‐up (IQR 4.2–9.0 years). There were 4002 invasive cancers, including 3077 (77%) ER/PR+HER2−, 300 (8%) TNBC, 342 (9%) ER/PR+HER2+, and 126 (3%) ER/PR−HER2+ subtype. In multivariate models, Black women had 2.7 times higher risk of TNBC than white women (HR = 2.67, 95% CI 1.99–3.58). Breast density was associated with increased risk of all subtypes. BMI was more strongly associated with ER/PR+HER2− and HER2+ subtypes among postmenopausal women than premenopausal women. Breast density was more strongly associated with ER/PR+HER2− and TNBC among premenopausal than postmenopausal women. TNBC was more likely to be interval cancer than other subtypes. Conclusions: These results have implications for risk assessment and understanding of the etiology of breast cancer subtypes. Objective To investigate ethnic difference in the associations of BMI with comorbidity, mortality, and body composition between mainland Chinese and U.S. whites. Methods Ethnic-comparison study using data from China Health and Nutrition Survey, U.S. National Health and Nutrition Examination Survey, and data from Zhejiang University (China) and Columbia University (U.S.). Results Chinese people experienced greater odds of comorbidities than whites for a given BMI after standardizing for age and sex: 43% for diabetes, 30% for dyslipidemia, 28% for hypertension, 38% for metabolic syndrome, and 48% for hyperuricemia. Comparisons of BMI-mortality associations found that the U-shaped BMI-mortality curve shifted 1-2 kg m−2 to the left in Chinese compared to whites. Compared to whites at BMIs of 25 and 30 kg m−2, corresponding cutoffs in Chinese were 22.5 and 25.9 kg m−2 in men, and 22.8 and 26.6 kg m−2 in women after both fat and fat distribution were taken into account. Objective: Sleep disorders such as insomnia, apnea, and restless leg syndrome can negatively affect a person's overall health and may cause hypertension, heart failure, and coronary heart disease. Likewise, periodontitis, a gum disease, can lead to both physical and psychological health issues, exerting a considerable effect on one's overall well-being-periodontitis stands as a primary cause of tooth loss. Nevertheless, there has been insufficient research on the correlation between the amount of sleep individuals get and the occurrence of periodontitis/tooth loss among Americans. Therefore, this study aimed to assess the influence of sleep length on periodontitis in the American population., Methods: Periodontitis severity was classified (none, mild, moderate, and severe) using American Periodontal Association criteria. Sleep duration was assessed by self-reported data and categorized into three groups (deficient, adequate, and excessive). Tooth loss was assessed by the oral examination. To establish a connection between the duration of sleep and periodontitis/tooth loss, a weighted multivariable logistic regression analysis was employed. A GAM analysis and smooth curve fitting assessment were conducted to identify non-linear relationships. Subgroup, interaction, and mediation analyses were also performed., Results: The prevalence of tooth loss was significantly high, affecting 96.4% of the individuals, whereas 46.6% of the study sample experienced moderate to severe periodontitis. The average age of participants was 52.7 years. After adjusting for potential confounding factors, the analysis of weighted multivariable logistic regression revealed a significant association between sleep insufficiency and moderate/severe periodontitis (OR 1.15, 95% CI 1.01-1.30, P = 0.0298), as well as tooth loss (OR 1.16, 95% CI 1.01-1.33, P = 0.0371). Additionally, the research showed a correlation between the length of sleep and periodontitis that followed a U-shaped pattern. In addition, the analysis of mediation revealed that high blood pressure explained 7.0% (95% CI 4.0% to 12.9%; P < 0.0001) of the link between the amount of sleep and the likelihood of losing teeth., Conclusion: Sleep duration was independently correlated with moderate/severe periodontitis/tooth loss and had a non-linear relationship., (© 2023. Background: Cervical cancer remains a threat to female health due to high mortality. Clarification of the long-term trend of survival rate over time and the associated risk factors would be greatly informative to improve the prognosis of cervical cancer patients., Methods: This retrospective study was based on data extracted from the Surveillance, Epidemiology, and End Results (SEER) database of the United States. The 3-year and 5-year overall survival rates of patients with cervical cancer during 2002-2006, 2007-2011, and 2012-2016 were analyzed. Period analysis was used to assess the variation in survival rate stratified by age, race, and socioeconomic status during the 15-year study period and then predicted the relative survival rate in the following period from 2017 to 2021., Results: During 2002-2016, the 3-year relative survival rate of cervical cancer patients increased from 73.1% to 73.5% with a high jump between 2007 and 2011. This upward trend is expected to continue to 74.3% between 2017 and 2021. Patients older than 60 years, black ethnicity, or medium and high poverty status were likely to have a lower relative survival rate., Conclusion: This study confirmed the increased relative survival rate of cervical cancer patients over years and identified relevant risk factors. Area-based sociodemographic markers, such as census tract foreign-born population, have been used to identify individuals and communities with a high risk for tuberculosis (TB) infection in the United States. However, these markers have not been evaluated as independent risk factors for TB infection in children. We evaluated associations between census tract poverty, crowding, foreign-born population, and the CDC's Social Vulnerability Index (CDC-SVI) ranking and TB infection in a population of children tested for TB infection in Boston, Massachusetts. After adjustment for age, crowding, and foreign-born percentage, increasing census tract poverty was associated with increased odds of TB infection (adjusted odds ratio [aOR] per 10% increase in population proportion living in poverty: 1.20 [95% CI, 1.04-1.40]; P = 0.01), although this association was attenuated after further adjustment for preferred language. In separate models, increasing CDC-SVI ranking was associated with increased odds of TB infection, including after adjustment for age and language preference (aOR per 10-point increase in CDC-SVI rank: 1.08 [95% CI, 1.02-1.15]; P = 0.01). Our findings suggest area-based sociodemographic factors may be valuable for characterizing TB infection risk and defining the social ecology of pediatric TB infection in low-burden settings.

Background: Obesity is an established risk factor for severe COVID-19 outcomes. The mechanistic underpinnings of this association are not well-understood., Objective: To evaluate the mediating role of systemic inflammation in obesity-associated COVID-19 outcomes., Methods: This hospital-based, observational study included 3828 SARS-CoV-2-infected patients who were hospitalized February to May 2020 at Massachusetts General Hospital (MGH) or Columbia University Irving Medical Center/New York Presbyterian Hospital (CUIMC/NYP). We use mediation analysis to evaluate whether peak inflammatory biomarkers (C-reactive protein [CRP], erythrocyte sedimentation rate [ESR], D-dimer, ferritin, white blood cell count and interleukin-6) are in the causal pathway between obesity (BMI ≥ 30) and mechanical ventilation or death within 28 days of presentation to care., Results: In the MGH cohort (n = 1202), obesity was associated with greater likelihood of ventilation or death (OR = 1.73; 95% CI = [1.25, 2.41]; P = 0.001) and higher peak CRP (P < 0.001) compared with nonobese patients. The estimated proportion of the association between obesity and ventilation or death mediated by CRP was 0.49 (P < 0.001). Evidence of mediation was more pronounced in patients < 65 years (proportion mediated = 0.52 [P < 0.001] vs 0.44 [P = 0.180]). Findings were more moderate but consistent for peak ESR. Mediation by other inflammatory markers was not supported. Results were replicated in CUIMC/NYP cohort (n = 2626)., Conclusion: Findings support systemic inflammatory pathways in obesity-associated severe COVID-19 disease, particularly in patients < 65 years, captured by CRP and ESR. Background: Collecting duct renal cell carcinoma (CDRCC) is a rare type of renal cancer characterized by a poor prognosis. The aim of this work was to develop a nomogram predicting the overall survival (OS) and cancer-specific survival (CSS) for patients with CDRCC., Methods: A total of 324 eligible patients diagnosed with CDRCC from 2004 to 2015 were identified using the data from the Surveillance, Epidemiology, and End Results (SEER) database. The Kaplan-Meier curve was used to estimate the 1-, 3-, and 5-year OS and CSS of these patients. Univariate and multivariate Cox regression models were performed to identify the independent risk factors associated with OS and CSS. The nomogram was developed based on these factors and evaluated by the concordance index (C-index) and calibration curves using the bootstrap resample method. The predictive accuracy of the nomogram was also compared with the manual of the American Joint Committee on Cancer (AJCC)., Results: The estimated 1-, -3, and 5-year OS and CSS rates in the analytic cohorts were 56.4% and 60%, 32.5% and 37.3%, and 28.7% and 33.6%, respectively. The multivariate model revealed that age, tumor size, tumor grade, N stage, M stage, surgical type, and chemotherapy were independent predicted factors for OS, while tumor size, tumor grade, N stage, M stage, surgical type, and chemotherapy were independently linked to CSS. A nomogram was developed using these factors with relatively good discrimination and calibration. The C-index for OS and CSS was 0.764 (95% CI: 0.735~0.793) and 0.783 (95% CI: 0.754~0.812), which was superior to the AJCC stage (C-index: 0.685 (95% CI: 0.654~0.716) and 0.703 (95% CI: 0.672~0.734)). Patients were divided into low-risk, intermediate-risk, and high-risk groups according to the total points calculated by the nomogram. Patients in the low-risk group (97 mo and not reached) experienced significantly long median OS and CSS compared to the intermediate-risk (17 mo and 18 mo) and high-risk groups (5 mo for both). The calibration curves showed a good agreement between the predicted and actual probability related to OS and CSS., Conclusion: CDRCC has an aggressively biologic behavior with relatively poor prognosis. Rationale: Evidence linking saturated fat intake with cardiovascular health is controversial. The associations of unsaturated fats with total and cardiovascular disease (CVD) mortality remain inconsistent, and data about non-CVD mortality are limited., Objective: To assess dietary fat intake in relation to total and cause-specific mortality., Methods and Results: We analyzed data of 521 120 participants aged 50 to 71 years from the National Institutes of Health-American Association of Retired Persons Diet and Health Study with 16 years of follow-up. Intakes of saturated fatty acids (SFAs), trans-fatty acids, monounsaturated fatty acids (MUFAs), and polyunsaturated fatty acids (PUFAs) were assessed via food frequency questionnaires. Hazard ratios and 95%CIs were estimated using the Cox proportional hazards model. Overall, 129 328 deaths were documented during 7.3 million person-years of follow-up. In the replacement of carbohydrates, multivariable-adjusted hazard ratios of total mortality comparing extreme quintiles were 1.29 (95% CI, 1.25-1.33) for SFAs, 1.03 (1.00-1.05) for trans-fatty acids, 0.98 (0.94-1.02) for MUFAs, 1.09 (1.06-1.13) for animal MUFAs, 0.94 (0.91-0.97) for plant MUFAs, 0.93 (0.91-0.95) for PUFAs, 0.92 (0.90-0.94) for marine omega-3 PUFAs, 1.06 (1.03-1.09) for α-linolenic acid, 0.88 (0.86-0.91) for linoleic acid, and 1.10 (1.08-1.13) for arachidonic acid. CVD mortality was inversely associated with marine omega-3 PUFA intake ( P trend <0.0001), whereas it was positively associated with SFA, trans-fatty acid, and arachidonic acid intake. Isocalorically replacing 5% of the energy from SFAs with plant MUFAs was associated with 15%, 10%, 11%, and 30% lower total mortality, CVD, cancer, and respiratory disease mortality, respectively. Isocaloric replacement of SFA with linoleic acid (2%) was associated with lower total (8%), CVD (6%), cancer (8%), respiratory disease (11%), and diabetes mellitus (9%) mortality., Conclusions: Intakes of SFAs, trans-fatty acids, animal MUFAs, α-linolenic acid, and arachidonic acid were associated with higher mortality. Purpose: The TP53 tumor-suppressor gene is mutated in >95% of high-grade serous ovarian cancers. Detecting an autologous antibody response to TP53 that might improve early detection. Experimental Design: An immunoassay was developed to measure TP53 autoantibody in sera from 378 cases of invasive epithelial ovarian cancer and 944 age-matched healthy controls from the United States, Australia, and the United Kingdom. Serial preclinical samples from cases and controls were also assayed from the UK Collaborative Trial of Ovarian Cancer Screening (UKCTOCS). Results: Using a cutoff value of 78 U/mL to achieve a specificity of 97.4%, TP53 autoantibody was elevated in 30% of 50 cases from MD Anderson, 21.3% of 108 cases from the Australian Ovarian Cancer Study, and 21% of 220 cases from the UKCTOCS. Among 164 cases with rising CA125 detected with the UKCTOCS risk of ovarian cancer algorithm (ROCA), 20.7% had elevated TP53 autoantibody. In cases missed by the ROCA, 16% of cases had elevated TP53 autoantibody. Of the 34 ovarian cancer cases detected with the ROCA, TP53 autoantibody titers were elevated 11.0 months before CA125. In the 9 cases missed by the ROCA, TP53 autoantibody was elevated 22.9 months before cancer diagnosis. Similar sensitivity was obtained using assays with specific mutant and wild-type TP53. Conclusions: TP53 autoantibody levels provide a biomarker with clinically significant lead time over elevation of CA125 or an elevated ROCA value. Quantitative assessment of autoantibodies in combination with CA125 holds promise for earlier detection of invasive epithelial ovarian cancer. The design of "sham acupuncture" is necessary in clinical trials of acupuncture for confirming its effectiveness in the treatment of different disorders. The authors of the present paper made a comprehensive analysis on "sham acupuncture"or "placebo acupuncture" in clinical trials of acupuncture therapy in western countries from 1) the definition and background of sham acupuncture, 2) current state of acupuncture clinical trials with sham acupuncture design in different countries, 3) current situations of acupuncture clinical trials with sham acupuncture design in Germany, and 4) current state of large sample, randomized controlled clinical trials of acupuncture therapy with sham acupuncture design in other western countries. The authors of the present paper also make an analysis on the developing trends of acupuncture clinical trials with sham acupuncture design and put forward some problems existing in current acupuncture researches.

Objective: To compare rates of intracerebral hemorrhage (ICH) in HIV-infected and uninfected individuals in a large clinical care cohort and to assess risk factors associated with ICH., Methods: We identified incident ICH in HIV-infected and uninfected control cohorts from the Partners Health Care system using ICD-9-CM codes. We constructed Cox proportional hazards models to estimate adjusted hazard ratios for HIV infection and other predictors of ICH., Results: The incidence rate of ICH was 2.29 per 1,000 person-years in HIV-infected individuals compared with 1.23 per 1,000 person-years in uninfected individuals, with an unadjusted incidence rate ratio of 1.85 (95% confidence interval 1.37-2.47, p < 0.001). In a multivariable model, HIV infection was independently associated with a higher hazard of ICH, although its effect diminished with increasing age. Female sex was associated with a lower hazard of ICH in the uninfected cohort but not in the HIV cohort. Objectives: The objective was to evaluate the effectiveness of belt-positioning booster seats, compared with no restraint use and with seat belt use only, during motor vehicle crashes among U.S. children., Methods: This was a retrospective matched cohort study with data from the 1998 through 2009 National Automotive Sampling System (NASS) Crashworthiness Data System (CDS). The study sample consisted of children aged 0 to 10 years who were not seated in the front seat of the vehicle. We used Cox proportional hazards models to estimate the risk of overall, fatal, and regional body injury., Results: Children using seat belts in belt-positioning booster seats experienced less overall injury (Injury Severity Score [ISS] > 0, adjusted risk ratio [RR] = 0.73, 95% confidence interval [CI] = 0.55 to 0.96; Abbreviated Injury Scale [AIS] score of 2 or higher, adjusted RR = 0.30, 95% CI = 0.16 to 0.58; ISS > 8, adjusted RR = 0.19, 95% CI = 0.06 to 0.56), and less injury in most body regions except the neck (adjusted RR = 4.79, 95% CI = 1.43 to 16.00) than did children with no restraint use. Children using seat belts in belt-positioning booster seats had an equal risk of injury but higher risks of neck (adjusted RR = 1.86, 95% CI = 1.02 to 3.40) and thorax (adjusted RR = 2.86, 95% CI = 1.33 to 6.15) injury than did children restrained by seat belts only., Conclusions: Children using belt-positioning booster seats appear to experience a higher risk of AIS > 0 injury to the neck and thorax than do children using seat belts only. On the basis of MEDLINE and EMBASE database, through bibliometrics, the quantitative research was conducted on the published literatures on the acupuncture-moxibustion clinical trial abroad. The situation of published articles in each continent, country and institution was analyzed statistically. It was found that the number of published articles was higher in Germany, America, England, Sweden, Austria, Japan, South Korea, etc. In Europe, the clinical trial of acupuncture and moxibustion was in the tendency of more country participants, wider distribution and larger amount of research. In North America, America was the main country for the study. In Asia, Japan and South Korea played the leading role. Of those countries, some institutions in Germany America, and South Korea were on the top of the list. In future, the above-mentioned countries and institutions should be monitored specifically so as to launch the active cooperation and strategic project.

In the present paper, the authors analyze current state of clinical trials for acupuncture treatment of various types of diseases or clinical conditions in countries outside China in the past 30 years. The published top 5 more papers involve disorders of muscle-skeleton and connective tissues (295 papers), disorders of the nerve system (230), problems of the department of gynaecology and obstetrics (164), post-surgical complications (134), and addiction (70). Objective: This study provides insight into Occupational Medicine (OM) residency graduates and how residency programs are meeting their education goals., Methods: A survey of graduates from nine OM residency program was performed to evaluate the effectiveness of OM residency training in the United States and Canada., Results: Eighty percent of the OM residency graduates were currently practicing OM. Three-quarters worked in clinical practice for a mean of 20 hr/wk. Other activities varied and included management, teaching and consulting. Ninety-five percent were satisfied with their OM residency training. The competencies acquired were mostly ranked highly as practice requisites, although preparation in clinical OM might be better emphasized in training. Recent OM residency graduates were more likely to be board-certified in OM than other American College of Occupational and Environmental Medicine physician members (73% vs 41%)., Conclusions: OM residency graduates over the past 10 years were highly satisfied with OM residency training, with the training generally meeting practice needs.

Objective: Although some data regarding the use of thrombolysis are available from community-based studies, national estimates of the use of thrombolysis for ischemic stroke are not available. We performed this study to determine the frequency of thrombolysis for ischemic stroke in the United States and associated in-hospital outcomes., Methods: National estimates of thrombolysis, associated in-hospital outcomes, and mortality were obtained from National Hospital Discharge Survey data from 1999 to 2001. Patient numbers and frequency distributions were calculated for a nationally representative sample of patients hospitalized with a primary diagnosis of ischemic stroke. Thrombolysis was further stratified into thrombolysis with (intra-arterial) or without (intravenous) cerebral angiography., Results: There were 1,796,513 admissions for ischemic stroke between 1999 and 2001. Of these admitted patients, 1,314 (0.07%) underwent intra-arterial thrombolysis and 11,283 (0.6%) underwent intravenous thrombolysis. The days of hospitalization (mean +/- standard deviation) were significantly higher for patients admitted with ischemic stroke treated with intra-arterial thrombolysis (10.7 +/- 4.8) or intravenous thrombolysis (7.1 +/- 3.6) compared with non-thrombolytic admissions (5.4 +/- 5.7). We observed a trend toward a higher frequency of use of intravenous and intra-arterial thrombolysis and hospitals with greater number of beds (P < 0.01). The mortality rates for hospitalizations were not significantly different for admissions with ischemic stroke treated with intra-arterial or intravenous thrombolysis compared with other ischemic strokes. The rates of discharge to home after hospitalizations were significantly lower for patients treated with intravenous thrombolysis (19.8%) compared with those not treated with thrombolysis (53.1%)., Conclusion: The present study provides national estimates of patients undergoing thrombolysis for ischemic stroke. Further efforts need to be made to increase the proportion of patients with ischemic stroke who receive thrombolysis in the United States.

Objective: During the past decade, endovascular obliteration of intracranial aneurysms and new treatments for vasospasm and cerebral ischemia have been introduced. To analyze the effectiveness of these new strategies, we evaluated changes in morbidity and mortality rates in patients at least 18 years of age who were hospitalized for ruptured and unruptured intracranial aneurysms during the past 16 years., Methods: National estimates of hospitalization for subarachnoid hemorrhage (SAH) and unruptured intracranial aneurysms and associated in-hospital outcomes and mortality were obtained from National Hospital Discharge Survey data. All the variables pertaining to hospitalization were compared for three distinct time periods: 1986-1990, 1991-1995, and 1996-2001., Results: There were 94,692, 104,746, and 133,269 admissions for SAH during the periods 1986-1990, 1991-1995, and 1996-2001, respectively. Mortality rates for hospitalizations related to SAH demonstrated no significant change in mortality during the periods 1986-1990, 1991-1995, and 1996-2001 (27.6%, 24.6%, and 26.3%, respectively. Procedures performed for SAH from 1996 to 2001 included surgical clipping (28%), endovascular/wrapping (2%), and no procedure (70%). The number of admissions for unruptured intracranial aneurysms was 23,481 from 1986 to 1990, 28,017 from 1991 to 1995, and 51,904 from 1996 to 2001. There was an overall trend (P = 0.07) toward reduced in-hospital mortality during the three periods: 5.9%, 6.3%, and 1.4% for 1986-1990, 1991-1995, and 1996-2001, respectively., Conclusion: The mortality rate for unruptured intracranial aneurysms demonstrates a significant trend of reduction during the past 16 years. The mortality rate for SAH demonstrates limited change during the same period; it is presumed that this is attributable to the multitude of factors that influence outcome.
